RESUMEN
El despertar intraoperatorio ha sido una complicación anestésica infravalorada durante mucho tiempo en el adulto y mucho más en el niño. La escasa bibliografía disponible en pediatría muestra una incidencia mucho mayor que en el adulto. Actualmente se está reavivando la preocupación por este tema debido a la generalización del uso de los monitores de profundidad anestésica, que al menos en el adulto han demostrado reducir la incidencia de este evento pero que en el paciente pediátrico requieren mayor evaluación y posiblemente calibración para adaptarse a las condiciones madurativas del cerebro en las diferentes edades. La detección del despertar intraoperatorio en el paciente pediátrico presenta mayores dificultades que en el adulto y sus consecuencias que todavía no han sido bien evaluadas en el adulto, se desconocen en el niño. Revisamos los factores que pueden influir en la mayor incidencia en el niño así como las consecuencias y métodos de detección en el paciente pediátrico
ABSTRACT
Intraoperative Awareness (IA) has been an underestimated anaesthetic complication for a long time, in the adult patient and even more in the paediatric one. Limited references available show up a much higher incidence in paediatrics. Current revival of interest in awareness in adults has been stimulated by the availability of new technologies to assess anaesthetic depth, which has reduced the incidence of IA in adult patients. Nevertheless better assessment and calibration of daily used devices in children are required, in order to be adapted to maturational characteristics of the brain at different ages. Detection of IA in paediatric patients is far more difficult than in adults. Its consequences which have not been fully assessed in adults yet, are unknown in children. We review the factors that can have influence on the higher incidence of IA in paediatric patients, as well as its consequences and detection methods:
