RESUMEN
Objetivo. Analizar la importancia que la dependencia conjunta de alcohol y cocaína tiene en la presentación de diferentes trastornos psicopatológicos. Material y métodos. Se describe el caso de un paciente que solicitó una consulta en la unidad de alcoholismo por presentar dependencia de alcohol y cocaína. Asimismo, se mencionan los datos más relevantes que se observaron durante los 6 años en que el paciente estuvo en seguimiento. Resultados. A pesar de los diferentes tipos de abordaje terapéutico utilizados (desintoxicación hospitalaria y tratamiento ambulatorio), el paciente estuvo abstinente de alguna de las sustancias durante cortos períodos de tiempo. Durante el seguimiento presentó dos episodios que fueron diagnosticados como trastorno bipolar tipo dos. Además, presentaba un trastorno del comportamiento sexual, etiquetado como parafílico, cuyas primeras manifestaciones precedieron al consumo de drogas y se manifestaba clínicamente bajo su efecto. Conclusiones. El consumo de alcohol y cocaína incrementa la morbilidad psiquiátrica y empeora el pronóstico de la dependencia de ambas sustancias, el abordaje terapéutico es más complejo y el cumplimiento terapéutico es menor
ABSTRACT
Objective. To analyze the importance that both alcohol and cocaine dependence have in the appearance of different psychopathological disorders. Material and methods. The authors describe a patient’s case who demanded a consult in the alcoholism unit to treat alcohol and cocaine dependence. They also mention the most important data they observed during the six-year treatment of the patient. Results. The patient was treated with various kinds of therapeutic approaches (hospital detoxification, outpatient treatment) and he was off alcohol and/or cocaine for short periods of time. In the follow-up the patient had two events which were diagnosed as bipolar disorder, type two. He also had a sexual disorder labelled as paraphilias, which first symptoms appeared before drug use and at the present moment this disorder shows with drug taking. Conclusions. The alcohol and cocaine taking increases the psychiatric morbidity and the dependence prognosis of both substances is worse, the therapeutic approach is more complex and the compliance with the treatment is lesser
