RESUMEN
Introducción. La enfermedad cerebrovascular isquémica integra un conjunto de entidades que se producen por alteraciones de los componentes de la sangre, del flujo sanguíneo o de las paredes de los vasos sanguíneos y del corazón, tanto anatómicas como funcionales,e incluso mixtas. Caso clínico. Paciente de sexo masculino de 18 meses de edad, con enfermedad celíaca compensada, con ictus isquémicos de repetición en diferentes territorios, la arteria cerebral posterior y la arteria cerebral media derechas, en el curso de un estado de hipercoagulabilidad por trombocitosis esencial. Se le realizó una tomografía axial computarizada, angiorresonancia cerebral, hemoquímica completa y biopsia de médula ósea, y se constata el diagnóstico anteriormente citado. Se impuso un tratamiento con exanguinotransfusión, antiagregantes plaquetarios y un inhibidor de la producción de plaquetas (anagrelide). Actualmente, el paciente tiene 2 años y mantiene una ligera hemiparesia directa, total y con predominio faciobraquial, sin alteraciones del lenguaje. Conclusiones. Los infartos cerebrales en la infancia son excepcionales; sin embargo, su presentación obliga al clínico que lo atiende a la búsqueda de causas no habituales; en nuestro paciente, la hipercoagulabilidad se dio por una trombocitosis esencial, lo cual es raro en la infancia. La causa que motiva el infarto determina su probabilidad de recurrencia; el ácido acetilsalicílico no demostró ser efectivo para este fin. Sugerimos el uso de la carbamacepina para el tratamiento de las distonías cinesigénicas (AU)
ABSTRACT
Introduction. Ischemic cerebrovascular disease includes a set of entities that are produced by disorders in components of the blood, the blood flow, the walls of blood vessels or the heart, and can be anatomical, functional or even mixed. Case report. We describe the case of an 18-month-old male patient with compensated celiac disease, with repeated ischemic strokes in different territories, including the right posterior cerebral artery and middle cerebral artery, in the course of a hypercoagulable state due to essential thrombocytosis. Computerised axial tomography scans, brain angioresonance, a complete blood chemistry analysis and bone marrow biopsy were all performed and confirmed the previous diagnosis. Exchange transfusion, antiplatelet drugs and a platelet production inhibitor (anagrelide) were begun as therapy. At present, the patient is 2 years old and still has a slight direct hemiparesis, which is complete and predominantly faciobrachial, with no alterations to language. Conclusions. Cerebral infarctions in infancy are infrequent, and their presentation obliges the attending clinician to seek causes that are not usual. In our patient the hypocoagulability came about due to essential thrombosis, which is rare in infancy. The cause behind the infarction determines the chances of its recurring. Acetylsalicylic acid did not prove to be effective for this purpose. We suggest using carbamazepine for the treatment of kinesigenic dystonias (AU)
