RESUMEN
Objetivos. Análisis predictivo de los datos de la Unidad de Tabaquismo del Complejo Hospitalario de Albacete desde enero del 2008 hasta diciembre del 2009. Métodos. Estudio estadístico predictivo utilizándose tres modelos de clasificación árbol de decisión c4.5, modelo de regresión logística multinomial y red bayesiana para validar clasificadores capaces de predecir el éxito en la deshabituación. Las métricas utilizadas fueron la tasa de aciertos y AUC. Resultados. Árbol de decisión la predicción del éxito, considerado como abstinencia continua durante un año, a partir de las variables cooximetría, depresión y tipo de tratamiento, obtuvo una tasa de aciertos del 68,2% y AUC de 0,63. Si el tipo de tratamiento era vareniclina se predecía éxito en la deshabituación; no utilizar ningún tipo de tratamiento sugería que el paciente no tendría éxito en la cesación. Si el tratamiento era terapia sustitutiva nicotínica (TSN) o bupropion, el éxito dependería del nivel de cooximetría y de si el paciente padecía depresión. Regresión logística una mayor cooximetría, tener depresión, comorbilidad psiquiátrica, enfermedad cardiovascular (ECV) o enfermedad respiratoria y/o cardiovascular (ERESECV) aumentaban la probabilidad de no tener éxito; por el contrario, tener enfermedad respiratoria (ER) y EPOC, mayor probabilidad de tenerlo (tasa de aciertos del 72,8% y AUC = 0,67). Finalmente, mayor puntuación en el test de Richmond y tratamiento con vareniclina, mayor probabilidad de tener éxito mientras que un consumo mayor de cigarrillos y no tener síndrome de apneas-hipopneas del sueño (SAHS) aumentaba la probabilidad de no tenerlo (tasa de aciertos del 75,6% y AUC = 0,65). Conclusiones. Una motivación alta por el test de Richmond puede llevar al éxito en cesación tabáquica al igual que utilizar tratamiento farmacológico con vareniclina, bupropion y terapia sustitutiva nicotínica (TSN); la probabilidad de no tener éxito será mayor en pacientes con depresión, enfermedad cardiovascular (ECV) o ERESPECV, cooximetrías elevadas y aquellos sin SAHS (AU)
ABSTRACT
Objectives. Predictive analysis of the results obtained in a Tobacco Cessation Unit (TCU) in Albacete University Hospital to validate capable classifiers to predict success in tobacco cessation. Methods. Predictive statistical analysis used three classification models c4.5 decision tree, multinomial logistic regression model and Bayesian network to validate classifiers capable of predicting success in tobacco cessation. The metrics used were the hit rate and AUC. Results. Decision tree The prediction of success, considered as the maintenance of continuous abstinence during a year, for cooximetry, depression and type of treatment had a hit rate of 68.2% and AUC of 0.63. If treatment used was varenicline, a positive successful was predicted in tobacco cessation; however, if any treatment was not performed, it suggested that the patient could not quit. If treatment was nicotine replacement therapy (NRT) or bupropion, the success depended on cooximetry level and whether the patient was suffering from depression. Logistic regression model Mayor cooximetry, depression, psychiatric comorbidity, cardiovascular disease (ECV) and respiratory and / or cardiovascular disease (ERESECV) increased the possibility of not succeeding; on the other hand, respiratory disease (ER) and and COPD (hit rate, 73.9% and AUC, 0.7). Finally, higher score on Richmond test and varenicline therapy led to a higher probability of success while the consumption of more cigarettes per a day and not be dignosed as obstructive sleep apneas syndrome (OSAS) increased the probability of not success (hit rate of 75.6% and AUC = 0.65). Conclusions. High motivation measured by Richmond test leads to success in tobacco cessation if we use pharmacological treatment with varenicline, bupropion and nicotine replacement therapy; the probability of not success is higher in patients with depression, cardiovascular disease or ERESPECV, high cooximetry and in those patients not diagnosed with OSAS (AU)
