Repercusiones del cuidar en las necesidades básicas del cuidador primario de pacientes crónicos y terminales / Repercussions of caring on the basic needs of the primary caregiver for chronically and terminally patients

RESUMEN

OBJETIVO: Evaluar el nivel de repercusión total del cuidar, e identificar las repercusiones en las 14 necesidades básicas del cuidador primario de pacientes crónicos y terminales METODOLOGÍA: Tipo de estudio Estudio transversal, prospectivo y descriptivo. Población Cuidadores primarios de pacientes con enfermedad crónica incapacitante o terminal pertenecientes al programa de Atención Médica en Domicilio (AMED) del Hospital General de Zona No.47 del Instituto Mexicano del Seguro Social (IMSS) en el Distrito Federal. Tipo y tamaño de muestra Muestreo no probabilístico por conveniencia. Cálculo del tamaño de muestra para estudios descriptivos con nivel de significación del 95%. INTERVENCIONES Se aplicó ficha de identificación y cuestionario «Repercusiones del cuidar en el cuidador». Análisis estadístico Se utilizó la prueba estadística de Kruskal-Wallis. Nivel de significación de 0,05, programa SPSS versión 18 RESULTADOS: De los 83 cuidadores el 21,7% fueron hombres y el 78,3% mujeres con edad media de 52,2 años; el nivel de repercusión total en la población de estudio fue severo y alto en el 21,7%; medio en el 31,3%; y leve en el 25,3%. El tiempo de cuidado fue la única variable analizada con significación estadística (p = 0,044). CONCLUSIONES: En cerca del 75% de la población estudiada, el rol de «cuidador primario» es una experiencia que conlleva un deterioro general en la vida de quien lo ejecuta, exponiéndolo a sufrir alteraciones en sus necesidades básicas y vulnerando su integridad

ABSTRACT

OBJECTIVE: To assess the overall repercussions of caring, and to identify the impact on the fourteen basic needs of the primary caregiver of chronic and terminal PATIENTS METHODOLOGY: Type of study Cross-sectional, prospective and descriptive study. Population Primary caregivers of patients with disabling or terminal chronic illness, taking part in the Medical Care at Home Program (AMED) of General Hospital No.47 of the Mexican Institute of Social Security (IMSS) in Mexico City. Type and size of the sample Non-probability convenience sample using a sample size calculated for descriptive studies with a significance level of 95%. INTERVENTIONS: An identification sheet and the 'Impact of caring on the caregiver' questionnaire was used. Statistical analysis: The statistical Kruskal-Wallis test was used. Significance level 0.05, SPSS. Version 18. RESULTS: Of the 83 caregivers, 21.7% were men and 78.3% women, with a mean age of 52.2 years. The level of total impact in the study population was severe and high in 21.7%, with 31.3% average, and mild 25.3%. The time of care was the only variable analyzed with statistical significance (P = .044). CONCLUSIONS: In about 75% of the population studied, the role of "primary caregiver" is an experience that leads to a general deterioration in the life of the caregiver, exposing them to suffer disturbances in their basic needs and damaging their integrity
