RESUMEN

La propia estructura sanitaria favorece la cronificación de la patología dolorosa de columna debido a la falta razonable de unidad de criterios unido a las listas de espera tanto de consultas como para tratamientos. Se debería poder clasificar de forma rápida atendiendo a los signos guía de gravedad, tipo e irradiación del dolor y existencia de clínica neurológica. En ese momento se debe poner en marcha un protocolo de exploraciones urgentes y no urgentes para llegar al diagnóstico etiológico y de localización. Por otro lado no todas las pruebas complementarias tienen el mismo valor. Se abusa de pruebas de ‘última generación’, que presupone que son más fiables y más precisas. Se debe romper este mito, la historia clínica y la exploración sigue siendo la mejor y más económica prueba diagnóstica (AU)

ABSTRACT

The health system itself favors the chronification of painful spinal pathology due to the reasonable lack of standardized criteria, in addition to long waiting lists both for consultation and treatment. This type of pain should be easily classified according to the signs of pain severity, type and irradiation, as well as the presence of neural manifestations. A protocol of emergency and routine tests must be developed in order to establish the etiologic and location diagnosis. But not all the complementary tests have the same value. ‘Last generation’ tests are overused supposing that they are most reliable and accurate. Such myth has to be destroyed , since the clinical history and the exploration are still the best and most economic diagnostic tests (AU)
