RESUMO
Foram avaliados os efeitos da terapia de ressincronização cardíaca (TRC) sobre o controle neurovascular na insuficiência cardíaca avançada (IC). A atividade nervosa simpática muscular (ANSM) e a resistência vascular muscular (RVM) foram avaliadas durante o repouso, exercício isométrico moderado (EIM) e durante o isolamento do controle do metaborreceptor muscular (MM) em 11 pacientes com indicação clássica à TRC. A qualidade de vida (QV) e o consumo de oxigênio no pico do exercício (VO2 pico) também foram avaliados. Após 3 meses de TRC, observou-se redução da ANSM ao repouso e EIM sem modificação da RVM; normalização do controle do MM previamente atenuado e melhora da QV e VO2 pico. ABSTRACT
We evaluated the effects of cardiac resynchronization therapy (CRT) on neurovascular control in advanced heart failure (HF). Muscle sympathetic nerve activity (MSNA) and forearm vascular resistance (FVR) were recorded at rest, during moderate static exercising (MSE), and during isolation of muscle metaboreceptor (MM) control in 11 HF patients with classical indication for CRT. Quality of life (QoL) and peak oxygen consumption (peak VO2) were also evaluated. CRT results in uniform MSNA reductions at rest and during MSE. FVR was not different between groups. Before CRT MM control was blunted while it was normalized after CRT. Peak VO2 and QoL were improved. There was a high inverse correlation between MSNA and Peak VO2...
