RESUMEN
Claudicación intermitente (CI), síntoma clásico de la enfermedad arterial de extremidades inferiores. Toma cada día mayor importancia debido a su correlación con enfermedad ateroesclerótica sistémica y al aumento de su prevalencia, dado la mayor sobrevida de la población. Alrededor de un 20 por ciento de la población sobre 60 años presenta enfermedad arterial periférica, de los cuales sólo un 25 por ciento de ellos es sintomático. En los pacientes claudicadores se debe evaluar el sistema circulatorio en su totalidad, poniendo énfasis en estudio coronario y cerebrovascular (carotídeo). La medición del índice tobillo/brazo es útil para evaluar la severidad de la enfermedad así como factor pronóstico de la extremidad. El tratamiento médico consiste en control de sus factores de riesgo, especialmente el cigarrillo, ejercicio supervisado y uso de cilostazol en dosis de 1 00 mg/12 hrs. Si el paciente con CI continúa con limitación en su calidad de vida, a pesar de tratamiento médico, será candidato a revascularízación siendo la primera opción el tratamiento endovascular (angioplastía/stent).
ABSTRACT
Intermittent Claudication, a pivotal symptom of peripheral vascular disease is becoming a very important landmark of systemic atherosclerotic disease. Also, as life expectancy of the population increases, a large proportion of vascular unit's workload represents this type of patients. 20 percent of the population over 60 years has peripheric disease but only a quarter of them are symptomatic. These patients must be evaluated extensively, including ischemic heart disease and cerebral artery disease. The ankle-braquial index (ITB) is a useful tool to evaluate the arterial severity of the ischemic leg as well as a prognostic factor for the extremity. Medical treatment includes management of risk factors, specially smoking cessation, supervised exercise and cilostazol 100 mg bid. If the patient does not improves his walking ability and/or quality of life, regardless medical treatment, we must think about revascularization, being the endovascular treatment (angioplasty) the first line of treatment.
