RESUMEN
Introducción:
la leptospirosis humana constituye la zoonosis de mayor impacto en salud pública. En Cuba, durante los últimos 10 años se notificaron brotes en varias provincias.Objetivos:
estratificar la morbilidad y mortalidad por leptospirosis humana según variables epidemiológicas y estructura territorial en La Habana durante los años 2001-2010.Métodos:
se realizó un estudio ecológico de series temporales, utilizando el alisamiento exponencial para la tendencia, el indicador único para la estratificación y la estructura territorial basada en los criterios de ordenamiento territorial.Resultados:
en 2002 se registró el mayor riesgo de enfermar con 4,51/100 000 habitantes. Entre 2001 y 2005, Boyeros y Cotorro se ubicaron en el estrato de alto riesgo. Durante 2006-2010, todos los municipios alcanzaron incidencias por debajo de 3/100 000 habitantes. Según el riesgo de morir, Regla y San Miguel del Padrón se encuentran en los estratos más altos. La letalidad mostró el valor más elevado en 2006, la tendencia es al descenso.Conclusiones:
la incidencia de leptospirosis mostró diferencias en su distribución geográfica según estratos temporales, con un período de alta incidencia en tres municipios vinculados geográficamente y un período posterior de baja endemicidad en toda la provincia. La zona periférica según criterios de ordenamiento territorial concentra a la población con mayor exposición a los riesgos de enfermar y morir por leptospirosisABSTRACT
Introduction:
human leptospirosis is the zoonosis carrying the major impacts on the public health in Cuba. During the last 10 years, several outbreaks have been notified in different provinces.Objectives:
to stratify morbidity and mortality due to human leptospirosis according to the epidemiological and spatial structure of Havana from 2001 to 2010.Methods:
an ecological study of time series using the exponential smoothing for trend, the single indicator for stratification and the territorial structure based on land-use criteria.Results:
the year 2002 registered the greatest risk of becoming ill with 4.51/100 000 inhabitants. Between 2001 and 2005, Boyeros and Cotorro municipalities were in the high-risk stratum. During the 2006-2010 period, all municipalities had incidence rates lower than 3/100 000 inhabitants. According to the risk of dying, Regla and San Miguel del Padron were found in highest-risk strata. The fatality rate showed the highest value in 2006, but there has been a decreasing trend since then.Conclusions:
leptospirosis incidence showed differences in its geographical distribution by time-strata, with a period of high incidence in three geographically linked municipalities and subsequent periods of low endemicity throughout the province.
