Embarazo heterotópico en una paciente infértil con síndrome de ovarios poliquísticos / Heterotopic Pregnancy in an Infertile Patient with Polycystic Ovary Syndrome

RESUMEN

Paciente que inició su seguimiento en la Consulta de Infertilidad del Hospital General Universitario Vladimir Ilich Lenin de Holguín, a la edad de 23 años con antecedentes de salud aparente. Según los estudios sonográficos y humorales realizados se diagnosticó síndrome de ovarios poliquísticos y se prescribió tratamiento con cipresta por seis meses y metformina 1 000 mg diarios. Al terminar este tratamiento se inició tratamiento con los inductores de la ovulación, clomifeno (50 mg) una tableta diaria entre el quinto y noveno día del ciclo menstrual y gonadotropina coriónica humana 10 000 U al alcanzar el folículo un tamaño mayor de 18 mm. Con este esquema terapéutico la paciente ovuló al tercer mes y acudió con ocho semanas de amenorrea, pero con sangramiento genital y siete días más tarde se hace el diagnóstico de embarazo heterotópico, por lo que se realizó salpingectomia izquierda. La gestante continuó el embarazo hasta las 40,1 semanas en que se produjo parto espontáneo de un recién nacido femenino de buen peso y puntaje de Apgar.

ABSTRACT

This is a 23-year-old patient who began her follow up infertility consultation at V. I. Lenin General Teaching Hospital. She had a History of being a healthy woman. Taking into account the sonographyc and humoral studies the polycystic ovarian syndrome was diagnosed and the patient was prescribed with cipresta for 6 months and Metformin (1000 mg) daily. At the end of the treatment with cipresta the management with ovulation inducer, clomifeno (50 mg) treatment was given, once a day between the fifth and the ninth day of the menstrual period and human chorionic gonadotropin (10 000 U) when the follicle was bigger than 18 mm, getting to ovulation, and after three months of treatment she came to the consultation with eight weeks of amenorrhea, but with vaginal bleeding. A week later the diagnosis of heterotopic pregnancy was confirmed. A left salpingotomy was done, continuing the pregnancy up to 40, 1 weeks of gestation when the spontaneous delivery of a normal apgar and weight female neonate.
