ABSTRACT
The anterolateral thigh free flap, based on perforators of the descending branch of the lateral femoral circumflex artery, is ideal for soft tissue defects reconstruction. It may be harvested as a fasciocutaneous perforator flap or as a myocutaneous flap by including variable amounts of different kinds of tissues. It is very useful in head, neck and lower limb, but can be used in virtually every anatomic area. Its versatility, long and large pedicle, and minimal donor site morbidity are their main advantages.
