O cuidado familiar na perspectiva de cuidadores de idosos com demência de Alzheimer / Family care according to the perspective of carers of elderly relatives with Alzheimer's Disease

RESUMO

Este trabalho tem como objetivo compreender, a partir da perspectiva de cuidadores familiares de idosos com diagnóstico de Demência de Alzheimer, a situação de cuidado familiar em suas diferentes dimensões, à luz da história de vida dos cuidadores. Foram gravadas, literalmente transcritas e analisadas qualitativamente, entrevistas realizadas com 20 cuidadores cadastrados na Associação Brasileira de Alzheimer do Estado do Amazonas (ABRAZ-AM) na cidade de Manaus, através de roteiro semi-estruturado segundo a modalidade de história de vida temática. Os entrevistados têm entre 26 e 82 anos, são no geral provenientes de zona urbana, na quase totalidade mulheres (17), nível socioeconômico e de escolaridade médio para alto em sua maioria, a maior parte dos cuidadores (16) são casados, o grau de parentesco com o .idoso é de filiação em sua maioria (14). A história de relacionamento entre o idoso e o cuidador foi descrita como um elemento de grande importância frente ao bem-estar subjetivo no dia-a-dia do cuidado; os cuidadores experienciaram sentimentos positivos se contrapondo a sentimentos negativos no processo do cuidar, desde o momento do diagnóstico; a obrigação moral de cuidar, o sentimento de solidariedade e a motivação para retribuir experiências anteriores gratificantes se fizeram presentes na situação de cuidado. No que se refere ao cotidiano do cuidado, os cuidadores familiares mostraram passar por várias situações com as quais ainda não sabiam lidar. A condução solitária de tarefas relacionadas às Atividades da Vida Diária (AVDs) são as que mais geram sobrecarga aos entrevistados, associadas às suas privações particulares, sociais e profissionais. A falta de apoio no cuidado, a relação de indiferença, distanciamento e de agressividade dos familiares para com o idoso doente são fatores geradores de crise no contexto familiar. ABSTRACT

The objective of the present study was to understand, as from the perspective of family carers of elderly relatives diagnosed with Alzheimer's Disease, the situation of family care in its different dimensions, in the light of the life history of these carers. lnterviews with 20 carers registered in the Brazilian Association for Alzheimer of the State of Amazonas (ABRAZ- AM) in the city of Manaus, were taped, transcribed literally, and qualitatively analyzed using a semi-structured itinerary according to the modality of a life history theme. The interviewees were between 26 and 82 years of age, in general from urban areas and almost all (17) women. Most of them showed medium to high socioeconomic and educational levels, the majority (16) were married and in most cases (14) the relationship with the elderly person was that of son. The history of the relationship between the elderly person and the carer was described as an element of great importance considering the day-to-day wellbeing of the person cared for. The carers experienced positive feelings in opposition to negative feelings in the caring process, as from the moment of diagnosis, and the moral ob1igation to care for the demented person, sentiments of solidarity and of motivation to repay earlier gratifying experiences, were factors present in the caring situation. With respect to the daily caring routine, the family carers faced various situations which they were still unable to cape with. The solitary undertaking of tasks related to the Everyday Activities (EDAs) was what most caused an overload in the interviewees, associated with the need for personal, social and professional privation. The lack of support for the caring process, attitudes of indifference, distancing and aggressiveness by other relatives with respect to the sick elderly relative, were factors generating crises in the family context.
