O acompanhamento psicológico a óbitos em unidade pediátrica / Psychological assistance to death in pediatrics unit

RESUMO

Vivemos em uma sociedade propensa a ignorar ou evitar a morte (Kübler-Ross, 2005), pois é uma experiência que expõe o ser humano a sua própria impotência (Freitas, 2000). A confrontação com a perda, bem como a dor física e emocional advinda dessa experiência, parecem ser ainda mais intensos quando se trata da morte de um filho, visto que há interrupção de uma seqüência esperada (Freitas, 2000). Objetivo: Descrever aspectos da experiência do Serviço de Psicologia Clínica de um hospital de referencia em cardiologia no RS relacionados ao acompanhamento de familiares mediante óbito de crianças internadas em Unidades Pediátricas.Método: Relato de experiência profissional. Foram estudados 35 relatos de casos de óbitos, registrados nos prontuários do Serviço, atendidos nas Unidades Pediátricas do hospital, no ano de 2006. Os encaminhamentos são realizados pela equipe médica e de enfermagem das respectivas Unidades. O acompanhamento pode, em alguns casos, iniciar exatamente no momento da notícia do óbito que é anunciada pelo médico. Resultados: O impacto psicológico dessa situação de perda causa reações de revolta ou desnorteamento nas quais, por vezes, é necessária a contenção física. O Serviço de Psicologia é colocado como referência, oferecendo um espaço que contemple acolhimento e escuta. Além disso, o profissional encarrega-se de identificar o membro da família com condições de encaminhar os trâmites legais para o sepultamento, encaminhando-o ao Serviço Social ou a outros serviços de referência para realização da parte burocrática do óbito. Os familiares também são acompanhados no morgue do hospital, onde podem permanecer com a criança falecida. É nesse momento que a confrontação com a morte dá-se de forma direta. Acompanha-se o trânsito entre as fases iniciais do processo de luto, intervindo-se no sentido de mediar esse momento de tanta dor. ABSTRACT

We live in a society that tends to ignore or avoid death (Kübler-Ross, 2005), since it is an experience that exposes the human being to his own impotence (Freitas, 2000). The confront with the loss, as well as the physical and emotional pain which comes from this experience, seem to be even more intense when it is the loss of a child, because there is an interruption of the expected sequence (Freitas, 2000). Objective: To describe aspects of the experience of the Clinic Psychology Service of a reference hospital in cardiology in RS concerning the assistance given to family members during the moment of death of children attended to at Pediatric Units. Method: Professional experience report. 35 case-reports of death assisted in the Pediatric Units of the hospital, in 2006, registered on the files of the Service, were studied. The requests for psychological assistance are accomplished by the physicians and nurses in the respective units. Assistance can begin exactly in the moment the death is communicated by the physician, in some cases. Results: The psychological impact of the situation of loss causes reactions of revolt or confusion. Such reactions sometimes require physical contention. Psychology Service is offered as a reference, providing a space which contemplates listening and refuge. Besides, the professional identifies the member of the family that is emotionally capable of carrying on the legal procedures of the burial, directing him or her to the Social Service or to other reference services for the accomplishment of the bureaucratic part of the death. Family members are also followed to the morgue of the hospital, where they can remain with the death child. In such moment the confrontation with death happens in a direct manner. The transit through the mourning process initial phases are followed by the professionals, intervening as mediators in this painful moment(AU)
