Desempeño del método analítico laboratorial y control de calidad interno en pruebas del perfil bioquímico: glucosa, úrea y creatinina en el Hospital Nacional Edgardo Rebagliati Martins-EsSalud / Performance of laboratory analytical method and internal quality control in testing of biochemical profile: glucose, urea and creatinine at the National Hospital Edgardo Rebagliati Martins-EsSalud

RESUMEN

El objetivo del presente trabajo fue verificar el nivel de desempeño del método analítico laboratorial de las pruebas del perfil bioquímico glucosa, úrea y creatinina sérica en métrica six sigma en el laboratorio central del Hospital Nacional Edgardo Rebagliati Martins-EsSalud, año 2014. El tipo y diseño de investigación fue descriptiva, transversal, observacional y comparativa, el universo del estudio fueron todos los analitos del perfil bioquímico glucosa, úrea y creatinina sérica y la muestra estuvo conformada por la totalidad del universo. Las variables independientes fueron precisión, veracidad y error total del método analítico; mientras que las variables dependientes fueron nivel de desempeño del método analítico en métrica six-sigma y herramientas del control de calidad interno del método analítico. En la verificación de la precisión y veracidad se utilizó el protocolo EP15-A2 de las guías del CLSI y se realizó la medición de los analitos en dos niveles de concentración (nivel 1 y nivel 2) en el analizador automatizado ADVIA 1800 (Siemens Healthcare Diagnostics Inc.). El error total del método analítico se determinó en base al coeficiente de variación intralaboratorio y al bías obtenido para cada nivel del analito, éste error fue comparado con el error total máximo permisible aceptado por CLIA 88, posteriormente se determinó el desempeño del método en la escala métrica six sigma y luego se estableció el plan de control de calidad interno, utilizando el gráfico de función de poder, de acuerdo al desempeño obtenido en el nivel de concentración limitante, es decir en el nivel de peor desempeño de cada metodología analítica. ABSTRACT

The aim of this study was to verify the performance level of the laboratory analytical method for testing the biochemical profile in serum glucose, urea and creatinine in six sigma metric in the central laboratory of National Edgardo Rebagliati Martins-EsSalud, Hospital in 2014. The type and research design was descriptive, cross-sectional, observational and comparative, the universe of the study were all analytes of biochemical profile glucose, urea and serum creatinine and the sample consisted of the entire universe. The independent variables were precision, trueness and total error of the analytical method; while the dependent variables were level of performance of the analytical method in measuring six- sigma and tools of internal quality control of the analytical method. The EP15-A2 protocol CLSI guidelines was used in verifying for precision and trueness and the measurement of analytes was performed at two concentration levels (level 1 and level 2) in the automated ADVIA 1800 analyzer (Siemens Healthcare Diagnostics Inc.).
