Cisto dentígero acometendo terceiro molar inferior incluso em posição atípica: relato de caso clínico / Dentigerous cyst affecting lower third molar included in atypical position: report of a clinical case

RESUMO

Lesão cística é definida como uma cavidade patológica revestida por epitélio que apresenta em seu interior material fluido ou semifluido. Quando o epitélio odontogênico está associado à coroa de um dente não erupcionado e ocorre a separação do folículo ao redor deste dente, origina-se um cisto odontogênico denominado cisto dentígero. É uma lesão benigna, que acomete mais os terceiros molares inferiores e os caninos superiores, ocorrendo principalmente nas três primeiras décadas de vida, tendo um crescimento lento e assintomático. Pode atingir dimensões consideráveis, causando deformidade facial, deslocamento e impactação de dentes e/ou estruturas adjacentes. Apresenta características radiográficas não específicas, podendo fazer diagnóstico diferencial com ceratocisto e ameloblastoma unicístico. A remoção do dente associado e a enucleação cuidadosa do folículo é o tratamento definitivo para a maioria dos casos. Neste trabalho, relatamos um caso clínico de cisto dentígero em mandíbula, o qual foi tratado em bloco cirúrgico sob anestesia geral, devido a possibilidade de fratura patológica, realizado no Hospital Metropolitano Odilon Behrens pelo serviço de Cirurgia e Traumatologia Bucomaxilofacial da Universidade Federal de Minas Gerais. O caso encontra-se em controle radiográfico pós-operatório.

ABSTRACT

Cystic lesion is defined as a pathological cavity lined by epithelium that has fluid or semi-fluid material inside. When the odontogenic epithelium is associated with the crown of an unerupted tooth and separation of the follicle around this tooth occurs, an odontogenic cyst called dentigerous cyst. It is a benign lesion that affects more the lower third molars and the upper canines, occurring mainly in the first three decades of life, with a slow and asymptomatic growth. It can reach considerable dimensions, causing facial deformity, displacement, and impaction of teeth and/or adjacent structures. It presents non-specific radiographic features, being able to make a differential diagnosis with keratocyst and unicystic ameloblastoma. Removal of the associated tooth and careful enucleation of the follicle is the definitive treatment for most cases. In this work, we report a clinical case of a dentigerous cyst in the mandible, which was treated in the operating room under general anesthesia, due to the possibility of pathological fracture, performed at the Hospital Metropolitano Odilon Behrens by the Department of Oral and Maxillofacial Surgery and Traumatology of the Universidade Federal de Minas Gerais. The case is under postoperative radiographic control.
