Se realizó un estudio descriptivo transversal prospectivo, para determinar el comportamiento del intervalo QT corregido prolongado, en pacientes con síndrome coronario agudo sin elevación del segmento ST, en el Centro Diagnóstico Integral Los Arales, San Diego, año 2009. El universo fue de 48 pacientes y la muestra incluyó a 29 pacientes con ECG normal o sin nuevos cambios isquémicos. Al ingreso se determinaron los valores del intervalo QT corregido, las variables demográficas, antecedentes personales, niveles enzimáticos de CK-MB y el score TIMI. Estos resultados se correlacionaron con la aparición de eventos clínicos (muerte, infarto no fatal y revascularización) un mes después del ingreso. De los 29 pacientes analizados sólo 11 (38 por ciento) presentaron eventos clínicos, mientras que los restantes 18 (62 por ciento) no los tuvieron. Los pacientes con eventos presentaron valores del iQTc más prolongados por encima del punto de corte. En ambos grupos predominó el sexo masculino y el grupo de edad más frecuente fue el de 60 a 69 años. El tabaquismo, la hipertensión y la hipercolesterolemia fueron los más prevalentes para ambos grupos, así como el uso previo de aspirina. La estratificación de riesgo TIMI y los parámetros basales en los pacientes estudiados fueron mayores y revelan un mayor porcentaje en el grupo con eventos clínicos. Los resultados demuestran que el iQTc predice eventos adversos cardiovasculares hasta 30 días después del ingreso (AU)
ABSTRACT
A prospective, transversal and descriptive study was carried out to determine the performance of the corrected prolonged QT interval in patients with acute coronary syndrome without ST segment elevation, at Los Arales Integral Diagnosis Center in San Diego, 2009. The universe was formed by 48 patients and the sample included 29 patients with normal ECG or without new ischemic changes. At the admission, the values of the corrected QT interval were determined, the demographic variables, personal history, enzymatic levels of CK-MB and the TIMI score. These results were correlated with the appearance of clinical events (death, non fatal infarction and revascularization) one month after the admission. From the 29 analyzed patients, 11 (38 percent) suffered clinical events, while the remaining ones, 18 (62 percent) didn't have them. The patients with events presented prolonged corrected QT intervals above the cut point. In both groups the male sex prevailed and the most frequent age was from 60 to 69 years. The habit of smoking, hypertension and the hypercholesterolemia were prevalent for both groups, as well as the previous use of aspirin. The stratification of TIMI risk and the basal parameters in the studied patients were higher and they reveal a larger percentage in the group with clinical events. The results demonstrated that the prolonged corrected QT interval predicts cardiovascular adverse events up to 30 days after the admission (AU)
