ABSTRACT
INTRODUCTION:
faciocervicotorasic subcutaneous emphysema can be defined as an entity resulting from air infiltration within the subcutaneous cellular tissue. The word cervicotorasico refers to the regions involved.OBJECTIVE:
to describe negative pressure therapy based on hypodermic needles for the resolution of a subcutaneous faciocervicotrasic emphysema at the hospital level. CLINICAL CASE A 35-year-old female patient is presented, apparently healthy, originally and resident of the State of Mexico. Referred to the maxillofacial surgery service. On clinical examination, an increase in volume is observed in the region of the left face, bilateral cervical region, trapezoidal area, and bilateral supraclavicular and infraclavicular areas; Upon auscultation of these sites with the stethoscope and palpation, crepitation of the aforementioned areas is identified due to the fact that the patient presented severe desaturation, the immediate management was the application of negative pressure therapy based on hypodermic 32 mm 20 gauge needles.CONCLUSION:
Negative pressure therapy based on hypodermic needles is effective for the resolution of subcutaneous faciocervicotorasic emphysema, as long as the affected regions are delimited, also, it is an option to improve critical saturation values.RESUMEN
NTRODUCCIOÌN el enfisema subcutaÌneo faciocervicotoraÌsico, se puede definir como una entidad resultante de la infiltracioÌn de aire, dentro del tejido celular subcutaÌneo. La palabra faciocervicotoraÌsico se refiere a las regiones involucradas. OBJETIVO:
describir la terapia de presioÌn negativa a base de agujas hipodeÌrmicas, para la resolucioÌn de un enfisema subcutaÌneo faciocervicotoraÌsico a nivel intrahospitalario. CASO CLIÌNICO se presentoÌ una paciente femenina de 35 anÌos de edad, aparentemente sana, originaria y residente del Estado de MeÌxico. Referida al servicio de cirugiÌa maxilofacial. Al examen cliÌnico se observa un aumento de volumen en la regioÌn del hemicara izquierda, regioÌn cervical bilateral, aÌrea trapezoidal y zona supraclavicular e infraclavicular bilateral, a la auscultacioÌn de estos sitios con el estetoscopio y palpacioÌn se identifica crepitacioÌn de las zonas ya mencionadas, debido a que el paciente presento una desaturacioÌn grave, el manejo inmediato fue aplicando la terapia de presioÌn negativa a base de agujas de tipo hipodeÌrmica de calibre 20 G de 32 mm. CONCLUSIOÌN la terapia de presioÌn negativa a base de agujas de tipo hipodeÌrmica, es efectiva para la resolucioÌn del enfisema subcutaÌneo faciocervicotoraÌsico, siempre y cuando se delimiten las regiones afectadas, asiÌ mismo, es una opcioÌn para mejora los valores de saturacioÌn criÌticos.
