ABSTRACT

Background:Human immunodeficiency virus/ Acquired immunodeficiency syndrome (HIV/AIDS), has remained a disease of public health concern, with the largest burden being found in sub Saharan Africa. The advent of anti-retroviral therapy (ART) has significantly reduced the mortality of the disease, thereby transforming it to a chronic disorder, with significant co-morbid psychiatric sequalae.Objectives:To determine the prevalence and pattern of psychiatric morbidity among PLWHAattending Aminu Kano Teaching Hospital, Kano.Materials and methods:This was a cross sectional study of adult patients with HIV/AIDS attending outpatient clinic at the S.S. Wali Virology Centre of Aminu Kano Teaching Hospital who gave informed consent. Systematic random sampling technique was used.Patients aged 18years and above who had been on ARVdrugs for at least one year were included, while those who had a medical emergency and needed immediate attention were excluded.Socio demographic characteristics were obtained using a socio-demographic questionnaire and psychiatric morbidity was assessed with the MINI International Neuropsychiatry Interview.Results:Atotal of 420 participants were recruited in the study with a male to female ratio of 1:1.5 and mean age of 40.4±10.0 years. The prevalence of a psychiatric disorder was 22.1% out of whom 5.0% had more than one psychiatric diagnosis. Major depression was the most common (11%) psychiatric disorder. Generalized Anxiety disorder, substance abuse, post-traumatic stress disorder and alcohol abuse accounted for 7.6%, 5.5%, 2.4% and 1.7% of psychiatric disorders respectively.Conclusion:Psychiatric disorders are common in PLWHA, with major depression being the commonest.

ABSTRACT

Background:Moderate acute malnutrition (MAM) is a leading cause of childhood morbidity and mortality globally. The morbidity pattern of underfives with this condition is yet to be described. Objective: To describe the morbidity pattern of underfives with MAM.Method: Across sectional study was conducted in two Primary Health Centres in Uruan Local Government Area of Akwa Ibom State. Caregivers'brought children aged 6- 59 months to the health facilities following community mobilization. Eligible children were recruited into the study after obtaining parental consent. Avalidated proforma was used to obtain the biodata and symptoms of common illnesses in the children. Ageneral physical examination, anthropometric measurements and systemic examination were performed. Results: Atotal of 162 children were recruited into the study. Their mean (±SD) age was 20.4 ± 13.0 months. Over 70% of them were 6 - 23 months of age. Their mean (±SD) length/height was 77.3 ± 29.6 cm, mean (±SD) weight was 8.3 ± 3.4 kg and mean (±SD) mid upper arm circumference was 12.4 ± 4.5 cm. The main symptoms noted in the children were; fever 99 (61.1%), cough 84 (51.9%), weight loss 81 (50.0%), diarrhoea 40 (24.7%) and vomiting 40 (24.7%) while pallor 77 (47.5%), lymphadenopathy 56 (34.6%), hair changes 49 (30.2%), skin changes 27 (16.6%) were the main signs in them.Conclusion: The main symptomatology of underfives with MAM were fever, cough and weight loss

ABSTRACT

Background: Brain Fag Syndrome (BFS) is a culture-bound syndrome characterized by cognitive and somatic symptoms, commonly reported among African students. This study aimed to determine the prevalence of BFS among Nigerian university students and examine its associated factors. Methods: A cross-sectional survey was conducted among undergraduate students from a university in the northwest region of Nigeria. The study utilized a self-administered questionnaire to collect data on socio-demographic characteristics, stimulant use, course of study, and academic performance. The presence of BFS was assessed using standardized diagnostic criteria. Results:The study included a total of 625 participants, in their young adulthood. The prevalence of BFS among Nigerian university students was found to be 62.7%. The majority of affected students were aged 20-30, male, and from the Hausa ethnic group. No significant association was found between stimulant use and BFS. However, there was a significant relationship between the course of study and the occurrence of BFS. Academic performance (CGPA) showed a weak negative association with BFS. Other socio-demographic factors such as age, gender, ethnicity, relationship status, birth position, type of home, and family income did not predict the occurrence of BFS. Conclusion: The high prevalence highlights the need for attention to mental health issues among this population. The results emphasize the importance of considering the course of study and academic performance when studying BFS. Further research is warranted to explore the underlying mechanisms and develop effective interventions for students affected by BFS.

ABSTRACT

Objectives:To determine the pattern of ocular disorders among stroke patients in Federal Teaching Hospital, Lokoja (FTHL).Materials and Methods:Consecutive new stroke patients seen at the Federal Teaching Hospital, Lokoja over a 3-month period were studied. Data was collected with the use of a structured questionnaire divided into four major sections: bio-data, visual history, results of ocular assessment and NEI VFQ-25 quality of life questionnaire. Data analysis was done using SPSS for Windows, version 23.0. Univariate analyses were presented in the form of frequencies, percentages, means, ranges, and standard deviations, charts and tables.Results:Eighty-three patients were recruited with an age range of 40-71 years, a mean age of 55.3(±7.1) years. Thirty-nine patients (47.0%) were between the ages of 51 and 60 years. Thirty-four (41.0%) patients were females while 49(59.0%) were males. Of 46 (55.4%) patients that had neuro-imaging, stroke was ischaemic in 37(80.4%) and haemorrhagic in 9(19.6%). Of the 63 patients with left sided stroke, 2 (2.4%), 4(4.8%) and 57(68.7%) had severe, moderate and mild and normal visual impairment respectively while of the 20 patients with right sided stroke, 3(3.6%) each had severe and moderate visual impairment and 14(16.9%) had normal or mild visual impairment. Other ocular abnormalities included corneal anaesthesia and macular hole in 2(1.2%).Conclusion:Anterior and posterior segments, together with neuro-ophthalmic disorders were found among stroke patients in this study. Many of the ocular abnormalities are as a result of long-standing uncontrolled hypertension which caused the stroke. It is recommended that awareness should be created among the populace about uncontrolled hypertension. It is also advised that internists should refer hypertensive patients for routine ophthalmic screening.

ABSTRACT

Background: Infectious disease outbreaks are common in sub-Saharan Africa (SSA). Consequently, integrated public health surveillance has become increasingly essential for the region. Health surveillance systems enable early detection and monitoring of emerging and re-emerging disease outbreaks, thus informing preparedness and response measures. However, complex and intertwined factors obstruct a successful integrated public health surveillance in SSA, with dire consequences. Objectives: The objective of this article was to establish how big data analytics can be used to enhance integrated infectious disease surveillance and response in SSA. Method: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) was used to identify and select relevant articles. A total of 10 studies that addressed the article's objective were selected. Results: Findings reveal several barriers to the application of big data analytics for public health surveillance in SSA. These include the absence of regulatory and data governance frameworks for big data management in healthcare, disparities in digital health infrastructure across SSA's healthcare systems, and the digital and analytical skills required for data capture and interpretation. The development of regulatory frameworks is essential for the ethical application of analytical technologies such as artificial intelligence. Conclusion: This article's contributions emphasise the need for comprehensive strategies for the application of big data analytics for public health surveillance, as well as addressing barriers to its successful application by highlighting the requirements for an integrated infectious disease surveillance and response system in SSA. Contribution: The article contributes to the body of knowledge on the interplay between the public health space and digital health interventions by emphasising the beneficial applications of big data analytics for surveillance and response to address emerging and re-emerging infectious disease outbreaks in the health systems of sub-Saharan Africa.

ABSTRACT

Background: About 99.7% of cervical dysplasia and cancer cases are caused by persistent genital high-risk human papillomavirus (hrHPV) infection. Most HPV infections are subclinical and self-limiting but may persists in about 5 to 10% of infected women, resulting in pre-cancerous lesions that can progress to invasive cancer years later. This study is aimed at detecting hrHPV among apparently healthy women of reproductive age in Kaduna State, thus providing more information for effective control of HPV and cervical cancer in Nigeria. Methodology: Cervical smears were taken from 515 randomly selected apparently healthy women across selected secondary and tertiary facilities from 3 Local Government Areas (LGAs) in each Senatorial Zone of Kaduna State, Nigeria. Liquid-based cytology (LBC) technique was used to collect cervical smears and prepare smears for cytology study, while the remaining samples were stored at -80oC for molecular studies. HPV DNA were extracted from the samples and amplified by convectional PCR using specific hrHPV (HPV 16,18,31 and 45) primer sets and a broad spectrum MY09/11 and GP5+/6+ primers for a wider range of HPV genotypes. Data were analysed using the Statistical Package for Social Sciences (SPSS) version 23.0 and relationship between prevalence of hrHPV and socio-demographic factors such as age and marital status were determined using Chisquare or Fisher Exact test with p<0.05 considered statistically significant. Results: The prevalence of total HPV and hrHPV infections in the study population was 11.8% (61/515) and 9.3% (48/515) respectively. A total of 100 HPV genotypes were detected by PCR in the 61 positive smears, with 66 hrHPV types from 48 women, and 34 other HPV types from 13 women. The frequency of hrHPV genotypes detected was HPV 31 (5.8%, n=30), HPV 45 (4.1%, n=21), HPV 16 (1.7%, n=9), and HPV 18 (1.2%, n=6), with other HPV genotypes (6.6%, n=34). The frequency of cervical dysplasia was 6.4% (33/515), which was significantly associated with all HPV genotypes except HPV 16. Single HPV infection was seen in 31 (51.8%) women while multiple infections were seen in 30 (49.2%), with double infection in 21 (34.4%) and triple infections in 9 (14.7%). Conclusion: The prevalence of hrHPV infection was high among women in Kaduna State, Nigeria. DNA-based screening for hrHPV genotypes and production of new vaccine that will protect against the predominant hrHPV genotypes are thus recommended for the prevention of cervical cancer in Nigeria, Africa and beyond.

ABSTRACT

Background: Scientific information on the impact of malaria on the risk of developing type 2 diabetes mellitus (T2DM) after recovery from the coronavirus disease 2019 (COVID-19) is limited in the Ghanaian context. The purpose of this study was to examine the association between selected risk markers of T2DM in falciparum malaria patients post-COVID-19 or not at a tertiary hospital in Ghana. Methodology: This was a descriptive cross-sectional comparative study of 38-recovered COVID-19 adult participants with malaria and 40 unexposed COVID-19 adults with malaria at the Tamale Teaching Hospital, Ghana. Demographic, anthropometric and levels of glucose, insulin, C-reactive protein and lipid profiles were measured in the two groups of participants under fasting conditions. Parasitaemia was assessed microscopically but insulin resistance and beta-cell function were assessed by the homeostatic model. Results: The COVID-19 exposed participants were older (p=0.035) with lower parasitaemia (p=0.025) but higher mean levels of insulin, insulin resistance, and beta-cell function compared with their unexposed counterparts (p<0.05). Parasitaemia correlated positively with a number of the measured indices of diabetogenic risk markers in the COVID-19 exposed group only, and predicted (Adjusted R2=0.751; p=0.031) by beta-cell function, C-reactive protein and triglycerides with the model explaining about 75% of the observed variation. Parasitaemia could only be predicted (Adjusted R2=0.245; p=0.002) by C-reactive protein with the model explaining just about a quarter of the observed variation in the COVID-19 unexposed group. Insulin resistance and sub-optimal beta-cell function were detected in both groups of participants. Conclusion: Falciparum malaria is associated with risk markers for development of T2DM irrespective of COVID-19 exposure. Insulin resistance, inflammation and sub-optimal beta-cell secretory function may drive the risk. The observed diabetogenic risk is higher in the recovered COVID-19 participants.

ABSTRACT

Background: Globally, 296 million people were infected by hepatitis B in 2019, with 1.1 million deaths. Africa is one of the endemic regions. Good knowledge and awareness of hepatitis B remain pivotal to the biosafety of medical students. This study sought to determine the levels of knowledge and awareness of hepatitis B among students of Pamo University of Medical Sciences (PUMS), Port Harcourt, Nigeria, and the predicting factors associated with this knowledge and awareness. The is with the aim of providing recommendations for improving and sustaining biosafety levels for medical and other health-related students of the University. Methodology: The study was a descriptive cross-sectional design conducted amongst 528 randomly selected medical students of PUMS, Port Harcourt, Nigeria. Structured questionnaires were interviewer-administered to collect socio-demographic information and participants' responses to questions on knowledge and awareness of hepatitis B. Data were analysed using SPSS version 26.0 and relationships of socio-demographic characteristics and predictive factors with knowledge and awareness of hepatitis B were tested using binary logistic regression analysis with p value for statistical significance set at <0.05. Results: A total of 528 students participated in the study, 202 (38.3%) males and 326 (61.7%) females. Most participants (296, 56.1%) were between 15-19 years of age with mean age of 19 ±2.43 years. The mean (±SD) of participants responses with good knowledge of hepatitis B was 249±121.5 while for good awareness, it was 181±88.3. The percentage average for good knowledge and good awareness was 47.2% and 34.2% respectively, with positive correlation between knowledge and awareness of hepatitis B (r=0.720, p<0.0001). Age was significantly associated with participants percentage average knowledge (OR=0.77, 95% CI 0.70-0.84, p<0.0001) and awareness of hepatitis B (OR=0.84, 95%CI 0.78-0.90, p=0.004). No other factor was significantly associated with knowledge and awareness of hepatitis B except Ijaw tribe (OR=0.4, 95%CI 0.24-0.66, p=0.034) and attendance of Federal Government College (OR=0.4, 95% CI 0.24-0.68, p=0.046). Conclusion: The percentage average good knowledge of 47.2% and awareness of 34.2% for hepatitis B in this study are low, although most participants in the study were between the ages of 15-19 years and in their first and second year of study. This gives room for improvement in knowledge and awareness of hepatitis B with progression in age and year of training. Good knowledge and awareness of hepatitis B are central to the biosafety of medical students. It is recommended that the National Universities Commission (NUC) and the Medical and Dental Council of Nigeria (MDCN) review the current medical school curriculum to increase the teaching of medical and health-related students that will impact more on knowledge and awareness of infectious diseases and infection prevention and control.

L'hypertension artérielle est une maladie à forte progression reste un problème de santé publique. Mais, les pratiques de sa prise en charge se heurtent à différents obstacles. Cette recherche questionne les problèmes qui caractérisent les pratiques de prise en charge de l'hypertension artérielle au Centre Hospitalier et Universitaire d'Abomey-Calavi au Bénin. Pour y parvenir, nous avons opté pour une analyse basée sur les méthodes quantitatives et qualitatives. L'échantillon est constitué de 130 personnes enquêtées. De l'analyse des résultats collectés, des difficultés éprouvées entre patients et agents de santé dans la prise en charge de l'hypertension artérielle, se caractérise par le manque de relation soignant-soigné. De même, 90% des enquêtés estiment avoir peu de ressources humaines qualifiées et du faible pouvoir d'achat des patients pour faire face aux coûts élevés du traitement de l'hypertension (86,75%). Ainsi, le manque de plateau technique et les frais de consultations spécialisées posent problèmes y compris les suivis de l'éducation hygiéno-diététique. Cet état de fait compromet les pratiques de prise en charge et les formations globales que le système soin est supposé assurer aux usagers qui le fréquentent. Ces résultats suggèrent l'urgence de formations pour le renforcement des capacités pour repérer la précarité et la réorganisation des mesures de prise en charge de l' hypertension artérielle dans le périmètre sanitaire béninois.


Arterial hypertension remains a rapidly growing public health problem. However, management practices face a number of obstacles. This research questions the problems that characterize arterial hypertension management practices at the Centre Hospitalier et Universitaire d'Abomey-Calavi in Benin. To achieve this, we opted for an analysis based on quantitative and qualitative methods. The sample consisted of 130 respondents. From the analysis of the results collected, of the difficulties experienced between patients and health workers in the management of arterial hypertension, most of those surveyed claimed to have a complexity that characterizes the training of health workers. Similarly, 90% of respondents felt that they had few non-cardiologist practitioners, and that patients had little purchasing power to meet the high costs of treating hypertension (86.75%). As a result, the cost of specialized consultations and complementary examinations poses a problem, including follow-up health and diet education. This state of affairs compromises management practices and the comprehensive training that the healthcare system is supposed to provide for its users. These results suggest the urgent need for training to identify precariousness, and the reorganization of hypertension management measures within the Beninese health perimeter.

ABSTRACT

In Benin, nurses complain of insufficient job satisfaction. This study is the third part of a mixed research project. The aim of this article was to develop an intervention to improve nurses' job satisfaction. Participatory action research with intervention mapping was carried out in 2021. Purposive sampling was used. Interviews and panel discussions were held with 40 participants, nurses and administrative staff from the Djidja and Covè area hospitals. A number of internal factors contributed to the nurses' job satisfaction. However, external factors also militated in favor of insufficient job satisfaction among nurses. This confirms the results of quantitative evaluation of nurses' job satisfaction, in line with a number of authors. The panelists agreed ona major workshop and satisfaction improvement program. They agreed on quantified objectives, a logic model, terms of reference, a logical framework and an evaluation plan. Participants also agreed on the development of nursing work management tools. The determinants of nurses' job satisfaction are examined in depth, some of which are confirmed, also confirming a lack of job satisfaction. The workshop will focus on three interventions selected from the literature, merged and adapted by the participants. None of these interventions has been tested in sub-Saharan Africa. Intervention implementation and evaluation will be the subject of future research. ABSTRACT

Background: Sudan's history is marred by ongoing sociopolitical challenges, with deep cultural divisions fueling numerous wars. A new conflict erupted on April 15, 2023, pushing the country closer to a full-scale civil war. This war has severely crippled Sudan's already fragile healthcare system, rendering 70% of hospitals in combat zones nonoperational, causing 12,000 deaths, thousands of injuries, and leaving 11 million in dire need of healthcare. More than seven million people are displaced, half of which are children, and are facing severe health challenges, especially vulnerable groups. Heath situation is threatening with unchecked spread of outbreaks of communicable diseases that were previously controlled and marked failure in meeting the health demands of patients with noncommunicable diseases, reproductive and child health issues, and people with serious conditions that require adequate follow-up. Moreover, the projections threaten with more catastrophic consequences including famine, environmental destruction, and further displacement of people. This review article highlights the urgency of the situation and explores potential solutions to enrich global understanding of crisis management. Methods: To comprehensively assess the impact of the crisis and propose a way forward, we drew data by exploring search engines and databases such as Google, Humanitarian Agencies Websites, Google Scholer, and PubMed along with some relevant reports. The search terms included are "Sudan's war", "impact of war on healthcare systems", and "Sudan's armed conflicts". Results: More than 19 publications on the impact of the war on health in addition to periodic reports from international organizations and governmental authorities were reviewed. Conclusion: Although all publications point to the gravity of the situation and the need for prompt response, this crisis offers a unique opportunity to rebuild Sudan's struggling healthcare system with the principles of social accountability. Through domestic and international collaboration, this sector can become a model for similar nations, meeting the needs of its people and promoting sustainable development.

ABSTRACT

The sensitivity and specificity of five rapid HIV antibody test kits commonly used in Nigeria were evaluated. The kits were selected based on their high percentage frequency of use as compared to others. A total of 100 EIA HIV-1and RNA HIV-1 positive sera were used as positive gold standard; while 100 EIA HIV-1 and RNA HIV-1 negative sera were used as negative gold standard. The positive gold standard sera were pooled; serially diluted and analysed to determine the sensitivities of the kits. The methods used were strictly as provided by the manufacturers. Of the 100 positive gold standard serum samples used; Immunocomb-II gave false negative results with 10 (Sensitivity = 90); while HIV-SAV; Hexagon; Determine and SD-Bioline were false negative with 12 specimens; representing 88 sensitivity for each. On the other hand; of the 100 negative gold standard sera; Immunocomb-II gave 6 false positive results (Specificity = 94); HIV-SAV 12 (Specificity = 88); Hexagon 2 (Specificity = 98); Determine 12 (Specificity = 88); while SD-Bioline had no false positive result (specificity = 100). In analytical sensitivity; Immunocomb-II detected the highest serum titre of 30 000; making it the most sensitive. Two of the five test kits (Immunocomb and SD-Bioline) demonstrated excellent analytical sensitivity and specificity respectively. The two could be recommended for use as combination test algorithms instead of EIA/Western Blot algorithm; which is time-consuming; expensive and often not technically feasible in a developing country like ours. This study shows that not all the analytical performance indices cited in the literature from the manufacturers of diagnostic kits are necessarily reproducible in end-user laboratories

ABSTRACT

L''Adénofibrome est l'affection la plus fréquente des pathologies bénignes du sein. L'objectif était d'identifier les aspects clinique et thérapeutique de l'adénofibrome dans le service de gynécologie obstétrique et chirurgie générale du CHU Gabriel TOURE. Patientes et Méthodes : L'étude était descriptive avec récolte rétrospective des données du 1er juillet 2018 au 31 juillet 2021. Ont été inclus, les dossiers de patientes prises en charge pour adénofibrome selon les principes éthiques garantissant l'anonymat et la confidentialité des données. Résultats : Nous avons recensé 701 cas de pathologie mammaire parmi lesquelles nous avons retenu au total 112 cas d'adénofibrome durant la période d'étude soit 15,9%. Nous avons enregistré 682 cas de tumeur mammaire, l'adénofibrome a représenté 16,42 %. Ainsi parmi toutes les 154 tumeurs bénignes du sein, l'adénofibrome (112 cas) a représenté 72,7%. L'âge moyen était de 27 ans avec un écart type de 14,8 ans (13 ans et 62 ans). La tranche d'âge de 20 à 35 ans était la plus représentée soit 44%. La fréquence de l'adénofibrome diminuait avec l'augmentation de la gestité et de la parité. L'obésité ou le surpoids ont été retrouvés chez 63 des patientes. Le motif principal de consultation était la présence d'une tuméfaction mammaire. L'atteinte unilatérale gauche prédominait dans 46% et la localisation au quadrant supéro- externe représentait 50 %. La résection chirurgicale a représenté la modalité de prise en charge la plus fréquente avec 56,2 % des cas. Conclusion : L'adénofibrome du sein est une affection bénigne assez fréquente. Son diagnostic oblige à une surveillance. Le traitement chirurgical doit être discuté selon l'âge, la taille et la présence de facteurs de risque de malignité


Fibroadenomas are the most common benign breast disorders. The aim of this study was to identify the clinical and therapeutic aspects of fibroadenoma in the obstetric gynecology department and General Surgery of Teaching hospital Gabriel TOURE in Bamako Mali. Patients and Methods: The study was descriptive with retrospective data collection from July 1, 2018 to July 31, 2021. The records of patients treated for fibroadenoma were included in accordance with ethical principles guaranteeing anonymity and confidentiality of data. Results: A total of 112 patients were selected for the study period, representing 15. 9% of all breast pathologies. We recorded 642 cases of breast mass, with fibroadenoma accounting for 16.72% of these breast tumors. Among all 154 benign breast tumors, fibroadenoma accounted for 72%. The mean age was 27 years, with a standard deviation of 14.8 years (13 years and 62 years). The 20-35 age group was the most represented, at 44%. The frequency of fibroadenoma decreased with increasing gestational age and parity. Over 60% of patients were overweight. The main reason for consultation was the presence of breast swelling. Unilateral left breast swelling was predominant in 46% of cases, and location in the upper-outer quadrant accounted for 50%. Surgical excision was the most frequent management modality, accounting for 56,2% of cases. Conclusion: fibroadenoma of the breast are a fairly common benign condition. Diagnosis requires surveillance. Surgical treatment should be discussed according to age, size and the presence of risk factors for malignancy.

Introduction : La vaccination de routine est une stratégie clé dans la prévention des maladies évitables par la vaccination. L'objectif de l'étude est d'apprécier les connaissances, attitudes des mères vis-à-vis de la pratique de la vaccination en milieu rural et urbain. Méthodes : Il s'agit d'une étude transversale descriptive comparative qui a eu lieu dans deux aires de santé (urbaine et rurale) sur une période de trois (3) mois d'octobre à décembre 2022. Elle a concerné les mères ou gardiennes d'enfants venus pour la vaccination de routine. Résultats : La moyenne d'âge des femmes était de 28, 76±6,85 ans en milieu urbain et 26,51±7,37 ans en milieu rural. Environ72, 00% et 43,00% des femmes respectivement en milieu urbain et rural avaient donné une bonne définition de la vaccination. 88,00% des femmes en milieu urbain connaissaient au moins une maladie cible du PEV contre 55,00% en milieu rural. Le calendrier vaccinal n'était connu que par 31,00% des femmes en milieu urbain contre 12,00% en milieu rural. La majorité des femmes en milieu urbain (97,00%) et rural (67,00%) trouvaient nécessaire de rattraper un rendez-vous de vaccination manqué les séances prochaines. Elles jugeaient majoritairement d'envoyer les enfants au centre de santé en cas de manifestations post- vaccinale (urbain : 80,00%, rural : 53,00%). Conclusion : Une bonne connaissance et pratique de la vaccination permettrait ainsi de protéger les enfants contre les maladies évitables par la vaccination


Background: Routine immunization is a key strategy in the prevention of vaccine-preventable diseases. The objective of the study was to assess the knowledge and attitudes of mothers regarding the practice of vaccination in rural and urban areas. Methods: This is a comparative descriptive cross-sectional study that took place in two health areas (urban and rural) over a period of three (3) months from October to December 2022. It involved mothers or guardians of children who came for routine vaccination. Results: The mean age of the women was 28.76±6.85 years in urban areas and 26.51±7.37 years in rural areas. About 72.00% and 43.00% of the women in urban and rural areas, respectively, had given a good definition of vaccination. 88.00% of women in urban areas knew at least one EPI target disease, compared to 55.00% in rural areas. Only 31.00% of women in urban areas knew the vaccination schedule, compared to 12.00% in urban areas. Majority of women in urban (97.00%) and rural (67.00%) areas found it necessary to make up for missed immunization appointments in next sessions. Majority of them considered it necessary to send their children to the health center in the event of postimmunization symptoms (urban: 80.00%, rural: 53.00%). Conclusion: A good knowledge and practice of vaccination would allow to protect children against vaccine preventable diseases

Introduction : La maladie thromboembolique veineuse (MTEV) regroupe : la thrombose veineuse profonde(TVP) et l'embolie pulmonaire(EP).Ce travail a été initié pour identifier les particularités féminines si elles existent. Matériels et Méthode : il s'agissait d'étude rétrospective, descriptive réalisée dans le service de cardiologie du CHU de Kati sur une période allant du 01 janvier 2014 au 31 décembre 2021. Ont été inclues les patientes hospitalisées durant la dite période pour maladie thromboemboliqueveineuse. Les variables étudiées étaient les données sociales démographiques, les facteurs de risque, les paramètres cliniques et paracliniques, les aspects thérapeutiques et évolutifs. Résultats : Au total nous avons enregistré 68 cas de MTEV dont 40 (58,8%) femmes. Il s'agissait 12 (30%) cas de TVP, 25 (62,5%) d'EP et leur association dans 3 (7,5%) cas. La majorité (82,5%) des patientes était des femmes au foyer.Le post partum représentait 10% des cas. La probabilité clinique, selonle score de Wells était élevée chez 93% des TVP, intermédiaire pour les cas d'embolie pulmonaire avec 50%. L'angioscanner révélait que l'embolie pulmonaire était bilatérale dans 80% des cas. L'héparine et l'AVK ont été les plus utilisés. La mortalité était de 7,5%. Conclusion : les femmes sont plus victime des maladies thromboemboliques veineuses que les hommes. Le post partum est une situationparticulière chez la femme. Les récidives ne sont pas rares


Introduction: Venous thromboembolic disease (MTEV) includes: deep venous thrombosis (DVT) and pulmonary embolism (PE).This work was initiated to identify the female particularities if they exist.Materials and Method: This was a retrospective, descriptive study conductedin the cardiology department of the Kati university hospital over a period from January 01, 2014 to December 31, 2021.Patients hospitalized during the period for venousthromboembolic disease were included.The variables studied were demographic social data, risk factors, clinical and para-clinical parameters, therapeutic and evolutionary aspects. Results: In total we recorded 68 cases of MTEV of which 40 (58.8%) women. There were 12 (30%) cases of deep vein thrombosis, 25 (62.5%) cases of pulmonary embolism and their association in 3 (7.5%) cases.The majority (82.5%) of patients were housewives.Thepostpartum accounted for 10% of cases.The clinical probability according to the Wells score was high in 93% of deep vein thrombosis, intermediate for cases of pulmonary embolism with 50%.Angio-CT showed that pulmonary embolism was bilateral in 80% of cases. Heparin and anti-vitamin K were the most commonly used. Mortality was 7.5%. Conclusion: women are more victims of venous thromboembolic diseases than men. Postpartum is a special situation for women. Recurrences are not uncommon

Objectifs : étudier les facteurs de risque de mortalité des enfants malnutris aigus sévères (MAS) dans les 72 premières heures d'hospitalisation. Méthode : Nous avons mené une étude rétrospective sur des données d'enfants MAS de 0-59 mois, hospitalisés du 1er juin 2016 au 31 mai 2019 au CHUSourô Sanou (CHUSS) de Bobo-Dioulasso. Une régression logistiquea été réalisée pour déterminer les facteurs de risque de décès. Les variables avec un odds ratio (OR) >1, et p 3s [OR :3.55, 95%IC: 1.50-8.27], le sepsis [OR : 3.39, 95%IC: 1.33-8.50], étaient les facteurs de risque prédictifs de mortalité. Conclusion : Une formation et supervision continue du personnel soignant sur la prise en charge des urgences pédiatriques, l'utilisation du protocole OMS de prise en charge de la MAS, la disponibilité de kits d'urgence pourraient améliorer la prise en charge des MAS. Objectives: to study risk factors for mortality in SAM children within the first 72 hours of hospitalization. Methods: We conducted a retrospective study on data from 0-59-month-old SAM children hospitalized from June 1th, 2016, to May 31th, 2019, at CHUSourô Sanou of Bobo-Dioulasso. A logistic regression was performed to determine risk factors for death. Variables with odds ratio (OR) > 1, and p 3seconds [OR :3.55, 95%IC: 1.50-8.27], sepsis[OR :3.39, 95%IC: 1.33-8.50]were the risk factors predictive of mortality. Introduction L'échographie, modalité plus accessible dans notre pays constitue une alternative incontournable au diagnostic des hépatopathies sur polyarthrite rhumatoïde (PR). L'objectif de ce travail était de déterminer les caractéristiques de ces hépatopathies ainsi que l'apport de l'échographie dans leur diagnostic au CNHU-HKM. Méthode Etude transversale descriptive et analytique portant sur les patients suivis pour une PR, vus en consultation dans le service de rhumatologie du CNHU-HKM, ayant bénéficié d'une échographie abdominale, pendant la période d'étude allant du 15 Juillet 2020 au 15 Octobre 2020. La prévalence, les caractéristiques clinico-biologiques et échographiques des hépatopathies ont été étudiées. Résultats Ont été colligés 42 sujets. la fréquence hospitalière des hépatopathies était de 40,5%. La sex-ratio était de 0,8 et l'âge moyen de 50,9 ans. Sur le plan clinique, des douleurs de l'hypocondre droit (9,5%) , une hépatomégalie (14,3%) et un syndrome métabolique (38%) avaient été trouvées. Une cytolyse hépatique était notée chez 01 patient ; l'Ag HBs positif chez 02 patients. A l'échographie une hépatomégalie (26,1%), une stéatose hépatique (16,7%) et un kyste hépatique (2,4%) étaient retrouvés. Les étiologies principales évoquées étaient : hépatomégalie isolée (41,2%), métabolique (35,5%) et virale ( 11,4%). Conclusion Les hépatopathies sur PR sont fréquentes. Le profil clinique n'est pas spécifique. L'échographie dans notre contexte constitue un moyen fondamental d'aide au diagnostic chez ces patients.


Ultrasound, a more accessible modality in our country, is an essential alternative to the diagnosis of liver disease on rheumatoid arthritis (RA). The objective of this work was to determine the characteristics of these liver diseases as well as the contribution of ultrasound in their diagnosis in CNHU-HKM. Method Descriptive and analytical cross-sectional study of patients followed for RA, seen in consultation in the rheumatology department of the CNHU-HKM, who underwent an abdominal ultrasound, during the study period from July 15, 2020 to October 15, 2020. The prevalence, clinico-biological and ultrasound characteristics of liver disease were studied. Results A total of 42 subjects were collected. The hospital frequency of liver disease was 40.5%. The sex ratio was 0.8 and the mean age was 50.9 years. Clinically, right hypochondria pain (9.5%), hepatomegaly (14.3%) and metabolic syndrome (38%) were found. Hepatic cytolysis was noted in 01 patient; HBsAg positive in 02 patients. On ultrasound, hepatomegaly (26.1%), fatty liver disease (16.7%) and a liver cyst (2.4%) were found. The main etiologies were isolated hepatomegaly (41.2%), metabolic (35.5%) and viral (11.4%). Conclusion: Liver disease in RA patients is common. Ultrasound in our context is a fundamental means of aiding in the diagnosis of these patients

ABSTRACT

Objectif : Déterminer la fréquence et les facteurs associés à la dénutrition en postopératoire dans deux hôpitaux universitaires publics au Bénin. Méthode : Il s'agissait d'une étude observationnelle transversale descriptive et analytique, qui s'est déroulée de mai à juillet 2022. Un recrutement exhaustif des patients devant être pris en charge chirurgicalement pour une pathologie digestive et ayant consenti à l'étude, a été effectué. Des données anthropométriques et de consommation alimentaire, des données cliniques ont été recueillies grâce à un questionnaire standardisé. Résultats : Un total de 51 patients a été inclus, avec une prédominance d'adultes âgés de moins de 50 ans (70,6%), un sex-ratio de 0,9 ; 72,5% avait un niveau d'instruction ≥ secondaire, 60,8% vivait en couple. Le niveau de revenu était inférieur au salaire minimum interprofessionnel garanti (SMIG) dans 52,9% des cas. La fréquence de la dénutrition était de 29,4% à l'admission contre 54% en postopératoire. Seuls les patients dénutris avaient présenté des complications postopératoires (36%). L'évaluation de l'état nutritionnel n'avait été consignée dans aucun des dossiers médicaux. En analyse multivariée, l'insuffisance en apport protéique et un bas niveau de revenu étaient associées à la dénutrition postopératoire (p = 0,002). Conclusion : La dénutrition en chirurgie digestive est une réalité dans les hôpitaux publics du Bénin et constitue un facteur de risque de complications postopératoires. Son dépistage et sa prise en charge doivent être systématiques et consignés dans le dossier médical du malade


Objective: To determine the frequency and factors associated with postoperative undernutrition in two public teaching hospitals in Benin. Method: This was a descriptive and analytical cross-sectional observational study conducted from May to July 2022. An exhaustive recruitment of patients to be managed surgically for a digestive pathology and who consented to the study, was carried out. Anthropometric, food consumption and clinical data were collected using a standardized questionnaire. Results: A total of 51 patients were included, with a predominance of adults under 50 years of age (70.6%), a sex ratio of 0.9; 72.5% had an educational level ≥ secondary, 60.8% were living as a couple. Income was below the guaranteed minimum wage in 52.9% of cases. The incidence of undernutrition was 29.4% on admission, compared with 54% postoperatively. Only the undernourished patients (36%) presented postoperative complications. Assessment of nutritional status was not recorded in any of the medical register. In multivariate analysis, insufficient protein intake and low income were associated with postoperative undernutrition. Conclusion: Undernutrition in digestive surgery is a reality in public hospitals in Benin, and constitutes a risk factor for postoperative complications. Its detection and management must be systematic and recorded in the patient's medical register.

Introduction Les dermatoses tumorales sont des néoformations tissulaires dues à une prolifération cellulaire excessive, anormale, anarchique et plus ou moins autonome, aux dépens de la peau et de ses annexes. Ces dermatoses tumorales peuvent être bénignes, malignes ou à malignité limitée. L'objectif de la présente étude est de répertorier l'ensemble des dermatoses tumorales vues en consultation dermatologique au CNHU-HKM de Cotonou. Patients et méthodes Etude transversale rétrospective et descriptive ayant porté sur les dossiers des patients vus pour des tumeurs cutanéo-muqueuses, dans le service de Dermatologie-vénérologie du Centre National Hospitalier Universitaire Hubert Koutoukou Maga (CNHU-HKM) de Cotonou, de 2009 à 2023. Les données épidémiologiques et cliniques ont été recueillies à partir des registres de consultations puis saisies et analysées avec les logiciels Excel, EPI-DATA et SPSS. Résultats Au total, 15627 nouveaux patients ont été enregistrés, dont 1221 atteints de dermatoses tumorales (7,81%). L'âge moyen était de 33 ans ± 18,71 ; la tranche d'âge la plus représentée était celle de 20 à 39 ans (46,30%). La sex-ratio était de 1,08. Les lésions étaient acquises chez 93,94% des patients. Trente-cinq types de dermatoses tumorales ont été colligés. Les tumeurs bénignes représentaient 95,43% des patients. Elles étaient dominées par les chéloïdes (25,34 %) suivies parles condylomes (17,50 %), les verrues (10,12%), le molluscum contagiosum (7,54 %), l'ensemble formé par les nævi et hamartomes (5,18 %) et les kystes sébacés (4,57 %). Les tumeurs malignes étaient rares et représentées par les carcinomes spinocellulaires (1,14%), basocellulaires (0,30%) et les mélanomes (0,30%). Les tumeurs à malignité limitée sont essentiellement représentées par la maladie de Kaposi retrouvée chez 2,05% des patients. Conclusion Les dermatoses tumorales sont relativement fréquentes en milieu hospitalier au Bénin. Elles sont dominées par les tumeurs bénignes avec à leur tête les chéloïdes et les condylomes

These tumoral dermatoses can be benign, malignant, or of limited malignancy. The aim of this study is to catalog all the tumoral dermatoses seen in dermatological consultation at NUHC- HKM in Cotonou. Patients and Methods This was a retrospective, descriptive cross-sectional study based on the files of patients seen for cutaneous or mucosal tumors in the Dermatology-Venereology department of the National University Hospital Center Hubert Koutoukou Maga (NUHC-HKM) in Cotonou, from 2009 to 2023. Epidemiological and clinical data were collected from consultation registers and analyzed using Excel, EPI-DATA, and SPSS software. A total of 15,627 new patients were registered, of which 1,221 were affected by tumoral dermatoses (7.81%). The average age was 33 years ± 18.71; the most represented age group was 20 to 39 years (46.30%). The sex ratio was 1.08. Thirty-five types of tumoral dermatoses were collected. The lesions were non congenital in 93.94% of patients. Benign tumors accounted for 95.43% of the patients. They were dominated by keloids (25.34%) followed by condylomas (17.50%), warts (10.12%), molluscum contagiosum (7.54%), the group formed by nevi and hamartomas (5.18%), and sebaceous cysts (4.57%). Malignant tumors were rare and represented by squamous cell carcinomas (1.14%), basal cell carcinomas (0.30%), and melanomas (0.30%). Tumors with limited malignancy were primarily represented by Kaposi's disease found in 2.05% of patients. Conclusion Tumoral dermatoses are relatively frequent in the hospital setting in Benin. They are dominated by benign tumors, headed by keloids and condylomas

Introduction: L'altération de la qualité de vie est l'une des conséquences de l'infection chronique au virus de l'hépatite C. L'objectif de cette étude était d'évaluer l'impact du traitement par les antiviraux à action directe (AAD) sur la qualité de vie des patients atteints d'hépatite C à Cotonou. Méthodes : Il s'agissait d'une étude transversale descriptive et analytique, avec un recueil de données à la fois rétrospectif et prospectif sur une période de 5 ans allant du 1er décembre 2015 au 1er septembre 2020. Etaient inclus tous les patients atteints d'hépatite C traités par les AAD dans la Clinique Universitaire d'hépato-gastroentérologie du Centre National Hospitalier et Universitaire Hubert Koutoukou Maga (CNHU-HKM) de Cotonou. L'efficacité était évaluée par la réponse virologique soutenue (RVS) à 12 semaines après la fin du traitement. Le questionnaire SF-36 était utilisé pour évaluer la qualité de vie des patients atteints d'hépatite C, avant et après traitement. Résultats : Pendant la période d'étude, 206 patients ont été colligés. Ils étaient constitués de 33,5% d'hommes et 66,5 % de femmes, soit une sex-ratio de 0,5. La moyenne d'âge des patients était de 62,0 ± 1,7 ans (extrêmes de 18 et 87 ans). La charge virale moyenne en UI/ml était de 3.507.336,9 ± 1.156.443,98 (extrêmes de 3422 et 75.674.348 UI/ml). Le score SF-36 était statistiquement plus élevé pour tous les items après le traitement par AAD (p˂0,05). Conclusion: Les antiviraux à action directe constitue un traitement qui améliore significativement la qualité de vie des patients atteints d'hépatite C à Cotonou.


Background: Impaired quality of life is one of the consequences of chronic infection with the hepatitis C virus. The aim of this study was to assess the impact of treatment with direct-acting antivirals (DAAs) on quality of life in patients with hepatitis C in Cotonou. Methods: This was a descriptive and analytical cross-sectional study, with both retrospective and prospective data collection over a 5-year period from 1 December 2015 to 1 September 2020. All patients with hepatitis C treated with DAAs at the University Hepato-Gastroenterology Clinic of the National and University Hospital Hubert Koutoukou Maga (CNHU-HKM) in Cotonou were included. Efficacy was assessed by the sustained virological response (SVR) at 12 weeks after the end of treatment. The SF-36 questionnaire, the reference measurement instrument, was used to assess the quality of life of hepatitis C patients before and after treatment. Results: A total of 206 patients were enrolled during the study period. They comprised 33.5% men and 66.5% women, giving a sex ratio of 0.5. The mean age of the patients was 62.0 ± 1.7 years, (extremes 18 and 87 years). The mean viral load in IU/ml was 3,507,336.9 ± 1,156,443.98 (extremes 3,422 and 75,674,348 IU/ml). The SF-36 score was statistically significantly higher for all items after DAA treatment (p˂0.05). Conclusion: Direct-acting antivirals are a treatment that significantly improves the quality of life of hepatitis C patients in Cotonou.
