ABSTRACT
Objectives: Patients with AIDS who present with an acute abdomen pose a new challenge to surgeons. The purpose of this study was to determine the nature and outcome of intra-abdominal catastrophes in AIDS patients. Methods: A prospective clinical study was conducted on HIV-positive patients who presented with an acute abdomen. Results: Thirteen patients were included. Average age was 36 years. Seven patients presented with advanced AIDS. In 10 the ileo-caecal region was affected by a chronic inflammatory process resulting in ulceration and necrosis. Intestinal tuberculosis was found in 7 patients. Despite optimal treatment more than half the study group died in hospital. Conclusion: The 'acute AIDS abdomen' proved to be different in nature and prognosis than has been described previously. Intestinal tuberculosis can reasonably be suspected. Operative mortality is unacceptably high. Other treatment options are being investigated

Le Congo est dans la zone ou le SIDA sevit da facon endemique. La Chirurgie dans cette zone est peut-etre a l'origine de plusieurs contaminations accidentelles (transfusions; blessures). Quelle attitude adopter vis-a-vis des malades sero-positifs qui presentent des affections chirurgicales ou des malades immuno-deprimes pour lesquels les confreres medecins nous demandent de faire des biopsies ganglionnaires. Pour nous; le SIDA ne constitue pas une contre-indication operatoire du moins pour la chirurgie d'urgence. Il faudrait bien sur redoubler de vigilance; travailler avec des gants corrects; du materiel bien sterilise; et des locaux desinfectes regulierement. Les blocs operatoires ne doivent souffrir d'aucune rupture de detergents; ni d'antiseptiques

Reports of various studies conducted in one of the three general surgical firms of Muhimbili Medical Centre on opportunistic diseases common in surgical practice occurring in HIV/AIDS subjects is presented. Cervical lymphadenopathy in these subjects due to tuberculosis; non-specific adenitis; and Kaposi's sarcoma were found to be common; while lymphomas were rare. Pyomyositis; Kaposi's sarcoma; throactic empyema and pericardial effusion occur in a significant number of these patients. An attempt is made to establish the pattern of the various clinical manifestations in HIV/AIDS subjects
