ABSTRACT

Objectif : Déterminer la fréquence, le pronostic et la morbidité des patientes opérées pour cancer du sein.Matériel et Méthodes : Il s'agit d'une étude rétrospective, descriptive et analytique, sur une période de trois ans, allant de janvier 2013 à janvier 2017. Elle a porté sur les patientes opérées pour cancer du sein dans le service de gynécologie obstétrique et à l'Institut de Cancérologie du Centre Hospitalier Universitaire d'Angondjé (CHUA) de Libreville. Les informations recueillies ont été analysées à l'aide du logiciel R. Résultats : Un total de 51 patientes a été colligé. La moyenne d'âge des patientes était de 50 ans avec des extrêmes de 27 et 88 ans. Le délai moyen de consultation était de 56 semaines avec des extrêmes de 3 et 288 semaines. Le délai moyen entre la consultation et la preuve histologique était de 60 semaines avec des extrêmes de 2 et 288 semaines. Au moment du diagnostic, 65,2 % des patientes étaient au stade III de la classification de l'Union Internationale Contre le Cancer et 66% avaient un carcinome canalaire infiltrant. La mastectomie totale selon Madden et le curage ganglionnaire ont été réalisés respectivement dans 78,4% et 86,3 des cas. L'envahissement ganglionnaire a été constaté dans 60% des femmes. En tout, 33,3% des patientes ont présenté des complications post-opératoires, dont 13,7% de lymphocèle.Conclusion : la prise en charge chirurgicale des cancers du sein est dominée par la mastectomie modifiée type Madden, avec une faible morbidité post opératoire

ABSTRACT

The study aimed to establish the common reasons for non-adherence to drug treatment among breast cancer patients at the Obafemi Awolowo University Teaching Hospitals Complex; Ile-Ife; Nigeria over a ten-year period (Jan 1993 - Dec 2002). Patients' adherence and reasons for non-adherence to chemotherapy were evaluated using descriptive statistics. Two hundred and twenty-five breast cancer patients including 199 females and 6 males with breast cancer were recruited. 12(5.3) patients had Stage I disease while 126 (56) were in stage IV. The non-adherence rate was 80.9and 111(73) of the non-adherent patients were eventually not seen again. Of the one hundred and one patients who gave reasons for non-adherence; 45complained of financial difficulties; 18thought they were well enough; 15were fearful of subsequent operation and 11were unable to further bear the drug side effects. In conclusion; the study demonstrated poor economic status of the patients as one of the major reasons for non-adherence to cancer chemotherapy in Nigeria. Government subsidy of breast cancer treatment; improved health education and advocacy complemented by home visiting to encourage hospital attendance would be required for better adherence to chemotherapy

ABSTRACT

Breast cancer is rare in young women aged 30 and below. Out of a total of 374 patients admitted and managed for breast cancer in a period of 12 years; 14 (3.74) were aged 30 and below (mean 26.31 years); Their parity ranged between 0 and 4 (mean 1.5) and the duration of symptoms before presentation ranged between 3 weeks and 3 years. 6 cases involved the right breast; 4 the left and 3 were bilateral. All the patients presented with advanced breast cancer (stage 4 disease). Ten had invasive ductal Ca while one each had lobular carcinoma; lymphoma and malignant phyllodes tumour respectively. Management was multimodal: neovadjuvant chemotherapy; tamoxifen; mastectomy and referral for radiotherapy. Two developed features of depressive illness in the course of treatment; 6 died within 6 months of presentation; 1 after 3 months; 1 refused mastectomy discharged herself against in favour of traditional healers. The remaining 6 were lost to follow-up. It is confirmed that as else where; in Nigerians breast cancer in younger women is associated with a high degree of morbidity and mortality as well as physical and emotional stress

ABSTRACT

La Prise en charge des cancers de sein revient a apprecier les orientations strategiques basees sur les principes suivants : - definir les facteurs de pronostic - preferer le traitement radical au traitement conservateur a defaut de la radiotherapie - savoir associer un traitement adjuvant : chimiotherapie ou hormonotherapie en fonction de l'age et de l'etat hormonal de la patiente. Une telle attitude permettra de prolonger la survie des patientes
