ABSTRACT
L'etude est prospective et s'etend sur une periode de 24 mois. L'objectif general est d'exposer les effets benefiques de la ligature de l'ACE. Les objectifs specifiques sont : -la recherche de l'exsanguinite du champ operatoire; avec comme consequence la reduction des pertes sanguines dans les interventions tres hemorragiques; reduisant ainsi la dependance en produits sanguins ; montrer l'interet de cette technique dans les hemorragies cervico-faciales menacant le pronostic vital. Il ressort de cette etude que la ligature de l'ACE est une methode chirurgicale simple qui peut etre un procede d'hemostase de choix dans certaines situations: tumeurs benignes et malignes; hemostase d'urgence. Ses avantages sont non negligeables: sa rapidite; l'exsanguinite du champ operatoire avec economie de la consommation en produits sanguins; il n'y a aucune complication

Dans l'etiologie des ruptures uterines spontanees; l'existence d'une cicatrice uterine occupe une place preponderante: cesariennes iteratives segmentaires ou corporeales; curetages repetes et myomectomies. De symptomatologie souvent bruyante; ces ruptures uterines entrenaient presque toujours la mort foetale et constituent une cause non negligeagle de mortalite maternelle au Zaire

The Enugu experience of surgical treatment for persistent patency of the ductus arteriosus (PDA) over the period April 1986 to March 1992 is reviewed. A total of 322 patients with PDA had left thoracotomy. Ligation alone was carried out in six patients (1;86 per cent); division and suture closure was done in 312 (96;89 per cent); while in 4 patients (1;24 per cent); the PDA was not closed because of the presence of severe pulmonary hypertension and right to left shunting at the ductal level. The commonest associated anomaly was ventricular septal defect requiring pulmonary artery banding in addition to ductal closure in 59 patients (18;30 per cent). There were 2 deaths in the series (0;62) with occured in neonates with other malformation. It is concluded that operative closure of PDA is a safe procedure

In carefully selected patients H.A.L is the best surgical of palliation of this disease in our experience. It is relatively simple and the post operative complications are neglible. It is emphasized that with early detection of the tumour more cases could benefit from this procedure
