ABSTRACT

Background: The testis can be biopsied either for a diagnostic or therapeutic purpose. Objective: The aim of this study is to characterize the common indications for testicular biopsy and determine the pattern of testicular lesions. Materials and Methods: This was an 8­year retrospective study of all testicular and paratesticular specimens that were histologically diagnosed in the Department of Histopathology in University of Uyo Teaching Hospital between January 2008 and December 2015. Results: Sixty­four cases of testicular specimens were received in the histopathology laboratory, accounting for 1.3% of all received specimens. The youngest patient was 4 years, while the oldest patient was 86 years with a mean age of 54.4 ± 21.62. The most common presenting complaints seen in the nonfertility, nonprostate cancer­related cases were testicular swelling, pain, and small­sized testis as seen in 50%, 31.3%,and 12.5% of cases, respectively. In 60.9% of cases, the clinical diagnosis was prostatic cancer, while primary and secondary infertility accounted for 12.5% and 1.6%, respectively, with testicular/paratesticular tumor been the clinical diagnosis in 7.8% of cases. The mean ages for surgical castration patients, male infertility patients, and malignant lesion patients were 68.9, 41.7, and 46.5 years,respectively. Hypospermatogenesis was the most common histopathologic diagnosis of testicular biopsies in infertile men (33.4%). Four malignant lesions were seen, with embryonal rhabdomyosarcoma accounting for 50% of cases. Conclusion: Benign neoplastic lesions of the testis are very rare in study population, while embryonal rhabdomyosarcoma is the most common malignant lesion seen. Most men do not present to health facility for infertility management

ABSTRACT

Objectives: The present study is undertaken to describe the spectrum of histopathological features and age distribution of non-neoplastic testicular and paratesticular lesions in the University of Maiduguri Teaching Hospital (UMTH). Materials and methods: A retrospective descriptive study of 70 testicular and paratesticular nonneoplastic lesions was conducted over a period of 10 years; between January-2005 and December2014 in the Department of Histopathology, UMTH. Histopathological examination was done after routine processing and staining with Haematoxylin and Eosin. Special stain (Ziehl-Neelsen stain) was done to confirm the presence of acid fast bacilli in cases of tuberculous epididymoorchitis. Results: There were of which (24.3%) followed by testicular torsion and infarction (14.3%). The youngest patient was 2 years old and the oldest was 80 years of age. The highest incidence occurred in the age range of 30 ­ 59 years with a total of 25 cases representing 35.7%. The lowest incidence was observed in the elderly (= 60 years) with a total of 21 cases (30%). Conclusion: This study shows that inflammatory disorders are the predominant causes of the testicular and paratesticular non-neoplastic lesions capable of interfering with fertility and mimicking malignancy. There is also the need to emphasise the necessity of proper evaluation and treatment of acute orchitis and microabscess to avoid unnecessary orchidectomy.70 cases of non-neoplastic testicular and paratesticular lesions the majority were inflammatory disorders accounting for 53 cases (75.7%). They included acute orchitis (4.3%), tuberculosis (12.9%), schistosomiasis (8.6%), hydrocoele (24.3%), chronic orchitis (10.0%), epididymal cyst (11.4%) and tumoral calcinosis (4.3%). Other categories of the lesions included congenital abnormalities (10.0%) and traumatic disorder (14.3%). Long-standing hydrocoele was the commonest lesion

ABSTRACT

Background: To analyze the spectrum of testicular tumors in prepubertal children and the therapeutic resultants in an unselected population. Materials and Methods: Our hospital database was analyzed for testicular tumors from January 1995 to December 2010 concerning clinical presentation; treatment and therapeutic results. Results: Eight patients were operated on because of testicular tumors. In six cases (75) the tumor was benign: benign teratoma (four cases); epidermoid cyst (one case) and immature teratoma (one case). Two patients (25) had a malignant tumour: yolk-sac tumour (two cases). All this children underwent surgery. Radical inguinal orchidectomy was performed in six cases and conservative surgery was performed in two cases. One patient has received adjuvant chemotherapy. Follow-up was uneventfully three years after primary surgery. Conclusion: In prepubertal children; most testicular tumours are benign. If tumour markers were negative testis-preserving surgery can be proposed; complete excision of the tumour should be ascertained. In the case of testicular teratoma; the possibility of contralateral tumour should be considered in the follow-up

ABSTRACT

Le seminome spermatocytaire est une tumeur rare; representant moins de 2des cancers du testicule; survenant essentiellement chez le sujet age. Nous rapportons une nouvelle observation d'un patient age de 48 ans. La tumeur se presentait comme une proliferation de cellules en nappes compactes; avec 3 types cellulaires; des cellules de petite taille; des cellules intermediaires et des grandes cellules. Il n'a ete retrouve ni contingent sarcomateux; ni seminome classique. L'analyse en immun histochimie n'a retrouve aucune expression des cellules tumorales pour les anticorps classiques testes; notamment l'Ac anti PLAP et les marqueurs lymphoides. Le seminome spermatocytaire doit etre reconnu; car son evolution est tres favorable et ne necessite qu'une simple orchidectomie; en l'absence d'un exceptionnel contingent sarcomateux ou de metastase ou une chimiotherapie s'impose

ABSTRACT

Introduction: Liposarcomas are neoplasms of mesodermal origin derived from adipose tissue and correspond to 10-14of all soft tissue sarcomas. Paratesticular liposarcoma is very rare. Case report: We report a 60-year old man who presented with a left testicular tumor 20 cm in diameter. Initial incisional biopsy was reported as fibromatosis. Chest and abdominal CT scan did not show distant metastases. Through an inguinal incision orchidectomy with homolateral inguinal node dissection was performed. Histopathological examination showed a paratesticular myxoid liposarcoma. Adjuvant radiotherapy without chemotherapy was administered. The patient remains well at 11 months followup; with no evidence of recurrence. Conclusion: Complete surgical extirpation reduces the risk of local recurrence. Neoadjuvant chemotherapy or radiotherapy may reduce the tumor size; thus facilitating complete excision

ABSTRACT

Objectif : Le but de cette étude est d'étudier les caractéristiques histologiques du cancer du testicule en Côte d'Ivoire. Matériel et méthodes : Cette étude rétrospective porte sur les dossiers de 54 patients âgés en moyenne de 23,41 ans (extrêmes 13 mois et 68 ans) avec des tumeurs malignes du testicule, colligés en 25 ans dans les services d'Anatomie pathologique de Côte d'Ivoire. Les renseignements suivants ont été évalués: l'âge, l'origine géographique des patients ainsi que les caractéristiques cliniques des tumeurs telles leur localisation, leur nature et leur type histologique. Certaines données telles que les marqueurs tumoraux et la stadification de la maladie n'ont pas été prises en compte. Résultats : En moyenne deux tumeurs ont été diagnostiquées par an. Sur le plan histologique, 87% des tumeurs sont primitives. On y distingue: 46,3% de tumeurs germinales parmi lesquelles les tumeurs germinales non séminomateuses sont prédominantes avec un taux de 24.07% de l'ensemble des cas et les séminomes 22.22% des cas; et 40.74% de tumeurs non germinales avec 18,5% de rhabdomyosarcomes. Les tumeurs secondaires représentent 13,9% des tumeurs du testicule de notre série. Il s'agit essentiellement de localisations secondaires des lymphomes de Burkitt (5 cas sur 7). Enfin, l'atteinte bilatérale est importante. La tumeur est bilatérale, d'emblée dans 31% des cas. Conclusion : Il ressort que les tumeurs du testicule sont rares en Côte d'Ivoire, avec une incidence annuelle d'environ deux cas. Elles surviennent chez l'adulte jeune âgé en moyenne de 23 ans et sont souvent de mauvais pronostic à cause de la bilatéralité de leur atteinte. En effet, 31% des tumeurs testiculaires atteignent simultanément les deux testicules
