ABSTRACT

Patients consent for medical care is a legal requirement. Consent is a document that patient must be necessary aware about all risks of surgery, treatment strategy, Substitution and the risks involved by physician. This study was done to evaluate status of filled consent form of hospitalized patients in Ardabil - Northwest of Iran. This descriptive cross sectional study was done by interview with patients and accompained patients in hospitals of Ardabil - Northwest of Iran during 2010. Patient demographics and other variables gathered by a questionnaire. Data was analyzed by Chi-Square test. 86 [57%] of patients were female and 64 [43%] male. 35 [23%] were illiterate and 92 [61%] under diploma. 45% of patients get information about their illness from doctors. According to patient ideas, 13% of reception personals did not complete consent form. 65 [43.3%] of patients stated that the context of consent form was unreadable and long. Rate of completing form for patients were 87%. 40% of completed forms was not signed by witnesses. The patients and relatives were not satisfied with the form itself, and this observation statistically was significant [P<0.001]. This study showed that validity of completed consent forms are low and it is necessary to design programs for improvement the quality of filling the consent forms

ABSTRACT

Leukocyte adhesion deficiency syndrome [LADS] is a rare inherited immunodeficiency in which the frunction of leukocytes, particularly the phagocytes are disturbed. In the present study, we report three cases with LADS [2 male infants and 1 female child] who had a history of delay in separation of umbilical cord. One infant admitted for skin infection and two patients admitted in hospital following bacterial and fungal infections in nose and finger. All cases had a remarkable leukocytosis, neutrophilia and decreased of CD 11 and CD 18. LADS is a congenital syndrome causes recurrent bacterial and fungal infections. Usually all patients have a history of delayed separation of umbilical cord

ABSTRACT

Cerebral metastases from choriocarcinoma are poor prognostic indicator of outcome in both the World Health Organization and FIGO classification systems. Although gestational trophoblastic neoplasia has become the most curable gynecological malignancy, failure rate among 'high-risk' patients is still high despite the use of aggressive multidrug regimens. A 27 year old woman [G4P2Ab1] presented with hemiplegia due to brain metastases of choriocarcinoma one year after spontaneous abortion. She underwent craniotomy and was treated with nine courses of multiple agent etoposide, methotrexate, actinomycin-etoposide and cisplatinum [EMA-EP] regimen combined with whole brain irradiation. She delivered a term healthy child two years after termination of treatment. Multiagent EMA-EP chemotherapy and whole brain irradiation with craniotomy in selected patients preserves fertility and may improve a patient overall prognosis

ABSTRACT

Developmental dysplasia of the hip [DDH], formerly known as congenital dislocation of the hip [CDH] is a common congenital malformation which occurs worldwide with variable frequencies in respect to the race and geographic distribution. To determine the incidence of DDH, its predisposing factors and the diagnostic value of click for its detection. In a cross-sectional study of over 9-month duration, all of neonates born in the four teaching hospitals of Mashhad were examined during 48 hours after birth by Ortolani test. For all newborns with unstable joints, data regarding maternal history were collected. If clunk was felt on the first examination or click on repeated examinations, sonography was performed for more accurate diagnosis. All infants with definite CDH or unstable hip was referred to orthopedic clinic. A total number of 6576 newborns were examined over a period of nine months. Clunk or click was found in 3% [197 neonates]. Only 10 newborns had CDH by clinical and sonographic findings. Of these, eight had clunk at birth and two had persistent click on repeated examinations. These were finally diagnosed as CDH. Eleven neonates were born in breech position. CDH was bilateral in 60% of patients, and solely left-sided in 30%. The incidence of congenital dislocation of the hip was 1.5/1000. There was a significant association between the first parity and CDH [p<0.05], but not with maternal age. The incidence of CDH in the sample studied [1.5/1000] is similar to other studies reported so far. Many of unstable hips showed no abnormality or click on repeated examinations and were thus of no value in diagnosis of CDH
