ABSTRACT

OBJECTIVES@#To investigate the diversity of peripheral blood T cell receptor (TCR) β chain complementarity-determining region 3 (CDR3) based on immune repertoire sequencing in neonates with sepsis and the possible pathogenesis of neonatal sepsis.@*METHODS@#A total of 12 neonates with sepsis were enrolled as the case group, and 9 healthy full-term infants, matched for gestational age, birth weight, and age, were enrolled as the control group. Omega nucleic acid purification kit (SQ blood DNA Kit II) was used to extract DNA from peripheral blood samples, TCR β chain CDR3 was amplified by multiplex PCR, and then high-throughput sequencing was performed for the products to analyze the diversity of TCR β chain CDR3 and the difference in expression.@*RESULTS@#The length and type of TCR β chain CDR3 were similar between the case and control groups, and Gaussian distribution was observed in both groups. With D50 and Shannon-Wiener index as the evaluation indices for diversity, the case group had a significantly lower diversity of TCR β chain CDR3 than the control group (@*CONCLUSIONS@#There is a significant change in the diversity of TCR β chain CDR3 in the peripheral blood of neonates with sepsis, suggesting that it might be associated with the immune pathogenesis of neonatal sepsis.

ABSTRACT

Objective@#To investigate the factors associated with the patient's adherence to screening in the five years before the diagnosis of hepatocellular carcinoma (HCC) related to chronic hepatitis B ( CHB ), so as to provide reference for improving the screening rate.@*Methods@#From June 2016 to April 2018, the patients with newly diagnosed HCC and a history of CHB for more than five years in Southwest Hospital in Chongqing were interviewed. The information about socio-demographic characteristics, health status, medical care and HCC screening in the past five years were collected. A multivariate logistic regression model was used to analyze the factors associated with adherence to screening. @*Results@#Among 420 participants, 140 ( 33.33% ) adhered to HCC screening, 124 ( 29.53% ) had irregular/incomplete screening, while 156 ( 37.14% ) never had screening. The proportion of early-stage HCC at diagnosis was significantly higher in patients who adhered to screening ( 77.14% ) than that in patients who had irregular/incomplete screening (35.48%) or no screening ( 12.82% ) and the differences were statistically significant ( P<0.05 ). The multivariate analysis demonstrated that five factors were significantly associated with patient's adherence to screening, including education level of high school and above ( OR=2.346, 95%CI: 1.370-4.017), family history of HCC ( OR=2.795, 95%CI: 1.457-5.362 ), history of chronic diseases ( OR=3.860, 95%CI: 2.052-7.262), acceptance of antiviral therapy ( OR=17.816, 95%CI: 9.702-32.716 ) and specialized clinic visits ( OR=8.332, 95%CI: 1.588-43.710 ).@*Conclusions@#Adherence to screening is conducive to the early detection of HCC, but the screening rate is low in the patients with CHB. Education level, history of HCC, health status and medical status are significantly related to screening adherence.

ABSTRACT

Purpose@#This study sought to understand Taiwanese women’s decisional experiences regarding prenatal screening procedures and diagnostics. @*Methods@#A hermeneutic phenomenological design guided semistructured interviews with 33 women who were 36 weeks pregnant. Data were collected between February and October 2016. Verbatim transcripts were analyzed following hermeneutic circle to cocreate an understanding of Taiwanese women’s decision-making in prenatal screening and diagnostics. @*Results@#Women’s existential experiences were derived from their decision-making process on prenatal screening procedures and diagnostics for chromosomal aneuploidy. These decisional experiences were captured by four theme clusters and eight themes, which were inductively derived from 16 meaning units: (1) accessing health information; (2) considering what was best for my baby; (3) considering family finance; and (4) feeling anxiety posttest. @*Conclusion@#Participants made informed choices on several prenatal screening procedures, ostensibly, based on their personal values and considerations. During the decision-making process, often-cited benefits of genetic screenings were emphasized, but test limitations were often unheeded. A fundamental need for supportive information in decision making was further identified with recommended strategies. Hence, a revision of traditional genetic counseling approaches is recommended. As genomics technologies are increasingly available during antenatal services, women should be sufficiently educated about them to support decision making.

ABSTRACT

OBJECTIVETo investigate the skill and evaluate the accuracy for application of guide combined with probing the internal wall of pedicle screw trajectory for subaxial cervical pedicle screw placement.

METHODSSubaxial cervical pedicle screw was inserted in 11 patients by the guide combined with probing the internal wall of pedicle screw trajectory from January 2014 to October 2016, including 7 males and 4 females with an average age of 48.1 years(ranged 32 to 63 years). There were 4 cases with cervical spondylotic myelopathy, 4 with fracture and dislocation of cervical vertebrae, 1 with cervical cord injury without fracture and dislocation, and 2 with atlantoaxialfracture and dislocation. The target pedicle's diameter, optimal entry point, sagittal angle and cross-sectional angle were measured by CT before operation. During operation, the pedicle screw inserted angle was controlled by a guide with a self-designed protractor and probed the internal wall of pedicle screw trajectory as medial safety margin of insertion screw. The accuracy of cervical pedicle screw was evaluated by CT with classification of four grades and assessed whether there was injury of spine cord or vertebral artery postoperatively.

RESULTSSeventy-one cervical pedicle screws were placed among 11 patients, and no one had been found with clinical manifestations of injury of spine cord (or nerve root) or vertebral artery after operation. According to postoperative CT scan for evaluating the grade of screw position, 52 screws were in grade 0, 13 in grade 1, 4 in grade 2, 2 in grade 3, and 91% (65/71) located in good position. In total, 6 screws were incorreted in placement, and 4 cases of them broke medial wall and 2 cases broke lateral wall.

CONCLUSIONSThe method of probing the internal wall of pedicle screw trajectory for subaxial cervical pedicle screw placement is safe and reliable, but the studying curve is long. Probing the internal wall of pedicle screw trajectory and controlling the insertion angle by guide with a protractor are key points of this technology.

ABSTRACT

OBJECTIVE: To investigate the blocking effect of bergapten on cell cycle in nasopharyngeal carcinoma (NPC) cells. METHODS: The inhibition was tested by CCK-8 assay after the NPC cells (CNE-2 and HONE-1) being exposed to bergapten for 48 h. The bergapten concentrations were as follows: 1, 10 and 100 μmol·L-1. The cell cycle was assayed by PI staining and following flow cytometry analysis, and the CDK4, CDK6, CDK2, Cyclin D1, Cyclin E2, β-catenin, GSK-30 and p-GSK-3p (S9) protein levels were detected by Western blotting. RESULTS: Bergapten was able to significantly inhibit CNE-2 and HONE-1 NPC cancer cell proliferations, and the effect was dose-(CNE-2: r=0.926, P<0.05; HONE-1: r=0.959, P<0.05) and time-dependent (CNE-2: r=0.991, P<0.05; HONE-1: r=0.963, P<0.05). Drug treatment induced a block in the G0/G1 phase and decreased protein levels of CDK4, CDK6, CDK2, Cyclin D1, Cyclin E2 and β-catenin, but the protein levels of GSK-3β and the phosphorylation of p-GSK-3β (S9) didn't change. CONCLUSION: Bergapten can block cells cycle and inhibit NPC cells' proliferations, and those effects might be related to suppressing of cells cycle-related protein and interfering with the wnt/p-catenin signal pathway.

ABSTRACT

Although guidelines and formulas have been developed through clinical practice to define infusion rate and volume, over- and under-resuscitation are still common, followed by increasing morbidity and mortality. In order to establish an effective management for early fluid resuscitation, the clinical decision support system (CDSS) has been established. The CDSS, by utilizing information systems coupled with decision support technology, could provide recommendations for the amount of fluid to be infused based on measured biological response. The results showed that patients treated with CDSS had a significantly lower mortality, increased ventilator-free days, and ICU-free days as compared with those treated with traditional fluid management. This article reviews the concepts as well as the result of recent clinical studies of CDSS for burn patients.

ABSTRACT

OBJECTIVETo obtain allelic loss mapping and define the minimal lost region on chromosome 1q21 in gastric carcinomas, and explore role of 1q21 loss of heterozygosity (LOH) in the development and progression of gastric carcinogenesis.

METHODSUsing 7 high-density microsatellite markers and PCR method, lq21 LOH was analyzed in 30 paired specimens of fresh gastric carcinoma, and the relation between 1q21 LOH and the clinicopathological features of the malignancy was tested.

RESULTSThe LOH frequency on chromosome 1q21 from these gastric carcinoma tissues reached 60% (18/30). The LOH frequencies of the microsatellite markers D1S514, D1S2696, D1S498, D1S305, D1S2624, D1S2635 and D1S2702 were 13.3%, 10%, 20%, 23.3%, 33.3%, 40% and 23.3%, respectively. The minimal lost region on 1q21 LOH in the gastric carcinoma tissues was located in the region between D1S2624 and D1S2707, in the vicinity of D1S2635. No significant relations of 1q21 LOH to the patients' age, gender, location of the primary foci, clinical staging, or the tumor differentiation were noted (P>0.05), but 1q21 LOH was correlated to lymph node metastases of the malignancy (P<0.05).

CONCLUSIONHigher frequency of 1q21 LOH occurs in gastric carcinoma cells, suggesting the presence of potential tumor suppressor genes closely associated with gastric carcinogenesis near the region of D1S2635.

OBJECTIVETo investigate the stress intensity and distribution in the dentine of intact endodontically treated mandibular second premolar under three different restorations.

METHODSFour three-dimensional finite element models were constructed by the data of spiral CT: 1) Mandibular second premolar restored directly by full crown: 2) Fill the perforating cavity with resin material and full crown restoration; 3) Cast nickel chromium (NiCr) alloy post core plus full crown restoration; 4) Fiber post and resin core plus full crown restoration. Load was applied on the buccal inclined surface near the top of the buccal cusp and the intensity and distribution of the maximum tensile stress and von Mises stress in the dentine were compared.

RESULTSWhen the tooth was restored with NiCr alloy post core, an obvious stress concentration area was observed in the dentine next to the top of the post. When the fiber post was used, there was little stress concentration area around the top of the post, and the stress distribution in the dentine was almost the same as the control group. No difference of the intensity and distribution of the stress was found with the control group when no post was used and the perforating cavity was just filled with resin material.

CONCLUSIONCompared with cast metal post, fiber post is more suitable for post-core crown restorations. In cases with sufficient coronal tooth substance, post is not absolutely necessary and just filling the perforating cavity with resin material followed by crown restoration can be one of the alternative restorations.
