ABSTRACT

Mesenteric pseudocyst is rare. This term is used to describe the abdominal cystic mass, without the origin of abdominal organ. We presented a case of mesenteric pseudocyst of the small bowel in a 70-year-old man. Esophago-gastro-duodenoscopy showed a 3.5 cm sized excavated lesion on the posterior wall of angle. Endocopic biopsy confirmed a histologic diagnosis of the poorly differentiated adenocarcinoma, which includes the signet ring cell component. Abdominal computed tomography scan showed a focal mucosal enhancement in the posterior wall of angle of the stomach, a 2.4 cm sized enhancing mass on the distal small bowel loop, without distant metastases or ascites in rectal shelf, and multiple gallbladder stones. The patient underwent subtotal gastrectomy with gastroduodenostomy, segmental resection of the small bowel, and cholecystectomy. The final pathological diagnosis was mesenteric pseudocyst. This is the first case report describing incidentally detected mesenteric pseudocyst of the small bowel in gastric cancer patients.

ABSTRACT

The purpose of this study is to measure the standard volume of the ventricle of normal Koreans in their 20s and 40s and find out difference in the volume of the ventricle according to age and gender. We collected MR brain images from 118 people in their 20s (male 58, female 60)and 100 people in their 40s (male 41, female 59), so a total of 218 people.For each of sagittal, coronal and axial sections, the ventricular part was segmented using automatic and manual methods, and the volume was measured. In order to observe difference according to age and gender and to observe the interactive effect between age and gender, two-way ANOVA was carried out using age (2 levels)and gender (2 levels)as independent variables. The average volume of the ventricle of Koreans in their 20 s was 16.2 [cm3]and that of Koreans in their 40s was 23.1 [cm3].The average volume of the ventricle of male Koreans in their 20s and 40s was 21.9 [cm3]and that of female Koreans was 17.5 [cm3].The volume of the ventricle was significantly larger in Koreans in their 40s than those in their 20s, and in male Koreans than n female ones. Moreover, the increase of the volume of the ventricle with age was significantly larger in male Koreans than in female ones. According to the results of measuring the volume of the ventricle of normal Koreans in their 20s and 40s, the volume was significantly different according to age and gender, and the increase of the volume of the ventricle with age was significantly larger in men than in women.

ABSTRACT

BACKGROUND/AIMS: Immunogene therapy is extensively studied for a therapeutic modality of various cancers. This study was conducted to investigate the efficacy of immunogene therapy using the T-cell costimulatory molecule and human B7-1 (CD80, hB7-1) in an in vivo human hepatocellular carcinoma (HCC) model. METHODS: The stable HCC cell line expressing hB7-1 gene was established using retroviral vector (Huh-7/hB7-1). Of fourteen BALB/c nude mice, 7 were subcutaneously injected with 2 X 10(6) Huh-7/hB7-1 cells, while the other 7 were injected with 2 X 10(6) Huh-7/mock cells as a control group. After the injection, the mice were observed weekly for three months for subcutaneous tumor formation. Assay for natural killer (NK) cell cytotoxicity and serum IFN-gamma was performed at 1 and 2 weeks after inoculation. RESULTS: In BALB/c nude mice inoculated with Huh-7/hB7-1 cells, no tumor growth was observed. BALB/c nude mice inoculated with Huh-7/hB7-1 cells showed significantly increased NK cell activities of splenocytes compared with those with Huh-7/mock cells. Serum IFN-gamma was not measurable at 1 week, but significantly increased at 2 weeks after inoculation to the level of 470 pg/ml in BALB/c nude mice with Huh-7/mock cells and 521 pg/ml in BALB/c nude mice with Huh-7/hB7-1. CONCLUSIONS: Our results demonstrate the in vivo anti-tumor immunity and NK cell activation by transfer of hB7-1 gene into human HCC in xenogeneic BALB/c nude mice model. This approach may provide a tool for the development of immunogene therapies against human malignant tumors.

ABSTRACT

BACKGROUND: Although hypoalbuminemia is common in patients with tsutsugamushi disease, acute rickettsial infectious disease, its impact on the severity of disease is not reported. Therefore, we studied the role of hypoalbumiemia as a marker of the severity of disease in patients with tsutsugamushi disease. METHODS: We retrospectively reviewed clinical data of 95 patients with tsutsugamushi disease who were admitted between January 1994 and December 1999 at Uijongbu St. Mary's hospital. We compared clinical and laboratory findings, complications, and mortality between hypoalbuminemic group (serum albumin or = 3.0 g/dL). RESULTS: Of the total 95 patients, 50 patients (52.6%) had hypoalbuminemia. In hypoalbuminemic group, the incidence of hypotension was higher (20.0% vs 2.2%, p=0.006) and the duration of admission was longer (12.0+/-4.5 vs 8.8+/-2.7 days, p<0.001), compared to control. The degree of thrombocytopenia in hypoalbuminemic group was severer than that in control (84,000+/-46,000 vs 138,000+/-75,000/mm3, p<0.001). Hypoalbuminemic group showed higher incidence of interstitial pneumonia (64.0 vs 13.3%, p<0.001), hypoxia (40.0 vs 6.6%, p<0.001), metabolic acidosis (12.0 vs 0%, p=0.018), and acute renal failure (18.0 vs 4,4%, p=0.038), compared to control. Four patients who had hypoalbuminemia died due to septic shock and multiorgan failure. CONCLUSION: This study shows that hypoalbuminemia could be used as a marker of the severity of disease in patients with tsutsugamushi disease.

Although cimetidine causes a transient rise in serum creatinine without reduction of renal function, acute renal failure due to acute interstitial nephritis is rare in patients after cimetidine treatment. We here present a case of acute renal failure and acute interstitial nephritis that occurred during cimetidine treatment. A 38-year old woman was referred to our hospital because of nausea and general weakness. She had been taking cimetidine for 3 weeks because of epigasric discomfort. On admission, serum creatinine was 3.9 mg/dL and urinalysis showed mild proteinuria and hematuria. There was no history of pyelonephritis, diabetes mellitus, hypertension, toxin exposure. Renal biopsy showed severe interstitial infiltration of lymphocytes without definite glomerular change. After withdrawal of cimetidine, renal function completely recovered.

ABSTRACT

We report a case of ileal perforation associated with Cytomegaloviral enteritis in a 62 years old man who was diagnosed as AIDS lately. The perforated lesian was located at distal ileum approximately 50 cm above ileo-cecal valve. The lesion was simply closed by surgery. There were numerous erythematous patches on the serosal surface of jejunoileum. His gastrofiberscopic examination revealed multiple shallow ulcers in distal esophagus and stomach. Mieroscopic findings of both surgical and endoscopic specimens showed characteristic cytonegalic cells with inclusion bodies. There were numerous cytomegalic endothelial cells found in the ileal specimen without direct evidence of vasculitis. The diagnosis was confirmed by colorimetric detection of cytomegalovirus DNA using in situ hybridization.

ABSTRACT

The major purpose of this study was to assess the post-operative quality of life after performing a pylorus-preserving gastrectomy (PPG) in early gastric cancer patients, comparing the results with patients who underwent a conventional gastrectomy (CG). There were 48 patients with gastric cancer who underwent surgical intervention at the Department of Surgery, Taejon St. Mary's Hospital, The Catholic University of Korea School of Medicine from November 1995 through June 1996. Out of 48 gastric cancer patients, 13 patients (27.1 %) were early gastric cancers. Seven patients of these 13 patients underwent a PPG and the others underwent a CG. The indications for PPG were ; 1) a lesion at least 5 cm proximal to the pyloric ring, 2) a lesion in the middle third of the stomach, 3) a lesion less than 40 mm in maximum length, and 4) especially a lesion located at the greater curvature. Localization of the lesion was precisely performed through a gastrotomy during the operation. Dissections of the No. 7, 8, and 9 nodes (D1 +) were performed in cases of ulcerated lesions (type III) or poorly differentiated lesions. The proximal and distal portions of the stomach were anastomosed together 2-3 cm proximal to the pyloric ring to preserve the pyloric function. Gastrografin studies were performed on the sixth or the seventh day following PPG, and disclosed that the pylori were almost normal in appearance and function. Compared to a CG, the duration of nasogastric decompression was longer by one day, the start of oral feeding was delayed by two days, and the duration of the hospital stay was longer by five days after PPG. Three months following PPG, the amount of food taken in a meal was 70-90 % compared to that of the pre-operative amount, oral feedings took place three to four times a day, and the performance status was excellent (grade 0). Endoscopic examination revealed two cases of food stasis without subjective symptoms and a case of bile reflux with minimal mucosal edema after the PPG. After the CG, three out of the six cases had bile reflux with mucosal redness and erosion. In conclusion, it seemed that for early gastric cancer a pylorus-preserving gastrectomy, with selective lymphnodal dissection, was superior to the standard operation in terms of post-operative quality of life.

ABSTRACT

Gastritis cystica profunda is a rare condition in which mature glandular epithelium extends ino the tissues beneath the muscularis mucosae. It is analogous to similar conditions affecting the small and large intestines that are called, enteritis and colitis cystica profunda. Macroscopically, gastritis cystica profunda may present not only as a submucosal tumor or as solitary or diffuse polyps, but also as a giant gastric mucosal fold rarely These lesion have been found not only at the site of a gaatroenterostony but also in the stomach of patients without previous surgery. We present herein a patient having gastritis cystica profunda without any previous surgery, in which it present as a giant gastric mucosal fold.

ABSTRACT

The Endoscopic Injection Sclerotherapy(EIS) has been widely adopted as a primary treatment for acute hemorrhage from esophageal varices. The long term repeated EIS could achieve esophageal variceal obliteration which has been regarded as the endpoint of sclerotherapy by many authors. Several studies showed that repeated EIS improved the long-term survival, and reduced the bleeding episodes. However, in recent prospective randomized trials, the frequency of bleeding was not reduced and even increasd without improvement of survival in patients undergoing prophylactic sclerotherapy.

ABSTRACT

Endoscopic variceal ligation(EVL) is an accepted alternatives to endoscopic injection sclero- therapy(EIS) in many patients with bleeding esophageal varices. Esophageal hematoma is rare and an unusual complication after EVL or EIS. We present a patient with liver cirrhosis who developed an obstructive esophageal hematoma after EVL.

ABSTRACT

A clinical and hematological observation was performed on 136 newborn infants who were admitted to dept. of Pediatrics of Chosun University Hospital from Junly, 1976 to June, 1980 and were diagnosed of neonatal hyperbilirubinemia. The following results were obtained 1) The incidence of hyperbilirubinemia in male (64.0%) was more than female. 2) The highest monthly incidence was seen in June with 23 cases. (16.9%) 3) In the observation of the gestational period, its incidence was highest in full-term neonate with 103 cases. (79.3%). The mean value of serum bilirubin was highest in postmature neonate (19.70mg%). 4) In the obseervatio of the delivery type, the mean value of serum bilirubin was highest in vacuum and forcep delivery (17.81mg%). 5) The mean value of serum bilirubin was more higher in those of low birth weight group. 6) In the distribution of peak value of serum bilirubin, a group of 10.1~20.0mg% occupied 75.7%. 7) Idiopathic hyperbilirubinemia was the most common cause of pathological hyper bilirubinemia, with 79 cases. (58.1%). In the group of blood incompathibily, visible jaundice was begun earliest and mean value of serum bilirubin was highest. 8) Sepsis among the infection was the most important factor in the cause of neonatal hyper bilirubinemia with 18 cases (51.4%). 9) The most common blood types of mother-baby in ABO blood incompatibility was O-B with 8 cases (61.5%). 10) The exchange transfusions were performed on 13 cases (9.5%) and the rate of performance was the highest in blood incompatibility.

We experienced one suspected case of chondrodystrophic calcificans congenita of 2 days aged newborn infant. The patient manifested short stature, articular contracture on both lower and upper extremities, moderate jaundice and irritabilities. A brief review of related literature is also presented.
