ABSTRACT
The objective of the present study was to determine the relationship between nitric oxide synthases (NOS) and heart failure in cardiac tissue from patients with and without cardiac decompensation. Right atrial tissue was excised from patients with coronary artery disease (CAD) and left ventricular ejection fraction (LVEF) <35 percent (N = 10), and from patients with CAD and LVEF >60 percent (N = 10) during cardiac surgery. NOS activity was measured by the conversion of L-[H ]-arginine to L-[H ]-citrulline. Gene expression was quantified by the competitive reverse transcription-polymerase chain reaction. Both endothelial NOS (eNOS) activity and expression were significantly reduced in failing hearts compared to non-failing hearts: 0.36 ± 0.18 vs 1.51 ± 0.31 pmol mg-1 min-1 (P < 0.0001) and 0.37 ± 0.08 vs 0.78 ± 0.09 relative cDNA absorbance at 320 nm (P < 0.0001), respectively. In contrast, inducible NOS (iNOS) activity and expression were significantly higher in failing hearts than in non-failing hearts: 4.00 ± 0.90 vs 1.54 ± 0.65 pmol mg-1 min-1 (P < 0.0001) and 2.19 ± 0.27 vs 1.43 ± 0.13 cDNA absorbance at 320 nm (P < 0.0001), respectively. We conclude that heart failure down-regulates both eNOS activity and expression in cardiac tissue from patients with LVEF <35 percent. In contrast, iNOS activity and expression are increased in failing hearts and may represent an alternative mechanism for nitric oxide production in heart failure due to ischemic disease.

As dissecçöes agudas da aorta representam uma entidade clínica não rara e de extrema gravidade. O diagnóstico precoce pode ser realizado e, para tanto, é necessário alto grau de suspeita clínica, que irá direcionar os exames a serem realizados com presteza, levando sempre em consideração a experiência da instituição com diversos exames complementares. Recomenda-se a utilização do método diagnóstico de maior acurácia e que proporcione menor estresse ao paciente. Assim que for levantada suspeita clínica de dissecção da aorta, deve ser instituída terapêutica clínica. Com a confirmação diagnóstica de diassecção envolvendo a aorta ascendente, é mandatório o tratamento cirúrgico imediato. Nos casos de dissecção da aorta descendente, o tratamento clínico é o de escolha, a menos que haja indícios de complicaçöes decorrentes da dissecção, Recentemente, novas perspectivas terapêuticas têm sido desenvolvidas e testadas com resultados bastante promissores, entre as quais destacam-se a técnica cirúrgica a tromba de elefante e o tratamento com prótese endovascular ("stent"), pela via percutânea, para correçäo das dissecçöes da aorta descendente.

Background. The recent study of the variations of the origin of the sinoatrial node and on the "arterial network of the perinodal sinusal area" in normal hearts points out the importance of this network. Purpose. Report on a case of patient with syncope of ischemic etiology. Conclusion. In this patient the arterial network did not protect the node from the ischemia caused by the obstruction of the artery of the sinoatrial node.

OBJETIVO - Avaliar as alteraçöes de hemostasia encontradas em crianças submentidas a cirurgia cardíaca com circulaçäo extracopórea (CEC). MÉTODOS - Estudamos 17 crianças no pré e pós-operatório (PO) imediato, no 1§ PO e entre o 4§ e 7§ PO, analisando o hematócrito, tempo de protrombina (TP), tempo de tromboplastina parcial ativado, fibrinogênio, contagem de plaquetas, fator V e tempo de lise de euglobulina (TLE). Os pacientes foram divididos em grupos com e sem sangramento execessivo no PO. RESULTADOS - Houve alteraçöes significantes no intraoperatório com aumento do TP e tempo de tromboplastina parcial ativado e reduçäo do fibrinogênio, fator V e do TLE. Seis (35 por cento) crianças sangraram execessivamente. A contagem de plaquetas foi significantemente menor no intra-operatório; neste grupo o tempo de CEC foi menor. CONCLUSÄO - Alteraçöes de hemostasia no intra e PO ocorrem como conseqüência da ativaçÝo da cascata de coagulaçäo e fibrinólise. A incidência de sangramento execessivo é alta nesta faixa etária. Os grupos com e sem sangramento execessivo diferenciam-se pelo maior tempo de CEC e contagem reduzida de plaquetas.

OBJETIVO - Avaliar o valor do ecocardiograma Doppler (ECO) transtorácico na identificação de perviabilidade da anastomose entre artéria torácica interna esquerda (ATIE) e interventricular anterior, realizada pela técnica de revascularização miocárdica pela minitoracotomia sem circulação extracorpórea. MÉTODOS - Estudaram-se os primeiros 12 pacientes, consecutivos, no período de pós-operatório intra-hospitalar pelo ECO, utilizando-se transdutores de 5 MHz, pela via paraesternal esquerda, preferencial. Foram analisadas velocidades máximas e integrais de velocidade dos componentes sistólico e diastólico das curvas espectrais de fluxo Doppler. Todos pacientes foram submetidos à cinecoronariografia, enquanto hospitalizados. RESULTADOS - O ECO foi exeqüível em 93 'por cento' dos pacientes. Nos com anastomose pérvia (6/7), observou-se ao estudo do Doppler amplo componente diastólico (padrão A). Naqueles com anastomose obstruída (4/4) o padrão observado foi de predomínio sistólico (padrão B) (p=0,003*). CONCLUSÄO - O ECO da ATIE anastomosada com a artéria interventricular anterior, após cirurgia de revascularização miocárdica pela técncia de minitoracotomia, permitiu caracterizar precocemente, com recisão, a pervibilidade da anastomose.

Uma das grandes barreiras a serem vencidas em vários programas de transplantes é o diagnóstico e controle da rejeiçäo. Apesar de todo progresso nesse campo, a biópsia endomiocárdica continua sendo ainda a técnica mais objetiva e confiável para detecçäo e monitorizaçäo de rejeiçäo no transplante cardíaco. Por tratar-se de método invasivo com alguns inconvenientes, continuam a ser pesquisadas técnicas alternativas menos agressivas que possam dar diagnóstico precoce. Os autores fazem uma revisäo sobres aspectos clínicos, radiológicos, eletrocardiográficos e ecocardiográficos, além de mapeamento com radionuclídeos, ressonância nuclear magnética, testes bioquímicos na urina e monitorizaçäo cito-imunológica do sangue periférico. Cumpre lembrar que após o advento da ciclosporina A, tornou-se mais difícil a detecçäo da rejeiçäo precoce e esses métodos näo-invasivos, apesar de ajudarem, perderam relvância, tendo ainda a biópsia papel fundamental.

The utilization of thrombolytic agents during the first hours of acute myocardial infarct has been accepted as one of the main methods to limit infarct extension. However, persistence of residual stenosis requires a complementary revascularization method. Twenty-five patients with a diagnosis of acute myocardial infarct were successfully treated within six hours from onset of symptioms with intravenous streptokinase, followed 1 to 21 days (mean of 8 days) afterwards by surgical revascularization of the myocardium without extracorporeal circulation. The mean age of the group was 53.8 years and the mean ejection fraction 60 per cent; 10 patients were uniarterial, 14 biarterial and 1 triarterial; in 15 patients the infarct was located at the anterior wall and in 10 at the posterior wall. Eight patients received one bypass and 17, two. Our nosocomial mortality was 0 per cent and in none was there a need for reoperation due to bleeding. Surgical revascularization of the myocardium withou using extracorporeal circulation is thus a method that can be used in some patients with acute myocardial infarct with excellent results regarding morbidity and nosocomial mortality

The first anterior septal arteries from 41 cadavers were studied. By the idealized classification they could be divided into 3 main types according to the path in the interventricular septum: Type I - 25 (60.98 per cent); Type II - 7(17.07 per cent); Type III - 9 (21.95 per cent). Several other parameters were studied regarding the 3 types: distance between the ostium and the bifurcation of the left coronary artery trnk; realtion with the second anterior septal artery; number of anterior septal arteries per heart; the greatest caliber of the anterior septal artery; relation of diameter with that of the anterior descending artery and angle of emergence at the anterior descending artery. These parameters were analyzed in view of a better knowledge about this coronary branch for surgery and clinical practice
