ABSTRACT

ObjectiveTo study the chemical structure of gardenia blue pigment and its inhibitory activity against monoamine oxidase B (MAO-B), in order to seek a potential feasible way for rational utilization and value enhancement of iridoids in Gardeniae Fructus. MethodIridoid glycosides in Gardeniae Fructus were hydrolyzed by cellulase to obtain their aglycones and reacted with amino acids. Then, the products were purified by column chromatography packed with D101 macroporous resin and preparative liquid chromatography to obtain gardenia blue pigments, and the gardenia blue pigments were identified by nuclear magnetic resonance (NMR) and mass spectrometry (MS). Benzylamine was used as the reaction substrate of MAO-B and in vitro incubated with gardenia blue pigment monomers, high performance liquid chromatography (HPLC) was employed to determine the production of benzaldehyde for evaluating the inhibitory effect of gardenia blue pigments on MAO-B, the mobile phase was methanol (A) -50 mmol·L-1 potassium phosphate buffer (B, pH 3.2) (2∶3), and the detection wavelength was 245 nm. ResultEight compounds of gardenia blue pigment A-H were synthesized and identified. In MAO-B inhibition test, compared with geniposide, the inhibitory activity of gardenia blue pigment D and E was significantly enhanced (P<0.05). Compared with the 6β-hydroxygeniposide, the inhibitory activity of gardenia blue pigment G and H was significantly enhanced (P<0.05, P<0.01). All the four gardenia blue pigments showed better MAO-B inhibitory activity than the prototype compounds. ConclusionGardenia blue pigment is a simple compound formed by one molecule of amino acid and one molecule of iridoid. Some gardenia blue pigments have better MAO-B inhibitory activity than the prototype compounds. The activity of gardenia blue pigment produced by different substrates is different, and the high-value gardenia blue pigment can be prepared based on experimental optimization, which can expand the application range of gardenia blue pigment and enrich the comprehensive utilization of iridoids from Gardeniae Fructus.

ABSTRACT

OBJECTIVES@#To study the clinical application effect of "kindergarten effect" in radiotherapy for children with tumor based on the psychology of preschool children aged 3-5 years.@*METHODS@#A total of 30 children, aged 3-5 years, who were admitted to the Department of Radiotherapy, Tianjin Medical University Cancer Institute and Hospital, from January 2020 to August 2021 were enrolled in this prospective study. The children were randomly divided into a control group and a test group, with 15 children in each group. The children in the test group were treated in "kindergarten mode", i.e., all children were treated together at a specified time and left together after all children completed treatment. Those in the control group were treated alternately with adult patients according to the treatment time based on the type of radiotherapy fixation device. The treatment compliance was evaluated for both groups, and the two groups were compared in terms of the setup errors in the superior-inferior (SI), left-right (LR), and anterior-posterior (AP) directions.@*RESULTS@#Compared with the control group, the test group showed a significantly shorter time for finishing the treatment (P<0.05) and a significantly lower proportion of children with treatment interruption (P<0.05). Compared with the control group, the test group showed smaller mean errors in the SI, LR and AP directions after image-guided radiotherapy, with significant differences in the mean errors in the SI and LR directions (P<0.05).@*CONCLUSIONS@#With the application of the "kindergarten effect", most children can actively cooperate in radiotherapy, and it can also improve the accuracy and repeatability of positioning and help to achieve the desired treatment outcome.

ABSTRACT

@#AIM:To observe curative effect and safety of slanted bilateral lateral rectus recession for convergence insufficiency-type intermittent exotropia.<p>METHODS:This study included 29 patients who underwent slanted bilateral lateral rectus recession for convergence insufficiency-type intermittent exotropia in our hospital between October 2017 and November 2019 with a follow-up of 6mo, the deviation, the surgical success rate, the binocular vision function and the complications were observed.<p>RESULTS:In our study, the mean near deviations, the mean distance deviations and the near-distance deviation differences reduced from -41.72±3.35PD, -23.28±9.75PD and 16.90±2.47PD before surgery to -5.97±4.85PD, -2.66±4.78PD, 3.28±1.10PD 6mo after surgery, the surgical success rate was 76%. The Grades I and Ⅱ binocular vision function improved 6mo after surgery(<i>P</i><0.05), the distance stereopsis and the near stereopsis were not statistically significant(<i>P</i>>0.05). None of the patients developed A-V pattern, limitation of eye movement, restrictive strabismus, vertical strabismus, and rotated diplopia, some patients had transient horizontal diplopia, which disappeared within 2-3wk after surgery.<p>CONCLUSION:Slanted bilateral lateral rectus recession may successfully reduce the near exodeviations, the distance exodeviations and the near-distance deviations difference without obvious complications, proved to be a safe and effective procedure for the treatment of convergence insufficiency-type intermittent exotropia.

ABSTRACT

UDP glucosyltransferase (UDPGT) catalyzes the synthesis of secondary metabolites and plant hormones to regulate plant growth and development, pathogen defense and environmental adaptability. In this study 18 members of the RcUDPGT gene family were cloned from Tibetan Rhodiola crenulata and analyzed using bioinformatics. The tissue-specific expression, abiotic stresses and plant hormones (abscisic acid, auxin, methyl jasmonate) induced expression patterns were identified by real-time quantitative PCR. The bait vector of RcUDPGT (JX228125.1) was constructed to select interacting proteins from an Arabidopsis yeast library. The results of the bioinformatics analysis revealed that RcUDPGT nucleotide sequences were about 1 400 bp and encoded 452-498 amino acids. In the primary protein sequences, C-terminal sequences were more conserved compared with N-terminal regions, which held a PSPG (plant secondary product glycosyltransferase) domain. In the tertiary structures, RcUDPGTs contained a UDP sugar donor recognition binding site. In addition, all genes had multiple phosphorylation sites. The results of qRT-PCR showed that RcUDPGTs genes were expressed in root, stem and leaf. The expression levels were regulated by low temperature/ultraviolet light and various plant hormones (ABA, IAA, MeJA), but the expression patterns were quite different among them. For example, RcUDPGT6, RcUDPGT11, and RcUDPGT17 had the highest expression in leaves and were induced by all three hormones, suggesting that the functions of these genes might be to respond to environmental changes. RcUDPGT9, RcUDPGT10, RcUDPGT14 were most abundantly expressed in roots and were significantly induced by ABA and MeJA hormones, indicating that these genes may be involved in the synthesis and accumulation of salidroside. Yeast two-hybrid results showed that RcUDPGT did not exhibit autoactivation and cell toxicity, and two significant interactional genes were identified, AtKCR1 (AT1G67730.1) and AtSNL4 (AT1G70060). The AtKCR1 gene encodes a β-ketoacyl reductase (KCR) involved in synthesis of very long chain fatty acids. The AtSNL4 gene encodes a homolog of the transcriptional repressor SIN3, which could participate in the ABA hormone signaling pathway and enhance the transcriptional repression of AP2/EREBP class factors in Arabidopsis. These results suggest that the accumulation of the secondary metabolite salidroside in Rhodiola crenulata might be affected by several regulatory mechanisms. The above results may lay the foundation for understanding the adaptive mechanism of Rhodiola crenulata in a high altitude environment and stimulate an in-depth study of the synthesis and accumulation of secondary metabolites in this species.

ABSTRACT

A common pathological features of Alzheimer's disease (AD) is the excessive activation of inositol-requiring enzyme-1α (IRE1α) downstream of endoplasmic reticulum stress (ERS) and the abnormal expression of microRNA (miRNA) with a variety of cellular physiological functions. Studies have found that IRE1α and miRNA are not alone in the AD pathologenesis. IRE1α can regulate the expression of miR-200, miR-7, miR-17 and miR-34, and participate in most AD brain diseases such as Aβ deposition, Tau protein hyperphosphorylation and neuronal apoptosis. It is suggested that IRE1α-miRNA signal abnormality is one of the pathological mechanisms of AD. Exercise can prevent and delay AD, but its mechanism is unclear. Studies have found that exercise could interfere with AD by regulating the IRE1αmiRNA signaling pathway. The specific mechanisms of action include: (1) Exercise improves the adaptation of AD brain energy metabolism and alleviates the excessive activation of brain IRE1α signals. (2) Exercise regulates the expression of miRNA in the brain, exerts epigenetic effects, and reduces pathologies such as Aβ and Tau protein aggregation. (3) The IRE1α-miRNA pathway and its downstream protein changes can mediate exercise to resist the development of AD. This article will review the relationship between IRE1α-miRNA and AD pathology and its exercise feedback mechanism, aiming to provide evidence and ideas for AD diagnosis and treatment strategies.

ABSTRACT

OBJECTIVE@#To review the therapeutic effect of acupuncture and moxibustion on allergic rhinitis based on the network Meta-analysis.@*METHODS@#The randomized controlled trials of acupuncture and moxibustion for allergic rhinitis were retrieved from the databases, starting from the date of establishment to August 17, 2020, i.e. the PubMed, EMbase, Cochrane Library, CNKI, Wanfang and VIP. The traditional Meta-analysis and network Meta-analysis were performed by RevMan5.3 and GeMTC0.14.3.@*RESULTS@#A total of 50 RCTs were included, including 4260 patients, involving 5 kinds of acupuncture and moxibustion therapies, such as acupuncture, moxibustion, acupoint application, acupoint thread-embedding and auricular point therapy.①In term of total effective rate, acupuncture, moxibustion and acupoint thread-embedding were superior to western medication and auricular point therapy (@*CONCLUSION@#The therapeutic effect of acupuncture and moxibustion on allergic rhinitis is better than western medication, and acupoint thread-embedding has the best curative effect.

ABSTRACT

Objective@#: To generate synthetic spine magnetic resonance (MR) images from spine computed tomography (CT) using generative adversarial networks (GANs), as well as to determine the similarities between synthesized and real MR images. @*Methods@#: GANs were trained to transform spine CT image slices into spine magnetic resonance T2 weighted (MRT2) axial image slices by combining adversarial loss and voxel-wise loss. Experiments were performed using 280 pairs of lumbar spine CT scans and MRT2 images. The MRT2 images were then synthesized from 15 other spine CT scans. To evaluate whether the synthetic MR images were realistic, two radiologists, two spine surgeons, and two residents blindly classified the real and synthetic MRT2 images. Two experienced radiologists then evaluated the similarities between subdivisions of the real and synthetic MRT2 images. Quantitative analysis of the synthetic MRT2 images was performed using the mean absolute error (MAE) and peak signal-to-noise ratio (PSNR). @*Results@#: The mean overall similarity of the synthetic MRT2 images evaluated by radiologists was 80.2%. In the blind classification of the real MRT2 images, the failure rate ranged from 0% to 40%. The MAE value of each image ranged from 13.75 to 34.24 pixels (mean, 21.19 pixels), and the PSNR of each image ranged from 61.96 to 68.16 dB (mean, 64.92 dB). @*Conclusion@#: This was the first study to apply GANs to synthesize spine MR images from CT images. Despite the small dataset of 280 pairs, the synthetic MR images were relatively well implemented. Synthesis of medical images using GANs is a new paradigm of artificial intelligence application in medical imaging. We expect that synthesis of MR images from spine CT images using GANs will improve the diagnostic usefulness of CT. To better inform the clinical applications of this technique, further studies are needed involving a large dataset, a variety of pathologies, and other MR sequence of the lumbar spine.

ABSTRACT

Objective@#To investigate the influence of the Rho/ROCK signaling pathway on the anti-cryodamage ability of human sperm and provide some theoretical evidence for the development of high-efficiency semen cryoprotectants.@*METHODS@#We collected semen samples from 25 healthy males, each divided into a fresh, a normal cryopreservation control and an Rho-inhibition group. Before and after freezing, we detected sperm motility, viability, membrane integrity, morphology, DNA fragmentation index (DFI), acrosomal enzyme activity (AEA) and mitochondrial membrane potential (MMP) and determined the expressions of RhoA and ROCK proteins in the sperm by immunofluorescence staining.@*RESULTS@#Compared with the normal cryopreservation control, the frozen-thawed sperm of the Rho-inhibition group showed significantly increased sperm motility ( [51.20 ± 7.70]% vs [57.50 ± 6.83]%, P = 0.002), survival rate ( [52.87 ± 5.07]% vs [60.24 ± 5.53]%, P = 0.001), membrane integrity ([59.78±5.56]% vs [67.10 ± 4.43]%, P = 0.001), percentage of morphologically normal sperm ([4.83 ± 1.11]% vs [7.46 ± 1.28], P = 0.001) and MMP (56.30 ± 4.28 vs 63.11 ± 2.97, P = 0.001), but decreased DFI ([27.64 ± 6.64]% vs [18.87 ± 4.07]%, P = 0.001). There was no statistically significant difference in the AEA of the frozen-thawed sperm between the control and Rho-inhibition groups (97.65 ± 9.31 vs 98.30 ± 11.33, P > 0.05). Immunofluorescence staining revealed extensive expressions of RhoA and ROCK proteins in the head and neck of the sperm.@*CONCLUSIONS@#The Rho/ROCK signaling pathway plays a role in the cryodamage to human sperm, and inhibiting the activity of Rho/ROCK can significantly improve the ability of sperm to resist cryodamage.

ABSTRACT

@#AIM: To explore the application of anterior transposition of inferior oblique muscle with unequal excision in treatment of segregated vertical strabismus with asymmetric inferior oblique hyperfunction.<p>METHODS: A total of 22 patients(10 males and 12 females, 28 eyes in all)who underwent anterior transposition of the unequal excisional inferior oblique muscle for dissociated vertical deviation with asymmetric inferior oblique muscle overaction were studied retrospectively. The patients were admitted from June 2015 to June 2017,with mean age at 12.32±6.81 years old(ranging from 3 to 28 years old). The binocular vision, the curative effect of DVD, the curative effect of inferior oblique muscle overaction, eye position and compensatory head posture, complications were examined postoperatively, follow-up survey was conducted at 6 to 18mo, meanly at 10.05±3.87mo.<p>RESULTS: Recovery of binocular vision: 11 cases did not have binocular vision(50%); 6 cases recovered to the first level of binocular vision(27%); 3 cases recovered to the second level of binocular vision(14%); 2 cases recovered to the third level of binocular vision(9%). The curative effect of DVD shows: 17 cases with satisfaction(77%), 5 cases with improvement(23%), and 0 case with no avail. The curative effect of inferior oblique muscle overaction shows: 13 cases with cured(59%), 9 cases with improvement(41%), and 0 case with no avail. Postoperative eye position and compensatory head posture: 1)Horizontal eye position: 1 case of under correction after exotropia correction surgery, 1 case of under correction after esotropia correction surgery, 0 case of overcorrection, 20 cases of correction; 2)Vertical eye position: 13 cases show vertical strabism deviation less than 5△, 9 cases show vertical strabism deviation between 5△ to 10△ meanly at 5.55△±2.35△, vertical eye position without overcorrection; 3)Compensatory head posture: 6 cases were disappeared, 1 case was improved. Complications: Mild anti-elevation syndromeoccurred in 1 case, narrowed palpebral fissueoccurred in 1 case, and narrowed quantity less than 1mm.<p>CONCLUSION: In general, the curative effect of anterior transposition of the unequal excisional inferior oblique muscle for dissociated vertical deviation with asymmetric inferior oblique muscle overactionis satisfactory, standard preoperative examination, individualized surgical design and surgical techniques are necessary.

@#AIM: To investigate the compliance and the relative influencing factors of orthokeratology lens in the treatment of teenagers' and children's myopia in Dongguan,and provide a scientific basic for prevention and control of myopia.<p>METHODS: Totally 757 patients of teenagers' and children's myopia with orthokeratology lens in our hospital from January 2017 to December 2017 were collected. Data from medical records and return visits by telephone, short message and Wechat were collated and summarized, then grasp the compliance and the relative influencing factors of orthokeratology lens in the treatment of teenagers' and children's myopia in Dongguan by analyzing re-examine time and persistence in wearing lens.<p>RESULTS: Totally 757 patients with mean age at 11.82±2.234 years old, including 363 males, mean aged 11.66±2.256 years old, and 394 females, mean aged 11.96±2.206 years old, there was no significant difference in age comparison of boys and girls(<i>t</i>=-1.855, <i>P</i>>0.05). In terms of re-examination compliance and persistence on wearing lens, significant difference was observed in educational stage, studying way and parents' education level(<i>P</i><0.05), no significant difference was observed in gender, area, family economic status, vision before and after wearing lens and myopia control effect(<i>P</i>>0.05). Multivariate Logistic regression analysis showed that educational stage, studying way and parents' education level were correlated with re-examination compliance and persistence on wearing lens(<i>P</i><0.05).<p>CONCLUSION: Compliance of orthokeratology lens in the treatment of teenagers' and children's myopia in Dongguan was correlated with educational stage, studying way and parents' education level, to improve the compliance of orthokeratology lens in the treatment of teenagers' and children's myopia, Standardize fitting, standardize care, normative propaganda and education, review on time and regular return visits were important.

ABSTRACT

SUMMARY OBJECTIVES To evaluate the epidemiological characteristics of acute pancreatitis (AP) and explore potential relationships between these factors and severity. METHODOLOGY Data-sets of 5,659 patients with AP from health statistics and the Information Center of Jiangsu province, between 2014 and 2016, were analyzed. A self-organizing map (SOM) neural network was used for data clustering. RESULTS Biliary acute pancreatitis (BAP) (86.7%) was the most frequent etiological factor. A total of 804 (14.2%) patients had severe acute pancreatitis (SAP). The mean age of patients was 53.7 + 17.3 (range 12~94y). Most of the AP patients were married (75.4%); 6% of mild /moderately severe AP (MAP/MASP) patients were unmarried, which was less than SAP patients (P=0.016). AP patients with blood type AB in the general population (8.8%) was significantly lower than that of AP cases (13.9%) (P=0.019) and SAP cases(18.7%) (P=0.007). The number of AP patients in southern Jiangsu was much higher than that in northern Jiangsu province, especially in Nanjing (1229, 21.7%). The proportion of acute alcoholic pancreatitis (AAP) in the north of Jiangsu (Xuzhou 18.4%) was much higher than that in southern Jiangsu (Suzhou 2.6%). The whole sample was divided into five classes by SOM neural network. If BAP patients were male, old, divorced, and blood type AB or B, they were more likely to develop SAP. Middle-age, unmarried or divorced male patients with blood type B/AB who suffered from HAP or AAP were also more likely to develop SAP. CONCLUSIONS The number of unmarried patients with MAP/MASP was smaller than that of SAP. Blood types AB and B were more frequent in AP, especially in SAP. The differences between southern Jiangsu and northern Jiangsu, in number of AP patients and the proportion of AAP, were significant. In class I and class IV, the ratio of SAP was much higher than in other classes and the whole sample.

ABSTRACT

Objective To establish and evaluate a rat model of inhalation lung injury induced by ship smog.Methods A rat model of inhalation lung injury was established by analyzing the composition of ship materials after combustion.Fortytwo healthy male Wistar rats were randomly divided into normal control group and 2,6,12,24,48 and 72h groups (6 each)after inhalation,these rats were killed at each time point,and the changes of arterial blood gas,coagulation function,the lung water content (%) were detected.Macroscopic and microscopic changes in lung tissues were observed to judge the degree of lung injury.Results The main components after combustion of 7 kinds of nonmetal materials on ship included CO,CO2,H2S,NOx and other harmful gases in this study,AIKE in one gas detector was used to monitor O2,CO,CO2 and H2S,and their concentrations remained relatively stable within 15 minutes,and the injury time was 15 minutes.The rats presented with shortness of breath and mouth breathing.Smoke inhalation caused a significant hypoxemia,the concentration of blood COHb reached a peak value 2h and the lung water content (%) did 6h after inhalation (P<0.05).It is metabolic acidosis in the early stage after inhalation,but metabolic acidosis combined with respiratory acidosis in the later period.Histopathological observation showed diffuse hemorrhage,edema and inflammatory cell infiltration in the lung tissue as manifestations of lung injury,and the injury did not recover at 72h after inhalation,the change of blood coagulation function was not statistically significant.Conclusion A rat model of inhalation lung injury induced by ship smog has been successfully established,and has the advantages of easy replication,stability and reliability,thus can be used to research and treat inhalation lung injury induced by ship smog in naval war environment and other cases.

Objective To analyze the fingerprints of serum peptides in bloodstream infection induced by Candida albicans (C.albicans),with matrix assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS),and establish the corresponding diagnostic model Methods To establish ICR mice model of C.albicans bloodstream infection,and collect the serum samples which were purified by weak cation exchange beads.The serum peptide finger printing was recognized by MALDITOF MS,and BioExplorer software was used for analysis between infection group and normal control group.Furthermore,the peptide fingerprints were analyzed between C.albicans infection group,Escherichia coli (E.coli) infection group and Staphylococcus aureus (S.aureus) infection group.Results Comparison between C.albicans infection group and normal control group,there were 135 polypeptide peaks,of which 5 polypeptides up-regulated,6 down-regulated and 7 up-regulated first and down-regulated afterwards.The diagnostic model was established by combination of 6 peaks (i.e.m/z 1610.9,1742.3,2666.4,2778.0,3345.1 and 4528.8),which possessed an accurate rate of 80.0% in diagnosis of C.albicans infection.It was found by comprehensive comparison between C.albicans,E.coli and S.aureus infection groups that there were 5 polypeptides expressed collectively,i.e.m/z1513.8,2910.8,3538.1,3884.9 and 5007.3.Conclusions MALDI-TOF MS can be used to distinguish the C.albicans infection and normal polypeptide peaks.The collective polypeptides expressed in the infection groups can be further used to diagnose the infection.Establishment of corresponding diagnostic models may be helpful for early diagnosis of fungal bloodstream infection and provide a basis for reasonable clinical medication.

Objective To establish a clinical method for early detection of the bloodstream infection bacteria based on matrix-assisted laser ionization time of flight mass spectrometry (MALDI-TOF MS).Methods After consulting the Laboratory Information System statistics and domestic related literature,We chose 10 kinds of bacteria (Escherichia coli.,Pseudomonas aeruginosa,Acinetobacter baumannii,Klebsiella pneumoniae,Enterococcus faecium,Enterococcus faecalis,Staphylococcus aureus and Staphylococcus epidermidis,Proteus mirabilis,Streptococcus pneumoniae) as target.The MALDI-TOF MS was established using simulated bacterial infection blood samples.From March to May 2017,33 blood samples of suspected sepsis patients from Emergency Department,General Hospital of PLA,were tested.Results The MALDI-TOF MS whose detecting sensitivity was 100CFU/ml had the same negative detection rate with the blood culture (27cases/27cases,100%).Besides the 2 samples of Morganella morganii infection and Staphylococcus hominis infection were out of the range,the results of the remaining 4 positive samples were consistent (100%).Conclusion Compared to the blood culture and biochemical identification,MALDI-TOF MS can rapidly detect 10 kinds of bloodstream infection bacteria with high sensitivity and high accuracy.

ABSTRACT

Three features of ethics review in Chinese medicine (CM) and integrative medicine (IM) were put forward in this paper. It is consistent with the principles of ethical review in Western medicine; it has to be compliant with the laws of CM and IM; emphasis should be laid on the review of clinical practice facts and experience. Three problems were pointed out. The characteristics of CM and IM are not distinctive enough, operation procedures need to be refined and effectiveness remains to be improved. Based on the mentioned above, seven measures were proposed to improve the level and quality of ethics review in CM and IM, including better brand awareness, considerable tolerance, treatment based on disease differentiation and syndrome differentiation, scientific review and toxicity and side effects of CM, perfection of the ethics review system, reasonable procedures of ethics review and more specialized ethics review workers.

ABSTRACT

@#Oral lichen planus (OLP), a common and chronic disorder, has no effective treatment or cure because its etiology is complicated and has until now remained unclear. Therefore, effectively controlling the activity of the disease and preventing its recurrence are two primary purposes of clinical treatment for OLP. The available evidence supports the notion that topical steroids are the preferred drug for treating this disease. However, some cases have poor clinical curative responses to steroids or other medicines, mainly because of drug resistance and other factors. Therefore, scholars have explored new drugs and methods with the aim of achieving more effective and safe treatment effects. The current research evidence indicates that glucocorticoid remains the front-line drug for OLP treatment. Topical tacrolimus, pimecrolimus and retinoic acid combined with glucocorticoid can be used as alternative therapies, especially in cases with glucocorticoid drug resistance. The systemic application of glucocorticoid or immunosuppressive agents, such as methotrexate, mycophenolate mofetil, and sulfoxide, should be limited to extensive, refractory cases or those with lesions involving regions outside the oral cavity. Surgery and cryotherapy can be applied in a specific, limited range of lesions. Some previous randomized controlled clinical studies have had limitations, including a small sample size, short treatment and clinical follow-up periods, a lack of reliable questionnaires and differences in evaluation standards. Therefore, future studies should include the following: randomized controlled clinical studies with large sample sizes that offer reliable evidence of a curative effect; assessments of treatment outcomes should be extended as long as possible; the follow-up period should be long enough to assess the recurrence rate; and the methods and standards used to evaluate treatment effects should be standardized.

BACKGROUND: Data on the mechanical ventilation (MV) characteristics and radiologic features for the cases with H7N9-induced ARDS were stil lacking. METHODS: We describe the MV characteristics and radiologic features of adult patients with ARDS due to microbiologically confirmed H7N9 admitted to our ICU over a 3-month period. RESULTS: Eight patients (mean age 57.38±16.75; 5 male) were diagnosed with H7N9 in the first quarter of 2014. All developed respiratory failure complicated by acute respiratory distress syndrome (ARDS), which required MV in ICU. The baseline APACHE II and SOFA score was 11.77±6.32 and 7.71±3.12. The overall CT scores of the patients was 247.68±34.28 and the range of CT scores was 196.3–294.7. The average MV days was 14.63±6.14, and 4 patients required additional rescue therapies for refractory hypoxemia. Despite these measures, 3 patients died. CONCLUSION: In H7N9-infected patients with ARDS, low tidal volume strategy was the conventional mode. RM as one of rescue therapies to refractory hypoxemia in these patients with serious architectural distortion and high CT scores, which could cause further lung damage, may induce bad outcomes and requires serious consideration. Prone ventilation may improve mortality, and should be performed at the early stage of the disease, not as a rescue therapy.

There is high demand for a sensitive method for miRNA detection in clinical diagnosis. In this work, we developed a method for miRNA detection based on the surface plasmon-enhanced energy transfer ( SPEET ) between gold nanoparticles ( AuNPs ) and silver nanoclusters ( AgNCs ) , coupled with DNA polymerase and nicking enzyme-assisted isothermal amplification for target recycling. Two DNA probes ( Probe a and Probe b) were assembled onto the surface of AuNPs to form Probe b-Probe a-AuNP conjugates. Probe a consisted three domains:the complementary sequence of miRNA, the specific site of the nicking enzyme, and the self-assembly sequence for AgNCs. The 3′ end of Probe a was modified with thiol as a binding site for AuNPs. The SPEET of AgNCs and AuNPs was inhibited when miRNA was added to produce the dumbbell shaped template by polymerase. The template could promote synthesis of AgNCs, resulting in replacement and subsequently recycling of the target molecule for signal amplification. In comparison with the traditional method of miRNA detection with commercial RT-PCR kits, this method avoided the process of reverse transcription and was easy to perform. In addition, this method with a detection limit of 2. 5×10-11 mol/L was cost-effective, label-free, and highly selective for detecting miRNA, and could be applied to the analysis of miRNA in biological samples.

ABSTRACT

Objective To investigate the secondary metabolites from diethyl sulfate(DES)mutant 3d10-01 derived from Peni-cillium chrysogenum S-3-25. Methods The secondary metabolites were isolated by multiple separation techniques,such as silica gel,Sephadex LH-20 column chromatography and HPLC. The structures of the compounds were determined by ESI-MS and NMR spectroscopic data. The antitumor activity was assayed by the MTT method. HPLC-UV analysis was used to determine if compounds 1-7 were newly produced by the mutant. Results Seven secondary metabolites,including methyl 2,4-dihydroxy-3,5,6-trimethylbenzo-ate(1),6-hydroxymethyl-2,2-dimethylchromanone(2),regiolone(3),(3S,4S)-3,4-dihydro-3,4,8-trihydroxy(2H)naphthalenone (4),cerebroside C(5),cerebroside D(6)and dankasterone A(7),were isolated from the fermentation products of the mutant 3d10-01. Compounds 1,4 and 7 showed strong inhibitory effects on the five tested cell lines,but 2,3 and 6 only exhibited the proliferation of HL-60 cells to some extent. Compounds 1-4 and 7 were newly produced by the mutant 3d10-01. Conclusion Compounds 1-7 are firstly isolated from P. chrysogenum. The inhibitory effects on certain tested antitumor cell lines of compounds 1-4,6 and 7 are report-ed for the first time.

ABSTRACT

Objective To investigate the expression and variation of MIP 1β,MIP-2,and IL-12p70 in mice with bloodstream infection caused by 4 kinds of bacteria.Methods CD-1 (ICR) mouse models of bloodstream infection with Staphylococcus aureus (S.aureus),Enterococcus f aecalis (E.f aecalis),Escherichia coli (E.coli),and K lebsiella pneumoniae (K.pneumoniae) were established.After mice in each trial group and PBS control group were infected by bacteria for 0.5h,1h,3h,6h,12h,24h,and 48h,concentrations of MIP-1β,MIP-2,and IL-12p70 were detected by Luminex liquid suspension chip system.Results Concentrations of MIP-1β increased significantly 1h after bacteria was in blood,S.aureus,E.faecalis,E.coli,K.pneumoniae,and control groups were (134.5 ± 18.3),(61.5 ± 15.4),(3 354.0 ±809.0),(6 888.4 ± 1 100.2),and (28.9 ± 4.6) pg/mL respectively;the peak values of IL-12p70 were (389.3 ± 118.1),(127.6 ± 10.0),(42.2 ± 3.5),(62.8 ± 8.4),and (4.8 ± 0.3) pg/mL respectively.Concentrations of MIP-1β and MIP-2 in E.coli and K.pneumoniae groups were significantly higher than other trial groups and control group (all P<0.01),while concentrations of IL-12p70 in S.aureus and E.faecalis groups were both significantly higher than E.coli,K.pneumoniae,and control groups (all P<0.01).Conclusion Concentrations of MIP-1β and MIP-2 in E.coli and K.pneumoniae groups were both significantly higher than those in S.aureus and E.faecalis groups,while concentrations of IL-12p70 in S.aureus and E.faecalis groups were both significantly higher than those in E.coli and K.pneumoniae groups.The combination detection of multiple cytokines or chemokines are valuable in predicting gram-positive or gram-negative bacterial infection,and can provide basis for treatment of early infection.
