ABSTRACT

OBJECTIVE: This study explores the relationship between patients' self-assessment and physicians' evaluation regarding clinical stability. METHODS: This cross-sectional study was carried out at the general outpatient clinic of the Instituto de Psiquiatria da Universidade Federal do Rio de Janeiro (IPUB-UFRJ) in a large sample (1,447) of outpatients, of which 67.9% were patients with severe mental disorders (SMD). We collected information using a structured questionnaire developed for this purpose, filled in by the patient's physician. Clinical stability was assessed by means of five psychiatric instability criteria and by the physician's global clinical impression over the six previous months. The patients' self-assessment was based on a question about how they evaluated their health status: stable/better, worse, does not know. For the analyses, patients' self-evaluation was considered as our standard. RESULTS: The sample was composed of 824 (57%) women with an average age of 49 years. The most prevalent diagnoses within the SMD category corresponded to 937 patients, of whom 846 (90.3%) assessed themselves as stable/better. The physicians' evaluations agreed more with patients with bipolar disorders and less with schizophrenics regarding stability. As for patients with depressive disorder, physicians agreed more with them regarding instability. CONCLUSION: The data analysis confirms our hypothesis that the self- -assessment made by patients with SMD was accurate regarding their health condition, and that the self- -assessment made by patients who considered themselves stable agree with the physicians' evaluation.


OBJETIVO: Este estudo explora a relação entre a autoavaliação dos pacientes e a avaliação dos médicos quanto à estabilidade clínica. MÉTODOS: Trata-se de um estudo transversal realizado no ambulatório do Instituto de Psiquiatria da Universidade Federal do Rio de Janeiro (IPUB-UFRJ) em uma ampla amostra de pacientes (1.447), dos quais 67,9% eram portadores de transtornos mentais graves (TMG). Coletamos informações por meio de um questionário estruturado desenvolvido para esse fim, preenchido pelo médico assistente. A estabilidade clínica foi avaliada por meio de cinco critérios de instabilidade psiquiátrica e pela impressão clínica global do médico, nos seis meses anteriores. A autoavaliação dos pacientes baseou-se em uma pergunta sobre como eles avaliavam seu estado de saúde: estável/melhor, pior, não sabe. Para as análises, a autoavaliação dos pacientes foi considerada como nosso padrão. RESULTADOS: A amostra foi composta por 824 (57%) mulheres, com idade média de 49 anos. Os diagnósticos mais prevalentes na categoria TMG corresponderam a 937 pacientes, dos quais 846 (90,3%) se avaliaram como estáveis/melhores. As avaliações dos médicos concordaram mais com pacientes portadores de transtorno bipolar e menos com esquizofrênicos em relação à estabilidade. Quanto aos pacientes com transtorno depressivo, os médicos concordaram mais com eles em relação à instabilidade. CONCLUSÃO: A análise dos dados confirma nossa hipótese de que a autoavaliação feita por pacientes com TMG foi precisa quanto à sua condição de saúde e que a autoavaliação feita por pacientes que se consideravam estáveis concorda com a avaliação dos médicos.

ABSTRACT

ABSTRACT Chagas disease is among the 21 neglected diseases according to the World Health Organization. This study aimed to investigate the morbidity and mortality distribution of Chagas disease for identifying areas with greater prevalences and deaths of the disease in Northeast Brazil. A population-based ecological study was performed from 2016 to 2018 using data on acute Chagas disease patients from the Disease Notification Information System, chronic cases from the Chagas Disease and the referral Heart Failure Outpatient Clinic in Pernambuco, and Chagas disease-related mortality from the Mortality Information System. The unit of analysis were Pernambuco State mesoregions. The indicators were spatialized into thematic maps on the occurrence and mortality of the disease per 100,000 inhabitants. No cases of acute disease were reported in the period analyzed. Data on 801 chronic Chagas disease patients were analyzed. The population showed an average age of 62 years, with female predominance. The most prevalent comorbidity was systemic arterial hypertension and cardiologic involvement without ventricular dysfunction. The average chronic disease occurrence rate was 3.2/ 100,000 people/ year. As for deaths in the mortality system; in total, 350 deaths were recorded, showing male predominance, age ≥ 60 years, and chronic disease with cardiac involvement as the main mortality cause. The annual average mortality proportion was 1.6/100,000 people. The chronic case distribution showed spatial heterogeneity, with the highest rates of chronic disease and deaths observed in two mesoregions, with the main cause of death being heart-related. This highlights the need for more specialized services in areas with higher burden of the disease to avoid delay in the patients' care.

ABSTRACT

OBJECTIVE: To describe the history of creation, development, and topics covered by the Study Center of the Institute of Psychiatry of the Federal University of Rio de Janeiro (CE ­ IPUB/UFRJ) over its 70 years. METHODS: Research in newspapers of the Hemeroteca Brasileira Digital, internal documents of IPUB/UFRJ, and interviews with eyewitnesses of the functioning of the CE. RESULTS: The Study Center has been operating on an uninterrupted basis for 70 years, every week. 472 events have been identified since the founding of the CE, but numerous other meetings have taken place. The findings were described in three major groups: 1. Academic meetings in the first half of the 20th century and insertion of the CE in the history of IPUB; 2. Topics discussed and presentations; 3. Changes in periodicity and format. CONCLUSIONS: The CE produces cultural and scientific dissemination continuously since its foundation. The type and format of events have changed over time, adapting to the needs of their community, but always serving as an important beacon for the training of specialists in mental health, dissemination of research, and tendencies about psychiatry worldwide, Latin America, and Brazil.


OBJETIVO: Descrever a história da criação e do desenvolvimento e os temas abordados pelo Centro de Estudos do Instituto de Psiquiatria da Universidade Federal do Rio de Janeiro (CE ­ IPUB/UFRJ) ao longo de seus 70 anos. MÉTODOS: Pesquisa em jornais da Hemeroteca Brasileira Digital, documentos internos do IPUB/UFRJ e entrevistas com testemunhas oculares do funcionamento do CE. RESULTADOS: O Centro de Estudos funciona de maneira ininterrupta há 70 anos, semanalmente. Desde sua fundação, 472 eventos foram identificados, mas outros inúmeros encontros ocorreram. Os achados foram divididos e descritos em três grandes grupos: 1. Encontros acadêmicos na primeira metade do século XX e inserção do CE na história do IPUB; 2. Temas debatidos e apresentações; 3. Mudanças de periodicidade e formato. CONCLUSÕES: O CE produz divulgação cultural e científica continuamente desde sua fundação. O tipo e o formato dos eventos mudaram ao longo do tempo, adaptando-se às necessidades de sua comunidade, mas sempre servindo como um importante farol para a formação de especialistas em saúde mental e divulgação de pesquisas, apresentando as tendências da psiquiatria mundial, latino-americana e brasileira.

Abstract Objective: To investigate the association between birth weight and excess weight among students aged 6-14 years, adjusted for life course confounding factors. Methods: Cross-sectional study with 6-14-year-old schoolchildren in 2010; 795 school children from two public schools. In addition, a sub-sample was selected using a case-cohort study approach. Sociodemographic, breastfeeding, food introduction, previous weight gain, family history, current clinical and behavioral variables as well as maternal variables related to pregnancy, were collected. Multivariable weighted logistic regression was used to evaluate the association between birth weight and overweight. All prevalent cases of overweight (n = 160) were selected to compose the case group and a random sub-sample of all students participating in the study (n = 276 students, of whom 88 were cases) were the control group. Results: An unadjusted 6% increase in the excess weight prevalence ratio (p-value = 0.004) was found for each 100 g increase in birth weight. With adjustment for age, sex and behavioral variables (models 1 and 2), the association of birth weight with excess weight was positive and statistically significant, but it was no longer significant in the final model (model 3) when clinical variables were considered. Conclusions: Although some of the secondary associations were statistically significant, we could not identify a significant association between birthweight and excess weight in adolescents.

ABSTRACT Objectives The primary objective was to measure the effect of psychiatry rotation in students self-confidence (SC) for managing mental health (MH) issues. Methods An eighteen questions version of "Preparation for Hospital Practice Questionnaire" (PHPQ) adapted for MH was applied before, after and six months later the psychiatry rotation. Sociodemographic and experience with mental illness was measured as confounding factors. Results Hundred and ten students were recruited and four factors were identified: "Diagnosis elaboration and basic care" (F1), "Crisis management and prevention" (F2), "External sickness determinants" (F3) and "Personal distress with clinics" (F4). Cronbach Alpha ranged from 0.71 to 0.90. Previous MH training were not frequent (9.09%), and associated with better SC in F2 (after p < 0.05, 6m p = 0.03). Previous mental disorder was frequent (42.16%), and associated with more SC on F2 (p < 0.01) and F3 (p = 0.03) before course, but only on F3 after (p < 0.01) and not 6 months later. Male gender had more SC in F4 (p < 0.01) before course, but after course and 6m later female gender became more SC in F1 (after p = 0.02, 6m p = 0.01) and equivalent in F4. All factors had higher scores after and 6 months later (p < 0.001). The class considered that an interview script is very important for their SC, and improves assistance (mean > 9.0/10.0). Conclusion Obligatory rotation in MH improved SC in students. Previous training and gender were related with long lasting effects in SC.


RESUMO Objetivos Avaliar os efeitos de empoderamento do internato em saúde mental (SM) na autoconfiança (AC) dos alunos de Medicina. Métodos Uma versão adaptada para a saúde mental do "Questionário de Preparação para Prática Hospitalar" foi aplicada antes, depois e seis meses após o internato. Resultados Cento e dez alunos participaram e quatro fatores foram extraídos: "Elaboração diagnóstica e cuidados básicos" (F1), "Gestão e prevenção de crise" (F2), "Determinantes externos de adoecimento" (F3) e "Sofrimento pessoal com a clínica" (F4). Treinamento prévio em SM é incomum (9,09%), mas foi associado com pontuações mais altas em F2 (p = 0,05 e 6m p = 0,03). Tratamento prévio em SM (42,16% dos alunos) foi associado a valores mais altos em F2 (p < 0,01) e F3 (p = 0,03) antes, mas apenas em F3 (p < 0,01) após o curso. O gênero masculino apresentou valores mais positivos que o feminino em F4 (p < 0,01) antes, mas não após o curso, quando apresentaram valores mais baixos em F1 (após p = 0,02, 6m p = 0,01). Todos os fatores apresentaram valores mais altos após o curso (p < 0,001). Os alunos consideraram o uso de uma entrevista estruturada muito importante para sua autoconfiança e qualidade da assistência (média > 9,0/10,0). Conclusão O internato em SM aumentou a AC nos alunos. Treinamento prévio e gênero estiveram associados com efeitos duradouros na AC.

ABSTRACT To evaluate knee and ankle disorders during functional gait assessment in individuals with Mucopolysaccharidosis type VI (MPS VI). 19 subjects were included in this cross-sectional study and allocated in three groups according to age: Children Group (n=11); Adolescent Group (n=4); and Adult Group (n=4). Subphases of one gait cycle were analyzed: Initial contact; Medium support, and Pre-Balance. All volunteers with MPS VI presented greater knee and ankle flexion angles, in all gait subphases, when compared to the normal values defined by literature (p<0.05). Initial contact subphase: knee flexion angle ranging from 8.5º to 15º; Ankle = Child Group −23.73º ± 8.53º; Adolescent Group = −25º ± 11.22º; Adult Group = −27.75º ± 3.3º. Medium support subphase: Knee = Child Group 19.64º ± 10.47º; Adolescent Group 16.75º ± 10.34º; Adult Group = 21.25º ± 12.84º. Ankle = Child Group −18.82º ± 8.91º ± 8.53º; Adolescent Group = −16.5º ± 9.33º; Adult Group = −22.25º ± 4.19º. Pre-Balance subphase: Knee = Child Group 22.72º ± 13.49º; Adolescent Group 21.25º ± 7.97º; Adult Group = 27º ± 16.27º. Ankle = Child Group -15º ± 9.76º; Adolescent Group = −15.75º ± 5.31º; Adult Group = −14.75º ± 3.86º. In this study, MPS VI individuals presented hyperflexion of knee and ankle as the main joint disorders during functional gait, regardless of age.


RESUMO O objetivo deste estudo foi avaliar distúrbios articulares de joelho e tornozelo durante a marcha funcional na Mucopolissacaridose tipo VI (MPS VI). 19 indivíduos foram incluídos neste estudo transversal e alocados em três grupos de acordo com a idade: Grupo Crianças (n=11); Grupo Adolescentes (n=4); e Grupo Adultos (n=4). Foram analisadas as subfases de um ciclo da marcha: contato inicial; apoio médio e pré-balanço. Todos os voluntários com a MPS VI apresentaram maiores ângulos de flexão do joelho e tornozelo, em todas as subfases da marcha, quando comparados aos valores normais definidos pela literatura (p<0,05). ABSTRACT

RESUMEN El cuidado de personas diagnosticadas con trastornos esquizofrénicos y delirantes representa un desafío, especialmente, en territorios de gran vulnerabilidad social. En Río de Janeiro, con la promulgación de la Ley Federal 10216 de 2001 sobre la Reforma Psiquiátrica, las últimas dos décadas se han caracterizado por la transición del modelo centrado en el hospital al modelo basado en los servicios comunitarios. Utilizando el caso de una comunidad de Río de Janeiro, se seleccionaron cinco servicios de salud pública y se extrajeron datos de los registros médicos de 94 pacientes con diagnósticos de esquizofrenia, trastornos esquizotípicos y trastornos delirantes (codificados en la Clasificación Internacional de Enfermedades 10 edición entre los códigos F20 a F29) y se analizaron indicadores de calidad del tratamiento ofrecido en el período 2003-2016. Como resultado de la transición se observa que los hospitales psiquiátricos quedaron casi vacíos con un bajo número de pacientes actualmente desatendidos. Además, la atención a la crisis y las hospitalizaciones a largo plazo se reemplazaron por la atención territorial y las comorbilidades clínicas se monitorean y se tratan en unidades de salud familiar.


ABSTRACT Care for people diagnosed with schizophrenia and delusional disorders involves many challenges, especially in territorial contexts of pronounced social vulnerability. In Rio de Janeiro, the two decades after the passing of Federal Law 10,216 on Psychiatric Reform in 2001 have been characterized by the transition from a hospital-centered model to one based on community services. Taking the case of a community in Rio de Janeiro, data was extracted from the medical records of 94 patients diagnosed with schizophrenia, schizotypal disorders, and delusional disorders (ICD-10 codes F20 to F29) in five public health services. Covering the period from 2003 to 2016, indicators of the quality of treatment provided were analyzed. As a result of this transition, psychiatric hospitals have all but emptied and a low number of currently untreated patients can be observed. In addition, crisis care and long-term hospitalizations have been replaced by territorial care, and clinical comorbidities are monitored and treated at Family Health Units.

Objective: Human immunodeficiency virus (HIV) prevention efficacy trials with psychiatric patients have been conducted in research settings in high-resourced countries, establishing short-term efficacy for reducing sexual risk behavior. None has been implemented within systems of care. In the last decade, overcoming this research-to-practice gap has become a focus of implementation science. This paper describes the first and only HIV Prevention intervention trial for psychiatric patients conducted in real-world outpatient psychiatric settings facilitated by trained clinic-based providers. Methods: The HIV Prevention intervention, which uses the Information-Motivation-Behavioral Skills model to achieve sexual risk-reduction, was rigorously adapted to the local context and clinic services' needs. Participants from eight clinics were randomized to HIV Prevention or Health Promotion conditions. Results: HIV Prevention participants showed significant improvement in Information-Motivation-Behavioral domains; in this group, behavioral intentions were associated with significantly fewer unprotected sex occasions, but reduction of unprotected sex occasions was similar in both conditions. Conclusion: Our trial was conducted before implementation studies became widely funded. Transporting an intervention to a new culture or into real-world practice settings may require adaptations. Our results demonstrate that clear guidelines are needed regarding whether to conduct efficacy, effectiveness, and/or implementation research as the most appropriate next step. ABSTRACT

As Diretrizes Curriculares do curso de graduação em Medicina de 2014 incluem a Saúde Mental (SM) como uma das áreas obrigatórias do internato. O objetivo deste trabalho é apresentar o internato integrado de SM e de Medicina de Família e Comunidade (MFC) da Universidade Federal do Rio de Janeiro (UFRJ) e a fundamentação da escolha da Atenção Primária à Saúde (APS) como cenário de formação dos internos. Trata-se do relato da experiência do internato integrado de SM e MFC da UFRJ e da discussão de seus marcos teóricos. A alta prevalência de sofrimento psíquico e transtornos mentais na APS, o fato de ela ser a porta de entrada do Sistema Único de Saúde (SUS) e o fato de que a faculdade de Medicina deve formar médicos generalistas fazem deste cenário lócus privilegiado para o treinamento dos internos de Medicina em SM.(AU)


Las Directrices Curriculares del Curso de Graduación en Medicina de Curso de 2014 incluyen la salud mental (SM) como una de las áreas obligatorias del internado. El objetivo de este trabajo es presentar el internado integrado de SM y Medicina de Familia y Comunidad (MFC) de la UFRJ y la fundamentación de la Escuela de Atención Primaria de la Salud (APS) como escenario de formación de los internos. Se trata del relato del internado integrado de SM y MFC de la UFRJ y de la discusión de sus marcos teóricos. La alta prevalencia de sufrimiento psíquico y trastornos mentales en la APS, el hecho de que la APS es la puerta de entrada del sistema único de salud (SUS) y el hecho de que la facultad de medicina debe formar a médicos generalistas, hace que este escenario sea un locus privilegiado para el entrenamiento de los internos de medicina en salud mental.(AU)


The 2014 Curricular Guidelines of the medical undergraduate courses include Mental Health (MH) as one of the required areas of internship. The objective of this paper is to present the integrated internship for MH and Family and Community Medicine (FCM) of UFRJ, as well as the rationale for choosing Primary Health Care (PHC) as the setting for the internship training. It presents a report of the experience of the integrated internship for MH and FCM of UFRJ and the discussion of its theoretical frameworks. The high prevalence of psychological distress and mental disorders in PHC, the fact that PHC is the gateway to the unified national health system (SUS) and the fact that the medical school must train general practitioners, make PHC a privileged locus for the training of medical interns in mental health.(AU)

ABSTRACT

A regionalização do cuidado perinatal deve considerar as estruturas assistenciais existentes e facilitar o acesso. Este estudo identificou fluxos assistenciais intermunicipais de nascimentos e óbitos perinatais ocorridos na Região Metropolitana do Rio de Janeiro, Brasil, em 2011 e 2014, definiu parâmetros e sistematizou propostas para a organização da regionalização da assistência perinatal. Estudo ecológico espaço-temporal. As fontes de dados foram os Sistemas de Informações sobre Nascidos Vivos e sobre Mortalidade, o Cadastro Nacional de Estabelecimentos de Saúde e o Censo Demográfico de 2010. Foram identificadas relações existentes entre municípios de residência e ocorrência dos nascimentos e óbitos perinatais. Cada município foi analisado em separado e em pares - residência/ocorrência - segundo o evento vital, recursos assistenciais e critérios pragmáticos de ameaça à vida. Foram realizadas análises descritivas de fluxos dominantes, fatorial exploratória de componentes principais e cluster. Identificaram-se as redes assistenciais existentes, e as 47 variáveis analisadas foram resumidas em três fatores (dimensões analíticas) - disponibilidade de leitos, situação de ameaça à vida e condições socioeconômicas - responsáveis, respectivamente, por 60/80%, 20/30% e 13/22% da variância, relativas a cada ano analisado. Os fatores foram utilizados para a formação dos clusters, classificados de 3 a 5 estratos. Três propostas de regiões de saúde perinatal foram elaboradas. A principal contribuição deste estudo foi apresentar parâmetros para o acompanhamento da regionalização e a reavaliação desse processo sistematicamente com base nos registros administrativos.


The regionalization of perinatal care should consider existing healthcare structures for facilitating access. This spatial-temporal ecological study identified intermunicipal flows of perinatal births and deaths in Greater Metropolitan Rio de Janeiro, Brazil, in 2011 and 2014, defined parameters, and systematized proposals for organizing the regionalization of perinatal care. The data sources were the Brazilian Information System on Live Births, Mortality Information System, National Registry of Healthcare Establishments, and 2010 Population Census. The study identified existing relations between the mothers' municipalities of residence and the occurrence of perinatal births and deaths. Each municipality was analyzed singly and pairwise (residence/occurrence) according to the vital event, healthcare resources, and pragmatic criteria of life-threatening conditions at birth. We conducted descriptive analyses of dominant flows, exploratory principal components analysis, and cluster analysis. The existing healthcare networks were identified, and 47 variables were summarized in three factors (analytical dimensions) - availability of beds, risk of life-threatening conditions, and socioeconomic status - accounting for 60%/80%, 20%/30%, 13%/22%, respectively, of the variance pertaining to each year analyzed. The factors were used to form clusters, classified in 3 to 5 strata. Three proposals were drafted for perinatal health regions. The study's principal contribution was having presented parameters for monitoring the regionalization and systematic reevaluation of this process based on administrative records.


La regionalización del cuidado perinatal debe considerar las estructuras asistenciales existentes y facilitar el acceso a las mismas. Este estudio identificó flujos asistenciales intermunicipales de nacimientos y óbitos perinatales, ocurridos en la región metropolitana de Río de Janeiro, Brasil, en 2011 y 2014, definió parámetros y sistematizó propuestas para la organización de la regionalización de la asistencia perinatal. Se trata de un estudio ecológico espacio-temporal. Las fuentes de datos fueron: Sistema de Información sobre Nacidos Vivos y sobre Mortalidad, registro nacional de establecimientos de salud y Censo Demográfico de 2010. Se identificaron las relaciones existentes entre municipios de residencia y ocurrencia de nacimientos y óbitos perinatales. Cada municipio se analizó por separado y por pares -residencia/ocurrencia-, según el evento vital, recursos asistenciales y criterios pragmáticos de amenaza para la vida. Se realizaron análisis descriptivos de flujos dominantes, así como análisis factoriales exploratorios de componentes principales y clúster. Se identificaron las redes asistenciales existentes y las 47 variables analizadas se resumieron en tres factores (dimensiones analíticas): disponibilidad de camas, situación de amenaza para la vida y condiciones socioeconómicas; responsables respectivamente de un 60%/80%, 20%/30% y 13%/22%, de la variancia, relativas a cada año analizado. Los factores fueron utilizados para la formación de los clústeres, clasificados de entre 3 a 5 estratos. Se elaboraron tres propuestas de regiones de salud perinatal. La principal contribución de este estudio fue presentar parámetros para realizar un seguimiento de la regionalización, así como la reevaluación de este proceso sistemáticamente, a partir de registros administrativos.

ABSTRACT

Resumo O objetivo deste artigo é investigar a visão dos profissionais de saúde a respeito da coordenação do cuidado na Atenção Primária à Saúde (APS) e os desafios frente ao seu exercício. Pesquisa qualitativa com profissionais de duas equipes de APS que atuam em territórios com vulnerabilidades e desigualdades no Rio de Janeiro. Utilização do método de Análise de Conteúdo, modalidade temática com realização de entrevistas semiestruturadas e observação participante. Coordenar o cuidado é estar envolvido com as atividades que proporcionam a prestação de um cuidado individualizado e integral, visando a continuidade do cuidado. Os desafios da coordenação do cuidado pela APS são a superação da fragmentação da rede, a baixa oferta de vagas para especialistas, a frágil comunicação entre os serviços, o prontuário eletrônico não integrado, a baixa qualificação profissional e o desconhecimento do papel da APS pelos outros serviços. O fortalecimento da APS como porta de entrada, coordenadora e ordenadora da rede requer esforços de gestores, profissionais e sociedade, a fim de que esse modo de organização dos sistemas possa trazer benefícios no que tange à equidade, à acessibilidade, à eficácia clínica e sanitária, à eficiência econômica e que possibilite a integração dos cuidados.


Abstract The objective of this article is to investigate the viewpoint of health professionals regarding the coordination of care in Primary Health Care (PHC) and the challenges for performing it. Qualitative research with professionals from two PHC teams working in territories with vulnerabilities and inequalities in Rio de Janeiro. Use of the Content Analysis method, thematic modality with semi-structured interviews and participant observation. To coordinate care is to be involved with the activities that foster the provision of individualized and comprehensive care, aiming at the care continuity. The challenges of coordinating care by the PHC are overcoming the network fragmentation, the low supply of openings for specialists, the fragile communication between the services, the non-integrated electronic medical record, the low professional qualification and the unawareness of the PHC role by other services. The strengthening of the PHC as the gateway, coordinator and organizer of the network requires efforts by managers, professionals and society, so this way of organizing the systems can bring benefits regarding equity, accessibility, clinical and sanitary effectiveness, economic efficiency and allow care integration.

ABSTRACT

Abstract INTRODUCTION: Chagas disease (CD) is a neglected disease caused by the parasite Trypanosoma cruzi. One-third of infected patients will develop the cardiac form, which may progress to heart failure (HF). However, the factors that determine disease progression remain unclear. Increased angiotensin II activity is a key player in the pathophysiology of HF. A functional polymorphism of the angiotensin-converting enzyme (ACE) gene is associated with plasma enzyme activity. In CD, ACE inhibitors have beneficial effects supporting the use of this treatment in chagasic cardiomyopathy. METHODS: We evaluated the association of ACE I/D polymorphism with HF, performing a case-control study encompassing 343 patients with positive serology for CD staged as non-cardiomyopathy (stage A; 100), mild (stage B1; 144), and severe (stage C; 99) forms of Chagas heart disease. For ACE I/D genotyping by PCR, groups were compared using unconditional logistic regression analysis and adjusted for nongenetic covariates: age, sex, and trypanocidal treatment. RESULTS: A marginal, but not significant (p=0.06) higher prevalence of ACE I/D polymorphism was observed in patients in stage C compared with patients in stage A. Patients in stage C (CD with HF), were compared with patients in stages A and B1 combined into one group (CD without HF); DD genotype/D carriers were prevalent in the HF patients (OR = 2; CI = 1.013.96; p = 0.04). CONCLUSIONS: Our results of this cohort study, comprising a population from the Northeast region of Brazil, suggest that ACE I/D polymorphism is more prevalent in the cardiac form of Chagas disease with HF.

Resumo Introdução Violência contra mulheres constitui uma violação dos direitos humanos e um problema de saúde pública. Objetivo Analisar as notificações de violência contra mulheres (10 anos de idade ou mais), residentes em Niterói/RJ (2010-2014). Método Estudo descritivo de série de casos. As fontes de dados foram o Sistema de Informação de Agravos de Notificação (SINAN) e o Departamento de Informática do Sistema Único de Saúde (DATASUS). Foram descritas as distribuições das características de violência e calculadas as taxas de incidência. Resultados Foram notificados 307 episódios de violência com incidência de 26,2/100 mil, decrescente com a idade. O percentual de incompletitude variou de 0 a 65%. Violência física (186) predominou, seguida de violências psicológica (121) e sexual (96) e de negligência (64). As adolescentes foram as principais vítimas de violência sexual (56,3%). Cônjuges, ex-cônjuges, namorados(as), ex-namorados(as) foram autores em 42,6% das agressões físicas, indicando ocorrência de violência entre parceiros íntimos. Mães foram responsáveis por 50,7% das negligências, refletindo a ausência de compartilhamento e a sobrecarga feminina com relação à criação dos filhos. Conclusão As notificações de violência interpessoal/autoprovocada possibilitam intervir caso a caso, e a análise sistemática das informações poderá respaldar a elaboração de políticas e estratégias de prevenção e enfrentamento da violência contra mulheres.


Abstract Background Violence against women is a violation of human rights and a public health problem. Objective To analyze surveillance reports of violence against women (aged 10 years or more), living in Niterói, RJ, from 2010 to 2014. Method Descriptive study based on case series was carried out. Case reports and population estimates were provided, respectively, by the National Surveillance Information System (SINAN) and the Brazilian Ministry of Health (DATASUS). Incidence rates and proportions were calculated according to violence related characteristics. Results There were 307 reported cases of violence with incidence rate 26.2 per 100 thousand in the period studied decreased with age. Data incompleteness ranged from zero to 65%. Physical violence (n=186) was the most frequent type, followed by psychological (n=121), sexual (n=96) and negligence (n=64). Adolescents were the main victim of sexual violence (56.3%). Husband, ex-husband, boyfriend or ex-boyfriend were perpetrators in 42.6% of physical assaults, denoting the occurrence of violence between intimate partners. Mother was considered as the perpetrator in 50.7% of negligence cases, reflecting lack of sharing and female burden related to raising children alone. Conclusion Interpersonal/self-induced violence surveillance allow individual interventions and provide data for policy development towards prevention of violence against women.

ABSTRACT

RESUMO Objetivo Avaliar o efeito do internato em saúde mental nas atitudes dos alunos de medicina quanto ao portador de transtorno mental (PTM). Métodos Atitudes foram avaliadas por questionário antes e depois do internato, por meio dos fatores: "aceitação social de PTM" (F1), "não acreditar em causas sobrenaturais para doença mental" (F2), "papéis sociais comuns para PTM" (F3), "acreditar em causas psicossociais para doença mental" (F4), "intimidade" (F5). Diferenças foram avaliadas por meio de testes t, fatores confundidores por ANOVA e correlações entre expectativa de melhora e fatores por Pearson. Resultados 74 de 85 alunos responderam ao questionário. Houve redução significativa em quatro fatores avaliados (F1, p < 0,001, F2, p = 0,002, F3, p = 0,04, F5, p < 0,001). Uma associação entre ter um amigo PTM e F3 foi identificada antes do curso (p = 0,04), porém não após (p = 0,13). Houve correlação positiva entre crenças de melhora e atitudes negativas com o F2 antes do curso (p = 0,01), mas não após. F5 esteve relacionado com a expectativa de melhora (p < 0,001) após o curso, indicando melhores atitudes quando melhor expectativa. Observou-se a melhora da expectativa quanto a resposta ao tratamento da esquizofrenia (p = 0,02), transtorno bipolar (p = 0,03) e transtorno de ansiedade (p = 0,03). Conclusões O internato esteve associado à redução de atitudes negativas com relação aos PTMs. O contato direto com o paciente parece ter influência direta nessa redução. Acreditamos que, mais importante do que possíveis efeitos de esclarecimento sobre causas do adoecimento, a desconstrução do mito sobre o louco violento é essencial para a melhora das atitudes. Estudos com populações de outras regiões do Brasil e voltadas para avaliação do medo de violência são necessários para a confirmação dessa hipótese e do efeito do internato sobre os alunos.


ABSTRACT Objective To evaluate and measure the effects of mental health internship on Medicine Students (MS) attitudes towards people with mental illness (PMI). Methods MS was submitted to an attitude questionnaire previously and after the mental health internship. Their attitudes were measured inside five factors: (F1) "social acceptance of PMI", (F2) "normalizing roles for PMI", (F3) "non-belief in supernatural causes for mental illness", (F4) "belief in bio-psychosocial causes for mental illness," and (F5) "near contact with PTM". T-tests were used to evaluate factor differences, confounding factor were analyzed by ANOVA and correlations through Pearson's correlation test. Results 74 of 85 students responded. There were a significant reduction in four factors (F1, p < 0.001, F2, p = 0.002, F3, p = 0.04, F5, p < 0.001). An association between having a PMI friend and F3 was identified before the course (p = 0.04), but not after (p = 0.13). A positive correlation was identified between belief in disease improvement and negative F2 attitudes before course (p = 0.01), but not after (0.40). F5 was related with disease improvement after course (p < 0.001), suggesting positive attitudes when improvement is expected. There were an increase in improvement expectations after course for schizophrenia (p = 0.02), bipolar disorder (p = 0.03) and anxiety (p = 0.03). Conclusions Mental health internship was related to a decrease in negative attitudes towards PMI. Personal contact seems to influence this improvement. We believe that the reduction of fear toward PMI is more powerful to reduce stigma than the acquisition of knowledge about its natural causes. More studies with a regionally distinct population in Brazil and aimed to measure the impact of fear are necessary to confirm this hypothesis.

ABSTRACT

Objective: To estimate the prevalence of major depressive episode (MDE) in patients with presumptive pulmonary tuberculosis (pre-PTB, defined by cough lasting ≥ 3 weeks) and compare it between patients with pulmonary tuberculosis (PTB) and without PTB. Methods: Patients with pre-PTB (n=260) were screened for depression using the Patient Health Questionnaire (PHQ-9). Those individuals with scores ≥ 10 were subsequently assessed with the depression module of the Mini International Neuropsychiatric Interview (MINI-Plus) to confirm diagnosis. Associations of categorical variables with PTB and MDE were calculated using the chi-square test and OR. Results: PTB was confirmed in 98 patients (37.7%). A high proportion of both groups (active PTB and no PTB) screened positive for depression (60.2 vs. 62.1%, respectively). Among 159 patients who screened positive for depression, a subset of 97 (61.0%) were further evaluated with the MINI-Plus; current MDE was confirmed in 54.6% (53/97). On univariate and multivariate analysis, female sex was the only factor associated with the diagnosis of current MDE (p = 0.04). Conclusion: The prevalence of MDE was high among individuals with prolonged respiratory symptoms, independent of PTB diagnosis. This is consistent with other studies of depression in primary care in Brazil.

ABSTRACT

Several Latin American countries have made remarkable strides towards offering community mental health care for people with psychoses. Nonetheless, mental health clinics generally have a very limited outreach in the community, tending to have weaker links to primary health care; rarely engaging patients in providing care; and usually not providing recovery-oriented services. This paper describes a pilot randomized controlled trial (RCT) of Critical Time Intervention-Task Shifting (CTI-TS) aimed at addressing such limitations. The pilot RCT was conducted in Santiago (Chile) and Rio de Janeiro (Brazil). We included 110 people with psychosis in the study, who were recruited at the time of entry into community mental health clinics. Trial participants were randomly divided into CTI-TS intervention and usual care. Those allocated to the intervention group received usual care and, in addition, CTI-TS services over a 9-month period. Primary outcomes include quality of life (WHO Quality of Life Scale - Brief Version) and unmet needs (Camberwell Assessment of Needs) at the 18-month follow-up. Primary outcomes at 18 months will be analyzed by Generalized Estimating Equations (GEE), with observations clustered within sites. We will use three-level multilevel models to examine time trends on the primary outcomes. Similar procedures will be used for analyzing secondary outcomes. Our hope is that this trial provides a foundation for planning a large-scale multi-site RCT to establish the efficacy of recovery-oriented interventions such as CTI-TS in Latin America.


Diversos países latino-americanos já alcançaram avanços notáveis na oferta de assistência em saúde mental para pessoas com psicoses. No entanto, as clínicas de saúde mental geralmente realizam atividades de extensão muito limitadas dentro das comunidades, tendem a ter vínculos fracos com a assistência primária, raramente envolvem os próprios pacientes nos cuidados e poucas vezes prestam serviços orientados para a recuperação. O artigo descreve um estudo piloto randomizado e controlado sobre a Critical Time Intervention-Task Shifting (CTI-TS), que teve como objetivo analisar essas limitações. O estudo piloto foi realizado em Santiago (Chile) e no Rio de Janeiro (Brasil). Teve como meta a inclusão de 110 pessoas com psicose, recrutadas no momento da entrada em clínicas comunitárias de saúde mental. Os participantes foram randomizados para o CTI-TS ou para os cuidados usuais. Aqueles alocados ao grupo da intervenção receberam os cuidados usuais e os serviços de CTI-TS ao longo de 9 meses. Os desfechos primários incluíram a qualidade de vida (WHO Quality of Life Scale - Brief Version) e as necessidades não atendidas (Camberwell Assessment of Needs) no acompanhamento aos 18 meses. Os desfechos primários aos 18 meses serão analisados com a técnica de Equações de Estimação Generalizadas (GEE), com as observações agrupadas dentro dos locais do estudo. Serão utilizados modelos em três níveis para examinar as tendências temporais nos desfechos primários. Procedimentos semelhantes serão utilizados para analisar os resultados secundários. Espera-se que o estudo forneça uma base para planejar um estudo randomizado e controlado em grande escala e em múltiplos locais para estabelecer a eficácia da intervenção orientada para a recuperação, a exemplo da CTI-TS, na América Latina.
