ABSTRACT

Objective To assess effects of stress dynamic CT myocardial perfusion imaging (CT-MPI) combined with coronary CT angiography (CCTA) on the diagnosis of myocardial perfusion defects in coronary artery disease (CAD). Methods Patients with CAD diagnosed by CCTA underwent ATP stress CT-MPI examination. Single-photon emission computed tomography (SPECT) myocardial perfusion imaging (SPECT-MPI) was performed within one week and set as the reference standard. CT-MPI results were qualitatively analyzed, and myocardial blood flow (MBF), myocardial blood volume (MBV) as well as time to peak (TTP) were quantified according to CT-MPI. Effects of CCTA, CT-MPI, and CT-MPI combined with CCTA on predicting myocardial perfusion defects were assessed in comparison with NMPI. Results Thirty patients [(54.8±8.4)years] were enrolled in our study, 20 were men (68%). MBF [(79.3±18.0) versus (135.1± 35.2) ml·100 ml-1·min-1] and MBV [(8.9±2.9) versus (13.8±8.9) ml/100 ml] were significantly decreased in hypoperfused segments compared with normal segments, while TTP was increased in hypoperfused segments [(13.9 ± 2.5)s] compared with normal segments [(9.1 ± 2.1)s] (t=0.302, 0.866 and 0.024 respectively, all P values<0.01). The sensitivity, specificity of CT-MPI for identifying segments with perfusion defects were 91.3%(147/161), 84.6%(281/332), respectively. On a per-vessel basis, the area under the receiver operating characteristic curve for predicting myocardial perfusion defects were 0.635(95%CI:0.517—0.753) for CCTA, 0.709(95%CI:0.599—0.819)for CT-MPI, and 0.837(95%CI:0.749—0.925)for CT-MPI combined with CCTA, respectively. Conclusions The performance of stress dynamic CT-MPI in the diagnosis of myocardial perfusion defects in CAD was good. One-stop examination of CT-MPI combined with CCTA improves the diagnostic accuracy for identifying flow-obstructing stenosis.

ABSTRACT

Objective To evaluate the mechanism of Fumai decoction in treating type 2 diabetes mellitus combined with acute cerebral infarction. Methods 68 cases of type 2 diabetes mellitus with acute cerebral infarction patients were divided into treatment group (34 cases) and western medicine control group (34 cases) at random. The serum levels of Endothelin (ET), Calcitonin gene related-pepitide (CGRP), Thromboxane-B2 (TXB2), 6-ketone-prostagla-ndins F1? (6-keto-PGF1?) and cerebral blood flow variation, clinical therapeutic effect were observed. Result Compared 2 groups, there was significant difference in improving neurological impairment, cerebral artery stenosis, cerebral circulation insufficiency and reducing levels of ET and TXB2, enhancing levels of CGRP and 6-keto-PGF1? (P0.05). Conclusion Fumai decoction can adjust the serum levels of ET, CGRP, TXB2 and 6-keto-PGF1?, and improve cerebrovascular function. Probably this is one of the mechanisms in treating type 2 diabetes mellitus with acute cerebral infarction.

ABSTRACT

The changes of hepatocyte and skeletal muscle membrane potential.the membrane fluidity of microsomes,microviscosity.and the order coefficient of the arrangement of membrane lipid molecules were determined in rats after their cecum was ligated and punctured to induce septic shock.Lipid peroxides of plasma and the liver,phospholipase A2 and salivary acid were determined as well.It was found that both in the first 10 hours and the next 20 hours of septic shock,the resting membrane potential of hepatocytes and skeletal muscles was significantly suppressed as compared with that of the sham-operated rats.The membrane potential of hepatocytes was suppressed prior to the decrease of blood pressure and much earlier and much more severe than that of skelatal muscles.Lipid peroxides of the liver were increased by 1.6 fold(P

ABSTRACT

This paper is to report the determination of the skeletal muscle cellular membrane potential (SMCMP) in vivo with the technique of "Semi-floating"glass microelectrode. The changes of SMCMP and the distribution of extra- and intracellular electrolytes and water in different phases of irreversible hemo-rrhagic shock in rabbits were studied systematically.It was found that SMCMP decreased significantly and immediately as soon as hypotension occurred, which means that the functions of the cellular membrane begin to deteriorate in the early stage of shock; SMCMP continued to decrease when the shock was progressing, which reflects that besides the failure of the cellular membrane function, there is also the degradation of the functions of the cell proper; the leakage of the intracellular potassium across the cellular membrane and the retention of the intracellular sodium and water occurred three hours after the onset of the shock, which indicates that the inability of cells to regulate their volume.
