ABSTRACT

Objective:To understand the social networking using status of medical students, develop the social networking using status scale (SNUSS) and evaluate its reliability and validity. Methods:In March 2018, 665 valid questionnaires were collected via cluster sampling. The scale was retest in 46 subjects after an interval of 2 months. Factor analysis, correlation analysis and t-test were used to calculate the structure validity and content validity of the scale to evaluate the scale validity, and internal consistency reliability, broken half reliability and retest reliability were used to evaluate the scale reliability. Results:The result of exploratory factor analysis showed NSUSS had two subscales: the dimension of maintaining existing social relations and social network use and the dimension of expanding circle of friends and social network use. The result of confirmatory factor analysis showed that the factor load value of each item was greater than 0.500 ( P < 0.05), and the model fitted well ( χ2/ df = 2.873, GFI = 0.970, CFI = 0.971, RMSEA = 0.074). The r value of correlation between SNUSS and MSPSS was 0.295. I-CVIs were all equal to 0.90, and S-CVI was 0.90, the internal consistency reliability was 0.764, the split-half reliability was 0.555, and the retest reliability was 0.526. Conclusion:SNUSS has good reliability and validity for the use of social network among medical college students, and the use of social network is related to perceived social support.

ABSTRACT

Objective:To analyse the clinical features of encephalitis patients with antibodies against the α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor (AMPAR).Methods:Three anti-AMPAR encephalitis patients diagnosed in Tangdu Hospital, the Air Force Military Medical University between January 2020 and May 2021 were retrospectively reviewed. The clinical symptoms, supplementary examination, treatment options and outcomes with knowledge from literature were summarized in this study. Results:Three patients aging from 12 to 70 years presented with symptoms ranging from cognitive impairment, personality change to headache and paralysis. The lung occupying lesion was pathologically proved to be small cell lung cancer in case 1. Antibody to AMPAR (AMPAR-ab) was positive in both blood and cerebrospinal fluid of case 1, with coexisting antibodies against sex-determining region of Y chromosome-related high mobility group box 1 in blood, and the symptoms persisted but did not recur following therapy with corticosteroids. AMPAR-ab was detected only in serum in case 2, with the lesion located in both frontal and temporal lobes, centrum semiovale and lateral ventricle, combined with classic imaging features of intracranial hypotension, and the syndrome was partially improved following treatment with corticosteroids. The lesions were located in the pons and middle cerebellar peduncle, accompanied by cerebellar atrophy in case 3. Spinal cord magnetic resonance imaging showed long hyperintense lesions involving the cervical and thoracic cord, extending from C 2 to Th 10 level on T 2-weighted images. AMPAR-ab was positive in both serum and cerebrospinal fluid. And the symptoms improved significantly following treatment with corticosteroids and intravenous immunoglobulin. Conclusions:The clinical manifestations of anti-AMPAR encephalitis are highly heterogeneous, and brainstem and spinal cord can also be involved in addition to the limbic system, accompanied by brain atrophy. Combining with concurrent antibodies, especially the intracellular antibodies, malignancy needs to be closely monitored; the immunotherapy is effective and the presence of tumor superimposed with multiple antibodies may be associated with poor prognosis.

ABSTRACT

Objective:To study the effects of cisapride on digestive symptoms and gastrointestinal hormones in elderly peritoneal dialysis patients.Methods:Forty-two elderly patients with renal failure undergoing peritoneal dialysis in our hospital from July 2017 to December 2017 were randomly selected as the study group.Meanwhile, 20 healthy elderly people in the corresponding time period were selected as the control group.Control group received continuous ambulatory peritoneal dialysis(CAPD)and other conventional basic therapy, and study group received cisapride as add-on therapy to treatment for control group.Serum levels of somatostatin(SS), motilin(MOT)and vasoactive intestinal peptide(VIP)were compared between the two groups.Gastrointestinal symptoms, serum levels of gastrointestinal hormones and biochemical indexes were compared before and after treatment in the study group. Results:The score of acid reflux, nausea, abdominal distension, belching and constipation were lower in the study group after treatment than before treatment( t=4.42, 4.32, 6.80, 6.29 and 6.76, all P=0.00). Before treatment, levels of MOT, SS and VIP were higher in the study group than in the control group[(636.65±32.02)pmol/L vs.(228.47±28.74)pmol/L, (64.02±16.32)mg/L vs.(42.38±6.42)mg/L, (118.64±17.68)ng/L vs.(58.62±11.63)ng/L, t=48.44, 7.47 and 15.93, all P=0.00]. The level of MOT was lower after than before treatment[(385.36±19.64)pmol/L vs.(636.65±32.02)pmol/L, t=43.36, P=0.00], and the levels of SS and VIP had no significant difference before versus after treatment( t=-0.11 and -0.42, P=0.91 and 0.68). After treatment, the level of MOT was still higher in the study group than in the control group[(385.36 ±19.64)pmol/L vs.(228.47 ±28.74)pmol/L, t=22.08, P=0.00]. There was no significant difference in blood urea nitrogen(BUN), creatinine, haemoglobin and kt/v levels between before and after treatment( P>0.05). The level of albumin(ALB)was higher after than before treatment[(38.60±1.89)g/L vs.(37.71±1.96)g/L, t=2.12, P=0.04]. Conclusions:Gastrointestinal symptoms are common in elderly patients with peritoneal dialysis, and the accumulation of gastrointestinal hormones is obvious, which leads to gastrointestinal dysfunction. The conventional treatment in combination with cisapride can improve gastrointestinal symptoms and reduce serum MOT level in these patients.

ABSTRACT

Objective:To evaluate the value of 99mTc-MIBI SPECT/CT and ultrasonography in preoperative localization of secondary hyperpa-rathyroidism. Methods:A retrospective analysis of 85 patients with secondary hyperparathyroidism due to uremia who underwent total parathyroidectomy or total parathyroidectomy + autologous transplantation was performed. All patients underwent color Doppler ultrasound and SPECT/CT before operation. Taking the surgical pathological results as the gold standard, the significance of SPECT/CT examination and color Doppler ultrasound examination for the preoperative localization of parathyroid nodules was analyzed. Results:The sensitivity of color Doppler ultrasound and SPECT/CT for diagnosing ectopic parathyroid nodule were 70.9% and 66.9%, respectively, and the difference was not statistically significant (χ 2=2.773, P=0.428). The combined of color Doppler ultrasound and SPECT/CT was superior to the single application of color Doppler ultrasound (χ 2=5.161, P=0.023) and SPECT/CT (χ 2=3.735, P=0.041) for diagnosing ectopic parathyroid nodules. Conclusions:For SHPT patients who require parathyroidectomy, the combined application of ultrasound and SPECT/CT before surgery can improve the sensitivity and specificity of the detection of parathyroid nodules, thereby increasing the success rate of surgery.

ABSTRACT

Objective To explore potential related factors of left ventricular hypertrophy in maintenance hemodialy?sis (MHD) patients. Methods Sixty patients were enrolled in the study. Echocardiography was performed to exam LVH in?dexes. There are forty-three patients with LVH (LVH group), and seventeen patients without LVH (non-LVH group). Clini?cal data were collected to exam the levels of hemoglobin (HGB), creatinine (Cr), parathyrin (iPTH), albumin (ALB), triglycer?ide (TG), cholesterol(CHO), calcium(Ca), phosphorus(P), advanced oxygen protein products (AOPP) and C reactive protein (CRP). Results Compared with non-LVH group, age, BP, iPTH, AOPP were higher in LVH group patients while BMI and other biochemistry index does not show any statistical difference. Multiple linear stepwise regressions analysis shows that SBP, iPTH and AOPP are correlated factors to left ventricle mass index (LVMI). Conclusion LVH is prevalent in MHD pa?tients. BP, iPTH and AOPP are the risk factors for LVH.

ABSTRACT

BACKGROUNDIt has been proven that clusterin is a newly apoptosis-related factor and upregulates in many tumors. It plays important roles in carcinogenesis and tumor progression. The antiapoptosis of clusterin seems to be relative to other antiapoptosis factors. The aim of this study is to investigate the expression and significance of clusterin in non-small cell lung cancer (NSCLC) tissue.

METHODSThe expression of clusterin, p53 and Bax in NSCLC were detected by immunohistochemical SP method and Western blot assay.

RESULTSPositive expression rate of clusterin was 79.25% (42/53) in NSCLC tissues which was much higher than that in normal lung tissue (2/16, 12.50%) (Chi-square=23.68, P < 0.05). The expression of clusterin was closely related to pathological differentiation (rs=0.464, P < 0.01), clinical stage (rs =0.320, P < 0.01) and lymph node metastasis (rs=0.255, P < 0.05), but not correlated to the sex (Chi-square=0.007, P > 0.05), age (Chi-square=0.707, P > 0.05) and histological type (Chi-square=0.702, P > 0.05). The expression of clusterin in NSCLC was positively correlated to the expression of p53 (rs=0.589, P < 0.01), but was negatively related to the expression of Bax (rs =-0.346, P < 0.01). The relative expression level of clusterin protein in NSCLC was significantly higher than that in normal lung tissue (0.541±0.010 vs 0.201±0.020) (P < 0.05).

CONCLUSIONSClusterin may play an important role in the biological characteristics of NSCLC by the antiapoptosis pathway.

ABSTRACT

Human interleukin-15 (IL-15) is a proinflammatory cytokine to suppress neutrophil apoptosis, which is a potential therapeutic agent. The modulatory effect of TNFα was investigated in IL-15-induced suppression of human neutrophil apoptosis. TNFα was shown to reverse the ability of IL-15 to delay neutrophil apoptosis within certain time course. Moreover, this reverse effect by TNFα might be associated with a reduction of the expression of the anti-apoptotic Bcl-Xl protein detected by Western blotting. It is concluded that TNFα can be used to modulate IL-15-induced suppression of neutrophil apoptosis within certain time course.

ABSTRACT

OBJECTIVE: To establish the fingerprints of Lonicera similis from Sichuan and Chongqing.METHODS: Chlorogenic acid was taken as the reference substance. The separation was carried out on Dikma Kromasil C18(250 mm?4.6 mm,10 ?m) column with methanol-0.1% phosphoric acid solution as mobile phase(in gradient elution) under a detection wavelength of 326 nm.RESULTS: A total of 11 common peaks appeared in the 10 batches of L.similis samples, and the parameters of the common peaks were up to the standard of the specification for fingerprints of Chinese medicine injections.CONCLUSION: The established HPLC fingerprints can be used as a reference for the quality evaluation and variety discrimination of L.similis.
