ABSTRACT

Objective To evaluate the value of the systemic immune-inflammatory index (SII), CEA, Cyfra21-1, and NSE in predicting and diagnosing bone metastasis of lung cancer. Methods The clinical data of 618 patients with lung cancer were retrospectively analyzed. According to the bone metastasis at baseline, the data of the diagnosis group (patients with bone metastasis at baseline and patients without bone metastasis during follow-up) and the prediction group (patients with bone metastasis during follow-up and patients without bone metastasis during follow-up) were analyzed to determine the correlation between the above indicators and lung cancer bone metastasis. Results Predictive group: SII≥850 and NSE≥58.64 ng/ml were independent risk factors and independent predictors for lung cancer bone metastasis. The AUC of the combined SII+NSE model was 0.662, with a sensitivity of 54.5% and a specificity of 74.5%; it was superior to the predictive value of single factor (95%CI: 0.596-0.728; P < 0.001). Diagnostic group: lung adenocarcinoma, SII≥951.6, CEA≥5.14 ng/ml, NSE≥20.15 ng/ml, and Cyfra21-1≥3.94 ng/ml were independent risk factors for bone metastasis in lung cancer patients (P < 0.05). The AUC of SII alone in the diagnosis of lung cancer bone metastasis was 0.754. The AUC of the SII+Cyfra21-1 combined model was 0.82 which was the largest, with a sensitivity of 74% and a specificity of 78.5%; it was superior to any univariate AUC (P < 0.05). Conclusion The levels of SII, CEA, Cyfra21-1, and NSE in the bone metastasis group are significantly higher than those in the non-bone metastasis group. The predictive and diabnostic values would be improved further when SII combined with other single risk factors.

ABSTRACT

Multiple markers that related to gastric cancer has been widely used in clinical, to predict its occurrence, development and prognosis. Serum level of fibrinogen (FIB) and neutrophil to lymphocytes ratio (NLR) respectively as representative indicators of coagulation system and inflammation, can predict the prognosis of patients with gastric cancer and survival time. The rise of fibrinogen is associated with enhanced capacity of the gastric tumor cells proliferation, invasion and migration. High NLR is associated with significantly induced survival of patients with gastric cancer. The combination of fibrinogen and NLR (F-NLR) combines the advantages of each individual indicator in predicting the occurrence, development and prognosis of cancer. It has shown a great prognostic potential in gastric cancer, and can be widely applied in prospect.

Targeted therapy and immunotherapy are important methods for the treatment of metastatic melanoma, but monitoring the patient′s disease progress and treatment response during the treatment is the key to accurate treatment and personalized treatment.Circulating tumor DNA (ctDNA) as a non-invasive " liquid biopsy" method, due to its high sensitivity and specificity for gene mutations, and the advantages of being qualitative, quantitative and traceable, which has shown great application value in monitoring metastatic melanoma progression, evaluating efficacy, and predicting prognosis during targeted therapy and immuno-therapy.

ABSTRACT

ABSTRACT

Objective To detect the expression of osteopontin (OPN) in the pancreatic tissues of chronic pancreatitis (CP) rats,and analyze the relationship between the expression of OPN and pancreatic fibrosis.Methods A total of 20 male Wistar rats were randomly divided into control group and CP model group.Control group was treated by tail vein injection of absolute ethanol and glycerol,and CP model group received a single tail vein injection of dibutyltin dichloride (DBTC) to construct CP model.Six weeks after the procedure,the rats were sacrificed to obtain pancreatic tissue,and pathological examinations were routinely performed.Sirius-red staining was used to assess the degree of pancreatic fibrosis.The expression of OPN,α-SMA and Col I in the pancreas of CP rats was detected by immunohistochemistry and Western blot,and the relationship between the expression of OPN and the degree of fibrosis was analyzed.Results In CP model group,3 rats died.In the 7 rats which survived,light microscopic examination observed great proliferation of fibrous tissue in pancreatic interlobular space,vacuolization and necrosis of acinous cells,the infiltration of inflammatory cells in the pancreatic interlobular space and dilation of pancreatic duct.Results of Sirius-red staining showed that a large number of fresh red stripe and reticular collagen fibers were deposited in the interlobular space of the pancreatic tissues in the CP group,whereas there were only a few fiber deposited around the blood vessels and the pancreatic duct in the control group and the stained area in CP group (51 ± 11)% was significantly larger than that of control group (9 ± 4)% and the difference was statistically significant (P < 0.01).The results of immunohistochemistry showed that the expression of OPN in CP group was strong positive in 3 rats and positive in 4 rats,which in control group was weak positive in 3 rats and negative in 7 rats.OPN expression in CP group was significantly higher than that in control group (Z =-5.170,P < 0.01),and the expression of OPN was positively correlated with the Sirius-Red staining area (r =0.790,P < 0.01).Western-blot results showed that the expression of OPN,α-SMA and Col I in CP group was significantly higher than that in control group (0.70 ±0.22 vs 0.24 ±0.11,0.71 ± 0.10 vs 0.06 ± 0.01,2.83 ± 1.42 vs 0.39 ± 0.07),and the differences were statistically significant (all P < 0.05).Conclusions A single tail vein injection of DBTC can be used to successfully establish CP rat model.The expression of OPN is significantly increased in the CP group and is positively correlated with pancreatic fibrosis.

ABSTRACT

Circulating tumor DNA (ctDNA) refers to the DNA segment of the free tumor located in the peripheral circulation,which carries the biologic information derived from the tumor.In recent years,the innovation of the second generation sequencing technology has expanded the prospect of the clinical application of ctDNA analysis.Detection of ctDNA has made a great progress in the field of radiation therapy,especially in the aspect of evaluating the sensitivity of radiotherapy,ctDNA in peripheral blood is characterized by convenient extraction and high specificity.Detection of ctDNA is not only conducive to find minimal residual disease and metastasis,but also help clinicians better evaluate the curative effect and judge prognosis after radiotherapy,which will provide a new way of thinking for the diagnosis and treatment strategy of tumor.

ABSTRACT

Objective To observe the changes of expression levels of NMDA receptor NR2A and NR2B in anterior cingulate cortex (ACC) of chronic pancreatitis (CP) rats and explore its roles in the pathogenesis of pain in CP.Methods Male adult SD rats were randomly divided into CP group,control group and normal group using random number method.CP rat model was established by injecting 8mg/kg dibutyltin dichloride (DBTC) through tail vein.Control group was injected with a equal volume mixture of ethanol and glycerol via tail vein.After 4 weeks,yon Frey hair of different sizes (3.85,5.50,7.05,10.4,17.8 g) were used to stimulate the abdomen of the rats and the percentage of the positive pain response was recorded.Then the rats were sacrificed.The pancreas was collected for pathological examination.NR2A and NR2B subunit mRNA and protein expression in ACC was detected by realtime-PCR and Western Blotting,respectively.Results The success rate of CP model establishment was 60%.As Van Frey hair used to stimulate the rat abdomen increased from 3.85 g to 17.8 g,the percentage of positive pain response increased from (38.33 ±7.53)% to(73.33±8.17)% in CP group,from (7.80±6.70)% to (34.44±5.27)% in control group,from(6.57±5.48) % to(33.33 ± 5.00) % in normal group,which was increased with the increase of the stimulation intensity.For each stimulation intensity,the percentage of positive pain response in CP group was all obviously higher than those in control and normal group,and the differences were all statistically significant (all P < 0.01),but there was no statistical difference between control group and normal group.The protein expression of NR2A of ACC in CP group,control group and normal group was 1.25 ± 0.78,0.95 ± 0.14 and 0.91 ± 0.09,respectively.The protein of NR2B in three groups were 1.44 ± 0.12,0.93 ± 0.08 and 0.94 ±0.04,respectively.NR2A and NR2B protein expressions in the CP group were both significantly higher than those in control group and normal group,and the difference was statistically significant (P<0.01).The mRNA expression of NR2A in three groups were 1.43 ± 0.20,0.80 ± 0.10 and 1.01 ± 0.13,respectively.The mRNA expression of NR2B in three groups were 1.40 ± 0.09,0.98 ± 0.14 and 0.94 ± 0.05,respectively.The mRNA expressions of NR2A and NR2B in CP group were significantly higher than those in normal and control group,and the difference was statistically significant (P <0.01).Conclusions The expression of glutamate receptor NR2A and NR2B in ACC were upregulated in CP rats and may be involved in the development of the pain.

Objective To explore the role of the visceral afferent nerve hyperesthesia and acid-sensing ion channel 1 (ASIC1) in rats with reflux esophagitis (RE).Methods Sixty male Sprague-Dawley rats were selected and animal model was established.Rats were divided into control group (n=20) and RE group (n=40).The esophageal mocosa biopsy were routinely performed in two groups.The esophageal specific DRG neurons were identified by 1,1'dioctadecyl-3,3,3',3'-tetramethylindocarbocyanine perchlorate tracing method and the whole-cell patch clamp assay was performed.The expression of ASIC1 in esophageal mucosa and thoracic spine cord three to five segments at protein level and mRNA level were detected by Western blotting and quantitative real time-polymerase chain reaction (qPCR).Two independent samples t test was performed for statistical analysis.Results The body weight of RE group was significantly lower than that of control group ((179.41±-16.38) g vs (290.75 ±-22.20) g),and the difference was statistically significant (t=17.090,P< 0.01).Esophageal basal cell hyperplasia,papillary elongation,vascular dialation and congestion,inflammatory cells infiltration were found in RE group rats.The results of whole-cell patchclamp showed depolarization of the resting potential of esophageal-specific DRG neurons of RE group was more significant than that of control group (-(46.20 ± 1.92) mV vs-(51.60 ± 1.52) mV),and the difference was statistically significant (t=4.930,P<0.01).The threshold current of RE group was much lower than that of control group ((18.00±13.04) pAvs (80.00±12.25) pA),and the difference was statistically significant (t=7.750,P<0.01).When stimulated with two to three times the threshold current,the frequency of action potential of RE group significantly increased (5.80 ±1.48 vs 3.00 ±1.58,10.60±2.30 vs 5.20±1.92),and the differences were statistically significant (t=2.890 and 4.030,both P<0.01).The results of Western blotting indicated that the expression of ASIC1 in esophageal mucosa of RE group was significatly lower than that of control group (0.614±0.120 vs 0.976±0.283),and the difference was statistically significant (t =2.885,P< 0.05),while there was no statistically significant difference in the expression of ASIC1 in DRG between RE group and control group (0.804 ± 0.182 vs 1.032±0.316;t=1.528,P>0.05).The results of qPCR showed that the expression of ASIC1 mRNA in esophageal mucosa of RE group was lower than that of control group (0.694 ± 0.118 vs 1.036 ±0.137),and the difference was statistically significant (t=4.642,P<0.01).However there was no statistically significant difference in ASIC1 at mRNA level between RE group and control group (1.002± 0.074 vs 0.985±0.120;t=0.294,P>0.05).Conclusion The sensitivity of esophageal visceral afferent nerve of rats in RE group increases and ASIC1 may negatively regulate the formation of esophageal visceral hypersensitivity.

ABSTRACT

Objective To evaluate the adenoma detection rate( ADR)of fecal occult blood test (FOBT)-positive population in colorectal cancer screening programme and to analyse potential influence of“resection and discard”strategy on ADR. Methods Data of patients who paticipated in the Shanghai color-ectal cancer screening programme with FOBT-positive and received colonoscopy in the Digestive Endoscopic Center of Changhai Hospital from July 2013 to July 2014 were retrospectively analysed. ADR was calculated and compared by different genders. Multivariate logistic regression model was used to analyse the risk factors of polyp resection without retrieval. Results A total of 222 FOBT-positive patients were involved with 36. 5% male proportion. The total ADR was 19. 8%,higher in male(28. 4%)than in female(14. 9%)(P=0. 015). The independent risk factors of polyp resection without retrieval were diminutive polyp( OR =15. 256,95% CI:4. 159-55. 957),located in rectum( OR = 3. 663,95% CI:1. 427-9. 398) and polyp number >2(OR= 3. 988,95%CI:1. 562-10. 187).Conclusion ADR of FOBT-positive population is approx-imately 20%in our center. Low male proportion and“resection and discard”strategy may lead to lower ADR. ADR should be calculated by different genders and advanced endoscopic technology should be employed rou-tinely to predict the pathological diagonosis of the lesions.

ABSTRACT

Objective To investigate the role of p38 MAPK in the trypsin-induced injury in human esophageal epithelial cells.Methods Primary cultured human esophageal epithelial cells were stimulated with trypsin (20,40,and 80 μg/ml) for4 hours,phosphorylation of p38 MAPK was evaluated by Western blotting.Primary cultured human esophageal epithelial cells were stimulated with trypsin (40 μg/ml) and treated with p38 MAPK inhibitor (SB203580,1 and 10 μmol/L) simultaneously.Four hours later,the cells were collected for analysis.Results Western blotting results revealed that stimulation with trypsin enhanced phosphorylation of p38 MAPK,indicating that trypsin activated p38 MAPK in esophageal epithelial cells.SB203580 treatment suppressed trypsin-induced expression of pro-inflammatory cytokines including interleukin-8 (IL-8),cyclooxygenase 2 (COX2),and tumor necrosis factor alpha (TNFcα).Finally,SB203580 treatment suppressed trypsin-induced upregulation of protein expression of inducible nitric oxide synthase (iNOS),and subsequently reduced nitric oxide (NO) levels.Conclusions The regulation of p38 MAPK was involved in the trypsin-induced injury in esophageal epithelial cells.

ABSTRACT

Objective To investigate the risk factors of multiple recurrent bile duct stones in patients with common bile duct (CBD) stones and received endoscopic sphincterotomy (EST ) treatment . Methods From June 1st ,2007 to June 1st ,2011 ,the patients received EST treatment were followed up , the follow‐up time was three to seven years .A total of 2 738 patients were successfully followed up , including 24 patients with two or more than two times of recurrent bile stone after EST who were enrolled in multiple recurrence group .One hundred patients without recurrence were randomly selected by table of random number as control group .The clinical data of two groups were compared by t test or Chi‐square test .The factors were analyzed with multivariate Logistic regression analysis and try to find out the risk factors of multiple recurrence .Results The comparative analysis between two groups showed that the three factors of juxtapapillary duodenal diverticula [17 .0% (17/100 ) vs 41 .7% (10/24 )] ,history of biliary tract surgery [13 .0% (13/100) vs 45 .8% (11/24)] and the angle enclosed between the horizontal portion of the CBD and the horizontal plane (angleofbile duct) less than 45° [2 .0% (2/100) vs 20 .8% (5/24)] were statistically significant (χ2 = 6 .914 、13 .37 、9 .595 ,all P < 0 .01 ) . Furthermore , logistic regression analysis indicated that juxtapapillary duodenal diverticula (B = 1 .421 ,OR = 4 .142 ,P = 0 .01) , history of biliary tract surgery (B = 1 .612 ,OR = 5 .011 ,P= 0 .004) and the angle of bile ductless than 45° (B= 2 .661 ,OR= 14 .31 ,P= 0 .005) were risk factors of multiple recurrence .Conclusion Juxtapapillary duodenal diverticula , history of biliary tract surgery , and the angle of bile duct less than 45° are independent risk factors of multiple recurrence of CBD stones .

ABSTRACT

ObjectiveTo objectively evaluate the clinical efficacy of three-step acupuncture-cupping therapy for cervicogenic headache.MethodSixty patients were randomly allocated to treatment and control groups. The treatment group received three-step acupuncture-cupping therapy and the control group, conventional acupuncture based on “Acupuncture Therapeutics”. Both groups were treated for three courses (10 days as acourse with two days of rest between two courses) and then the clinical therapeutic effects were evaluated. The evaluation included the overall therapeutic effect on the symptoms, the VASscore and the follow-up recurrence rate at six months after treatment.ResultThe total efficacy rate was 96.7% in the treatment group and 83.3% in the control group; there was a statistically significant difference between the two groups (P<0.01). A follow-up of six months after treatment in the two groups showed that the recurrence rate was 16.7% in the treatment group and 46.7% in the control group;there was a statistically significant difference between the two groups (P<0.05).ConclusionThree-step acupuncture-cupping therapy has a definite effect on cervicogenic headache. Its immediate and long-term effects are superior to those of conventional acupuncture. The recurrence rate in six months is lower in this therapy than in conventionalacupuncture.

ABSTRACT

Objective: To explore clinical observation of magnesium sulfate and nifedipine treatment of pregnancy-induced hypertension. Methods:Retrospective analysis from December 2009 to November 2011 in our hospital was conducted. Sixty cases of pregnant women with severe pregnancy-induced hypertension divided into two groups. The clinical efficacy and incidence of adverse reactions were observed in patients. Results: The overall response rate(90.0%)was significantly higher overall response rate (73.3%) in observation group. The observation group patients(13.3%) was significantly lower than the control group, the totalefficiency (23.3%),the result is significant(t=2.240, P<0.05). Conclusion: Magnesium sulfate and nifedipine treatment of pregnancy-induced hypertension has significant efficacy and good safety.

Objective To establish in vitro model for perineural invasion (PNI) of pancreatic cancer,and observe the process of PNI.Methods Mouse dorsal root ganglia (DRG) and pancreatic cancer Capan-2 cell line were co-cultured in Matrigel matrix.Two models were constructed:model 1 for observing areas of cell colonies and model 2 for observing distance of cell migration.Neurite outgrowth and cell colony growth were observed by invert microscope,images were analyzed by using Image pro plus software.Results Neurite outgrowth was not affected by co-culture.DRG neurite appeared radial growth under either co-culture or single culture.It was observed that Capan-2 cell could migrate to DRG,and grows around the nerve fiber as spindleshaped,then continue to migrate to DRG.At the 5th day,in the co-culture group of model 1,the colony area was (309.28 ±19.11) μm2,which was significantly bigger than that in single culture model [(208.57±7.94) μm2,P <0.01].At the 5 th day,in the co-culture group of model 2,Capan-2 migrated (284.1 ±12.9) μm to DRG,while in the side of blank Matrigel,cell migrated less than 150 μm,the difference was statistically significant (P <0.01 ).Conclusions The in vitro models for perineural invasion in pancreatic cancer were constructed successfully,which lay a good foundation for future studies.

ABSTRACT

In total, 4006 women aged 25 -65 years at Qiaozi Town, Huairou District, Beijing were screened for cervical cancer during April to June, 2009. Results showed that 54. 52% of the women screened suffered from varied gynecological illness, highest in those aged 30 - 34 years ( 80. 90% ) and lowest in those aged 50 -54 years. The top three gynecological morbidity among them were cervicitis (34. 77% ), virginities ( 12. 88% ) and uterine leiomyoma (8. 34% ). Precancerous lesions in 12 women,endometrial cancer in one woman and cervical cancer in one woman were detected by cervical cytological examinations.

ABSTRACT

Objective To investigate high risk human papillomavirus(HR-HPV)prevalence among married women in Beijing and to study the high risk flactors.nethods During March 2007 to September 2008.a total of 6185 married women sampled from 137 communities in 12 districts were screened bv HR-HPV DNA test and cytogical test.The interview was carried out with unified questionnaires.The databage was set up and twice entered in EpiDam 3.0.After checked up,the data were analyzed in SPSS 15.0.Results (1) The HR-HPV infection rate was 9.89%.The HR-HPV infection rate of the city zone,the suburb and the exurb were 9.34%,10.51% and 9.51% (P>0.05).The HR-HPV infection rate of the native and the oudander were 9.53%,11.30% (P<0.05).(2) The age distribution of HR-HPV infection was that the rate was around 10% among 25 to 44 age groups,which was the highest(11.21%) in 30 to 34 age group;then the rate was descended as the age raising,the rate of 50 to 54 age group was the lowest(7.78%).(3) Multiple logistic regression showed that the related risk factors of HR-HPV infection mainly included 1000 RMB and above of family income per person per month.possessing more than 1 sexual partner of her husband,outlander and hish levels of education.(4) The prevalence of cervical intraepithelial neoplasia(CIN)in HR-HPV positive group wag significantly higher than that in HR-HPV negative group(29.76% vs 3.32%,P<0.01).Conclusions(1)The HR-HPV infection rate among aged 25 to 54 years was 9.9% and there was no significant difference in area distribution.(2)The hish risk population which should strengthen screening was the married bearing-age women with high level of family income,outlander,high levels of education and her husband possessing more than 1 sexual partner.(3)HR-HPV infection is the main risk factor for CIN and cervical cancer.while does not provide a causal relationship with them.The high risk population should be checked regularly to understand the development of HR-HPV infection and CIN incidence.

ABSTRACT

Objective To study the detection and significance of serum ICAM-1 and E-selectin in patients with chronic obstructive pulmonary disease (COPD).Methods Double antibody sandwich quantitative enzyme fined immunosorbent assay (ELISA) method was used to measure the serum levels of serum ICAM-I and E-selectin in 60 patients with COPD,52 exacerbated patients,52 stable patients and 30 normal control subjects.Results The levels of serum ICAM-1 and E-selectin in COPD group and exacerbated patients were significantly higher than those in normal controls (P<0.01) ; these in stable patients were significantly lower than those of exacerbated patients (P<0.01),and significantly higher than those exacerbated patients(P<0.05).Conclusion ICAM-1 and E-selectin were involved in occurrance and development process of COPD.

ABSTRACT

Objective To develop a pathology information system and provide a pathology testing platform with digitalization,normalization and optimization. Methods Based on the principal and requirement of information engineering theory to analyze the profession,flow-sheet management and data needs of pathology information system. The pathology examination and related testing were analyzed mainly,included some special requirements in installation of work position,quality control,cost management,which were transformed conceptual and logical structure of database and realized physical composition on Oracle database. The reciprocal software codes were researched by PB program,which can build database in servers and install application software in front end computer. Results The system that was specified the components of the clinical data entry,pathology examination registry,clinical document repository,EMR application server and the e-node accessing server. The system was tested and the result indicated that the system was an effective and scalable. Conclusion The pathology information system can be not only used realize the sharing of EMR documents across multiple hospital,but also support the collection clinical data within hospital clinical system integration.
