ABSTRACT

Objective@#To investigate the status of knowledge about acquired immune deficiency syndrome(AIDS) among college freshmen in Jiangsu Province, and to analyze the influences of AIDS-related knowledge on their sexual attitudes and behaviors.@*Methods@#A total of 8 633 freshmen from 20 colleges and universities in Jiangsu Province were enrolled by multi-stage stratified sampling. The questionnaire for young students about the eight items of AIDS basic knowledge were used to investigate the AIDS-related knowledge, sexual attitudes and behaviors. Multivariate Logistic regression model was used to analyze the influencing factors of the AIDS-related knowledge and to adjust the influences of AIDS-related knowledge on sexual attitudes and behaviors.@*Results@#The overall qualified rate of AIDS-related knowledge among college freshmen in Jiangsu Province was 87.6% and the qualified rate of 8 items were between 59.8% and 98.3%. The types of schools, nationality, mother s education level and whether only child or not were identified as independent factors for qualified rate of AIDS-related knowledge(P<0.05). The awareness of AIDS-related knowledge had significant impacts on the sexual attitudes and behaviors of college freshmen. Students with insufficient knowledge were more likely to approve premarital sexual behaviors(OR=1.31, 95%CI=1.11-1.63), willing to use condoms consistently(OR=1.67, 95%CI=1.43-1.92) and to accept the HIV tests(OR=4.54, 95%CI=3.13-6.52). Students with unqualified knowledge were more likely to have sexual behavior(OR=1.89, 95%CI=1.42-2.49), while those with sexual experiences and insufficient AIDS knowledge were more likely to use condoms consistently(OR=2.13, 95%CI=1.22-3.78).@*Conclusion@#The education on AIDS-related knowledge for teenagers was helpful to construct correct sexual concepts and develop healthy sexual behaviors. However, the AIDS-related knowledge of college freshmen in Jiangsu Province is not sufficient and needed to improve.

[Abstraet] Alteplase intravenous thrombolysis is the most effective drug treatment for acute ischemic stroke at present.Hemorrhagic transformation after thrombolysis,especially symptomatic intracranial hemorrhage (sICH),will seriously affect the outcomes after intravenous thrombolysis,and even threaten the life of patients.Therefore,the prediction and treatment of sICH after intravenous thrombolysis is particularly important.This article reviews the prediction and treatment of sICH after intravenous thrombolysis.

ABSTRACT

Objective To study the association between serum homocysteine (Hcy) level and cognitive impairment in PD patients.Methods Eighty-one PD patients admitted to our hospital were divided into normal cognition group (n=25),mild cognitive impairment group (n=32) and dementia group (n=24) with 19 healthy persons undergoing physical examination served as a control group.Their cognitive impairment was assessed according to their medical history,detailed physical examination data,Hoehn-Yahr scale score,MMSE score,and CDR score.Their serum levels of Hcy,folic acid and vitamin B12 were measured.The association of serum Hcy,folic acid and vitamin B12 levels with cognitive impairment was analyzed.Results The serum Hcy level was significantly higher in normal cognition group,mild cognitive impairment group and dementia group than in control group (P<0.05,P<0.01) and in dementia group than in normal cognition group (14.8±3.9 μmol/L vs 12.5±3.3 μmol/L,P<0.05).The serum Hcy level was associated with oral levodopa and oral benserazide daily dose,oral levodopa and oral benserazide taking time,course of PD and age (r=0.298,P=0.000;r=0.280,P=0.000;r=0.301,P=0.000;r=0.184,P=0.019) but not associated with the severity of PD.Conclusion High serum Hcy level is one of the risk factors for cognitive impairment in PD patients,and control of oral levodopa dose contributes to the prevention and treatment of dementia in PD patients.

ABSTRACT

Objective To compare the clinical effect of minimally invasive plate osteosynthesis (MIPO) and open reduction internal fixation(ORIF)for treating proximal humeral fractures.Methods The clinical data of 65 patients with proximal humeral fractures treated by surgery from February 2011 to January 2013 were collected.The patients were divided into MIPO group (26 cases) and ORIF group (39 cases) according to the surgery mode.The situation of operation and hospitalization was compared between the two groups.The patients were followed up by clinic and telephone modes.The Neer scores and EuroQol 5 dimensions scores (EQ-5D) were adopted to evaluate the function of shoulder joint and quality of life after discharge.The visual analogue scale (VAS) was adopted to evaluate the pain degree.Results Compared with the ORIF group,the MIPO group had less intraoperative bleeding,short operation time and hospital duration,while the incidence rate of complications showed no statistically significant difference between the two groups (P>0.05).All cases in both groups were followed up for more than 3 years and the mean follow-up period was (47.2±6.1) months.The shoulder joint function scores at last follow up in the MIPO group and ORIF group were (84.1±9.3) points and (82.8+11.5) points,the living quality scores were (0.91+0.09) points and (0.89+0.10) points and the VAS scores were (0.9±0.8) points and (1.2+0.7) points respectively.The scores of various items showed no statistically significant difference (P>0.05).Conclusion Both MIPO and ORIF obtain the good clinical effect in treating proximal humeral fracture,and the MIPO technique has the advantages of small trauma and rapid recovery.

ABSTRACT

BACKGROUND:Pathogenesis of Parkinson’s disease is not completely understood, and there is yet no effective therapy that can prevent the neurodegenerative process of the disease fundamentaly. OBJECTIVE:To explore the effects of pituitary adenylate cyclase-activating polypeptide (PACAP) on lactacystin-induced Parkinson’s disease dopaminergic PC12 cel apoptosis and its molecular mechanism. METHODS: Under induction by nerve growth factors, PC12 cels differentiated into dopaminergic neurons, and then were treated with different concentrations of lactacystin for different time. When the cel survival rate was about 50%,the concentration and action time oflactacystin were selected to establish cel models of Parkinson’s disease. In the study, there were control group, lactacystin group, PACAP1-27 group (intervention group 1) and PACAP1-27+PACAP6-27 co-intervention group (intervention group 2). Changes of cel morphology were observed under inverted microscope; cel viability was detected with MTT method; the expression of endoplasmic reticulum stress specific protein caspase-12 was detected by western blot. Then the action of PACAP1-27 and PACAP6-27 to the cytoxicity of lactacystin was observed. RESULTS AND CONCLUSION: With different concentrations and action time of lactacystin, the viability of PC12 cels presented a concentration- and time-dependent decline. When the lactacystin at 20μmol/L acted for 24 hours, the cel viability was declined by about 50%. Under same conditions of lactacystin concentration and action time (20 μmol/L, 24 hours), the cels in the lactacystin group appeared to have damaged changes, declined cel viability, and increased caspase-12 activity in comparison with the control group (P< 0.01). Compared with the lactacystin group, the cel damage was relieved and cel viability was increased significantly in the intervention group 1 as wel as the expression of caspase-12 was decreased (P < 0.01). Experimental findings in the intervention group 2 were similar to those in the lactacystin group. These results suggest that lactacystin, an ubiquitin proteasome inhibitor, can lead to cel damage; PACAP1-27 plays a protective role by regulating the above-mentioned signal pathway. As one PACAP1-27 receptor antagonist, PACAP6-27 can attenuate this effect of PACAP1-27.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the clinical feature of early newborn infants with congenital heart disease.</p><p><b>METHODS</b>We retrospectively analyzed the clinical features of 477 newborn infants with congenital heart disease born within seven days out of 28 050 live births in Shaoxing women and children hospital from October 1, 2009 to September 30, 2012. Infants with congenital heart disease were divided into single malformation group (240 cases), composite deformity group (199 cases) and multiple malformations group (38 cases). Differences of clinical feature were compared between the three groups.</p><p><b>RESULTS</b>Atrial septal defect was the most malformation 91.6% (437/477) .Incidence of preterm birth was higher in newborn inants with congenital heart disease [512.23/10 000(134/2 616)] than infants without without congenital heart disease [134.86/10 000 (343/25 434) , P < 0.01]. Proportion of preterm birth among the three congenital heart disease groups was similar (P > 0.05) . The incidence of small for gestational age in congenital heart disease group (10.90%, 52/477) was also significantly higher than those without congenital heart disease group (5.91%, 1 630/27 573, P < 0.01).Small for gestational age with congenital heart disease of complex malformations, multiple malformations groups was higher than that in the single malformation group (P < 0.01 and P < 0.05).</p><p><b>CONCLUSIONS</b>Early newborn with atrial septal defect is the most common neonatal congenital heart disease. The incidence of preterm is higher in newborn infants with congenital heart disease. Complex and multiple malformations are linked with small for gestational age birth weight.</p>

ABSTRACT

Objective To evaluatethe application value of APACHE Ⅱ on the prognoses in prediction of pin hole multi-directional intracranial hematoma aspiration to cure severe cerebral hemorrhage.Methods APACHE Ⅱ scoring system was conducted in randomly collected 58 severe cerebral hemorrhage patients' data (30 patients underwent surgical treatment,28 patients underwent conservative treatment)to evaluate the disease severity,prognosis and effect of the pin hole multi-directional intracranial hematoma aspiration.Results The APACHE Ⅱ score of treatment group on the first day was 28.00 ± 1.92 and the predicted mortality was 0.86 ±0.03.The score was decreased obviously on the tenth day(8.20 ± 0.76)and the predicted mortality was 0.57 ± 0.05.The actual mortality was 33.33%.Conclusion APACHE Ⅱ scoring system is suitable for judging the disease severity and predicting the prognosis of severe cerebral hemorrhage patients,it proves that surgical treatment effect of the pin hole multi-directional intracranial hematoma aspiration is remarkable.

ABSTRACT

Objective To investigate the alteration of phosphorylated GluR1Ser831 and behavioural effects in a rat model of levodopa-induced motor complications after Ca2 +/calmodulin-dependent protein kinase Ⅱ (CaMKⅡ) inhibitor KN-93 treatment. Methods The hemi-parkinsonian rat model was produced by injecting stereotaxically 6-OHDA to right medial forebrain bundle. Then, rats were intraperitoneally treated with levodopa ( 50 mg/kg with benserazide 12.5 mg/kg,twice daily) for 22 days. On day 23 ,rats received KN-93 before levodopa administration. Rotational duration was estimated. After sacrificed, subcellualr distribution of GluR1 and phosphorylated GluR1Ser831 were observed by western blot. Results The study showed that CaMKⅡ inhibitor KN-93 prolonged rotational duration. Moreover, KN-93 could regulate subcellular distribution of GluR1 and reduce hyperphosphorylation of GluR1 Ser831, which was closely associated with levodopa-induced motor complications. The expression of membrane GluR1 and phosphorylated GluR1Ser831 was (83.4 ±4.2)% and (47.2 ±5.2)% ,respectively. Conclusions These results indicated that activation of CaMKⅡ contributed to development of motor complications, through a mechanism that involved an increase in phosphorylated GluR1 Ser831. Pharmaceuticals which act to inhibit CaMKⅡ may be useful in the treatment of the motor complications in parkinsonian patients.

ABSTRACT

BACKGROUND: The key point in the treatment of acute cerebral infarction lies in inproving the blood supply of brain effectively as soon as possible as well as rescue ischemic and half diazonal nerve cells, so as to contract the local of infarction as great as possible.OBJECTIVE: To observe the effect of traditional treatment of cerebral piamatral microcirculation in gerbil with cerebral ischemic reperfusion injury by supplementing qi and activating blood circulation.DESIGN: Contrast animal experiment at random.SETTING: Zhabei Branch of Shanghai Changzheng Hospital.MATERIALS: The experiment was performed in the Institute of Naval Medicine (key laboratory of the whole army). Twenty-two healthy Monglian gerbils of clean grade (six months old) were selected and randomly divided into medicine group and control group with 11 gerbils in each group.METHODS: Animals were routinely anaesthetized and fixed in prone position to open the cranium window in median line and expose the pia mater. The cranium window was protected by being covered with a cotton ball containing artificial cerebrospinal fluid (ACSF) after operation. Gerbils were fixed in dorsal position with an median incision made in the lateral cervical nucleus to isolate and expose bilateral common carotid artery (CCA). The bilateral CCA were occluded by atraumatic micro-bulldog clamp, and the occluded blood flow in the distal end was observed under the microscope. Thirty minutes after occlusion of bilateral CCA, the vascular clamp was unclamped to recover the cerebral blood reperfusion. Gerbils in the medicine group were abdominally injected with 0.4 mL of Huangqi injection (containing 0.8 g of crude drug), 0.3 mL of compound Danggui injection (containing Danggui, Chuanqiong and Honghua, 0.09 g for each)and 0.3 mL of Danshen injection (containing 0.45 g of Shengyao). Animals in the control group were abdominally injected with same-volume of normal saline. The changes of pial microcirculation were quantitatively observed in two groups through cranium window respectively before blockage, at the reperfusion after blockage as well as in 120 minutes after injection by laser microcirculation microscope and the TV video system.MAIN OUTCOME MEASURES: The changes of piamatral microcirculation as well as comparison in venous blood flow velocity in gerbils of each group before and after ischemic reperfusion.RESULTS: All gerbils were involved in the analysis of results with no animal died in the experiment.①When the CCA of animals in the two groups were blocked, obvious contraction of piamatral microcirculation was seen and partial blood vessels were blocked up. Platelet and red blood cells etc.adhered to the arteriolae of grade Ⅰ and veinules below grade Ⅲ with microthrombus formed and large ischemic area appeared. The blood flow rate was obviously slowed down. Moderate and severe aggregations were found in red blood cells, and the white blood cells were significantly increased.There was obvious exudation in the periphery of microvessels. After CCA reperfusion, the arteriolae and veinule were in different thickness, and the thrombus in veinule of grade Ⅰ and Ⅱ formed in the cerebral ischemia were difficult to be removed by blood flow. Red blood cell moderately aggregated, and the number of white blood cells greatly increased. White small thrombus could be seen sometimes, while the exsudation in the periphery of microvessel was not significantly ameliorated. Great changes were found in gerbils of the medicine group after abnormal injection of supplementing qi and activating blood circulation medicine: almost all arteriolae and venulae were opened, and the rate of blood flow was accelerated with the blood flow rate remarkably increased. Deposit in the venulae began to decreased, and the thrombus was gradually dissolved. Red blood cells were moderately aggregated or normal. Blood flow in ischemic spot was improved significantly than that after reperfusion. The exudation in the periphery of microvessels was gradually lessened even dissolved. However,no obvious change was found in gerbils of the control group. ②The blood flow rate of arteriole in the medicine group was 1.04-1.50 mm, which was obviously higher than that in the control group with significant differences (P < 0.5-0.01).③The blood flow rate of veinule in the medicine group was 0.96-1.12 mm/s, which was remarkably higher than that in the control group with marked differences (P < 0.05-0.01).CONCLUSION: The effect on piamatral microcirculation of gerbil with cerebral ischemic reperfusion by supplementing qi and activating blood is exact, which is directly related with accelerated blood flow rate, expanded veinule and arteriolae as well as ameliorated blood-supply of brain.

ABSTRACT

Objective To investigate the effectiveness of repair of soft tissue defects of finger by using the digital artery island flap.Methods A total of 25 cases of soft tissue defects of finger were treated by using the digital artery island flap in this department from August 2003 to November 2004.The flap was 2.0~3.0 cm ? 1.5~3.0 cm in size.Results The flap had survived in all the 25 cases.The operation time was 60~90 min(mean,75 min),and the intraoperative blood loss was 25~40 ml(mean,32 ml).The skin graft necrosed by one-third of area in 2 cases,which was cured by a re-grafting.Follow-up observations in 23 cases for 6~18 months(mean,13.5 months) revealed the flaps with normal color,elasticity,and appearance.The static two-point discrimination was 6.5~8.8 mm(mean,7.5 mm).The digital pain and temperature sensation had recovered to normal,and the patients reported satisfactory sensory and motor functions.Conclusions Characterized by simple performance and satisfactory outcomes,digital artery island flap is an ideal choice for the coverage of soft tissue defects of finger.
