ABSTRACT
Objective To explore and evaluate an extracorporal method for inducing magnetic capsule endoscopy into small intestine. Methods 40 patients receiving magnetic capsule endoscopy were randomly divided in two groups: the control group: doctors stopped manipulating capsule after the examination of stomach, and the capsule entered small intestine by the natural gastrointestinal motility; and the study group: after the examination of stomach, the patient lay on the right side, doctors moved the capsule to the pylorus, and then moved magnetic ball to induce capsule into small intestine. Gastric inspection time, gastric residence time, small intestine transit time and the completion rate were compared between the two groups. Results The average time for checking stomach was (32.50 ± 11.71) min in control group and (31.75 ± 9.12) min in study group respectively, and the difference was not significant (P > 0.05). After the observation of stomach, the gastric residence time in the control group was (40.60 ± 21.43) min, and the completion rate was 40%, while the average gastric residence time in the study group was (13.55 ± 9.62) min, and the completion rate was 75%. The difference between the two groups was statistically significant (P 0.05). Conclusion By doctors moving magnetic ball and the patient lying on the right side after the observation of stomach, gastric residence time could be reduced and the completion rate could be elevated obviously.
ABSTRACT
Objective To summarize the clinical efficacy of Endu combined with intrapleural injection of cisplatin in the treatment of malignant pleural effusion of non-small cell lung cancer.Methods80 patients with malignant pleural effusion were selected from January 2014 to December 2015.The patients were randomly divided into observation group and control group (n=40).The control group was given Endu(40mg/time, 1 time/week) combined with cisplatin (40mg/time, 1 time/week) intraperitoneal injection treatment, the treatment group was given Endu intraperitoneal injection therapy, After 4 weeks of treatment, the clinical efficacy and adverse reactions were observed.ResultsThe clinical efficacy of the observation group was significantly higher than that of the control group (50.00%).The difference was statistically significant (P<0.05).The incidence of adverse reactions in the observation group was 15.00% higher than that in the control group, but the difference was not significant.The improvement rate of life quality in the observation group was significantly higher than that in the control group (52.50%).The difference between the two groups was statistically significant (P<0.05).ConclusionThe clinical efficacy of Endu combined with intrapleural injection of cisplatin in the treatment of malignant pleural effusion of non-small cell lung cancer is superior to that of simple analgesia, which can significantly improve the clinical symptoms and quality of life.It is worthy to be popularized in clinical practice.

Objective To observe the clinical efficacy of acupoint sticking with Chan Wu gel in managing cancer pain.Method A hundred patients were randomized into a treatment group of 50 cases and a control group of 50 cases. The treatment group was intervened by thethree-step analgesic ladder plus acupoint sticking with Chan Wu gel; the control group was by the three-step analgesic ladder alone. The decrease of the pain degree, action and lasting time of analgesia, and adverse reactions in the two groups were observed.Result The treatment group was superior to the control group in comparing the release of pain, action and lasting time of analgesia; the occurrence rate of adverse reactions in the treatment group was obviously lower than that in the control group(P<0.05).Conclusion The conventional three-step analgesic ladder plus acupoint sticking with Chan Wu gel is effective in mitigating cancer pain, and it can reduce the dose of the three-step analgesics and the adverse reactions.

Objective To explore the effect of token on the pain and compliance of wound care of postoperative children with hypospadias, and provide evidence for the pain management and psycho-behavior intervention of children. Methods Selected 80 cases of hypospadias and they were assigned to the experience group and the control group. The control group was guided by conventional nursing intervention method while the experience group was guided by token intervention. Compare the pain score and compliance of two groups. Results The pain score of the experience group were (2.65±1.89), (2.40± 1.73), (2.10±1.43) points, the control group were (4.10±2.52), (3.35±2.09), (2.90±1.75) points. There were significant differences between the two groups (t=2.912, 2.399,- 2.239, all P<0.05). There were significant differences between the two groups for the compliance (χ2=5.415, 7.587, 9.372, all P<0.05). Conclusion Token can relief the pain of wound care of postoperative children with hypospadias, increase the compliance of wound care of the children.
ABSTRACT
Objective To compare the clinical effect of Doxofylline and aminophylline in the treatment of bronchial asthma.Methods The clinical data of 112 patients with bronchial asthma treated in Jiaxing Second Hospital from February 2015 to December 2016 were retrospectively analyzed.The patients were divided into two groups: the treatment group(56 cases), the use of doxofylline treatment, and the control group(56 cases), the use of aminophylline treatment.After 1 w treatment, the curative effect and adverse reactions were compared between the two groups to observe the changes of lung function before and after treatment.Results The total effective rate of the treatment group and the control group were 91.07% and 76.79% respectively(P<0.05).After treatment, the FVC of the treatment group was(2.24±0.63)L, FEV1 was(1.59±0.47)L, VC was(2.94±0.77)L, the incidence of adverse reactions was 10.71%, which was superior to the control group, the difference was statistically significant(P<0.05).Conclusion Doxofylline treatment of bronchial asthma clinical efficacy is remarkable, worthy of clinical promotion.

Objectives:To analyze the influencing factors and status of hospital seeking behavior among floating population by focusing on the function of health insurance , and provide evidence to comprehensively promote tiered health care system in China .Methods:Using data from 2014 National Floating Population Dynamic Monitoring Survey , multivariate logistic regressions were applied to analyze the influencing factors of hospital seeking behavior for inpatient care .Chi-square tests were used to analyze the different proportion of having basic medical insurance reimbursement and reasons for not having it for different hierarchical inpatient care , and T test were used to analyze the ratio of basic medical insurance reimbursement for hospital costs .Results:The proportion of hospitalization at the county level and below was 56.4%among floating population , the proportion of inflow patients being 51.5%and the rest of the hospital population being counted at a proportion of 37.9%, lower than the corresponding proportion of 72.6%among local res-idents derived from the Fifth National Health Services Survey , but this ratio was 78.9%among registered population . The multivariate logistic regression results indicated that demographic , socio-economic and flow characteristics , hospital location and reasons for hospitalization have significantly affected the migrant inpatients facilities , but health insurance did not significantly influence the hospital seeking behaviors among floating population .Conclusions:The proportion of hospitalization at the county level and below was low among migrants and this proportion presented great regional differ -ences;and medical insurance didn't play a guiding role in the diversion of the internal floating population .

Objective To observe the clinical efficacy of acupoint application withChan Wu gel cream plus five notes therapy in treating cancer pain.Method Eighty patients with cancer pain were randomized into a treatment group and a control group, 40 cases in each group. The treatment group received acupoint application withChanWu gel cream at the corresponding acupoints of the affected organ in association with the five notes therapy; the control group was intervened by the three-step analgesic ladder recommended by World Health Organization (WHO). The quality of life and pain intensity were scored before and after the intervention.Result The quality of life and pain degree were significantly changed after the treatment in the two groups (P<0.01). The quality of life and pain intensity of the treatment group were significantly different from that of the control group (P<0.05).Conclusion Acupoint application withChan Wu gel cream plus five notes therapy is effective in mitigating cancer pain and improving the quality of life.

Objective We summarized the nursing care experience of spinal muscular atrophy complicated with pneumonia.Methods The measures of nursing care to the 9 children patients were comprised of preventing sputum blockage; preventing suffocating due to milk choking; paying special attention to the time being when the sputum blockage and milk choking occurred; early detection of paralysis of respiratory muscle; and training nursing skills to the families,and so on.Results All 9 children patients were cured and discharged from hospital.Conclusions The measure of targeting nursing intervention to the children with infantile spinal muscular atrophy and pneumonia could improve therapeutic effect and reduce mortality rate.

The stable quality of Chinese herbal medicines is a critical factor for their reliable clinical efficiency. An improved liquid-liquid extraction procedure and a liquid chromatographic method were developed to simultaneously analyze five anthraquinones (aloe-emodin, rhein, emodin, chrysophanol and physcion) in a Chinese traditional hospital preparation, Fuyankang mixture, in order to quantitatively control its quality in a more effective way.

Objective To investigate effect of Murphy's law on the nursing safety management of the hemodialysis patients. Methods 4598 hemodialysis cases in a blood purification center of a first class grade A hospital from April to July in 2011 was assigned as pre-implementation group.Then Murphy's law was used in the nursing practice.In the following,another 5683 hemodialysis cases from November in 2011 to January in 2012 were assigned as post-implementation group.The incidence of nursing risks and hemodialysis equipment failures were compared between the two groups.Result The incidences of nursing risks and hemodialysis equipment failures in the post-implementation group were significantly lower than those of the pre-implementation group(P<0.05).Conclusions Murphy’s law is effective for the declines of nursing manipulative defects and hemodialysis equipment failures.Besides,the law can be good for the strengthened awareness of safety and enhanced consciousness of all staff participating in the safety management so as to ensure the safety and quality of hemodialysis.

<p><b>OBJECTIVE</b>To explore the once sampling quantitation of Houttuynia cordata through its DNA polymorphic bands that carried information entropy, from other form that the expression of traditional Chinese medicine polymorphism, genetic polymorphism, of traditional Chinese medicine.</p><p><b>METHOD</b>The technique of inter simple sequence repeat (ISSR) was applied to analyze genetic polymorphism of H. cordata samples from the same GAP producing area, the DNA genetic bands were transformed its into the information entropy, and the minimum once sampling quantitation with the mathematical mode was measured.</p><p><b>RESULT</b>One hundred and thirty-four DNA bands were obtained by using 9 screened ISSR primers to amplify from 46 strains DNA samples of H. cordata from the same GAP, the information entropy was H=0.365 6-0.978 6, and RSD was 14.75%. The once sampling quantitation was W=11.22 kg (863 strains).</p><p><b>CONCLUSION</b>The "once minimum sampling quantitation" were calculated from the angle of the genetic polymorphism of H. cordata, and a great differences between this volume and the amount from the angle of fingerprint were found.</p>

OBJECTIVE@#To investigate the relationship between homocysteine (HCY) and congenital heart defects, and to observe the toxic effect of different doses of HCY on embryonic heart development in mammalian.@*METHODS@#A total of 30 SD pregnant rats were randomly divided into 3 groups: a high dose group [200 mg/(kg.d)], a low dose group [(100 mg/(kg.d)] and a control group (equal volume of physiologic saline, n=10 in each group). The HCY or vehicle was given intraperitoneally from 7 to 20 days after uterineincision delivery. The contents of HCY in serum were analyzed by high performance liquid chromatogram electrochem before the pregnancy and 20 days after the pregnancy. The structure changes of the newborn rats heart were observed by stereoscope. The ultrastructure changes of cadiomyocyte were observed through transmission electron microscope.@*RESULTS@#Comparing with the control, serum HCY in rats 20 days after pregnancy was significantly increased in the high or low dose group [(30.47 ± 1.12), (20.90 ± 1.08)vs(10.98 ± 0.77)μmol/L, P<0.01)], indicating that the hyperhomocysteinemia animal model was successfully established. The incidence rate of congenital heart defects in neonatal was significantly increased in the high or low dose group(14.13%, 9.57% vs 0.76%, P<0.01). The number of apoptotic cells were significantly increased in the high dose group.@*CONCLUSION@#Hyperhomocysteinemia may exert toxic effect on embryonic heart development in pregnancy rats, which led to congenital heart defects in the newborn rats. Hyperhomocysteinemia induced cardiomyocyte apoptosis may, at least partially, contribute to the heart defects.

<p><b>OBJECTIVE</b>To study the differences of HPLC fingerprints with the total quantum statistical moment for the Buyang Huanwu decoction (BYHWD) processed by various ways, and to verify the additive properties of total quantum statistical moment.</p><p><b>METHOD</b>The extracts of BYHWD were obtained by water extraction and alcohol precipitation firstly, and then were dissolved with five solvents of different solubility parameter from 11.4 Cal(1/2) x cm(-3/2) to 23.40 Cal(1/2) x cm(-3/2) respectively. Their chromatographic fingerprints were determined by HPLC, finally the total quantum statistical moment parameters and its superposition properties were manual calculated and analyzed by their expressions.</p><p><b>RESULT</b>As a contrast as the whole prescription, the similarities of the various processed samples with butanol (11.4 Cal(1/2) x cm(-3/2)), methanol (13.5 Cal(1/2) x cm(-3/2)), 68% methanol (16. 67 Cal(1/2) x cm(-3/2)), 34% methanol (20.03 Cal(1/2) x cm(-3/2)) and distilled water (23.40 Cal(1/2) x cm(-3/2)) were 0. 074, 0. 973, 0. 934, 0. 991, 0. 993, respectively and while the RSD of these total quantum zero moment, center moment and variance of each chromatographic fingerprints for them were 63.04%, 16.22%, 69.38%, which showed significant difference in these chromatographic fingerprints. The total quantum statistical moment parameters of the superimposed chromatographic fingerprint with each sole samples were 3.203 x 10(5) mAu x s, 29.85 min, 389.97 min2, whereas the whole prescription's were 6.548 x 10(4) mAu x s, 29.44 min, 389.00 min2, that suggested that the absolute difference percentages between the superimposed and the whole were 2.209%, 1.389%, 0.2484%, respectively.</p><p><b>CONCLUSION</b>The total quantum statistical moment of the chromatographic fingerprints is of characteristics with additive properties, it can be used in static and dynamic quality controlled analyses in the Chinese medicine multiple component systems.</p>

<p><b>OBJECTIVE</b>To establish and experimental verification the mathematical model of the balance groups that is the steady-state of traditional Chinese medicine in extraction.</p><p><b>METHOD</b>Using the entropy and genetic principles of statistics, and taking the coefficient of variation of GC fingerprint which is the naphtha of the Houttuynia cordata between strains in the same GAP place as a pivot to establish and verify the mathematical model was established of the balance groups that is the steady-state of traditional Chinese medicine in extraction.</p><p><b>RESULT</b>A mathematical model that is suitable for the balance groups of the steady-state of traditional Chinese medicine and preparation in extraction, and the balance groups which is 29 683 strains (approximately 118.7 kg) were gained with the same origin of H. cordata as the model drug.</p><p><b>CONCLUSION</b>Under the GAP of quality control model, controlling the stability of the quality through further using the Hardy-Weinberg balance groups of the H. cordata between strains, the new theory and experiment foundation is established for the steady-state of traditional Chinese medicine in extraction and quality control.</p>
