ABSTRACT

PURPOSE: Adiponectin is an endogenous modulator of vascular remodeling that suppresses vascular inflammation. However, the role of adiponectin in Kawasaki disease (KD) has not been elucidated. The purpose of this study is to investigate the correlation between serum adiponectin level and several parameters, such as interleukin (IL)-6, tumor necrosis factor (TNF)-alpha, lipid profile, and C reactive protein (CRP), and to clarify the association between adiponectin and cardiac function. METHODS: Twenty-two KD patients (22 patients in acute phase and 20 patients in subacute phase) were enrolled in the study group. The control group consisted of 31 subjects (13 febrile patients and 18 healthy children). Both groups underwent blood sampling and tissue Doppler imaging (TDI). RESULTS: CRP was significantly increased in the KD group compared with the control group. There were no significant differences in serum TNF-alpha, IL-6, and adiponectin levels between groups. However, a negative correlation was found between adiponectin level and CRP level or platelet count. Systolic myocardial velocity and A myocardial velocity measured by TDI were decreased significantly in the acute KD group compared with the subacute KD group and control group. Positive correlations were found between adiponectin level and systolic myocardial velocity or A myocardial velocity. CONCLUSION: In acute KD patients, low adiponectin level was related to severe inflammatory reactions and decreased left ventricular functions.

ABSTRACT

PURPOSE: This study was designed to determine the effects of a school-based obesity-management program on obese primary school children. METHODS: A total of 995 children (6-12 years old) in a primary school were screened in March 2008, and of those, 101 obese students (44 boys and 57 girls, body mass index (BMI) > or = 95 percentile) were enrolled for a study group. The school-based, obesity management program, which includes physical exercise and nutritional education, was conducted as part of an extracurricular program for 12 weeks. The measurement of height, weight, waist circumference, blood pressure (BP), and bioelectrical impedance analysis (BIA) was performed before and after the program. RESULTS: Height and weight increased significantly (P<0.05). The BMI and obesity index decreased significantly (P<0.01). Systolic and diastolic BP decreased significantly (P<0.01). BMI decreased in 61.4% of boys and 66.7% of girls. Protein and basal metabolic rate (BMR) increased significantly on the BIA (P<0.01). Fat decreased significantly (P<0.05). The total body water (TBW) and percent body fat (PBF) decreased significantly (P<0.01). The changes in protein, fat, TBW, PBF, and BMR significantly correlated to the change in BMI (P<0.05). In a multiple logistic regression analysis, BMI change was significantly correlated to the changes in protein and fat content (P<0.01). CONCLUSION: The school-based obesity management program is a very effective way to manage obesity for obese primary school children.

ABSTRACT

Poland syndrome is characterized by unilateral absence or hypoplasia of the pectoralis muscle and variable degree of ipsilateral hand anomalies. Mobius syndrome is a congenital neurological disorder characterized by complete or partial facial paralysis. Although the pathogeneses of these diseases are not well-characterized, diminished blood flow to the affected side is thought to play a role. A male infant weighing 2.670 g was born at 38+3 weeks of gestation with left facial paralysis, left chest wall defect with dextrocardia, and symbrachydactyly between the second and third fingers. The combination of Poland-Mobius syndrome is rare, and only 2 cases associated with dextrocardia have been reported worldwide. Here, we report the first case of Poland-Mobius syndrome associated with dextrocardia in Korea.

ABSTRACT

BACKGROUND: Ordering an excessive quantity of blood for elective surgery beyond the actual need causes unnecessary cross-matching and wastage or shortage of blood products. To prevent this problem, a maximum surgical blood order schedule (MSBOS) is beneficial. However, the application of a MSBOS is limited due to the complexity of accurate classification based on the name of the operation alone. In this study, we introduced an automatic cancellation program, in which the assigned blood is automatically cancelled after a certain period of time, andevaluated the practicality of a revised MSBOS. METHODS: We analyzed our hospital data involving transfused RBCs for each elective surgical procedure performed between January and June 2007. The MSBOS was organized based on the average number of units of RBCs transfused for the types of surgeries that had been performed >50 times during a certain period of time. The blood cancellation ratio, blood wastage ratio, and the causes of blood wastage were compared before and after adopting the automatic cancellation program and the revised MSBOS. RESULTS: After adopting the automatic cancellation program, the blood cancellation ratio decreased from 18.3% to 17.6% and the blood wastage ratio decreased from 2.67% to 2.00%. There was no significant change with respect to the causes of blood wastage before and after adoption of the automatic cancellation program. CONCLUSION: The implementation of an automatic cancellation program facilitates the efficient use of blood products during elective surgery. It would be useful to apply a revised MSBOS continuously, which is easily applicable and practical in order to decrease the blood wastage rate.

ABSTRACT

BACKGROUND AND OBJECTIVES: Because the mechanisms of the biological effects of statin and antiotensin converting enzyme inhibitor therapies differ, the vascular responses to these therapies were studied in hypercholesterolemic patients. MATERIALS AND METHODS: Simvastatin, 20 mg, placebo or ramipril, 10 mg, were administered daily for 2 months, with a 2 month washout, to 32 hypercholesterolemic patients. This was a randomized, double-blind, placebo-controlled, crossover in design study. RESULTS: Simvastatin alone, or in combination with ramipril, significantly changed the lipoproteins, and improved the percentage of the flow-mediated dilator response to hyperemia by 46+/-48% and by 59+/-66%, respectively, relative to the baseline measurements (both p<0.001). The plasma malondialdehyde levels were reduced, relative to baseline measurements, by 6+/-57% (p=0.045) and 13+/-47% (p=0.045 and p<0.001, respectively) and plasma levels of monocyte chemoattractant protein-1 by 3+/-27% and by 9+/-16%, respectively (p=0.113 and p=0.001, respectively). The C-reactive protein were also reduced, relative to baseline measurements, by 17+/-75% and by 17+/-37%, respectively (p=0.003 and p=0.001, respectively). However, simvastatin combined with ramipril changed, to a greater extent, but was statistically insignificant, the percentage of the flow-mediated dilator response to hyperemia, and the plasma monocyte chemoattractant protein-1 levels, than simvastatin alone. CONCLUSION: Compared with simvastatin alone, the addition of ramipril improved the endothelial function to greater extent, but was statistically insignificant, in hypercholes-terolemic patients.

ABSTRACT

After surgical operation in patients with arteriovenous malformation (AVM), normal pressure perfusion breakthrough (NPPB) is one of the major complications. Brain perfusion SPECT with acetazolamide stress was known to be useful to evaluate the vascular reserve in several neurological and neurosurgical conditions. The authors performed acetazolamide brain perfusion SPECT in patients with AVM and compared the brain perfusion in the post-operative clinical courses. The acetazolamide brain perfusion SPECT was helpful in defining the prognosis of the patients with AVM. We describe 4 patients with AVM who had acetazolamide brain perfusion SPECT to examine the prognosis.

ABSTRACT

OBJECT: The aim of this study is to determine whether exposure to chlorpromazine causes mutagenicity and genetic disorders. METHOD: Ames (Salmonella typhimurium) test and Rec assay (Bacillus subtilis) were used as indicators for DNA damage. Furthermore, the levels of umu operon expression by measuring the beta-galactosidase activity were monitered with the SOS umu test using S. typhimurium 1535 containing plasmid pSK1002. And the host-mediated assay was used to investigate the muta-genicity of chlorpromazine after the activation with in vivo metabolic systems. RESULTS: From the results, chlorpromazine did not affect DNA of S. typhimurium and B. subtilis strains and showed no mutagenicity at the all concentrations tested. These phenomena was also similar to that after metabolic activation of chlorpromazine in in vivo system. CONCLUSION: These results suggested that chlorpromazine did not show the mutagenicity and genotoxicity by four different methods used in this study.

ABSTRACT

Expander procedure is a newly developed extraocular muscle weakening technique which has several advantages such as good control of resection margin, easy reoperation, rare postoperative muscular paresis. For the study of usability of Gore-Tex(R) as a muscle expander, fifteen rabbits received expander procedure with Gore-Tex(R) in right eye and silicone retinal band in left eye. At postoperative 1, 3, 5, 7, 12 weeks, histopathologic examination was performed. There was no significant difference in inflammatory reaction in both groups. Inflammatory reaction decreased with time in both groups. In Gore-Tex(R) expander group, muscle regeneration from cut end was observed at the 3rd postoperative week and fibroblast ingrowth into the expander was observed at the 5th postoperative week. These findings were more prominent with time. Adhesion of expander to underlying sclera was not observed in both groups. It is concluded that expander procedure using Gore-Tex(R) will be a new and useful surgical method for the extraocular muscle weakening procedure.

ABSTRACT

Authers have experienced congenital punctal agenesis with syndactyly in sister and brother who complained purulent discharge and epiphora. Bilateral upper punctal agenesis, fistulas, and common canalicular obstruction in dacryocystogram were found in a 17 year-old brother. Bilateral upper and lower punctal agenesis was found in a 14 year-old sister. Syndactyly was found in both patients. In familial history, father of patients had syndactyly in upper and lower extremities and complained epiphora. Another sister of patients had experience of lacrimal system surgery. Conjunctivodacryocystorhinostomy using Jones tube was performed in both patients.

ABSTRACT

From Nov. 1990 to Dec. 1994, authors have experienced 12 patients(13 eyes) diagnosed as posterior lens dislocation. The clinical results were evaluated in aspeets of sex and age distribution, preoperative and postoperative visual acuity, operative method, postoperative complication. The average age of patients was 57.8 years old and average follow-up period was 11.5 months. Male were 58%. They have history of ocular trauma in all 13 eyes. Dislocated lens was removed by extraction with lens spoon or loop through corneoscleral incision in 3 eyes and pars planar vitrectomy followed by lens lift with needle and extraction through corneoscleral incision in 7 eyes or with perfluorocarbon liquid and extraction with spoon or loop in 3 eyes. The best corrected visual acuity was improved in 10 eyes and remained the same in 2 patient(3 eyes) who was visited our hospital more than 10 years after dislocation and associated with optic nerve atrophy, macular degeneration, bullous keratopathy. Postoperative complications included glaucoma, choroidal detachment, retinal detachment, persistent vitreous inflammation, vitreous hemorrhage, bullous keratopathy.

ABSTRACT

Primary chondrosarcoma of the extraskeletal soft tissue is extremely rare. Extraskeletal chondrosarcoma was first described as entity in 1953 by Stout and Verner, who were able to collect a series of seven cases. Because of the rarity of this lesion in extremities, neither the clinical course nor the preferred method of treatment has been clearly established. Authors report a case of extraskeletal chondrosarcoma which occurred on the right forearm of a thirty-nine-year-old man. The lesion was completely excised with free resection margins. There was no recurrence or metastasis for four years follow up.

ABSTRACT

This study was performed to investigate the toxicity of mitomycin C(MMC) on the iris and ciliary body of the cat. After anesthesia, MMC in 0.1, 0.2, or 0.4 mg/ml concenration was injected intracamerally and the equal volume of balanced salt solution(BSS) was injected intracamerally to the contralateral eye as a control. There were no significant difference between 0.1 mg/ml of MMC and BBS injection except for the enlargement of endoplasmic reticulum and destruction of cristae of ciliary body 14 days after MMC injection. Iris vessel was congested and mitochondria of ciliary body was enlarged and cristae was disrupted 7 days after 0.4 mg/ml of MMC injection. There were infiltration of inflammatory cell on the iris and increased cellular damage at the nonpigmented epithelium of ciliary body 21 days after 0.4 mg/ml of MMC injection. These results suggest that the caution is necessary to prevent high concentration of MMC from getting into the anterior chamber at the time of glaucoma filtering surgery in human.

ABSTRACT

To investigate the applicability of the Bartha strain of pseudorabies virus(PRV-Ba) as a neurotracer in the visual system, we performed immunohistochemical study of the rat brain after intravitreal injection of the PRV-Ba. Cholera toxin-horseradish peroxidase(CT-HRP), a well known neurotracer, was also injected into the vitreous cavity for comparison. After given survival time the brain was removed and processed immunohistochemical stain for PRV-Ba and neurohistochemical stain for CT-HRP. The CT-HRP positive reactions were observed almost contralaterally in optic chiasm, optic tract, suprachiasmatic nucleus(SCN), olivary pretectal nucleus(OPT), all three subdivision of lateral geniculate body, superficial gray layer and optic layer of superior colliculus and medial, lateral, dorsal terminal nucleus of accessory optic tract. PRV-Ba immunopositive reaction was observed in SCN and OPT, but not in nerve fiber system and dorsal lateral geniculate body. Also PRV-Ba positive cells were distributed mainly in intermediate layer and weakly distributed in superficial layer of the superior colliculus. This study suggests that PRV-Ba is not a useful neurotracer in studying the visual system of the rat.
