ABSTRACT

Osteonecrosis of the femoral head (ONFH) is a serious orthopedic disease caused by many factors. It is difficult to cure clinically and has a poor prognosis, which poses a serious threat to human health. The pathogenesis of this disease is still unclear. The ONFH caused by different causes involves the disorder of a variety of metabolic pathways in vivo. Abnormal proliferation and differentiation of bone mesenchymal stem cells (BMSCs), imbalance of bone metabolism, and increased destruction of bone trabeculae caused by abnormal transduction of bone metabolism-related signaling pathways may be the important causes of ONFH. BMSCs are pluripotent stem cells with self-renewal and multidirectional differentiation ability, which have good regeneration rate. Improving the osteogenic and differentiation ability of BMSCs is the key to inhibit bone absorption and promote bone matrix reconstruction, which plays an important role in bone remodeling. In recent years, there are many studies on the prevention and treatment of ONFH in traditional Chinese medicine(TCM), and it has been found that a variety of single herbs, monomers and compounds can regulate the differentiation direction and process of BMSCs by targeting signal molecules, with great potential for bone defect repair and anti-femoral head necrosis activity. Nowadays, prevention and treatment of ONFH by regulating bone metabolism signaling pathways has become a hot research topic. In this paper, the mechanism and related signal transduction pathways of TCM in preventing and treating ONFH were reviewed to explore some mechanisms of alleviating the rate of bone loss, promoting bone formation, and repairing bone defects, so as to provide reference for further research on the prevention and treatment of ONFH by TCM. The related clinical application studies also provided specific targets for gene-assisted therapy of ONFH.

ABSTRACT

Objective:To observe the antagonism of dexamethasone on radiation bystander effect and its re-transmission in rabbit lymphocytes. Methods:The plasmas of 2 New Zealand rabbits which were irradiated with 6 MV X-ray was taken to prepare the first generation of conditioned medium; the plasmas of 2 unirradiated New Zealand rabbits were taken to prepare the first generation of unconditioned culture medium; The lymphocytes of 5 unirradiated New Zealand rabbits were extracted. The lymphocytes from 5 unirradiated New Zealand rabbits were cocultured with the first generation of unconditioned medium or conditioned medium. After abandoning the culture medium, the lymphocytes were cultured in conventional medium for 24 h. The second generation of unconditioned medium or conditioned medium was taken. The lymphocytes were treated with four medium with or without dexamethasone (1 μg/ml), and the apoptosis rate of lymphocytes was detected by flow cytometry. Results:With or without dexamethasone, the apoptosis rates of lymphocytes treated with the first generation of conditioned medium was significantly higher than that with the first generation of unconditioned medium [without dexamethasone: (21.09±1.60)% vs. (8.06±0.65)%, t = -30.182, P < 0.05; with dexamethasone: (14.96±1.80)% vs. (6.25±0.54)%, t = -16.404, P < 0.05]. Dexamethasone could reduce the apoptosis rates of lymphocytes treated with the first generation of unconditioned medium and the first generation of conditioned medium, and the differences were statistically significant (both P < 0.05). With or without dexamethasone, the apoptosis rates of lymphocytes treated with the second generation of conditioned medium was significantly higher than that with the second generation of unconditioned medium [without dexamethasone: (28.70±2.14)% vs. (12.38±0.67)%, t = -33.351, P < 0.05; with dexamethasone: (20.21±1.96)% vs. (12.53±1.25)%, t = -14.145, P < 0.05]. Dexamethasone could reduce the apoptosis rates of lymphocytes treated with the second generation of conditioned medium ( P < 0.05), but it could not reduce the apoptosis rate of lymphocytes treated with the second generation of unconditioned medium ( P > 0.05). Conclusion:Dexamethasone can antagonize the injury of lymphocytes by radiation bystander effect in vitro, reduce the apoptosis rate of lymphocytes, and can also antagonize the re-transmission of radiation bystander effect.

ABSTRACT

To explore the medication rules of famous veteran traditional Chinese medicine practitioners in treating infertility based on medical cases of infertility collected from book series of Hundred Traditional Chinese Medicine Clinicians of Hundred Years in China and Prescription Proven by Traditional Chinese Medicine Masters. Researchers extracted the information of prescriptions from these cases according to the inclusion and exclusion criteria. Then, Excel 2010, SPSS Clementine(ver.12.0) and SPSS(ver. 22.0) were adopted respectively for frequency analysis, association rules analysis, cluster analysis and factor analysis. Cluster analysis was carried out by Ochiai algorithm of binary variable data, which was a systematic clustering method. And principal component analysis was used for factor analysis. Besides, KMO test and Bartlett spherical test were used for factor adaptation test. Finally, 151 medical cases and 396 prescriptions were included in total. A total of 60 kinds of frequently used herbs were identified according to the results of frequency analysis for medication, they were mainly used for activating blood and resolving stasis, tonifying and clearing heat respectively. The association rules analysis found out 25 drug pair association rules and 14 3-drug combination association rules. A total of 15 medicine groups were extracted by cluster analysis. KMO test and Bartlett spherical test indicated that the method was suitable for factor analysis, and 21 common factors were respectively extracted by factor analysis. Association rules indicated the characteristics of the therapeutic methods, like tonifying Qi and replenishing blood. The famous veteran traditional Chinese medicine practitioners utilized modified Siwu Decoction for tonifying blood and preferred Atractylodis Macrocephalae Rhizoma(Baizhu) for tonifying Qi. The results of both cluster analysis and factor analysis demonstrated the characteristics of the therapies for tonifying kidney, activating blood, tonifying spleen and dispelling dampness. In addition, factor analysis could reflect the therapies for nourishing Yin, tonifying kidney, warming the meridian, dissipating cold, nourishing blood and dispelling blood stasis. These results of analysis comprehensively showed out the medication characteristics of famous veteran traditional Chinese medicine practitioners of strictly following the pathogenesis, making good use of classical formulas and providing proper compatibility. In conclusion, data mining techniques(including frequency analysis, association rules analysis, cluster analysis and factor analysis) were used to comprehensively analyze the medication rules of famous veteran traditional Chinese medicine practitioners in treating infertility, which is helpful for guiding the clinical practice of treating infertility with traditional Chinese medicine.

ABSTRACT

Pancreatic cancer is a highly lethal disease in gastrointestinal malignant tumors. The mortality of pancreatic cancer closely parallels its incidence. Most patients with pancreatic cancer remain asymptomatic until the disease reaches an advanced stage. There is no program for screening patients at high risk of pancreatic cancer.Although CT,MRI,positron emission tomography, endoscopic ultrasonography, and endoscopic ultrasonography-guided fine-needle aspiration offer high diagnostic ability for pancreatic cancer,it cannot be found at the early stage easily.Surgical resection is regarded as the only potentially curative treatment and adjuvant chemotherapy is given after surgery. This article reviews epidemiology, risk factors, diagnosis and treatment for pancreatic cancer by summarizing relevant literature.

ABSTRACT

Small intestinal hemangioma is a rare condition that can be divided histologically into capillary, cavernous or mixed types, among which the cavernous type is the most common. Here we report a case of small intestinal cavernous hemangioma with chronic hemorrhage in 44-year-old man. The patient complained of weakness and dizziness for 2 years that aggravated 1 month before admission accompanied by intermittent melena. Laboratory tests suggest severe anemia, and computed tomography, gastroscopy and colonoscopy all revealed signs of anemia. Capsule endoscopy detected small intestinal erosions, bleeding lesions and prominent neoplasms. An exploratory laparotomy was performed, in which the segment of the jejunum with lesions was resected. Pathological examination of the resected jejunum identified the neoplasm as cavernous hemangioma of the small intestine, which was the cause of severe anemia.

ABSTRACT

Gastric cancer is one of the most common malignancies and chemotherapy is the main method for the treatment of advanced gastric cancer. Chemotherapy can improve the quality of life and prolong the survival time of patients with advanced or metastatic gastric cancer. The DCF regimen (docetaxel, cisplatin and 5-fluorouracil)is effective for patients with advanced gastric cancer. However, because of the severe hematological toxicity and gastrointestinal reaction caused by DCF regimen, its clinical application is still limited. This article reviewed the research progress of DCF regimen in treatment of advanced gastric cancer.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the protective effects of tea polyphenols (TP) against myocardial ischemia/reperfusion (IR) injuries and explore the possible mechanisms.</p><p><b>METHODS</b>Langendorff-perfused rat hearts were subjected to ischemia for 30 min followed by reperfusion for another 30 min. Myocardial function indices were measured by a left ventricular cannula via a pressure transducer connected to the polygraph in isolated Langendorff hearts and energy metabolism was measured using (31)P nuclear magnetic resonance (NMR) spectroscopy. Whole-cell atch-clamp technique was used to record calcium inward current (I(Ca-L)) in cultured rat cardiac myocytes.</p><p><b>RESULTS</b>Compared with the control hearts, the ex vivo rat hearts with 2.5 mg/L TP treatment showed significantly increased left ventricular developed pressure (LVDP), maximal rise rate of LVDP (+dp/d(tmax)), maximal fall rate of LVDP (-dp/dt(max)), and coronary flow (CF) (P<0.05). During both cardiac ischemia and reperfusion phase, ATP and PCr levels were elevated significantly in TP-treated hearts compared with those in the control hearts (P<0.05). In cultured rat cardiac myocytes, ICa-L was remarkably decreased by TP at the doses of 2.5 and 5.0 mg/L (P<0.01).</p><p><b>CONCLUSION</b>Our results support a possible protective role of TP against myocardial IR injury by improving myocardial energy metabolism and inhibiting I(Ca-L) in the cardiac myocytes.</p>

ABSTRACT

Objective To study the effect of rhodiola crenulata compound on serum corticosterone and myocardial glucocorticoid receptor ( GR) in rats exposed high sustained +Gz.Methods Seventy-two healthy SD rats were randomly divided into six groups: blank control group, stress control group, +10 Gz stress group and low, medium, high dose drug group, with twelve rats in each.20 mL/kg menstruum was fed to each rat once per day for 14 days.Low, medium and high dose drug group were fed with rhodiola crenulata compound at doses of 0.75 g/kg, 1.5 g/kg and 3.0 g/kg respectively, the other three groups were fed with equal volume saline.Rats were exposed to high +Gz in 15th day.The concentration of corticosterone in the serum of each group rats was detected with ELISA. Western blot was used to detect the expressions of GR in the myocardium of each group rats .Results The content of corticosterone was significantly elevated in +10Gz stress group, while the expression of GR in the myocardium was markedly declined (P <0.01,P <0.05).However, compound preconditioning could decrease concentration of corticosterone in the serum and enhance the expression of GR in the myocardium from rats after +10 Gz exposure. The corticosterone concentration of medium and high dose drug groups was significantly lower and the level of GR expression in the myocardium was significantly higher than that of +10 Gz stress group ( P <0.05 , P <0.01 ) . Conclusion Rhodiola crenulata compound preconditioning could regulate the concentration of corticosterone in the serum and the level of GR expression in the myocardium of the rats exposed +10 Gz, which may be related with its protective effect on high sustained +Gz-induced injury of myocardium.

ABSTRACT

BACKGROUND:It has been known that there exists a close relationship between leptin and obesity. Then, how are serum leptin and insulin related to obesity-related hypertension(ORH)? OBJECTIVE:To study the changes of serum leptin and insulin in elderly men with ORH,and investigate the relationship between leptin resistance and ORH in elderly men. DESIGN: Retrospective controlled study based on diagnosis. SETTINGS:The Department of Neurology and the Department of Nuclear Medicine,Affiliated Union Hospital of Tongji Medical College, Huazhong University of Science and Technology. PARTICIPANTS: Altogether 62 elderly male inpatients or outpatients aged from 60 to 82 years in the Affiliated Union Hospital of Tongji Medical College from October 2001 to June 2002 were selected. The diagnosis of hypertension was based on the diagnostic criteria established by the WHO in 1999:systolic blood pressure(SBP) ≥ 140 mm Hg(1 mm Hg=0.133 kPa) and(or) diastolic blood pressure(DBP) ≥ 90 mm Hg. INTERVENTIONS:The content of serum leptin and insulin in 41 hypertensive and 21 normotensive elderly men was determined by the method of radioimmunoassay. MAIN OUTCOME MEASURES:Blood pressure, body mass index (BMI),serum leptin and insulin level. RESULTS:The content of serum leptin and insulin in the hypertensive obese men were increased significantly by 1.8 μ g/L and 2.7 mIU/L,respectively,as compared with that in the normotensives (t=2.212,2.395,P< 0.01). The level of serum leptin and insulin in the ORH patients was increased by 2.7 μ g/L and 4.7 mIU/L,respectively,as compared with that of the normotensives with significant differences (t=3.348,5.113,all P< 0.001). The level of serum leptin in the hypertensive patients older than 70 years was significantly increased by 1.7 μ g/L compared with that of those aged 60 to 70 years(t=2.767,P< 0.05). CONCLUSION:Resistance to leptin and insulin exists in obesity-related hypertension of elderly men and leptin is closely associated with obesity-related hypertension.

ABSTRACT

<p><b>AIM</b>To determine whether repetitive exposure to high sustained +Gz acceleration induces persisting changes in the myocardial free radical metabolism and observe the protective effects of low-G training and antioxidant tea polyphenols (TP).</p><p><b>METHODS</b>Thirty-two male Wistar rats were randomly divided into four groups (n=8 each): group A, restrained, was only submitted to +1 Gz for 5 min. Group B, centrifuged, was exposed to five plateaus of 30 s at +10 Gz for intermittent times, three times a week, for three weeks. Group C, low-G trained, was exposed to +2 Gz for 5 min about 1 h prior to +10 Gz stress, and group D was orally given TP at dose of 200 mg/kg about 1 h prior to +10 Gz stress. On the next day morning after last centrifuge run, the rats were decapitated and the hearts were quickly removed. Malondialdehyde (MDA) concentration and superoxide dismutase (SOD) activity were measured. Additionally, CuZn-SOD and inducible NO synthase (iNOS) enzymatic contents were examined by immunohistochemical staining and their mRNA were analyzed by semi-quantitative reverse transcription polymerase chain reaction(RT-PCR).</p><p><b>RESULTS</b>Compared with group A, MDA concentration and iNOS enzymatic content in myocardial mitochondria were increased significantly (P < 0.05) in group B. Compared with group B, mitochondrial SOD activity was significantly increased in group C (P < 0.05). iNOS enzymatic content was significantly decreased in group C and D. There were no significant differences of CuZn-SOD content, CuZn-SOD and iNOS mRNA levels among the four groups.</p><p><b>CONCLUSION</b>Repeated high +Gz exposure can induce myocardial free radical metabolic disorder and mainly result in mitochondrial peroxidative injury. But low-G training and natural antioxidant TP have protective effects, and the former is better.</p>

ABSTRACT

Objective To explore the possible effects of nutritional supplements on brain function as reflected by Water Maze test performance in mice after +Gz exposure.Method Mice were arranged into control group (group A),+Gz group without nutritional supplementation (group B) and +Gz plus nutritional supplementation group (group C).Each group contains 12 mice.Mice in group A were not exposed to +Gz while mice in both group B and group C were exposed to 8 min +10 Gz.Distilled water was gavaged to group B mice 3 h before +Gz exposure. On the day before +Gz exposure pyridoxol fortified water was given and 3 h before exposure mixed amino acids solution were gavaged to group C mice.Water Maze test was done and scores were recorded in all groups. After the Water Maze test was completed, blood was collected through the eyes for serum amino acid determinations and brain tissue was collected by decollation for monoamine determination and γ-glutamyl transferase (GGT) activity evaluation. Result After +Gz exposure, longer completion time and more mistakes were observed in Water Maze test in group B as compared with group A and a trend of improvement in group C was noticed. The ratio of brain 5-HT to dopamine(DA) was significantly reduced in group C as compared with group B.Gamma glutamyl transferase (GGT) activity in brain tissue in group C and group B increased significantly. Conclusion High sustained +Gz exposure significantly reduces Water Maze test performance in mice (longer completion time and more mistakes). It seems that there is a trend of improvement in Water Maze performance in mice in dietary nutritional supplementation group, which might be due to significant reduction in ratio of brain 5-HT to DA in mice with nutritional supplementation.

ABSTRACT

<p><b>AIM</b>To observe the change of cardiovascular endothelium's ultrastructure and its intercellular adhesion molecules-1 (ICAM-1) expression in rat after repetitive high positive acceleration (+ Gz) exposures and further to explore the mechanisms of myocardial injuries induced by high + Gz stress.</p><p><b>METHODS</b>Thirty male Wistar rats were randomly divided into three groups: control group, + 1Gz group and + 10Gz group (n = 10 for each). The rats of + 10Gz group were exposed to five plateaus at + 10Gz for 30s with + 1Gz 1 min intervals, 3 times a week, for 3 weeks, while rats of + 1Gz group subjected to + 1Gz for 5 mim daily. The control group didn't undergo acceleration stress. The rats were decapitated in the next day after the last centrifuge run and myocardium were immediately dissected from left ventricles for ultrastructural examination using transmission electron microscope and immunohistochemical staining of ICAM-1.</p><p><b>RESULTS</b>The ultrastructural changes of the cardiovascular endothelium were observed in rats of 10Gz group, including endothelium edema and platelet aggregating in lumen of blood vessel. Also, the expression of ICAM-1 in + 10Gz stressed rats increased significantly (P < 0.05). While there was no difference between control group and + 1Gz group in ultrastructure of cardiovascular endothelium and its ICAM-1 expression.</p><p><b>CONCLUSION</b>The results suggested that repeated high + Gz exposures could injury cardiovascular endothelium of rat and increase ICAM-1 expression, which indicated cell adhesion molecules (CAMs) inducing inflammation took part in myocardial injuries induced by high + Gz stress.</p>

ABSTRACT

Objective To observe the protective effects of natural antioxidant tea polyphenols(TP) on myocardial free radical metabolic disorder in mice induced by inhalation of hypobaric pure oxygen under 5500 m hypobaric condition. Method Fourty-two male Kunming mice were randomly divided into three groups(n=14 each):group A, normal control; group B, inhalation of pure oxygen(>96%) at simulated altitude of 5500 m in an animal altitude chamber;group C(TP protection group), same as group B but 100 mg/kg of TP was given orally before the exposure. The exposure time was 2 h/d,3 d/wk for a total of 8 wk, and distilled water was given to groups A and B before exposure. After experiment, the mice were decapitated on the next day and the heart was quickly removed. Malondialdehyde(MDA) concentration, superoxide dismutase(SOD) activity and nitric oxide(NO) content were measured. In addition, Cu,Zn-SOD and inducible NO synthase(iNOS) enzymatic contents in myocardial tissue were qualitatively examined by immunohistochemical assaying. Result Compared with the control, MDA concentration, SOD activity and Cu,Zn-SOD enzymatic content in group B were significantly increased(P<0.05).But in TP protection group, myocardial MDA formation was significantly decreased(P<0.01) and SOD activity and Cu,Zn-SOD expression restored to normal. On the contrary, myocardial NO generation and iNOS expression were significantly reduced after repeated inhalation of hypobaric oxygen at 5500 m.NO metabolism regained to normal after repeated administration of TP. Conclusion Natural antioxidant TP had protective effects on myocardial free radical metabolic disorder induced by inhalation of hypobaric pure oxygen under 5500 m hypobaric condition.
