ABSTRACT

ABSTRACT Objective Intrathyroid injection of dexamethasone (IID) was used for decrease the relapse rate of hyperthyroidism in the treatment of Graves' disease (GD), but the mechanism is still unclear. We aimed to explore the effect of IID on T help (Th)1/Th2 cells and their chemokine in patients with GD. Subjects and methods A total of 42 patients with GD who were euthyroidism by methimazole were randomly divided into IID group (n = 20) and control group (n = 22). Thyroid function and associated antibody, Th1/Th2 cells proportion, serum CXCL10 and CCL2 levels, and CXCR3/CCR2 mRNA expression in peripheral blood mononuclear cells before and after 3-month IID treatment were tested by chemiluminescence assay, Flow cytometry, ELISA, and real-time PCR, respectively. Thyroid follicular cells were stimulated by IFN-γ and TNF-α and treated with dexamethasone in vitro. CXCL10 and CCL2 levels in supernatant were determined. Results After 3-month therapy, the proportion of Th2 cells and serum CCL2 levels, as well as TPOAb, TRAb levels and thyroid volume decreased in IID group (p < 0.05). However, the proportion of Th1 and CXCL10 levels had no change in IID group and control (p > 0.05). The CXCR3/CCR2 ratio had no change in both groups (p > 0.05). Conclusion IID therapy could inhibit peripheral Th2 cells via decreasing CCL2 level in peripheral blood, and this result partly explain the effects of IID therapy on prevention of relapse of GD. @#[摘 要] 目的:探讨miR-202-5p对口腔鳞状细胞癌(oral squamous carcinoma,OSCC)细胞生长、集落形成、迁移和侵袭的影响及其可能的机制。方法:通过qPCR法检测OSCC细胞系(Tca8113和SCC-4)和口腔角质细胞HOK中miR-202-5p和T细胞核因子c3(nuclear factor of activated T-cells isoform c3,NFATc3)mRNA的表达水平;将miR-202-5p mimic或/和NFATc3过表达质粒转染入Tca8113和SCC-4细胞,用MTT和集落形成实验检测转染对细胞增殖的影响,划痕伤口愈合实验和Transwell实验检测转染对细胞迁移和侵袭的影响,用Western blotting实验检测转染对NFATc3蛋白表达的影响;通过双荧光素酶报告基因实验验证miR-202-5p对候选靶基因NFATc3的直接调控作用。结果:与正常口腔角质细胞HOK相比,miR-202-5p在OSCC细胞Tca8113和SCC-4中呈低表达(均P<0.01),NFATc3 mRNA和蛋白质表达显著升高(P<0.01)。在Tca8113细胞和SCC-4细胞中,过表达miR-202-5p可显著抑制细胞生长、集落形成、迁移、侵袭以及细胞中NFATc3表达(均P<0.01)。NFATc3被证实是miR-202-5p的靶基因,过表达NFATc3能逆转miR-202-5p对OSCC细胞生长、迁移和侵袭的抑制作用。结论:miR-202-5p通过下调NFATc3表达发挥其肿瘤抑制功能,导致OSCC细胞的生长、迁移和侵袭受到抑制。

ABSTRACT Objective: The aim was to characterize blood glucose fluctuations in patients with fulminant type 1 diabetes (FT1DM) at the stable stage using continuous blood glucose monitoring systems (CGMSs). Subjects and methods: Ten patients with FT1DM and 20 patients with classic type 1 diabetes mellitus (T1DM) (the control group) were monitored using CGMSs for 72 hours. Results: The CGMS data showed that the mean blood glucose (MBG), the standard deviation of the blood glucose (SDBG), the mean amplitude glycemic excursions (MAGE), the blood glucose areas and the percentages of blood glucose levels below 13.9 mmol/L were similar between the two groups. However, the percentage of blood glucose levels below 3.9 mmol/L was significantly higher in the FT1DM group compared to the T1DM group (p < 0.05). The minimum (Min) blood glucose level in the FT1DM group was significantly lower than that of the T1DM group (p < 0.05). Patients with FT1DM had severe dysfunction of the islet beta cells and alpha cells compared to patients with T1DM, as indicated by lower C-peptide values and higher glucagon/C-peptide values. Conclusion: In conclusion, patients with FT1DM at the stable stage were more prone to hypoglycemic episodes as recorded by CGMSs, and they had a greater association with severe dysfunction of both the beta and alpha islet cells compared to patients with T1DM.

ABSTRACT

Objective:To explore the clinical features,surgical treatment and prognosis in acute myocardial infarction (AMI) patients with ventricular septal rupture (VSR) complicating cardiogenic shock (CS).Methods:A total of 77 AMI-VSR patients received surgical repair in our hospital from 2005-01 to 2015-05 were retrospectively studied.The patients were divided into 2 groups:CS group n=52 and Without CS group,n=25.Clinical features were compared between 2 groups and the outcomes in CS group were analyzed.Results:Both groups showed multiple aneurysm (73.0% vs 68.0%);CS was more occurred in patients with posterior VSR (48.0% vs 24.0%),P=0.044.Compared to Without CS group,CS group had the larger diameter of VSR (16.16±6.73) mm vs (11.86±4.62) mm,P=0.003;lower LVEF (45.0±8.8) % vs (47.9±12.3) %,higher pre-operative application rates of IABP (34.6% vs 0%) and vasoactive drugs (96.2% vs 28.0%),more patients received emergent surgery (42.3% vs 8.0%) and less patients received elective surgery (57.7% vs 92.0%),all P<0.05.In CS group,there were 3 in-hospital death,49 patients survived with the mean follow-up time at (4.5±3.1) years and 2 patients died during that period.Conclusion:Larger or posterior VSR were more likely to develop CS,the patients survived after surgical treatment may have good mid-term outcomes.

ABSTRACT

Accurate prenatal diagnosis of complex congenital cardiovascular anomalies,vascular ones in particular,is still challenging.A fetal cardiovascular cast model can provide a copy of the cardiac chambers and great vessels with normal or pathological structures.This study was aimed to demonstrate three-dimensional anatomy of complex congenital cardiovascular anomalies in fetuses by means of corrosion casting.Twenty fetuses with prenatal-ultrasound-diagnosed complex cardiovascular anomalies were enrolled in this study (19 to 35 gestational weeks).Fetal cardiovascular cast models were made by a corrosion casting technique.The specimens were injected with casting material via the umbilical vein,and then immersed in strong acid after casting fluid was solidified,to disclose the geometries of cardiovascular cavities.Nineteen cast models were successfully made from 20 specimens.The casts distinctly showed the morphological malformations and spatial relationship between cardiac chambers and great vessels.One hundred and eleven abnormalities were revealed by casting in the 19 specimens,including 34 abnormalities located in the cardiac chambers (3,4 and 27 anomalies in the atria,atrioventricular valves and ventricles,respectively),and 77 in the great vessels (28,20,24 and 5 anomalies in the aorta and its branches,the pulmonary artery,the ductus arteriosus and the major veins,respectively).Corrosion casting can display three-dimensional anatomy of fetal complex cardiovascular anomalies.This improves our understanding of related pathomorphology and prenatal diagnosis.

ABSTRACT

Objective To understand the characteristics of imported malaria cases in Qingcheng District,Qingyuan City and explore the strategies and priorities in prevention and control,so as to provide the evidence for improving the diagnosis, treatment and management of imported malaria. Methods The data of imported malaria as well as the case epidemiological in-vestigations were collected and retrospectively analyzed for the species composition,original countries,population distribution, regional distribution,onset situation,diagnosis,treatment,etc. in Qingcheng District from 2011 to 2016. Results The num-ber of imported malaria cases was 13 from 2011 to 2016. All the patients were confirmed by laboratory,and of which,9 patients infected with Plasmodium falciparum,1 with P. vivax,1 with P. ovale and 2 with mixed infections(P. vivax and P. falciparum). The yearly incidence of imported malaria presented an uptrend. The infection sources of all the patients were from African coun-tries,and the exported labor workers and travelers for business from malaria endemic areas were the high risk population. The re-ported time was mainly January,February,November and December(11/13,84.62%). All the patients were male,and the ma-jority of them(12/13,2.31%)were 21-60 years old. The median time from onset to seeing a doctor was 2.5 days and the median time from seeing a doctor to being diagnosed was 1.9 day. Six patients(46.15%)were diagnosed as other diseases at the first vis-it to a doctor,and one patient died of falciparum malaria because of delayed diagnosis. Conclusions The incidence of overseas imported malaria presents an uptrend in Qingcheng District. It is necessary to further strengthen the professional training in medi-cal staff to improve the diagnosis and treatment of malaria cases. It is also necessary to strengthen the multisectoral cooperation , establish the surveillance in the high risk population,etc.

ABSTRACT

Paragonimiasis is an infectious disease caused by Trematodes of the genus Paragonimus that is endemic in Asia, Africa, and South America. Most patients with paragonimiasis are cured by standard praziquantel treatment. However, several cases have been reported to have unsatisfactory responses to the standard praziquantel treatment. To probe the clinical characteristics, possible cause, and management of the paragonimiasis individuals improved by multiple therapies, we present a 12‑year‑old Chinese boy, who was infected with Paragonimus accompanied by arachnoid cyst involvement, as not having typical clinical symptoms, but repeatedly presenting with migrated lesions between the lung and pleura. He responded to treatment with 3 cycles of praziquantel and 1 cycle of albendazole.

Extraction is often used as part of orthodontic therapy, and good control of anchorage is a key step after extraction. Although microscrews can be implanted close to the extraction site in order to achieve orthodontic support, the efficiency of bone remodeling at the implant-bone interface near the extraction region is dubious. OBJECTIVE: The purpose of this study was to investigate bone remodeling of the bone-microscrew interface near the tooth extraction site, in the absence of loading. MATERIAL AND METHODS: Third and fourth premolars were extracted from the mandibles of beagle dogs, followed by placement of test microscrews near the extraction sites. Control microscrews were placed further away from the extraction site. All samples were collected after 1, 3, 8, or 12 weeks of healing following extraction. The bone remodeling process at the interface was evaluated using histologic and immunohistochemical analyses. RESULTS: Initially, a large number of inflammatory cells were aggregated at the interface. The expression levels of core binding factor (Cbfa1), osteocalcin (OC) and transforming growth factor beta (TGF-β) were inconspicuous in both groups, whereas tumor necrosis factor alpha (TNF-α) was strongly expressed, especially in the test groups (P<0.05). Subsequently, the expression levels of Cbfa1, OC and TGF-β were found to increase significantly, and active osteogenesis was observed. CONCLUSIONS: During week 1, inflammatory reaction is a major concern at the bone-microscrew interface near the extraction site. However, with healing, the influence of extraction on the remodeling of bone surrounding the microscrews decreases, thus facilitating successful treatment. .
