ABSTRACT

Objective To investigate the genetic polymorphisms of Plasmodium falciparum multidrug resistance protein 1 (PfMDR1), chloroquine resistance transporter (PfCRT) and Kelch 13 (PfK13) genes in Bioko Island, Equatorial Guinea, so as to provide insights into the development of the malaria control strategy in local areas. Methods A total of 85 peripheral blood samples were collected from patients with Plasmodium falciparum infections in Bioko Island, Equatorial Guinea in 2018 and 2019, and genomic DNA was extracted. The PfMDR1, PfCRT and PfK13 genes were amplified using a nested PCR assay. The amplification products were sequenced, and the gene sequences were aligned. Results There were no mutations associated with artemisinin resistance in PfK13 gene in Bioko Island, Equatorial Guinea, while drug-resistant mutations were detected in PfMDR1 and PfCRT genes, and the proportions of PfMDR1_N86Y, PfMDR1_Y184F and PfCRT_K76T mutations were 35.29% (30/85), 72.94% (62/85) and 24.71% (21/85), respectively. Conclusion There are mutations in PfMDR1, PfCRT and PfK13 genes in P. falciparum isolates from Bioko Island, Equatorial Guinea.

ABSTRACT

Objective@#To investigate the effect of the transverse incision of the lower segment of the upper uterus in the treatment of scar uterine redelivery, and to analyze the safety of the surgical method.@*Methods@#88 patients with scar uterus who received cesarean section were randomly selected as research subjects, aged 24-37 years old, weight 53.3-76.9kg.According to the method of random numbers, they were divided into observation group and the control group, 44 cases in each group.The observation group received transverse incision of the lower segment of the uterus.The control group received conventional cesarean section.The operation time, intraoperative conditions, surgical incision healing, overall hospitalization time and neonatal Apgar score were statistically observed.@*Results@#The intraoperative blood loss of the observation group[(50.1± 3.2) mL]was significantly higher than (48.9±1.7)mL of the control group, and the difference between the two groups was statistically significant(t=2.099, P<0.05). The fetal delivery time, postoperative ventilation time in the observation group were (9.5±3.3)min, (331±97)min, respectively which were significantly shorter than those in the control group, and the differences between the two groups were statistically significant(t=3.018, 4.733, all P<0.05). The neonatal Apgar score of the observation group (9.3±1.2) was significantly higher than (8.8±2.4) of the control group(t=4.800, P<0.05). In the observation group, newborn hypoxia 0 cases, 1 case of aspiration pneumonia, maternal postpartum hemorrhage in 2 cases.There was no statistically significant difference in incidence of complications between the two groups (χ2=1.03, P>0.05). In the observation group, maternal incision healing class A in 40 cases, scar clearly generated in 7 cases, which in the control group were 37 cases, 11 cases, respectively, the difference between the two groups was statistically significant(t=4.200, P<0.05).@*Conclusion@#Transverse incision of the lower segment of the uterus complied with the tension of skin line, is beneficial to maternal postpartum incision healing, and to some extent can relieve pain, promote parturient recovery, further promote the puerpera restoration, significantly improve the clinical outcome of neonatal birth, postoperative complications and reduce cesarean section, it is worthy of clinical promotion.

ABSTRACT

Objective To observe the clinical efficacy of terbinafine in the treatment of complicated vulvovaginal candidiasis.MethodsA case of 100 patients with complicated vulvovaginal candidiasis treated in our hospital from January 2015 to December 2016, were randomly divided into treatment group (terbinafine) and the control group (fluconazole),each 50 cases,the treatment cycle is four weeks, the therapeutic effect of two groups was observed.ResultsIn the treatment group 45 cases cured, three cases were markedly effective, two cases effective, the total effective rate was 96%;In the control group 41 cases cured, three cases were markedly effective, six cases effective, the total effective rate was 88%.The difference between the two groups was statistically significant.(P<0.01).ConclusionTerbinafine treatment of complicated vulvovaginal candidiasis was significant.

ABSTRACT

Objective@#To investigate the clinical effects of aripiprazole on sexual dysfunction induced by amisulpride or risperidone in male patients with schizophrenia.@*METHODS@#This study included 75 male patients with drug-induced secondary sexual dysfunction after treated with amisulpride or risperidone for first-episode schizophrenia between October 2014 and October 2016. We substituted aripiprazole for amisulpride or risperidone, gradually increased the dose from 10 to 30 mg/d within 2 weeks, and maintained 30 mg/d from the 3rd week. At 4 and 8 weeks after medication, we evaluated the sexual function of the patients, measured the levels of serum prolactin (PRL) and testosterone (T), obtained the scores of the Positive and Negative Symptoms Scale (PANSS), recorded adverse reactions, and compared the parameters with those before aripiprazole administration.@*RESULTS@#Compared with pre-aripiprazole administration, the patients showed significant increases after 4 weeks of medication in the sexual function score (24.3 ± 2.1 vs 32.6 ± 3.6, P 0.05). In comparison with pre-aripiprazole administration, the PANSS score was significantly decreased at 4 weeks after medication (62.1 ± 4.9 vs 57.2 ± 5.5, P <0.05) and even lower at 8 weeks (51.2 ± 5.2) (P <0.05). The incidence rates of medication-related excitation, dizziness, insomnia, and loss of appetite were 6.7%, 5.3%, 4.0% and 1.3% respectively, and no other serious adverse reactions were observed.@*CONCLUSIONS@#Aripiprazole is effective for the treatment of drug-induced sexual dysfunction in schizophrenic men by continuously alleviating their positive and negative symptoms and meanwhile improving their sexual function and restoring their sexual hormone levels.

ABSTRACT

Objective:To investigate the role of B cell adoptor protein with ankyrin repeats ( BANK ) in experimental autoimmune encephalomyelitis ( EAE) .Methods: C57BL/6 mice and BANK-deficient ( BANK-/-) mice were immunized with MOG peptide in CFA,and then observed the clinical symptoms and pathological severity .Results: The percentages of CD4+T cells,CD8+T cells and regulatory T cells in brain and spleen were analyzed by flow cytometry .BANK-/-mice showed significantly higher score at the peak and the plateau phase compared with wild-type mice(P<0.05).HE staining showed more widespread areas of inflammation and demyelination in BANK-/-mice when compared to wild-type mice on day 16.In addition,the frequency of CNS-infiltrating CD8+T cells was markedly higher in BANK-/-mice than in wild-type mice.In addition,the percentage of CD8+T cells from spleen in BANK-/-mice was also increased compared with wild-type mice (P<0.05).By contrast,the percentage of regulatory T cells and the ratio of CD 4/CD8 T cells from spleen in BANK-/-mice were significantly lower than in wild-type mice(P<0.05).Conclusion:Thus,BANK expression in B cells can inhibit the development of EAE .

ABSTRACT

Objective To investigate the therapeutic effect of 3D curvature traction on neck-type cervical spondylosis.Methods Totally 100 neck-type cervical spondylosis patients were divided randomly and equally into an observation group and a control group.The observation group underwent the treatment by 3D curvature traction combined with intermediate frequency therapy,and the control group went through the treatment by cupping and intermediate frequency therapy.Evaluation was carried out on neck pain,tenderness and cervical mobility before and after the treatment.Results The recovery rate was 88% in the observation group,which was significantly higher than that (40%) in the control group (P=0.007).The total effective rate was 100% in the observation group,which was obviously higher than that (88%) in the control group (P=0.029).The score differences for pain and cervical mobility before and after the treatment were 4.96±0.36 and 1.67±0.73 respectively in the observation group,which were statistically different from the score differences for pain (3.38±0.80) and cervical mobility (0.88±0.76) in the control group (P<0.05).Conclusion 3D curvature traction gains high clinical effect when used to treat neck-type cervical spondylosis.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the therapeutic effect of debridement and vacuum sealing drainage (VSD) of cavitas medullaris for the treatment of chronic osteomyelitis of tibia.</p><p><b>METHODS</b>From March 2006 to May 2009, 19 patients with chronic osteomyelitis of tibia were treated by debridment and VSD, then the second operation were performed to close the wound. Among them, 12 patients were male and 7 patients were female, the average age was 39 years (ranged from 25 to 68 years). The course of disease were from 10 months to 5 years. The main clinical symptoms were red swelling, tenderness and fluid of local soft tissue. There were prolonged unhealed sinus and pus; the X-ray showed osteosclerosis, increased bone mineral, and sequestrum and dead space was formed. The result of bacterial culture showed 3 cases were aeruginosus bacillus, 13 cases staphylococcus aureus, 1 case bacillus aerogenes and 2 cases beta streptococcus. Among them, 3 cases were methicillin resistant staphylococcus (MRS).</p><p><b>RESULTS</b>After debridement and VSD of cavitas medullaris 18-22 days later, the granulation tissue grow well and the wounds of the 19 patients all healed primarily with direct suturing of 17 cases, loco-regional flap of 2 cases. The standard of wound healing was the dryness, cleanness and no drainage. The X-ray revealed the bone tissue grew well and no relapse and fracture occurred during followed-up 6-12 months.</p><p><b>CONCLUSION</b>The debridement and VSD of cavitas medullaris is a very effective and safe treatment for chronic osteomyelitis of tibia.</p>

ABSTRACT

Objective To explore the acquired deficiencies of vitamin K-dependent coagulation factors in etiology, clinical characteristics and treatment. Methods Retrospective analysis was performed on the data of etiology, clinical manifestations of 45 patients with acquired deficiencies of vitamin K-dependent coagulation factor. All patients were treated with Vitamin K1 10 -40 mg/d, i. v. , for three months. Some patients with severe blooding were additionally treated with fresh freezing plasma or prothromibin complex. Prothrombin time(PT) and activated partial thromboplastic time(APTT) were measured using Stago automatic blood coagulation analyzer before and after treatment. Ⅱ , Ⅶ, Ⅸ and Ⅹ were measured in some patients. Results Among the 45 cases, no certain cause was found in 19 cases (42.2%), anticoagulant rodenticides poison was a common cause ( 11 cases,42.3% ). The main presentations was hemorrhage, the most common bleeding sites were mucosa (77.8%) (35/45)and hematuria (46.7%) ( 21/45 ). After vitamin K1 treatment, PT and APTT had shortened remarkably from ( 110.35 ± 35.36 ) s,(98.91 ±48.98)s to (13.48 ±2. 17)s,(33.25 ±6.95)s,respectively(t=19.10 and 6.19,Ps <0.01)and the activities of factor Ⅱ、Ⅶ、Ⅸ、Ⅹ had rapidly increased from ( 17.48 ± 10.93 ) %, ( 10.23 ± 5.68 )%, ( 11.98 ±4.69)%,(12.93±7.48)% to (70. 12 ±21.31)%,(92.76 ±29. 15)%,(88.64 ±40. 21)%,(63.97 ±20.11)%(t=12.13,14.43,13.27and9. 74,respectively,Ps<0. 01).Conclusions The histories of patients with acquired deficiencies of vitamin K-dependent coagulation factors are usually hiding, therefore it is easily misdiagnosed. It is worth of detecting PT and APTT in diagnosis and monitoring. Using vitamin K1 10 -40 mg/d is effective and safety.

ABSTRACT

<p><b>OBJECTIVE</b>The aim of this study is to evaluate whether the firing times affect the chroma of zirconia by adding rare-earth oxides.</p><p><b>METHODS</b>Six kinds (S1, S2, S3, S4, S5, S6) of tooth-like yttria stabilized tetragonal zirconia polycrystal (Y-TZP) were available by introducing internal colorating technology, the color were gauged with ShadeEye NCC chromatometry instrument, and one-way ANOVA was used to analysis the color of each kind of tooth-like zirconia after 1, 2, 3, 4, 5 times firing individually.</p><p><b>RESULTS</b>After 1, 2, 3, 4, 5 times firing respectively, the chromatic aberration ranged between 0.10-1.47 merely. The luminosity of three kinds (S1, S2, S6) of tooth-like zirconia were decreased (P < 0.05), the luminosity of the other three kinds (S3, S4, S5) of tooth-like zirconia were not obviously changed (P > 0.05), as for the hue and chroma, no significant differences were found (P > 0.05).</p><p><b>CONCLUSION</b>There are no significant influence on the color of tooth-like Y-TZP after 1, 2, 3, 4, 5 times firing respectively, the chroma of yttria stabilized tetragonal zirconia polycrystal by adding rare-earth oxides are with high stability.</p>

ABSTRACT

<p><b>AIM</b>To isolate triterpene saponins of polygalacic acid type from the roots of Platycodon grandiflorum (Jacq.) A. DC and to identify their structures.</p><p><b>METHODS</b>The compounds were separated by means of extraction, chromatography on silica gel, MPLC and HPLC, and their structures were elucidated on the basis of spectral analyses (FAB-MS, IR, 1H NMR, 13C NMR etc.).</p><p><b>RESULTS</b>Three triterpene saponins were isolated from the roots of Platycodon grandiflorum. They were identified as 3-O-beta-D-laminaribiosyl polygalacic acid (I), 3-O-beta-D-glucopyranosyl polygalacic acid (II), polygalacin D (III), separately.</p><p><b>CONCLUSION</b>Compound I is a new compound, compounds II, III are known triterpene saponins. The compound I and II were isolated from the plant for the first time, which is also the monodesmoside from the plant for the first time.</p>
