ABSTRACT

Objective:To study the clinical application of bronchoscopy finding causes of neonatal dyspnea after withdrawal of ventilation and determining intervention strategy.Methods:From November 2018 to August 2021, fiberoptic bronchoscopy results and further treatments of neonates with dyspnea after withdrawal of ventilation in our hospital were retrospectively analyzed.Results:A total of 45 patients experienced dyspnea after withdrawal of ventilation, including 16 cases of bronchial mucositis (35.6%) and 29 cases of extrapulmonary airway obstruction (64.4%). The 29 extrapulmonary airway obstruction cases included 15 cases of subglottic stenosis (11 caused by hyperplastic scar and 4 caused by sputum scab), 6 cases of simple laryngomalacia, 2 cases of nasal stenosis, 2 cases of laryngeal web, 1 case of laryngeal edema, 1 case of vocal cord paralysis, 1 case of vocal cord excrescence and 1 case of airway spasm. 16 patients with bronchial mucositis were cured after airway lavage and oxygen therapy or noninvasive respiratory support. For 11 patients of subglottic stenosis caused by hyperplastic scar, 9 cases were cured using tracheal tube expansion technique and 2 cases were cured with endoscopic intervention. One patient of nasal cavity stenosis, two patients of laryngeal web and one patient of vocal cord excrescence were cured after surgical treatment. The other 14 patients were cured and discharged after conservative treatment.Conclusions:For neonates with dyspnea after withdrawal of ventilation in neonatal intensive care unit (following proper withdrawal process), extrapulmonary airway obstruction should be considered, especially subglottic stenosis after intubation.

ABSTRACT

Objective To investigate and analyze the humane care quality and behavior of clinical nurses in informed consent, and provide theoretical basis about how to implement humane care in clinical nursing practice. Methods Using Nurse Humanities Care Quality Table which developed by Liu Yuxi , and Nurse Informed Consent Humanistic Behavior Self-rating Scale witch designed by self, To investigate 110 clinical nurses who picked out by hierarchical random sampling. Results In the humanistic care quality assessment, the score of the ability was the lowest in nurses′humanistic care quality, the average score (23.06 ± 1.72) accounted for 65.88% of the total score (35). the score of knowledge dimension was positively correlated with age and working years (r=0.339, 0.294, both P<0.01). the score of perceived dimension was positively correlated with age (r=0.250, P<0.01). In the humanistic care behavior assessment of informed consent, the score of the communication and emergency ability dimension were the lowest. The average score (21.65±1.93) accounted for 86.60% of the total score (25). The marital status of the clinic nurses was negatively correlated with respect and care (r=-0.305, P<0.01). The humanistic knowledge was positively correlated with support and communication and emergency response (r=0.422, 0.371, both P<0.01), the concept was positively correlated with respect and care, support and security (r=0.640, 0.587, both P<0.01). Conclusions The quality of nurses′humanistic care is significantly related to the humanistic care behavior in informed consent. It should improve the quality of nurses′humanistic care for cultivating humanistic care behavior in nursing practice.

ABSTRACT

Objective To explore the expression of RET in cervical squamous carcinoma tissues and evaluate its relationship with cervical squamous carcinoma clinical pathological indexes and its prognostic value. Methods The expression and distribution of RET in normal cervical tissues ,CINⅠ,CINⅡ& CINⅢ and cervical squamous carcinomas tissues were detected by immunohistochemistry in wax blocks. Clinical data and follow-up data are integrated to analyze its relationship with clinical pathological factors and prognosis value. Results The positive rate of RET protein in cervical squamous carcinomas tissues is higher than in other tissues.The positive rate was related to FIGO stage and lymph node metastasis(P0.05). The result of survival analysis with Kaplan-Meier method showed poorer disease-free survival time in the patients with high expression of RET (P < 0.05). Prognostic analysis of patients with cervical cancer through COX proportional hazard regression model suggested that RET expression was an independent prognostic factor. Conclusions RET has been involved in the progression ,FIGO stage and metastasis of cervical squamous cell carcinomas.

ABSTRACT

Objective:To study the effects of deoxyadenosine(dAdo) on methotrexate (MTX) induced suppression of inflammatory bone destruction.Methods:The culture system of whole bone marrow cells (WBMCs) was utilized to evaluate osteoclastogenesis.TRAP staining and μCT analysis were utilized to evaluate osteoclastogenesis and bone destruction in adjuvant arthritis rats.Results:In the bone marrow culture system,MTX-induced suppression of osteoclastogenesis was abrogated by the addition of dAdo.dAdo canceled MTX-induced suppression of osteoclastogenesis in the rats with arthritis,and significantly abolished the therapeutic effects of MTX on inflammatory bone destruction in the rats.Conclusion:The accumulation of dAdo may be one cause of the losing effectiveness of MTX in bone destruction.

ABSTRACT

<p><b>OBJECTIVE</b>To assess the effects of moxibustion at Shenque (CV8) in promoting gastrointestinal function recovery in rats following gastric perforation repair and explore the underlying mechanism.</p><p><b>METHODS</b>Thirty male SD rats with glacial acetic acid-induced gastric perforation underwent surgical repair of the perforation. The rats were then randomized 8 days later into model group (n=10), domperidone group (n=10), and moxibustion group(n=10) and treated with physiologic saline, domperidone suspension, and moxibustion at Shenque (CV8), respectively. Gastric antral myoelectric activities of the rats were observed and peripheral blood levels of TNF-α, IL-6, and T lymphocyte subpopulation were determined.</p><p><b>RESULTS</b>Gastric antral myoelectric activities in rats receiving moxibustion were stronger than those in the model group (P<0.05) but comparable with those in domperidone group (P>0.05). TNF-α and IL-6 level were decreased significantly and T lymphocyte subpopulations increased significantly in moxibustion group compared with those in the model and domperidone groups (P<0.05).</p><p><b>CONCLUSION</b>Moxibustion at Shenque (CV8) can promote the recovery of gastrointestinal functions in rats undergoing gastric perforation repair possibly by enhancing gastrointestinal electric activity, suppressing inflammation, and improving the cellular immune function, and can therefore serve as a simple and effective adjuvant therapy during the perioperative period.</p>

ABSTRACT

The changes of serum malondialdehyde, myeloperoxidase, total antioxidant capacity, superoxide dismutase activity, and glutathione peroxidase were observed in patients with Graves′ disease. The myeloperoxidase level increased in patients with Graves′ disease. The balance between oxidation and antioxidative defense was disrupted in patients with newly-onset Graves′ disease. Oxidative stress seems to be related to hyperthyroidism.

ABSTRACT

Objective To investigate clinical significance of intermittent sinusoidal feial heart rate at third trimester.Methods From Jan 2002 to Dec 2010,48 pregnant women at 33 to 41 gestational weeks undergoing electronic fetal heart rate(FHR)monitoring presented with intermittent sinusoidal FHR in Department of Obstetrics and Gynecology,Second School of Clinical Medicine,Jinan University were enrolled in this retrospective study.Twenty-one cases were categorized into continuous group(i.e.with sinusoidal feature and a constant duration≥10 minutes).while the other 27 cases were categorized iuto intermittent group(i.e.with a duration<10 minutes).In the mean time.76 normal cases were chosen randomly matched as control group.Blood gas and hemoglobin were measured in umbilical artery after fetal head delivery.General neurological system examination were performed in those fetus in hospitalization.The outcome of those fetuses was compared.Results (1)Neonatal complications:the rate of asphyxia,meconium-stained amniotic fluid and fetal anemia were 63%(17/27),33%(9/27)and 63%(17/27)in group of intermittent sinusoidal FHR,which were significantly higher than 1%(1/76),4%(3/76),3%(2/76)in control group(P<0.05).When compared with 67%(14/21),52%(11/21),76%(16/21)in group of continuous sinusoidal,the statistical difierence were not observed(P>0.05).(2)Blood gas in neonate:the rates of pH less than 7 were 18%(5/27)in intermittent group,52%(11/21)in continuous group and 0 in control group,which all reached statistical difference among those three groups(P<0.05).(3)Brain damage and death:the rates of brain damage and death were 48%(13/27)and 11%(3/27)in intermittent group,81%(17/21)and 43%(9/21)in continuous group,and 0 in control group,which all showed significant difference between them(P<0.05).Conclusion Intermittent and continuous sinusoidal FHR are typical graphics of severe fetal anemia at third trimester.Intermittent sinusoidal FHR is indicative of serious fetal hypoxia.
