ABSTRACT

ObjectiveTo establish a qualitative and quantitative analysis method for chemical constituents in Liu Junzitang(LJZT), and to clarify its material basis. MethodThe chemical constituents in LJZT were analyzed by ultra performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS), and the resulting compounds were identified by using databases, such as MassBank, PubChem, ChemSpider, Traditional Chinese Medicine Systems Pharmacology Database and Analytical Platform(TCMSP), and by combining with relevant literature. UPLC was used to establish a quantitative method for analysis of 9 compounds in LJZT, including liquiritin, hesperidin, lobetyolin, liquiritigenin, glycyrrhizic acid, nobiletin, tangeretin, atractylenolide Ⅱ and Ⅰ. ResultBy combining the relevant literature, database and MS information, a total of 79 compounds were identified from LJZT, including 31 flavonoids, 15 terpenoids, 14 nitrogen-containing compounds, 6 phenylpropanoids, 6 organic acids and 7 other compounds. The established quantitative analytical method for the nine representative components showed good linearity within their respective linear ranges, and the precision, stability, reproducibility and recovery were in accordance with the requirements. The quantitative results showed that the contents of liquiritin, hesperidin, lobetyolin, liquiritigenin, glycyrrhizic acid, nobiletin, tangeretin, atractylenolide Ⅱ and Ⅰ in LJZT were 0.376 5, 2.602 1, 0.082 6, 0.128 1, 1.778 6, 0.015 7, 0.006 7, 0.030 4, 0.003 2 mg·g-1, respectively. ConclusionThe established method can quickly, sensitively and accurately analyze the chemical constituents in LJZT, clarify that the material basis of LJZT is mainly flavonoids, terpenoids and nitrogen-containing compounds, and simultaneously determine the contents of the 9 components, which can lay a foundation for the research on quality control, mechanism and clinical application of LJZT.

ABSTRACT

Objective:To investigate the application value of intervention mapping-based health education on therapeutic cognitive perception of pulmonary arterial hypertension patients.Methods:Totally, 60 patients with pulmonary arterial hypertension from January 2018 to January 2021 in Huazhong Fuwai Central Vascular Disease Hospital were assigned to the experimental group and control group according to admission time, with 30 cases in each group. The control group received routine health education, while the experimental group implemented intervention mapping-based health education. The intervention effects were assessed by Beliefs about Medicine Questionnaire-Specific (BMQ-S), Morisky Medication Adherence Scale (MMAS), 6MWT as well as Borg Scale (BS).Results:At 1 month and 3 months after discharge, the scores of MMAS in the experimental group were significantly higher than in the control group[(5.43±1.17) points, (5.57±1.17) points vs (4.60±1.54) points and (4.87±1.41) points], the differences were statistically significant ( t=2.36, 2.10, both P<0.05). At 3 months after discharge, the scores of necessity belief subscale in the experimental group were higher than those in the control group [(22.93±2.63) points vs (21.27±2.73) points], concern belief subscale scores lower than in the control group [(12.40±2.54) points vs (14.13±3.01) points], 6MWT distance in the experimental group higher than in the control group [(532.13±38.51) m vs (507.03±51.13) m], BS scores lower than in the control group [(3.53±1.20) points vs (4.10±0.84) points], the differences were statistically significant ( t values were 2.12-2.41, all P<0.05). Conclusions:Intervention mapping-based health education can improve therapeutic cognitive perception and medication adherence of pulmonary arterial hypertension, it is helpful to promote rehabilitation of patients.

ABSTRACT

Objective:To investigate the application value of paradoxical intention therapy in patients undergoing PCI.Methods:A total of 116 patients who undergoing PCI in Fuwai Center China Cardiovascular Hospital from January 2018 to December 2019 were enrolled in the present study. They were divided into experimental group and control group with 58 cases in each group. The control group received routine therapy and nursing care, while the experimental group carried out paradoxical intention therapy based on the control group. The effects were evaluated by physiological stress index (blood pressure, heart rate), Symptom Checklist-90 (SCL-90) and General Comfort Questionnaire (GCQ) respectively.Results:At 1 day before surgery, once on the operative day and 1 day after surgery, the systolic pressure were (129.58 ± 13.89), (132.08 ± 12.28), (125.58 ± 12.28) mmHg(1 mmHg=0.133 kPa) in the experimental group, significantly lower than those in the control group (137.22 ± 11.18), (138.35 ± 14.94), (132.35 ± 14.89) mmHg, the differences were statistically significant ( t=3.14, 2.38, 2.59, all P<0.05). At 1 day before surgery and once on the operative day, the heart rate were (74.17 ± 4.18), (76.39 ± 5.14) times/minutes in the experimental group, significantly lower than those in the control group (76.62 ± 5.19), (78.78 ± 6.22) times/minutes, the differences were statistically significant ( t=2.69, 2.17, both P<0.05). After 4 weeks of intervention, the scores of somatization, compulsion, anxiety, depression and total SCL-90 were (1.25 ± 0.19), (1.42 ± 0.23), (1.19 ± 0.28), (1.20 ± 0.16), (121.66 ± 9.66) points in the experimental group, significantly lower than those in the control group (1.84 ± 0.25), (1.68 ± 0.37), (1.84 ± 0.39), (1.73 ± 0.33), (146.47 ± 8.64) points, the differences were statistically significant ( t values were 4.45-14.08, all P<0.05). The scores of physiological dimension, psychological dimension and total GCQ were (10.89 ± 2.22), (24.49 ± 4.92), (77.91 ± 6.20) points in the experimental group, significantly higher than those in the control group (8.51 ± 2.27), (21.84 ± 4.61), (72.09 ± 6.66) points, the differences were statistically significant ( t=5.50, 2.89, 4.69, all P<0.05). Conclusions:Paradoxical intention therapy can effectively alleviate physiological and psychological stress and decrease postoperative discomfort of patients undergoing PCI.

ABSTRACT

Objective:To investigate the effects of intervention mapping on cardiopulmonary function for patients with acute heart failure in compensatory period, so as to provide references for their early rehabilitation activities.Methods:A total of 90 patients with acute heart failure admitted to Fuwai Central China Cardiovascular Hospital from October 2018 to October 2019 were enrolled in the present study. They were divided into experimental group and control group according to draw-lots-method, with 45 cases in each group. The control group received the routine care and activity plan, while the experimental group implemented intervention mapping-based stage early rehabilitation program. The indicators included 6MWT, forced expiratory volume in one second (FEV 1), brain natriuretic peptide (BNP) as well as Minnesota Living with Heart Failure Questionnaire (MLHFQ), and the intervention effects were compared between the two groups. Results:There were no significant differences in FEV 1, 6MWT, BNP and MLHFQ scores between the two groups at admission ( P>0.05). On discharge, FEV 1, 6MWT were (2.17±0.44) L, (273.09±55.80) m in the experimental group, significantly higher than (1.94 ± 0.39) L and (236.44 ± 50.99) m in the control group; the plasma BNP were (676.79 ± 78.75) ng/L in the experimental group, significantly lower than (736.05 ± 78.77) ng/L in the control group; in addition, the physical demention, emotional dimenson, other demension scores and total scores of MLHFQ in the experimental group were (65.39 ± 5.02), (67.56 ± 4.99), (66.05 ± 4.16) and (66.33 ± 2.63) points, significantly higher than (59.79 ± 5.94), (64.33 ± 5.93), (62.76 ± 4.47), (62.36 ± 2.98) points in the control group, the differences were statistically significant ( t values were 2.56-6.51, all P<0.05). Conclusions:Designing and implementing stage early rehabilitation program using intervention mapping can promote cardiopulmonary function and quality of life of patients with acute heart failure in compensatory period.

ABSTRACT

Although primary vesical calculi is an ancient disease, the mechanism of calculi formation remains unclear. In this study, we established a novel primary vesical calculi model with d,l-choline tartrate in mice. Compared with commonly used melamine and ethylene glycol models, our model was the only approach that induced vesical calculi without causing kidney injury. Previous studies suggest that proteins in the daily diet are the main contributors to the prevention of vesical calculi, yet the effect of fat is overlooked. To assay the relationship of dietary fat with the formation of primary vesical calculi, d,l-choline tartrate-treated mice were fed a high-fat, low-fat, or normal-fat diet. Genetic changes in the mouse bladder were detected with transcriptome analysis. A high-fat diet remarkably reduced the morbidity of primary vesical calculi. Higher fatty acid levels in serum and urine were observed in the high-fat diet group, and more intact epithelia in bladder were observed in the same group compared with the normal- and low-fat diet groups, suggesting the protective effect of fatty acids on bladder epithelia to maintain its normal histological structure. Transcriptome analysis revealed that the macrophage differentiation-related gene C-X-C motif chemokine ligand 14 (Cxcl14) was upregulated in the bladders of high-fat diet-fed mice compared with those of normal- or low-fat diet-fed mice, which was consistent with histological observations. The expression of CXCL14 significantly increased in the bladder in the high-fat diet group. CXCL14 enhanced the recruitment of macrophages to the crystal nucleus and induced the transformation of M2 macrophages, which led to phagocytosis of budding crystals and prevented accumulation of calculi. In human bladder epithelia (HCV-29) cells, high fatty acid supplementation significantly increased the expression of CXCL14. Dietary fat is essential for the maintenance of physiological functions of the bladder and for the prevention of primary vesical calculi, which provides new ideas for the reduction of morbidity of primary vesical calculi.

ABSTRACT

Objective:To analyze the clinical characteristics of rare adrenal angiomyolipoma.Methods:Clinical data of 5 patients with adrenal angiomyolipoma admitted to West China Hospital of Sichuan University from April 2009 to April 2019 were retrospectively analyzed. There were 2 males and 3 females. Age ranged from 40 to 57 years old, with an average of 46.7(40-57) years old. Clinical manifestations included right upper abdominal mass in 1 case, pain in the lower back in 1 case, and no significant symptoms were found in the rest. One patient was complicated with hypertension, one patient was complicated with decreased activities of epinephrine, norepinephrine and renin activity (orthosis), and the other patients had no abnormal hormones. 2 patients underwent abdominal ultrasound with " strong echo mass in adrenal area" , and all underwent enhanced abdominal CT with " space occupying lesion in adrenal area" , which was specifically manifested as tumors with mixed density in fat, blood vessels, muscle and so on. The average tumor diameter was 5.8(2.3-9.1) cm, including 2 cases on the left, 3 cases on the right, and 1 case with renal angiomyolipoma. All patients underwent laparoscopic adrenal tumor resection.Results:All the 5 patients underwent surgical resection successfully and were diagnosed as adrenal angiomyolipoma by pathological examination of tumor specimens after surgery. There was no significant change in blood pressure level of patients with hypertension after surgery. Follow-up time was 2-7 years and no recurrence.Conclusions:Adrenal angiomyolipoma is an extremely rare benign nonfunctional disease, which is more common in middle age. Most of the patients had no clinical history and signs. The tumor has no hormone secretion function and can be basically diagnosed with CT and other imaging examinations. The final diagnosis depends on pathological examination. After operation, the prognosis of adrenal angiomyolipoma is good.

ABSTRACT

Early recognition and treatment for early warning electrocardiogram (ECG) of sudden death are very important to prevent and treat malignant arrhythmia and sudden death. Previous studies have found that R-on-T and T wave alternation, and QT interval prolongation are closely related to malignant arrhythmia or sudden death, which are included in the critical value of ECG.By analyzing the ECG characteristics of 4 patients with sudden death, we found that although the causes of the patients were different, there were transient prolongation of QT interval after premature contraction in 12 lead ECG, followed by malignant arrhythmia or sudden death. Thus, we thought that the transient prolongation of QT interval after premature contraction had a high value for warning malignant arrhythmia or sudden death. This phenomenon should be paid enough attention to reduce the risk of sudden death.

Our report reviews the literature and outlines of the treatment and management plans for patients with this rare and potentially dangerous disease.

Objective To investigate the distribution of blaOXA-51-like genes and the clonal relation-ship among Acinetobacter baumannii strains isolated from three teaching hospitals in Guangzhou , China. Methods Fifty-two Acinetobacter baumannii isolates were genotyped by multilocus sequence typing (MLST).eBURST algorithm was performed to define clonal complexes (CCs).blaOXA-51-like genes were am-plified by using polymerase chain reaction ( PCR) and sequenced .Results MLST grouped the A.bauman-nii isolates into 5 existing sequence types (STs) and 7 new STs.STn4 carried allele G1 with a T→C muta-tion at the 3rd nucleotide site (nt3) on the gpi111 locus.STn5 carried allele A1, possessing A→C muta-tions at nt156 and nt159 on the gltA1 locus.ST195 and ST208 accounted for 69.2%of all isolates.Clonal relationship analysis showed that ST 195 and ST208 belonged to CC92.Fifty-one A.baumannii isolates car-ried OXA-66 and the rest one carried OXA-199.Conclusion A.baumannii strains that belonged to CC92 and carried OXA-66 were the predominant genotype circulating in Guangzhou , China.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the immuno-effect of plasmacytoid dendritic cells (pDC) on bacteria infection induced spontaneous remission (SR) of leukemia.</p><p><b>METHODS</b>Both pDC and myeloid dendritic cells (mDC) were isolated and purified from leukemic patient with SR and healthy donor by combination of immunomagnetic beads and flow cytometry. pDC were cultured in RPMI1640 medium and stimulated with different bacteria. The T cells proliferation was detected by MTT, and cytokine production by ELISA kits.</p><p><b>RESULTS</b>The human bacterial pathogen Staphylococcus aureus and Pseudomonas aeruginosa stimulation for 48 h resulted in the maturation of pDC with production of high quantity of IFN-α at (15.34 ± 2.91) ng/ml and (10.38 ± 1.41) ng/ml, respectively, comparing with that of negative group at (1.36 ± 0.13) ng/ml (P<0.01). Activated pDC could promote the differentiation of naive CD4⁺ T cells to Th1 cells with secretion of IFN-γ at (2.16 ± 0.37) ng/ml and (2.73 ± 1.11) ng/ml, respectively, comparing with that of positive control at (2.55 ± 0.23) ng/ml (P > 0.05). Activated pDC showed higher T cell stimulatory capacities [proliferation index (PI) was 4.36 and 4.05, respectively] than that of non-activated pDC (PI was 1.23 and 0.13, respectively) (P < 0.01).</p><p><b>CONCLUSION</b>Staphylococcus aureus and Pseudomonas aeruginosa activated pDC may play a key role in SR of leukemia following severe infections.</p>

Objective To study the prevalence and sequence homology of virulence genes exoU and exoS in 53 strains of pan-drug-resistant Pseudomonas aeruginosa .Methods The virulence genes exoU and exoS were detected by PCR.Sequence homo-logy was analyzed by BOX-PCR.Results Of the 53 clinical isolates of pandrug-resistant Pseudomonas aeruginosa ,the exoS+/exoU- genotype was identified in 40 strains,exoU+/exoS - genotype in 10 strains,exoS +/exoU+ genotype in 1 strain, and exoS-/exoU- genotype in 2 strains.BOX-PCR results showed that 41 exoS+ isolates belonged to 24 genotypes,and 11 exoU+ strains could be grouped into 7 genotypes.Conclusions The prevalence of virulence genes is high in clinical isolates of pandrug-resistant Pseudomonas aeruginosa .BOX-PCR fingerprint analysis combined with sequence homology analysis is help-ful for effective monitoring and control of hospital pandrug-resistant pseudomonas aeruginosa infection.

ABSTRACT

Objective To explore the effect of designing and applying project teaching scheme in higher vocational nursing clinical curriculum. Methods Sophomores of nursing major in grade 2009 and 2010 were selected as experiment subjects. Lecture-based traditional teaching method was applied in tradition group (121 students in grade 2009) while project teaching was applied in reorga-nization group (128 students in grade 2010). The teaching method was evaluated through thoretical exam, vocational skill test, professional quality test and questionnaine surrey. Data were input by us-ing SPSS17.00. T-test and Ranksum test were applied. Results Vocational skill test results of tra-dition group and reorganization group were (76.67 ±13.072) and (85.48 ±10.085) respectively;professional quality test results were (74.77±13.865) and (87.04±8.982), respectively. Differences in both groups had statistical significances (P<0.001). Evaluation on the teaching method was carried out in both groups through a questionnaire. (716/1408) of students in reorganization group thought that project teaching can improve 11 items including comprehensive quality. Except for the item of the-oretical knowledge comprehension, the rest 10 items of both groups had statistical significances (P<0.001). Conclusion Implementation of project teaching in nursing clinical curriculum helps to im-prove students' professional core competence.

OBJECTIVE@#To evaluate the diagnostic value of vibration perception threshold (VPT) in diabetic peripheral neuropathy (DPN) by the receiver operating characteristic curve (ROC) and to establish its cut-off threshold.@*METHODS@#All patients had the VPT examination and nerve conduction velocity (NCV) examination. NCV examination showed that 283 patients with Type 2 diabetes were divided into a DPN group (n=151) and an NDPN group (n=132). The VPT diagnosis was evaluated by Youden index, sensitivity, specificity and the area under ROC curve. The best cut-off threshold was defined by the Youden index.@*RESULTS@#1) The NCV was significantly slower, while the VPT was higher in the DPN group than those in the NDPN group (both P values <0.05). 2) The VPT and NCV of both sides of the limb had no difference in all patients. 3) With NCV as the golden diagnosis criterion, the area under ROC of VPT was 0.707, the best cut-off threshold was 10.54 V, the sensitivity was 0.596, the specificity was 0.848, and the Youden index was 0.445. 4) The diagnosis ratio of NCV combined with VPT was 60.4%, significantly higher than that of NCV alone (P<0.05).@*CONCLUSION@#Compared with NCV examination, VPT has good diagnostic value for DPN. The best cut-off value is 10.54 V.

OBJECTIVE@#To explore the effectiveness of systematic self-management education on blood sugar level of patients in the community with type 2 diabetes.@*METHODS@#A total of 248 patients with type 2 diabetes from 10 communities of Changsha were randomized into intervention or control groups in February 2009. The intervention group received systematic self-management education, while the other received routine community education. Before and after intervention, fasting plasma glucose, 2 h postprandial plasma glucose and glycosylated hemoglobin were measured, and evaluated between the 2 groups.@*RESULTS@#Fasting plasma glucose, 2 h postprandial plasma glucose and glycosylated hemoglobin levels of patients with diabetes from the two groups were generally comparable at baseline (P>0.05). After the intervention, these three indicators were all statistically significant between the 2 groups (P0.05), 2 h plasma glucose and glycosylated hemoglobin levels were improved (P<0.01, P<0.05, respectively).@*CONCLUSION@#Systematic self-management education effectively encourages patients with type 2 diabetes to control their blood sugar levels, and deserves further promotion.

ABSTRACT

OBJECTIVE@#To explore the effect of systematic self-management education on quality of life, anxiety and depression of patients with Type 2 diabetes in communities.@*METHODS@#A total of 248 patients with Type 2 diabetes from 10 communities of Changsha from February 2009 to July 2010 were randomized into an intervention group and a control group. The intervention group received systematic self-management education, while the controls received routine community education. The quality of life, anxiety and depression were measured by adjusted diabetes-specific quality of life scale (A-DQOL), self-rating anxiety scale (SAS) and self-rating depression scale (SDS) respectively.@*RESULTS@#The quality of life, morbidity of anxiety or depression of the intervention and the control group were comparable at the baseline (P>0.05). One and half year later, there was significant difference between the two groups (P<0.01). Compared with the control group, the intervention group got better quality of life (P<0.01), less morbidity of anxiety (8.94% vs. 44.4%, P<0.01) and depression (23.58% vs. 56.00%, P<0.01).@*CONCLUSION@#Systematic self-management education can effectively improve the quality of life, reduce the level of anxiety and depression of patients with Type 2 diabetes in communities, which deserves further generalization.

ABSTRACT

Objective To observe the effects of jet nebulization with sodium bicarbonate on the children with bronchial asthma.Methods 128 children with bronchial asthma were collected and randomly divided into observation group and control group. Conventional treatment by anti-infection and jet nebulization with hormone and bronchodilators were used in control group. Jet nebulization with sodium bicarbonate combination with conventional treatment was used in observation group. The therapeutic efficacy, clinical symptom, physical sign, the stay time, the occurrence of adverse reaction and the levels of IL-6, IL-8 and TNF-α in peripheral blood were observed. Results There were no severity adverse reactions happened in the two groups. The excellence rate was 59.4% and total effective rate was 95. 3% in observation group which were significantly higher that in control group( P < 0. 05 ). The extinction time of clinical symptom and physical sign were significantly shorter than that in control group. The levels of IL-6, IL-8 and TNF-α in peripheral blood decreased notably after therapy than before treatment in the two groups. The decreased changes in observation group were significant than that in control group ( P < 0. 05 ). Conclusion Jet nebulization with sodium bicarbonate could improve the clinical symptom and physical sign and shorten the course of disease of children with bronchial asthma. It also could decrease the secrete of inflammatory factor and get satisfactory curative effect.

ABSTRACT

OBJECTIVE To know more clearly about the situation of Hepatitis B virus markers in clinical medical workers and take further interventional strategies to protect high risk medical workers.METHODS Hepatitis B virus markers in doctors,nurses and medical checkers who have contacted with patients′ blood,body fluid,or other occupational hazard situation,were detected by of ELISA.RESULTS Among the 587 medical workers detected,311 were with deficiency of active immunity(52.98%),196 were HBV infectors(33.39%).CONCLUSIONS Medical workers are in high risk groups of HBV infection.Medical institutions should attend to their self-protection and encourage them to take HBV vaccine to prevent iatrogenic transmission.

ABSTRACT

OBJECTIVE To investigate the genotypes of aminoglycoside-modifying enzymes(AMEs)genes of Acinetobacter baumannii.METHODS Clinical isolates of A.baumannii were collected from 2003 to 2006,and their resistance to gentamicin,amikacin and tobramycin were tested by K-B method.Twenty-three isolates were chosen because of their resistance to aminoglycoside antibiotics(at least resistant to one kind of the drugs).Nine types of the AMEs were detected by PCR.RESULTS Drug resistant rates of 23 isolates of A.baumannii to gentamicin,amikacin and tobramycin,were 86.96%,56.5% and 69.56%,respectively.The detection rates of the 9 AMEs,including ant(3')-Ⅰ,aac(3)-Ⅰ、aac(6')-Ⅰ,aph(3')-Ⅵ,aac(3)-Ⅱ,aac(6')-Ⅱ and ant(2″)-Ⅰ were 69.56%,60.87%,56.52%,47.82%,30.4%,26.09% and 21.73%,respectively.CONCLUSIONS The resistance to aminoglycoside antibiotics of A.baumannii is mainly caused by AMEs.

ABSTRACT

Objective To investigate the prevalence of 16S rRNA methylase genes in Klebsiella pneumoniae isolates from Guangzhou.Methods K-B test was used to determine the resistant rates of these stains.Five 16S rRNA methylase genes,armA,rmtA,rmtB,rmtC,and rmtD,were detected by PCR.Results All 55 K.pneumoniae isolates showed resistant to arbekacin,gentamicin,tobramycin,and neomycin.Susceptibility rates were 5.5%,20.0%,72.7%,and 100% to ceftazidime,ciprofloxacin,piperacillin/tazobactam,and imipenem respectively.ESBLs were positive in 52 of 55 (94.5%) isolates.Among 55 K.pneumoniae isolates,34 were positive for armA and 1 for rmtB.Conclusions In K.pneumoniae resistant to arbekacin,the positive rate of 16S rRNA methylase genes was high,predominantly with armA positive.These strains were highly resistant to some antibiotics.
