ABSTRACT

Objective To investigate the predictive value of peripheral blood neutrophil to lymphocyte ratio(N/L ratio) in patients with hypertension. Methods A total of 660 cases hypertension patients were enrolled from May 2013 to May2014.Peripheral blood N/L ratio was recorded. All patients were followed up to May 31,2016.According to the incidence of cardiovascular and cerebrovascular events, all patients divided into observation group(with cardiovascular and cerebrovascular events) and control group(without cardiovascular and cerebrovascular events),and white blood cell classification was compared.Logistic regression model was used to evaluate the value of N/L to predict cardiovascular events. Results The incidence rates of smoking, diabetes and hyperlipidemia in patients of the observation group were 87.5%,72.5% and 77.5%,higher than those of the control group(P<0.05),and the level of mean arterial pressure(MAP) was(95±12) mmHg,significantly higher than the control group(P<0.05).White blood cell count, percentage of neutrophils, mononuclear cell count,N/L of observation group were(11.6±2.5)×109/L,(70.8±5.9)%,(8.3±5.5)% and 3.7±1.5 were higher than control group(P<0.05).N/L increased by 10%(P=0.04),MAP increased by 10 mmHg(P=0.02) and the proportion of neutrophils(N%) increased by 15%(P=0.03) were independent risk factors for death in patients. Conclusion Neutrophil/lymphocyte ratio and mean arterial blood pressure were independent risk factors for hypertension patients with West-to-cerebral vascular events.

ABSTRACT

Objective To explore the postoperative secondary xerostomia′s uncomfortable experience of patients with long fating in gastrointestinal surgery, providing basis for medical staff to understand comfort needs of patients and to take targeted nursing intervention. Methods The phenomenological methodology was used in the study. Totally 12 patients with postoperative secondary xerostomia′s uncomfortable experience were interviewed, and tape recording was used. The data was analyzed by Colaizzi analysis method. Results Four themes were extracted: emotional response to patients with postoperative xerostomia, uncomfortable experience of mouth, the insufficient cognition of fating in perioperative period, desire for attention to solve the problem of dry mouth. Conclusions Medical staff should strengthen the awareness of the patients′experience with secondary and temporary dry mouth, and improve the nursing intervention measures according to need, in order to increase patients′comfort and to promote them recover soon.

ABSTRACT

OBJECTIVE:To provide a reference for setting codes in national New Rural Cooperative Medical System (NRC-MS) drug catalog. METHODS:Current conditions of drug catalog coding at home and abroad were introduced, the problems in the process of setting codes in national NRCMS drug catalog were analyzed and the countermeasures were put forward. RESULTS& CONCLUSIONS:Existing drug catalog codes mainly include those published by World Health Organization(WHO)in anatomi-cal,therapeutic and chemical classification system, and those in Chemical (Raw Material and Preparation) Classification and Code of China,Classification and Code of Drugs Covered by Social Insurance, meanwhile, the setting code method of relevant drugs can be also learnt from Chinese Pharmacopoeia and National Essential Drug List, etc. In setting codes in national NRCMS drug catalog, there were problems such as difficulty in classifying and mapping drug codes, insufficient authority of references for drug codes, too many reference standards for drug codes, and poor normalization of drug code extension. We should further refer to definitely authoritative standards for drug codes, establish appropriate rules on drug code integrated mapping, develop uniform and perfect drug code category set,and establish a system subject to continuous update and an effective evaluation mechanism,to solve the problems in the process of setting codes in national NRCMS drug catalog.
