ABSTRACT
OBJECTIVES: This study sought to evaluate the effect of simultaneous application of arthrocentesis and occlusal splint. MATERIALS AND METHODS: A retrospective study of 43 patients (3 males, 40 females) whose symptoms had improved was conducted at the Department of Oral and Maxillofacial Surgery, Dong-A University Hospital between 2008 and 2010. Subjects were divided into three groups: Group A (17 patients with arthrocentesis and occlusal splints simultaneously applied), Group B (13 patients whose symptoms did not improve with occlusal splints, undergoing arthrocentesis after occlusal splint use for 8 weeks), and Group C (13 patients that only used occlusal splints). We compared these groups in maximum comfortable opening (MCO) and the visual analogue scale of pain and noise. Follow-up was performed at 1 week, 1 month, 3 months, and 6 months. RESULTS: The improvement of symptoms was noted in all three groups, but Group A had a quicker improvement than the other groups, in terms of pain reduction and MCO increases. CONCLUSION: The simultaneous application of arthrocentesis and occlusal splints can reduce patient discomfort more quickly.

OBJECTIVES: Lateral cephalometry, computed tomography (CT) and full-night polysomnography were used to examine the correlation of obstructive sleep apnea (OSA) severity. MATERIALS AND METHODS: A total of 29 patients (5 females, 24 males) diagnosed with OSA were evaluated by lateral cephalometry, CT and full-night polysomnography. Lateral cephalometry was performed in the closed and open mouth states. The radiographic and polysomnography measurements of the patients with OSA were evaluated statistically to determine the association with OSA severity. RESULTS: A significant relationship was observed between the increased respiratory disturbance index and closing lateral cephalometry. With mouth opening, the airway space narrowed and the OSA worsened. Lateral cephalometry revealed OSA patients to have an inferiorly positioned hyoid bone, longer-than-normal soft palate and narrowing airway space. As OSA was severe, the airway shape was ovoid in the CT horizontal view. CONCLUSION: Polysomnography and the radiographic parameter can be used for diagnosing OSA.

This study was conducted to evaluate the antihyperglycemic effects of three phytoestrogens, genistein, coumestrol, and enterolactone, in type 2 diabetic animals. Forty male C57BL/KsOlaHsd-db/db mice were used as a diabetic animal model. The animals were divided into four groups and fed a phytoestrogen-free AIN-76 diet (control), or one of three phytoestrogen-supplemented (3.75 mg/100 g diet) AIN-76 diets for six weeks. During the experimental period, fasting blood glucose levels were measured on week 0, 2, 5, and 6 of the experiment, and oral glucose tolerance tests were performed on the 5th week. After the experimental period, blood concentrations of HbA1c, insulin, and glucagon were measured, and hepatic glycogen content and glucose regulating enzyme activities were analyzed. Fasting blood glucose, HbA1c level, and the area under the blood glucose curve in the oral glucose tolerance test were significantly lower in all of the phytoestrogen-supplemented groups compared to the control group. Plasma glucagon levels were also significantly lower in all of the phytoestrogen-supplemented groups compared to the control group. Hepatic glycogen level was significantly higher in the coumestrol-supplemented group compared to the other groups. However, there were no significant differences in the activities of glucokinase and glucose-6-phosphatase between the groups. These results suggest that all of the three major phytoestrogens tested in the present study were effective in lowering blood glucose levels in type 2 diabetic animals. However, further studies need to be conducted to elucidate the exact mechanism for the hypoglycemic effects of phytoestrogens.

PURPOSE: The purpose of this study was to compare the stress distribution characteristics of four different abutment connections on SS-III(R) fixture under occlusal loading, using 3-dimensional finite element method. MATERIALS AND METHODS: The fixture of SS-III(R) (Osstem, Korea) with 4 mm diameter and 11.5 mm length and 4 types of abutments were analyzed; Solid, Com-Octa, ComOcta Gold, and Octa abutment. The models were placed in the area of first molar in the mandible. The 4 loading conditions were; (1) the vertical loading of 100 N on the central fossa, (2) the vertical loading of 100 N on the buccal cusp, (3) the 30degreesinclined loading of 100 N to lingual side on the central fossa, and (4) the 30degrees inclined loading of 100 N to the lingual side on the buccal cusp. The 3G.Author program was used, the von-Mises stress was calculated and the stress contours were plotted on each part of the implant systems and the surrounding bone structures. RESULTS: Regardless of abutment types and loading conditions, higher stress concentration was observed at the cortical bone. In cancellous bone, the highest stress was observed at apical portion and the maximum stress occurred at the implant neck. The higher internal stress was observed in the fixtures than in the bone. The lowest stress was observed at loading condition 1 and the stress concentration was also lower than any other loading conditions. CONCLUSION: Within the limitation of the result of this study, it seems that the abutment connection type does not affect much on the stress distribution of bone structure.

The objectives of this study was to determine whether a new physically modified cornstarch by ultra-fine- or nanoscale pulverizer to reduce particle size offers better bioactive function than native cornstarch in weanling Sprague-Dawley rats. Male weaning Sprague-Dawley rats were fed diets containing native cornstarch (NAC), ultra fine pulverized cornstarch (UFC) or nano-scale pulverized cornstarch (NSC) for 4 weeks. In vitro rate of starch hydrolysis, growth performance, organ weight, intestine length intestinal proliferation and the fermentation by Bifidobacterium of rat cecum were evaluated. The diet with reduced particle size (UFC or NSC) significantly increased body weight gain and organ weight. Feed efficiency was increased in NSC fed rats and was not affected in UFC fed rats. Intestinal proliferation was decreased in NSC group. Reduction of particle size also increased cecal short chain fatty acid concentration and the growth and acidifying activity of Bifidobacterium. It is concluded that a reduction of particle size of starch granules by physically modification may increase growing performance and gut function.

The serum levels of antioxidant materials (alpha-carotene, beta-carotene, beta-cryptoxanthin, lutein, lycopene, alpha-tocopherol, gamma-tocopherol, retinol) of the healthy Korean middle-aged adults (n = 373) were measured and their relationships with the serum lipids and anthropometric indices were analyzed. The serum levels of beta-cryptoxanthin, beta-carotene, lutein were higher than those of lycopene and alpha-carotene. The levels of all measured carotenoids except lutein were significantly higher in females than in males, but retinol level was vice versa. There was a tendency of increase in serum levels of antioxidant vitamins with increasing age. The serum carotenoid levels had a positive correlation with serum cholesterol and a negative correlation with serum triglyceride. The serum levels of tocopherols or retinol showed a significantly positive relationship with blood cholesterol or triglyceride. In overall, anthropometric indices showed negative relations with serum carotenoids levels, but vice versa with serum tocopherol or retinol levels. Particularly, beta-carotene and lutein levels showed a significantly negative relation with blood pressure in male subjects. The serum levels of alpha-carotene, beta-carotene and beta-cryptoxanthin had significantly negative relations with body fat-related indices in female subjects. The results demonstrated that blood antioxidants levels differed by sex and age, and had significant relations with blood lipid levels and anthropometric indices. Therefore, the rationale and significance of the relationships need to be elucidated in the future study related to dietary intakes and life style.

OBJECTIVES: This study was conducted to investigate the associations of non alcoholic fatty liver with metabolic syndrome and the serum carotenoids. METHODS: This study was conducted in a general hospital in South Korea from November, 2004 to August, 2005. The study subjects were 350 sampled persons who were aged from 40 years and older (males: 180, females: 170). They were grouped into the normal, mild and severe groups according to fat accumulation in their livers, as determined by ultrasonography. We analyzed the association between non alcoholic fatty liver and metabolic syndrome by multiple logistic regression analysis and we analyzed the association between non alcoholic fatty liver and the serum carotenoids by a general linear model(ANCOVA). RESULTS: After adjustment for the effect of potential covariates, the prevalence of metabolic syndrome was associated with fat accumulation in the liver (p trend <0.001). If the odds ratio of normal group is 1.00, then that of the mild group is 2.80 (95% C.I=1.17-6.71) and that of the severe group is 7.29 (95% C.I=2.76-19.30). The prevalence of metabolic alterations fitting the criteria of metabolic syndrome, according to the class of fat accumulation in the liver, was significantly increased, except for criteria of high blood pressure, a large waist circumference and low HDL (high density lipoprotein) cholesterol level (p trend <0.001). The level of serum beta-carotene was decreased according to the class of fat accumulation in the liver (p trend=0.036), but the levels of serum alpha-carotene, lycopene, bata-cryptoxanthin and lutein were not decreased. CONCLSIONS: This study shows that non alcoholic fatty liver was associated with metabolic syndrome and with the serum beta-carotene level.

This study was performed to evaluate the effect of nutrient intake levels on the relative risk of breast cancer in the Daegu and Gyeongbuk area. The case subjects were 103 patients newly diagnosed as breast cancer at Kyungpook National University Hospital. The control subjects were 159 healthy women without breast cancer-related disease in the same community selected by frequency matching of age and menopausal status. The survey was administered by individual interviews by trained dietitians using semiquantitative food frequency questionnaires. The odds ratios were determined by using unconditional logistic regression after adjusting for the confounding factors found in the analysis of general characteristics of the subjects. In overall, it has been found that high consumptions of energy and carbohydrates significantly raised the relative risk of breast cancer, and that high intakes of total fat, plant fat, dietary fiber, all kinds of fatty acids, vitamin E and calcium significantly lowered the risk of breast cancer. The results of the study suggest that the possible protective factors to the breast cancer risk include a higher intakes of total fat, especially plant fat, dietary fiber, and micronutrients such as vitamin E, folic acid, calcium, phosphorus and potassium. On the other hand, high intakes of energy and carbohydrate appeared to be the risk factors.

The present study evaluated the effects of various dosages of soybean isoflavone extract on lipid profiles, lipid peroxidation and antioxidant activities in streptozotocin-induced diabetic rats. The one normal control group was fed an AIN-76-based experimental diet and four diabetic groups were fed the same diet, supplemented with four different levels of soybean isoflavone extract for seven weeks. The daily dosages of pure isoflavone for four diabetic groups were set to be 0 mg (diabetic control), 0.5 mg (ISO-I), 3.0 mg (ISO-II) and 30.0 mg (ISO-III) per kilogram of body weight, respectively. The plasma total cholesterol levels and the TBA-reactive substances contents in the liver and kidney were significantly lowered in ISO-II and ISO-III groups compared to those in the diabetic control group. The levels of plasma HDL-cholesterol, plasma vitamin A and hepatic superoxide dismutase were significantly increased in those two groups compared with the diabetic control group. The present study demonstrated the possibility that the diets supplemented with 3.0 mg and 30.0 mg of soybean isoflavone extract may have beneficial effects on the plasma lipids, tissue lipid peroxidation and partly on antioxidant system in diabetic animals and there were no significant differences between the ISO-II and ISO-III groups. The results suggest that the effective daily dosage level of isoflavone for improving lipid metabolism in diabetic rats may be above 3.0 mg per kilogram body weight.

BACKGROUND: Gastric cancer in elderly patients is one of the most important health issues in Korea, when considering its incidence and mortality rate. We have obtained the preliminary data concerning the diagnosis and treatment of gastric cancer in elderly people by analyzing the clinical and pathologic features. METHODS: We retrospectively analyzed 268 patients who were diagnosed with gastric cancer and who also underwent surgical procedures in Bundang Seoul National University Hospital from May 2003 to May 2005. The patients were divided into two groups: those aged or =65 (n=106). We compared the clinical and pathological characteristics between the older and younger groups. RESULTS: The elderly group had more patients with well differentiated carcinoma (11.8% vs. 33.0%, respectively, p=0.001) and the intestinal type (34.0% vs. 59.4%, respectively, p<0.001). The frequency of microsatellite instability-high (MSI-H) was higher in the elderly group (5.6% vs. 17%, respectively, p=0.010). There was no significant difference in the postoperative recovery time (p=0.374) and the operation-related complication rate between the two groups. CONCLUSIONS: Gastric cancer in elderly patients demonstrated more pathologic types that have a known favorable outcome when compared to the younger patient group. We should more frequently consider surgery for the elderly patient with operable gastric cancer because of they have similar postoperative recovery compared to the younger patients.

The present study examined the reproductive factors and food intake pattern which influence on the breast cancer risk in Daegu Gyungbuk area. The case subjects were 103 patients newly diagnosed as breast cancer at Kyungpook National University Hospital. The control subjects were 159 healthy women selected by frequency matching of age and menopausal status in the same community. The survey was conducted by individual interviews using questionnaires which include general and reproductive characteristics, dietary habits, and food frequency. The odds ratios were calculated by using unconditional logistic regression after adjusting for confounding variables. The mean age of the subjects was 50 yrs, and mean body mass index was significantly high in the patient group than in the control. The patient group had a significantly higher familial history of breast cancer and a significantly shorter breast-feeding period compared to the control group. It has been found no significant relationship between extrinsic hormone use such as oral contraceptives or estrogen replacement therapy and breast cancer risk. In regard of food habits, a high preference for the cooking method of steaming rather than frying, panbroiling or roasting was significantly associated with lower relative risk of breast cancer. The higher intake frequencies of fruits for all subjects, and seaweeds only for postmenopausal subjects were related with a significantly lower relative risk of breast cancer. The results of the study suggest that the possible risk factors for the breast cancer occurrence include high BMI, family history, less breast feeding experience, preference for the cooking method of frying, and less consumption of fruits and seaweeds. This study provides an useful data for nutrition education to prevent breast cancer for the residents in Daegu Gyungbuk area.

The present study evaluated the effect of various dosages of soybean isoflavone extract on body weight changes, glucose tolerance and liver function in streptozotocin-induced diabetic rats. One group of normal rats (normal control) was fed an AIN-76-based experimental diet and four groups of diabetic rats were fed the same diet supplemented with four different levels of soybean isoflavone extract for seven weeks. The daily dosages of pure isoflavone for four diabetic groups were set to be 0 mg (diabetic control), 0.5 mg (ISO-I), 3.0 mg (ISO-II) and 30.0 mg (ISO-III) per kilogram of body weight, respectively. The daily consumption of isoflavone at the level of 3.0mg per kilogram of body weight resulted in the suppression of body weight loss and increased the survival rate of diabetic animals one and half times compared to that of the diabetic control group. Blood glucose levels in a fasting state and after the oral administration of glucose were significantly lower in the ISO-II group during the oral glucose tolerance test. The ISO-II group showed a tendency to elongate the gastrointestinal transit time. The activity of serum aminotransferases, indicator of liver function, was not negatively affected by any intake level of isoflavone. The present study demonstrated that the soybean isoflavone extract may be beneficial to diabetic animals by improving their glucose tolerance and suppressing weight loss without incurring hepatotoxicity at the daily dosage of 3.0 mg per kg of body weight.

The present study determined the estimated dietary fiber (DF) intake per capita of Korean from 1991 to 2001 andanalyzed the major food sources of dietary fiber for Korean using the data on per capita consumption of each food reported in the Korean National (Health and) Nutrition Survey Reports and the recently established DF database by the Korean Nutrition Society. The estimated mean daily DF intakes of Korean during the last 11 years (1991~2001) were in the range of 19.25~21.22 g or 9.97~11.99 g/1,000 kcal with a small range of fluctuations. As of 2001, average DF intake per capita of Korean was estimated as 20.92 g or 10.59 g/1,000 kcal. The average DF intake level was under the Adequate Intake for DF (12 g per 1,000 kcal) of the Dietary Reference Intakes (DRIs) for Koreans established in 2005. The level of DF intake was relatively lower in small city residents compared to the large city or rural area. The top two major food group sources of DF for Korean were vegetables and cereals, in addition, legumes, fruits, seaweeds and seasonings were included among top five food groups. The ratio of DF intake from fruits was gradually increased but the ratio from legumes was steadily decreased during the last 11 years. The mostly contributed single foods for DF intakes of Korean were Kimchi and rice regardless of year and area. Percentage of DF intake from top ten single foods was continuously decreased from 65% in 1991 to 51% in 2001. The results of this study revealed that DF intakes of Korean as of 2001 is insufficient compared to the Adequate Intake for DF for Korean and the source of fiber in Korean diet has been more various. Therefore the beneficial health effects of DF and the increased consumption of DF from a variety of food sources should be continuously emphasized through the nutritional education.

The present study was conducted to analyze the status of food and nutrients intakes of the colorectal cancer patients in the Daegu.Kyungpook area and to find dietary risk factors related to the occurrence of colorectal cancer in this community. The case subjects (123) were selected from the patients recently diagnosed as colorectal cancer at Kyungpook National University Hospital, the control subjects (182) were selected from the patients of the Department of Orthopedic Surgery at the same hospital and from the healthy volunteers who did not have any gastrointestinal diseases. The food consumption survey was done by individual interviews using semi-quantitative food frequency questionnaire and nutrients intakes were analysed by CAN program. The results of the study suggested that dietary factors which are speculated as the risk factors of colorectal cancer in Daegu.Kyungpook area were high consumption of cereals and oils, low consumption of fruits and mushrooms, high consumption of energy and fat, especially animal fat, low consumption of dietary fiber, high percentage of energy intake from cereals and potatoes, high intakes of protein, fat, vitamin A and cholesterol from egg, low intake of calcium from vegetables, and high intake of iron from meats and eggs. These findings might be useful for the nutrition education to prevent colorectal cancer in the community. However it is recommended to conduct more extensive and systematic survey to reconfirm these dietary risk factors under taking into consideration of the dietary characteristics in this region.

The purposes of this study were to investigate the characteristics of lifestyle and dietary habits of the colorectal cancer patients in Daegu and Kyungpook areas and to collect the data useful for nutrition education for the prevention of colorectal cancer in this community. The case subjects of the study were 123 patients diagnosed recently as colorectal cancer at Kyungpook National University Hospital. The control subjects were 182 persons who did not have any gastrointestinal diseases, including the patients from the department of orthopedic surgery and healthy volunteers. The survey covering general characteristics, life style, dietary habits, eating behaviors, and food intake frequency was administered by individual interviews using questionnaires. The results of the study suggest that high BMI, daily life stress, pessimistic personality, lack of physical activities, and familial cancer history might be the possible risk factors for the incidence of colorectal cancer. Dietary factors suspected as risk factors for colorectal cancer in the present study included strong preferences to meats, salty and fatty taste foods, low intake of water, alcohol drinking, smoking, coffee intake and irregular eating habits. A high consumption of seaweeds, green-yellow vegetables, light-colored vegetables, and green tea was suggested as a preventive factor for colorectal cancer. It is recommended to conduct more extensive and systematic surveys in the near future to reconfirm the risk factors of colorectal cancer in consideration of the characteristic food culture in this community. The results of the present study may be applied to nutritional education for the prevention of colorectal cancer for the local residents.

The present study was conducted to investigate the effect of nutrition education program on nutrition knowledge, eating behaviors, food habit, nutrient intakes in obese children and their parents who live in Gumi city. The subjects were 16 obese boys, 20 obese girls with obesity index over 130% and 36 of their parents. The nutrition education for the subjects was carried out by the professional personnel such as doctors, professors, and dietitians. The children were provided with well-balanced lunch meals and had 40 min-lectures on the reasonable weight management, 40 min-games and also had regular exercises (stretching, swimming) for 90 mins everyday during 2 weeks of the program. The parents had 90 min-lectures on childhood obesity, diet therapy, behavior modification, and exercise for 6 times. The nutrition knowledge, eating behaviors and food habits were surveyed by using questionnaires before and after the education. The nutrition intakes of the children were surveyed before and after the education by 3-day food record method. The nutrition intakes of the parents were surveyed before and after the education by using semi-quantity questionnaires. After the education, the mean nutrition knowledge scores were significantly improved compared with the pretest scores in both children and parents. After the education, all subjects' eating behaviors were significantly changed positively and calorie and carbohydrates intakes were significantly decreased and vit. C was significantly increased in obese children. The food habits of the parents were significantly improved after the education. These findings show that the well-designed nutrition education program for obese children and their parents can be an effective approach to help them to improve their nutrition knowledge and to establish desirable food habits and eating behaviors.

The present study was conducted to investigate the effect of nutrition education program on anthropometric values and boichemical index in obese children who live in Gumi city. The subjects were 16 obese boys, 20 obese girls with obesity index over 130 and 36 of their parents. The nutrition education was carried out by the professional personnel such as doctors, professors, and dietitians. The children were provided with well-balanced lunch meals and had 40min-lectures on the reasonable weight management, 40 min-games and also had regular exercises (stretching, swimming) for 90 mins everyday during 2weeks of the program. The parents had 90 min-lectures on childhood obesity, diet therapy, behavior modification, and exercise for 6times. After the program, obesity index, BMI, % body fat were significantly decreased (p < 0.05) in boys and obese index, BMI, % body fat and WHR were significantly decreased (p < 0.05) in girls. Blood cholesterol and TG levels of girls were significantly decreased (p < 0.05). These findings show that the well-designed nutrition education program for obese children can be an effective approach to help them to improve their anthropometric values and biochemical index.

The present study was conducted to evaluate the food and nutrients intakes of stomach cancer patients in the Daegu and Gyeongbuk areas in order to find out the dietary risk factors for stomach cancer. The subjects of the study were 102 patients who had recently been diagnosed with stomach cancer at the Gyeongbuk National University Hospital. The control subjects were 105 people including patients from the Department of Orthopedic Surgery and healthy volunteers who did not have any gastrointestinal diseases. Estimates of nutrients intakes were determined from the food intake frequency data obtained by individual interviews using questionnaires. The mean daily calorie intakes of the control and the case groups were not significantly different. However the energy intake from protein was significantly higher in the case group as compared to the control group. With regard to the nutrients intakes, the case subjects consumed significantly higher amounts of nutrients such as protein, calcium, sodium, phosphorus and niacin than the control group. The case group showed a tendency to consume higher amount of protein, fat, calcium and iron from animal food sources. In the present study dietary factors which were suspected as being risk factors for stomach cancer included high consumption of animal foods, specific nutrients such as protein, sodium and niacin, specific food groups such as meat, spices, and low consumption of fruits and mushrooms. Therefore, it is recommended that more extensive and systematic surveys be conducted to confirm the risk factors for stomach cancer, taking into consideration the dietary cultural characteristics of this region.

The present study was conducted to evaluate the usefulness of common Korean legumes as a high-fiber supplement in therapeutic diets for diabetic patients. Streptozotocin-induced diabetic rats were used as animal models and four kinds of legumes, black soybean (BS), yellow soybean (YS), green pea (GP) and soybean curd residue (SCR) were tested as high-fiber supplements. Seven groups of normal and streptozotocin-induced diabetic rats were fed isocaloric experimental diets containing 8% dietary fiber from one of four legumes or purified cellulose and pectin for 6 weeks. The effects of the legumes on the glucose and lipid metabolism of diabetic rats was examined and compared with the effects of cellulose and pectin. The legume supplementation did not show any beneficial effect on glucose tolerance, however, it exhibited a plasma cholesterol-lowering effect in diabetic rats. The cholesterol-lowering action was especially strong in BS and the degree of the effect was comparable to that of pectin. The levels of total lipids, cholesterol, and triglyceride in the hepatic tissues of rats fed legume diets were similar to those of the pectin group. All legume supplements induced an increase in fecal steroid excretion. The fecal cholesterol contents were significantly high following the supplementations of YS and SCR (p<0.05). The excretion of fecal bile acids in the BS and YS groups was significantly higher than it was in the pectin group (p<0.05). Concentration of lipid peroxidation products in the blood and urine of diabetic animals was lower in the legume groups than in the cellulose group. The levels of hepatic lipid peroxidation products were significantly lower in the BS and YS groups than in the pectin group (p<0.05). From the results of this study, the plasma cholesterol-lowering effect of BS is possibly due to the significant (p<0.05) in-crease in fecal steroid excretion, which suggests that BS could be beneficial in improving abnormal lipid metabolism in diabetic rats.
