ABSTRACT

Introduction: The number of internet users in 2018 was 4.021 billion, increased 7 percent year-on-year. More than nine-tenths of Indian teens use Internet. Due to social distancing norms put forward due to COVID-19 dependency and availability of Internet usage has increased. Objective: To assess the extent of Internet addiction and determine its predictors among college students of Surat city. Method: A cross sectional study was conducted from March-September 2021 among 400 first- and second-year students selected by systematic random sampling from four colleges selected purposively. Outcome variables were Young's Internet addiction Test and Duke Health Profile Score which were assessed in terms of mean score. Univariate analysis was done, followed by t-test, ANOVA and Pearson Correlation to establish associations. Predictors were determined by Binary Logistic Regression. Results: Around one-tenth (12.8%) participants were seen with no Internet addiction, with majority being mild (44.8%) and moderately (36.5%) addicted. Severe addiction was seen in around one out of twenty participants (6.1%). Social networking (32%) and education (34.5%) were the major reasons for use of Internet. Higher pocket money (aOR=4.3), greater monthly internet expenditure (aOR=2.8), ownership of internet enabled mobile phone (aOR=3.9), lying down posture while accessing internet (aOR=4.8) and evening (aOR=2.2) or night time (aOR=8.7) of internet access were significant predictors for Internet addicts. Conclusion: In this study more than four fifth of the college students had Internet Addiction even at young age. Physical, Mental, Social and General health were significantly negatively correlated with internet addiction.

ABSTRACT

Introduction: L'objectif général de notre travail était d'apporter une description épidémiologique et clinicopathologique du cancer de l'endomètre au Gabon. Patients et méthode: Cette étude descriptive et rétrospective a été réalisée à l'Institut de Cancérologie de Libreville chez les patientes atteintes du cancer de l'endomètre sur une période de 7 années (de janvier 2012 à octobre 2018). Résultats: Cette étude a inclus 32 patientes et les résultats obtenus ontmontré que ce cancer au Gabon est plus fréquent chez la femme multipare, que histologiquement le type 1 est le plus souvent rencontré et que ce cancer est de haut grade de malignité. De ce travail ressort également la difficulté du suivi des patientes. Conclusion: Le cancer de l'endomètre étant un cancer souvent d'emblée de mauvais pronostic, il est d'intérêt de mettre en place une stratégie de prévention et de management adapté.Mots clés: cancer, endomètre, étude clinicopathologique, suivi, Gabon Abstract:Introduction:The general objective of our work was to provide an epidemiological and clinicopathological description of endometrial cancer in Gabon.

Patients and method: This descriptive and retrospective study was carried out at Cancer Institute of Libreville in patients with endometrial cancer over a period of 7 years (from January 2012 to October 2018) . Results: This study included 32 patients and the results obtained showed that this cancer in Gabon is more frequent in multiparous women, that histologically type 1 is most often encountered and that this cancer is of high grade malignancy. This work also shows the difficulty of following up patients. Conclusion: As endometrial cancer is often a cancer with a poor prognosis , it is interest to put in place an appropriate prevention and management strategy.

ABSTRACT

La pratique de la rachianesthésie est motivée par la qualité de l'analgésie post-opératoire qu'elle procure. Cependant, la rachianesthésie reste comme tout acte médical associée à certains accidents. Méthodes : C'est une étude prospective, descriptive et analytique qui s'est déroulée sur une période d'un (01) mois allant du 07 Mai 2022 au 07 Juin 2022 au CHU d'Angré. Résultats : la rachianesthésie a été réalisée dans 188 des cas, soit un taux de 60% de l'ensemble des interventions. Le sexe ratio était de 0,13 et l'âge moyen était de 33 ± 10 ans. L'antécédent le plus retrouvé était l'HTA. La bupivacaine était le seul anesthésique local utilisé avec comme adjuvant la morphine, le fentanyl ou l'association morphine -fentanyl. Les incidents peropératoires étaient dominés par les troubles hémodynamiques à type d'hypotension artérielle (98,7%) et de bradycardie. Les patients ayant présenté un syndrome de mal être post opératoire représentaient 70,8% de notre effectif. Le Syndrome de Mal Etre post opératoires était dominé par les nausées et vomissements suivis des prurits. Tous les patients ayant présenté un syndrome de mal être post rachianesthésie n'avaient pas bénéficié de prise en charge. La survenue du syndrome de mal être post rachianesthésie était statistiquement associé à la présence de la morphine comme adjuvant (p=0,016). Conclusion : La rachianesthésie est une remarquable technique d'anesthésie. Néanmoins la fréquence des évènements per et post opératoires survenant au décours de cette technique n'est pas négligeable, car ils sont la source d'inconforts notables

ABSTRACT

Contexte : la maladie thyroïdienne représente la plus fréquente endocrinopathie dans le monde après le diabète sucré. En Afrique, elle constitue un véritable problème de santé publique et sa prise en charge est souvent entravée par les moyens d'investigations. Le but était d'étudier le profil des affections thyroïdiennes à Bouaké. Méthodes : il s'agissait d'une étude rétrospective, descriptive, menée dans le service de consultation de Médecine Interne du Centre Hospitalier Universitaire de Bouaké du 1er janvier 2020 au 31 décembre 2021. Elle a porté sur les dossiers de 104 patients suivis pour une pathologie thyroïdienne. Résultats : Sur 2560 patients reçus, la pathologie thyroïdienne concernait 104 patients ; soit une fréquence de 4,1%. Il s'agissait de 93 femmes et 11 hommes avec un sex-ratio (H/F) de 0,12 L'âge moyen était de 42,03 ans ±15 ans. Dans plus de la moitié des cas (55,8%), le délai de consultation était supérieur à 06 mois. Sur le plan fonctionnel, 58 patients (55,8%) présentaient une hyperthyroïdie avec comme principales étiologies la maladie de Basedow (60,3%, n=35) et le goitre multinodulaire toxique (24,1%, n=14). L'hypothyroïdie était retrouvée chez 10 patients (9,6%), le plus souvent post thyroïdectomie (60%). Les goitres et nodules euthyroïdiens étaient retrouvés chez 36 patients (34,6%) dominés par le goitre multinodulaire euthyroïdien (36,1%, n=13) et le goitre simple (27,8%, n=10). Le traitement des hyperthyroïdies reposait essentiellement sur les antithyroïdiens de synthèse dans 96,7%. Seulement 1 patient hyperthyroïdien avait subi une thyroïdectomie. La majorité des patients (82,2%) étaient perdus de vue. Conclusion : La pathologie thyroïdienne au CHU de Bouaké est une maladie fréquente de la femme jeune. Les hyperthyroïdies dominent le tableau des thyréopathies avec comme principale étiologie la maladie de Basedow. On note un taux élevé de perdu de vue, d'où l'intérêt d'une éducation de nos patients.


Context: Thyroid disease is the most common endocrinopathy observed in the world. In Africa, thyroid diseases remain a real public health problem and their care is often hampered by the means of investigation. The purpose of this work was to study the profile of thyroid affections in Bouake. Methods: This was a descriptive study with retrospective data collection, carried out over the period from 1 January 2020 to 31 December 2021 at the Internal Medicine consultation at the University Hospital of Bouake. All epidemiological, clinical and therapeutic and evolutionary data from patients followed for thyroid pathology were collected. It focused on the records of 104 patients followed for thyroid disease. Results: Out of a total of 2560 consultations, 104 patients were involved in thyroid pathology, prevalence of 4.1%. They were 93 women and 11 men with a sex ratio of 0.12. The average age was 42.03±15.64 years. In more than half of the patients (55.8%) the evolution of the clinical symptomatology was more than six months. A total of 58 patients (55.8%) had hyperthyroidism. The etiologies of hyperthyroidism were dominated by Grave's disease and toxic multinodular goiter, with 60.3% and 24.1% of hyperthyroidism respectively. Hypothyroidism was confirmed in 10 patients (9.6%), among then 6 (60%) had undergone total thyroidectomy. The multinodular goiter was found in 36 patients and was dominated by multinodular goiter (36.1%) and single goiter (27.8%). The treatment of hyperthyroidism was exclusively with synthetic antithyroid drugs. Hypothyroidism was treated with hormone replacement therapy. The majority of patients (82.2%) were lost in view. Conclusion: The thyroid pathology at the University Hospital Center of Bouake is a frequent illness of the young woman. Hyperthyroidism were frequently found with as main etiology Grave's disease The majority of patients (82.2%) were lost in view hence the interest of patient's education.

ABSTRACT

Exercising autonomy allows nurses to demonstrate their knowledge and skills. However, more information is needed about the critical care nurses' perceptions of their professional autonomy in rural areas. Objective: To explore the perceptions, facilitators and barriers to professional autonomy among critical care nurses in rural Kenya. Methods: The hermeneutic phenomenological study design was used in this study. Data were collected in a critical care unit using a semi-structured interview guide. A sample of 10 participants were recruited. The study was conducted in Nyeri County Referral Hospital. Results: Three themes emerged from the study on the nurses' experiences of professional autonomy, perceptions of autonomy, facilitators of autonomy, and barriers to autonomy. Conclusion: Autonomy undeniably plays a pivotal role in defining the professional status of the nursing profession. Perceptions, facilitators and barriers to professional autonomy form the background of the current professional status. Nurses, therefore, exercise autonomy effectively when it aligns with patient care needs and when a conducive environment supports it.

Abstract The work aimed to evaluate the weight-length relationship and the condition factor, characterizing the biometry, anatomy, histology and volumetric density of the liver of Brycon amazonicus, in different stages of body growth. The experiment used twenty specimens in four stages of body growth (PI, PII, PIII and PIV) harvested every 90 days, containing five specimens, each group. The livers were dissected, weighed (g) and processed routinely using the hematoxylin and eosin technique. The data were submitted to analysis of variance, Pearson's correlation test and linear regression. The equation that represented the weight-length relation was W = 0.05902 x L2.63, with negative allometric growth, but with a relative condition factor equal to 1.0. The liver was divided into three lobes with the gallbladder close to the right lobe and its color varied from light red to dark red, not varying in relation to other fish species. The hepatosomatic relationship followed body growth until the PII group stage and then declined, demonstrating the behavior of its development in Brycon amazonicus. The organ consists predominantly of hepatocytes, followed by sinusoidal vessels and capillaries, with histological morphology similar to that of many species of fish. Melanomacrophage centers were found only in the most developed animals, but in small quantities, prompting the development of new research on this cell, in this species. In this way, research of this nature allows the characterization of fish species, helping to improve breeding methods, understanding pathological processes caused by diseases, and obtaining better productive capacity, serving an increasingly demanding and prosperous market.

O experimento utilizou vinte espécimes em quatro fases de crescimento corporal (PI, PII, PIII e PIV) colhidos a cada 90 dias, contendo cinco espécimes, cada grupo. Os fígados foram dissecados, pesados (g) e processados rotineiramente pela técnica da hematoxilina e eosina. Os dados foram submetidos à análise de variância, teste de correlação de Pearson e regressão linear. A equação que representou a relação peso-comprimento foi W = 0,05902 x L2,63, com crescimento alométrico negativo, mas com fator de condição relativo igual a 1,0. O fígado apresentou-se dividido em três lobos com a vesícula biliar próxima ao lobo direito e sua cor variou de vermelho claro a vermelho escuro, não variando em relação a outras espécies de peixes. A relação hepatossomática acompanhou o crescimento corporal até a fase do grupo PII e então declinou, demonstrando o comportamento de seu desenvolvimento em Brycon amazonicus. O órgão constitui-se predominantemente por hepatócitos, seguido de vasos e capilares sinusoidais, com a morfologia histológica semelhante ao de muitas espécies de peixes. Centros melanomacrófagos foram encontrados apenas nos animais mais desenvolvidos, mas em pequena quantidade, instigando o desenvolvimento de novas pesquisas sobre esta célula, nesta espécie. Desta forma, pesquisas desta natureza permitem a caracterização de espécies de peixes, auxiliando no aperfeiçoamento de métodos de criação, compreensão de processos patológicos provocados por enfermidades, e obtenção de melhor capacidade produtiva, atendendo um mercado cada vez mais exigente e próspero.

ABSTRACT

Introduction: Pleural effusion is one of the manifestation of a malignant disease which may be malignant pleural effusion with demonstrable malignant cells in the fluid or para-malignant pleural effusion which is reactive response or due obstruction of lymphatic drainage rather than invasion of pleural cavity. Various modalities are there to investigate this condition including routine microscopy, cytology, biopsy etc. Objective: To understand and compare the utility of cancer ratio, tumor markers, malignant cytology in cases of suspected malignant pleural effusion. Material and Methods: This Case Control Cross sectional study was conducted among patients attending respiratory OPD at Sir Sunder Lal Hospital, BHU, Varanasi, diagnosed with malignant pleural effusion and non-malignant pleural effusion. Results: Significant association was found between Cancer Ratio-Carcinoembryonic Antigen, CEA (p = 0.0069), CEA-Cytology (p = <0.01801)

ABSTRACT

The goal of this paper is to identify the trend of online apparel shopping as one of the motivating factors for women's purchasing. To achieve the study's objectives, structured questionnaires were distributed to online apparel customers who are aware of and purchase apparel products from online stores in and around the Davangere district of Karnataka. Convenience and snowball sampling techniques were used to collect primary data from online apparel women employees who are aware of, use, and buy apparel products from online stores. The data analysis and results were based on the completion of 180 usable questionnaires by online retail apparel women customers who actively participated in the marketing survey. Frequency analysis and graphs were used to determine trends in women's online apparel. Most women are interested in the latest trends and western clothes, and they prefer more ethnic clothes and dark shades. They prefer to buy online clothes during festivals and discounts and sales.

Conservation tillage has proven advantageous in improving soil health and productivity. However, the greenhouse gases (GHGs) emission and intensity from different conservation tillage and nutrient management systems under Indian conditions are less understood. Therefore, here, we compared the effect of tillage and nutrient management on GHGs emissions, net global warming potential (NGWP), and greenhouse gas intensity (GHGI) from a field experiment under five years in a soybean-wheat cropping system in the Vertisols. The tillage treatments comprised of reduced tillage (RT) and no tillage (NT). The three nutrient management treatments included application of 100% NPK (T1), 100% NPK + 1.0 Mg FYM-C ha-1 (T2), 100% NPK +2.0 Mg FYM-C ha-1 (T3). The results showed significantly higher SOC sequestration under NT (1388 kg ha-1 yr-1) followed byRT (1134 kg ha-1 yr-1) with application of FYM (2.0 Mg C ha-1) (T3) every year. Across tillage, integrated nutrient management(T2 and T3) lowered NGWP and GHGI compared to NPK (T1). The GHGI of NT system was less by 33% compared to RT. The results suggest that GHGs mitigation and sustained food production in the soybean-wheat system can be achieved in NT and RT with integrated use of organic and inorganic fertilizer as the major component of nutrient management.

EGFR?TKIs have changed the landscape of metastatic NSCLC treatment with a significant improvement in survival of EGFRm patients compared to wild?type EGFR. Even with the newer third generation EGFR TKIs like, Osimertinib, which has proven efficacy against the resistance mutation of EGFRm T790M, progression eventually occurs. There are limited treatment options for patients with metastatic EGFRm NSCLC with other acquired resistance. Therefore, novel therapeutic combination strategies are being researched to overcome potential resistance to EGFR?TKI?targeted therapy. The ICIs targeting the programmed cell death?1 pathway in patients with EGFRm NSCLC were greatly anticipated based on preclinical studies showing increased PD?L1 expression. In clinical settings, this increased expression did not translate into a survival benefit. Treatment with ICIs failed to positively affect EGFRm patients because of multiple reasons: nonsynonymous tumor mutational burden, lower PD?L1 expression in tumors, and cancer cells utilizing alternate immune escape mechanisms. The NCCN guidelines currently do not recommend immunotherapy in patients with metastatic EGFRm NSCLC. Recently, a subgroup analysis in the IMpower150 study provided a signal for overall survival of atezolizumab with bevacizumab plus chemotherapy in EGFRm?TKI progressed patients. Based on these encouraging findings, several combinations of ICIs and EGFR?TKIs are being evaluated in TKI?failed EGFRm patients. These regimens might provide a favorable therapeutic effect by combining higher response rates of TKIs and durable disease control of ICIs. However, further research is warranted to understand the exact underlying molecular and cellular mechanisms responsible for the clinical benefits. ABSTRACT

Background & objectives: Consumption of high glycaemic index (GI) food is associated with a high risk for diabetes. There is a felt need to understand the GI of common Indian traditional foods using standard GI protocols. The present study was aimed to analyse the carbohydrate profile of common traditional Indian food preparation and to determine their GI using standardized protocols. Methods: Twelve food preparations made of millets, wheat, maize and pulses were evaluated for nutrient composition including detailed carbohydrate profiling and tested for GI in healthy volunteers using standard methodology. Capillary blood glucose responses for the test foods containing 50 g available carbohydrates were recorded and compared to the reference food (50 g glucose). GI was calculated from the incremental area under the curve (IUAC) for the test and reference foods. Results: Available carbohydrate content of the food preparations ranged between 13.6 and 49.4 g per cent. Maize roti showed the highest total dietary fibre (7.5 g%). White chick pea ‘sundal’ showed highest resistant starch content (3.95 g%). Amongst the 12 test foods, five fell in the high GI category (finger millet balls, sorghum, pearl millet and maize roti), four in the medium GI category (sorghum idli, wheat dosa, methi roti and adai) and three in the low GI category (broken wheat upma, white peas sundal and white chick peas sundal). Interpretation & conclusions: Merely being a whole grain-based food does not qualify for a lower GI. The method of processing, food structural integrity and preparation could influence the GI. The type and quality of fibre are important than the quantity of fibre alone. Judicious planning of accompaniments using low GI legumes may favourably modify the glycaemic response to high GI foods in a meal.

BACKGROUND: Staphylococcus aureus is a cosmopolitan and pathogenic microorganism associated with various diseases spectra and antimicrobial resistance of public health importance.Aim: This study determined the phenotypic characteristics of S. aureus isolated from patients in healthcare institutions in Zaria metropolis.STUDY DESIGN: A cross-sectional hospital-based study was carried out in 5 healthcare institutions. Four hundred and twenty clinical samples were collected and analyzed. RESULTS: Majority of the patients (54.3%) were within the age range 21­40 years and mean age of 26.04 ± 12 years. Approximately, 70% of the respondents had history of antibiotic use prior to consultation in the hospitals and wereselfprescribed, and 91.2% were outpatients. The most commonly abused antibiotics were ampicillin-cloxacillin (19.5%) and cotrimoxazole (10.0%), and the mean duration of their use was 3.5 ± 1.3 days. The detection rate for S. aureus was 10% and 5.2% for MRSA. The S. aureus isolates showed the highest frequency of resistance against ampicillin 42 (100%), followed by penicillin G 39 (92.9%) and least was to gentamicin 5 (11.9%). The frequency of resistance for the MRSA were ampicillin 22 (100%), penicillin G 21(95.5%) and least was to gentamicin 2 (9.1%). The minimum inhibitory concentrations of oxacillin were greater than 128 µg /ml. CONCLUSION: The detection rate of S. aureus and MRSA strains are of great public health concern which requires continuous health education on rational use of antibiotics among others

ABSTRACT

Objectif : déterminer les étiologies des fièvres prolongées inexpliquées au service de médecine interne du CHU de Bouaké. Matériel et méthodes : Il s'agissait d'une étude rétrospective à visée descriptive mené de janvier 2019 à décembre 2020. Nous avons inclus les patients hospitalisés pour fièvre prolongée inexpliquée ou ayant présentés une fièvre prolongée durant l'hospitalisation. Les données anamnestiques, cliniques et paracliniques ont été analysées. Résultats : Nous avons retenus 204 patients sur 1650 patients hospitalisés (12,6%). L'âge moyen des patients était de 32±16,1 ans avec des extrêmes de 16 et 87 ans. Le sexe ratio était de 1,1. La fièvre (32,4%) et AEG (22%) constituaient les motifs d'hospitalisation les plus fréquents. La fièvre était le plus fréquent des symptômes physiques (94,1%) suivi de l'amaigrissement (50%), de la déshydratation (18,1%), de l'hépatomégalie (12,7%) et de l'obnubilation (12%). Les principales étiologies étaient les infections (82,3%), les tumeurs (10,3%), et les maladies inflammatoires (1,5%). Dans 5,9% des cas l'étiologie n'avait pas été retrouvée. Les principales infections étaient le VIH et ses complications (38,1%), la tuberculose (21,4%), et les infections urogénitales (19,9%). Les tumeurs étaient dominées par le cancer primitif du foie (33,4%) et le cancer de la prostate (19,4%). La polyarthrite rhumatoïde, le lupus érythémateux disséminé et la sclérodermie étaient maladies inflammatoires retrouvées. La létalité était de 36%. Conclusion : Les étiologies des fièvres prolongées inexpliquées sont variées. Elles sont dominées dans notre contexte par la tuberculose, l'infection à VIH et ses complications infectieuses.

ABSTRACT

Le profil épidémiologique de l'insuffisance rénale chronique (IRC) varie d'un pays à un autre et d'une région à une autre dans un même pays. L'objectif de la présente étude était de décrire les cas d'IRC observés dans la ville de Bouaké. Méthodes : il s'agissait d'une étude rétrospective réalisée dans le service de Médecine Interne du CHU de Bouaké durant la période allant du 1er Janvier 2016 au 31 Décembre 2020. L'IRC était définie par un DFG inférieur à 60 ml/minute évoluant depuis plus de trois mois et/ou une atrophie rénale. Résultats : nous avons recensé 150 cas donnant une prévalence hospitalière de 3,6%. L'âge moyen était de 38,8±10 ans avec une prédominance masculine. Les antécédents étaient dominés par l'automédication traditionnelle et l'hypertension artérielle dans 68% des cas chacun. Dans 91% des cas, l'IRC était classée grade 5. Les étiologies étaient dominées par l'hypertension artérielle (34%). Le traitement symptomatique était la transfusion dans 73% et l'usage des antihypertenseurs dans 73%. Le traitement de suppléance était indiqué dans 91% et réalisé dans 3%. La mortalité était de 25%. Les facteurs associés à la survenue du décès étaient une altération de l'état général, une hypercréatininémie, une hyperphosphorémie, des reins atrophiés et le recours à la transfusion sanguine. Conclusion: l'IRC était diagnostiquée à un stade sévère. La principale cause était l'hypertension artérielle. La mortalité demeure élevée, faute d'accès au traitement de suppléance. L'accent doit être mis sur la prévention de l'IRC au cours des pathologies à risque.


Background: The epidemiological profile of chronic kidney disease (CKD) varies from country to country and from region to region within, even in the same country. The objective of this study was to describe the cases of CKD found in the city of Bouake (Ivory Coast). Methods: We proceeded to a retrospective study carried out in the Internal Medicine department of the Bouake University Hospital from January 1, 2016 till December 31, 2020. CKD was defined by a GFR (Glomerular Filtration Rate) at less than 60 ml/ minute and small kidneys. Results: In all , we identified 150 cases corresponding to 3.6% as hospital prevalence . The mean age was 38 .8 ± 10 years with a male predominance. The history was dominated by traditional self-medication and high blood pressure (HBP) for respectively 68% of cases. In 91% of cases , the CKD was classified as grade 5. The etiologies were dominated by HBP (34%). The care was a symptomatic one using transfusion in 73% and antihypertensive medication was prescribed for 73% of the patients. The substitution treatment was indicated in 91% of cases and carried out in 3%. The rate of mortality was 25 % and factors associated with death were a deterioration of the general condition, increased of blood level of creatininemia, hyperphosphatemia, small size of kidneys and the use of blood transfusion. Conclusion: CKD was diagnosed at a severe stage. The main cause was high blood pressure. Mortality remains high due to lack of access to replacement therapy . Emphasis should be placed on the prevention of CKD in high-risk pathologies

Introduction Les maladies auto-immunes systémiques (MAIS) sont peu connues malgré les progrès diagnostiques et thérapeutiques réalisés ces dernières années. L'objectif de ce travail était de décrire le profil épidémiologique, diagnostique et thérapeutique des MAIS dans les services de Médecine Interne et de Dermatologie du Centre Hospitalier Universitaire de Bouaké (CHU). Méthodes Il s'agissait d'une étude transversale descriptive réalisée dans les services de Médecine Interne et de Dermatologie du CHU de Bouaké, sur une période de 10ans (janvier 2009- décembre 2018). Résultats : Sur 30906 patients, 50 présentaient une MAIS soit une prévalence hospitalière de 0,16%. Les MAIS les plus fréquentes étaient le lupus érythémateux systémique (50%) et la sclérodermie systémique (42%). L'âge moyen était de 39,5ans ±15ans et le sex-ratio de 0,19. Le délai moyen de consultation était de 26,2 mois. Le tableau clinique était dominé par les signes généraux (98%), les manifestations cutanéomuqueuses (96%) et les manifestations articulaires (90%). Chez 37 patients ayant réalisé l'hémogramme, l'anémie représentait 51,3% des cas. Le syndrome inflammatoire était objectivé chez 67% des 12 patients possédant un bilan inflammatoire. Les auto-anticorps réalisés chez 05 patients étaient contributifs chez 03 patients. Les corticoïdes par voie générale étaient prescrits dans 56% des cas et les perdus de vue étaient observés dans 90% des cas. Conclusion : Les MAIS étaient rares dans notre étude, dominées par le lupus érythémateux systémique et la sclérodermie systémique. L'amélioration du plateau technique et l'accessibilité du bilan immunologique et la sensibilisation paraissent indispensables afin d'améliorer la prise en charge des patients.


Introduction: Systemic autoimmune diseases (SAID) are little known despite the diagnostic and therapeutic progress made in recent years. The objective of this work was to describe the epidemiological, diagnostic and therapeutic profile of SAID in Internal Medicine and Dermatology departments of the university hospital of Bouake. Methods: This was a cross-sectional study carried out in the Internal Medicine and Dermatology departments of the university hospital of Bouake, over a period of 10 years (January 2009-December 2018). Results: Of 30,906 patients, 50 presented SAID with a hospital prevalence of 0.16%. The most common SAID were systemic lupus erythematosus (50%) and systemic sclerosis (42%). The mean age was 39.5 ± 15 years, and the sex ratio was 0.19. The average consultation time was 26.2 months. The clinical picture was dominated by general signs (98%), mucocutaneous manifestations (96%) and articular manifestations (90%). In 25 patients who performed the blood count, anemia represented 76% of cases. The inflammatory syndrome was objectified in 67% of the 12 patients with an inflammatory profile. The auto-antibodies made in 05 patients were contributory in 03 patients. Systemic corticosteroids were prescribed in 56% of cases and patients were lost to follow-up in 90% of cases. Conclusion: SAID were rare in our study, dominated by systemic lupus erythematosus and systemic scleroderma. Improvement of the technical platform and accessibility of the immunological assessment appears essential in order to improve patient's care.

Background:Type 2 Diabetes Mellitus is a disease of epidemic proportions and many patients are at a great risk of premature mortality and complication of atherothrombotic disorders affecting coronary, cerebral and peripheral arterial trees. Increased Plasminogen Activator Inhibitor Type 1 inhibits fibrinolysis and predicts cardiovascular risk in those living with Type 2 Diabetes. This study aimed to determine the effect of antidiabetic treatment on haemostatic and fibrinolytic parameters among Type 2 Diabetic subjects in Ilorin.Methods:This was a comparative cross-sectional study involving 78 Type 2 diabetic patients, (39 treatment naïve, 39 treatment experienced). Full blood count was performed using Sysmex XP300 while Prothrombin time was determined using one stage test of Owren. Activated partial thromboplastin time was determined by method of Proctor and Rapaport. Fibrinogen and Plasminogen Activator Inhibitor type-1 were assayed using AssayMax Human Fibrinogen ELISAand AssayMax Human PAI-1 ELISAkit. Data Analysis was done using SPSS version 25.0.Results:Mean PAI-1 levels were significantly higher in treatment naïve diabetics when compared to treatment experienced diabetics (2.44 ±2.57 vs 2.51±1.47 ng/ml p=0.002) as were fibrinogen levels (434.65±366.15 vs 482.24± 299.64mg /dl; p = 0.048). PAI -1 levels were lowest among diabetics treated with Metformin + DPP4 inhibitors, while fibrinogen levels were lowest among those treated with Metformin + sulfonylurea combination.Conclusion:Oral hypoglycaemic treatment, combination therapy in particular, improves fibrinolysis in type 2 diabetics thereby reducing the risk of cardiovascular disease in type 2 diabetes mellitus patients

Abstract The exponential rise in the Nigerian population has necessitated the use of agrochemicals for enhanced agricultural yields to meet the ever-rising demand for food. However, agrochemicals such as organochlorine pesticides (OCPs) have caused several devastating health and ecological challenges. The study was therefore aimed at assessing the bioaccumulation of OCPs and the associated parasitological and microbial susceptibility in P. obscura to determine the possible ecological impacts of the chemical. A total of 106 specimens of Parachanna obscura fish species were sampled between July and November 2019 from Lekki Lagoon in Lagos, Nigeria. Four culture media, namely nutrient agar (NA), MacConkay agar (MCA), eosin methylene blue (EMB), and sabouraud dextrose agar (SDA) were employed in microbial culture. These microbes were subjected to ceftazidime, ceftriaxone, cefuroxime, gentamicin, ofloxacin, augmentin, nitrofurantoin, ciprofloxacin, and erythromycin to test for resistance, susceptibility and intermediate statuses before and after curing. OCPs were tested in the water, sediment, and tissues of P. obscura using gas chromatography flame ionization detector (GC-FID). P. obscura sampled in the lagoon had poor growth exponent which was characterized by negative allometry (slenderness) in the sampled fish. Although the incidence of parasitic infection in the fish was not alarming, the situation might be aggravated if the prevalent anthropogenic activities persist, resulting in immunosuppression. Regulation of anthropogenic activities in the catchment area is recommended to forestall the prognosis of health and environmental hazards associated with the agricultural, industrial, pharmaceutical, and municipal activities around the lagoon. Bacteria that conferred the most resistance to the majority of the antibiotics were Staphylococcus sp., Micrococcus sp. Escherichia coli and Klebsiella sp., testing positive to plasmid profile. They conferred high resistance to the antibiotics before plasmid curing but became highly susceptible post- plasmid curing. This implies that the gene for resistance in the bacteria isolates was plasmid-mediated, that is, they were obtained from the environment. In the event of an outbreak of waterborne diseases such as cholera, typhoid, dysentery, and diarrhea, there may be non-response to treatment among the infected inhabitants. The incidence of antibiotic resistance in bacteria colonies recorded in this study is of great public health concern, given the possibility of the antibiotic-resistant bacteria strains being passed to humans through fish consumption, resulting in increased multi-drug resistance in humans. Regulation of anthropogenic activities around the lagoon is recommended to forestall prognosis of health and environmental hazards associated with OCPs from agricultural, industrial, pharmaceutical, and municipal sources.


Resumo O aumento exponencial da população nigeriana exigiu o uso de agroquímicos para aumentar a produção agrícola e, assim, atender à crescente demanda por alimentos. No entanto, agroquímicos como pesticidas organoclorados (OCPs) causaram vários problemas de saúde e ecológicos. Portanto, o estudo teve como objetivo avaliar a bioacumulação de OCPs e a suscetibilidade parasitológica e microbiana associada em Parachanna obscura, a fim de determinar os possíveis impactos ecológicos desse produto químico. Foi amostrado um total de 106 espécimes de P. obscura entre julho e novembro de 2019 da lagoa Lekki, em Lagos, Nigéria. Quatro meios de cultura, como o ágar nutritivo (NA), o ágar MacConkay (MCA), o ágar eosina azul de metileno (EMB) e o ágar sabouraud dextrose (SDA), foram empregados na cultura microbiana. Esses micróbios foram submetidos a ceftazidima, ceftriaxona, cefuroxima, gentamicina, ofloxacina, augmentin, nitrofurantoína, ciprofloxacina e eritromicina para testar resistência, suscetibilidade e status intermediário antes e depois da cura. Os OCPs foram testados na água, sedimentos e tecidos de P. obscura usando um detector de ionização de chama por cromatografia em fase gasosa (GC-FID). Os peixes amostrados de P. obscura da lagoa apresentaram um expoente de crescimento ruim, caracterizado por alometria negativa (esbelteza). Embora a incidência de infecção parasitária nos peixes não tenha sido alarmante, a situação pode ser agravada se as atividades antropogênicas prevalecentes persistirem, resultando em imunossupressão. Recomenda-se a regulamentação de atividades antropogênicas na área de captação para prevenir o prognóstico de riscos à saúde e ecológicos associados a atividades agrícolas, industriais, farmacêuticas e municipais ao redor da lagoa. As bactérias que conferiram maior resistência à maioria dos antibióticos foram Staphylococcus sp., Micrococcus sp., Escherichia coli e Klebsiella sp., com teste positivo para o perfil plasmidial. Elas conferiram alta resistência aos antibióticos antes da cura do plasmídeo, mas se tornaram altamente suscetíveis após a cura dele. Isso implica que o gene de resistência nos isolados de bactérias foi mediado por plasmídeo, ou seja, foi obtido do ambiente. No caso de surtos de doenças transmitidas pela água, como cólera, febre tifoide, disenteria e diarreia, pode haver não resposta ao tratamento entre os habitantes infectados. A incidência de resistência a antibióticos nas colônias de bactérias registradas neste estudo é de grande preocupação para a saúde pública, dada a possibilidade de que as cepas de bactérias resistentes a antibióticos sejam transmitidas aos seres humanos por meio do consumo de peixes, resultando em maior resistência a múltiplas drogas em seres humanos. Recomenda-se a regulamentação de atividades antropogênicas ao redor da lagoa para impedir o prognóstico de riscos à saúde e ecológicos associados aos OCPs de fontes agrícolas, industriais, farmacêuticas e municipais.

ABSTRACT

Abstract Contamination of Lekki lagoon by petrogenic chemicals such as polycyclic aromatic hydrocarbons (PAH) and benzene, toluene, ethylbenzene, and xylene (BTEX) has been widely reported. The study was aimed at investigating the bioaccumulation of PAH and BTEX in Malapterurus electricus collected from Lekki lagoon in Lagos, Nigeria. BTEX was analyzed in the intestine, water, and sediment samples using 8260B Agilent 7890B gas chromatograph coupled to a mass spectrometer (GC-MS). While PAHs were tested in the same media using a gas chromatograph coupled to a flame ionization detector (GC-FID). Histopathological analysis of the fish intestine was conducted using haematoxylin and eosin (H&E) stains. Parasite intensity, antioxidant enzymes, and lipid peroxidation activities were investigated in the fish. The parasitic infection detected in Malapterurus electricus was Electrotaenia malopteruri. The parasite showed weak and no depurative capacities for BTEX and PAH respectively. The decrease in superoxide dismutase (SOD), catalase (CAT), and reduced glutathione (GSH) in conjunction with an increase in Malondialdehyde (MDA) characterized relatively higher susceptibility among the male fish. Varieties of tissue injuries increased with the standard length of the fish groups. Results suggest that lengthier and uninfected M. electricus were more susceptible to PAH in Lekki lagoon than the shorter and infected ones. The study demonstrated a promising tendency of the enteric parasite, E. malopteruri to depurate chlorobenzene, ethylbenzene, o-xylene, m+p-xylene, and 1,4-dichlorobenzene from the intestine of its host fish, M. electricus, while PAHs were poorly mediated by the parasite.


Resumo A contaminação da lagoa Lekki por produtos químicos petrogênicos, como hidrocarbonetos aromáticos policíclicos (HAP) e benzeno, tolueno, etilbenzeno e xileno (BTEX), foi amplamente relatada. O estudo teve como objetivo investigar a bioacumulação de HAP e BTEX em Malapterurus electricus, coletados na lagoa Lekki, em Lagos, Nigéria. O BTEX foi analisado no intestino, na água e nas amostras de sedimento usando o cromatógrafo de gás Agilent 7890B 8260B acoplado a um espectrômetro de massa (GC-MS). Já os HAP foram testados no mesmo meio utilizando cromatógrafo gasoso acoplado a detector de ionização de chama (GC-FID). A análise histopatológica do intestino do peixe foi conduzida usando colorações de hematoxilina e eosina (H&E). Intensidade de parasitas, enzimas antioxidantes e atividades de peroxidação lipídica foram investigadas nos peixes. A infecção parasitária detectada em Malapterurus electricus foi Electrotaenia malopteruri. O parasita mostrou fraca e nenhuma capacidade depurativa para BTEX e HAP, respectivamente. A diminuição da superóxido dismutase (SOD), catalase (CAT) e glutationa reduzida (GSH), em conjunto com o aumento do Malondialdeído (MDA), caracterizou a suscetibilidade relativamente maior entre os peixes machos. Variedades de lesões nos tecidos aumentaram com o comprimento padrão dos grupos de peixes. Os resultados sugerem que os M. electricus maiores e não infectados foram mais suscetíveis aos HAP na lagoa Lekki do que os menores e infectados. O estudo demonstrou uma tendência promissora do parasita entérico E. malopteruri de depurar clorobenzeno, etilbenzeno, o-xileno, m + p-xileno e 1,4-diclorobenzeno do intestino de seu peixe hospedeiro, M. electricus, enquanto os HAP foram mal mediados pelo parasita.
