ABSTRACT

This article reviews the clinical studies about the treatment of allergic rhinitis (AR) with traditional Chinese medicine (TCM) in recent years. AR is a common and frequently occurring disease in the department of otolaryngology. The common manifestations of AR include nasal congestion, sneezing, runny nose, and nasal itching. AR, belonging to the category of rhinitis, is a dominant disease in the TCM treatment of otorhinolaryngologic diseases and head and neck diseases. The ancient literature has laid a solid theoretical basis on the etiology and pathogenesis of AR. According to the ancient literature, the theoretical basis, and their own experience, modern doctors classified the causes of AR into the disorders in Zang-fu organs and six meridians. Most of the explanations focus on the disorders in Zang-fu organs, especially the lung, spleen, and kidney. The clinical studies in this field mainly involve TCM treatment alone, integrated TCM and Western medicine treatment, and TCM external treatment. These therapies have good efficacy in the clinical treatment of AR. Among them, TCM treatment alone has significant advantages in alleviating the symptoms and nasal signs, declining the scores of related scales, and reducing the eosinophil count (EOS) of nasal secretions, with definite long-term efficacy. Integrated TCM and Western medicine treatment can complement with each other. The TCM external treatment methods include acupuncture, moxibustion, acupoint catgut embedding, acupoint application, and nasal irrigation with TCM, which are safe, simple, acceptable by patients and have good therapeutic effect. Finally, the clinical research status of TCM treatment of AR was summarized, and suggestions were put forward from three aspects: standardizing the clinical research protocol of TCM, encouraging pure TCM research, and exploring the mechanism of TCM treatment on the basis of frontier research achievements. This review aims to provide higher-level evidence for subsequent clinical research and promote the research on dominant AR diseases.

ABSTRACT

Proteasome is the eukaryotic organelle responsible for degradation of short-lived proteins and involved in maintaining cellular protein homeostasis. It has been reported that during the occurrence and development of hepatocellular carcinoma (HCC), the regulatory particle subunits of proteasome regulate a series of tumor-related proteins, and proliferation, survival-associated signaling molecules, including PTEN gene, P53, Bcl-2, Bcl-2 interacting mediator of cell death (Bim), cyclin-dependent kinase 4(CDK4), transforming growth factor β receptor (TGFBR), E2F1, growth factor receptor-bound protein 2 (GRB2) . Meanwhile, these subunits regulate some tumor-associated pathway protein, such as signal transducer and activator of transcription 3 (STAT3) and protein kinase B (AKT), inducing their malfunction to promote the occurrence, proliferation, invasion and metastasis of HCC. The core particle subunits are more to perform the degradation of HCC-related proteins, so inhibitors targeting the core particle show a good anti-tumor effect. This review summarizes the current research progress on the regulation and mechanism of proteasome subunits in promoting the occurrence and development .

ABSTRACT

Objective:Based on the perspective of supply and demand balance, to analyze the implementation effects of primary-feature medical services measures in medical alliances.Methods:From July to September 2019, two regions with different levels of economic development were chosen, and from each region one leading hospital and two community health service centers/township hospitals were selected from its medical alliance. A total of 213 medical staff and 703 residents were surveyed, to learn their awareness and performance appraisal of these feature services.Frequency and proportion were calculated in a descriptive statistical analysis. The scores of the performance appraisal of both supply and demand sides were tested by independent sample t. Results:In the performance appraisal of such measures, the average scoring by medical staff was 4.39±0.07, and that by residents was 3.85±0.06; the residents were more concerned with service capabilities and contents, while medical staff were more concerned with service content and delivery mode; supply and demand sides were found with consensus in terms of improving the medical service quality of chronic diseases in primary level and service accessibility.Conclusions:The county-level medical alliances should focus on enhancing their capacity of primary medical services, and improving their primary service mode with the help of medical insurance institutions. Meanwhile, it is imperative to optimize the primary medical service policy in view of supply and demand balance.

ABSTRACT

Objective@#To comparatively study intrahepatic cholangiocarcinoma (ICC) and hepatocellular carcinoma (HCC) with reference to clinical features and prognosis in Chinese Han population.@*Methods@#699 cases of HCC and 170 cases of ICC confirmed by surgical pathological files from 2009 to 2010 were included and followed-up. The differences in demographic characteristics, hepatitis B virus infection, clinical characteristics, biochemical indexes, tumor markers and prognosis of HCC and ICC were analyzed retrospectively by means of paired t-test, analysis of variance, chi-square test and Pearson’s correlation coefficient.@*Results@#Among 869 cases of primary liver cancer, HCC and ICC accounted for 80.43% and 19.57%. The old aged (P < 0.001) male incidence of HCC was higher than that of ICC (P < 0.001). The infection rates of hepatitis B virus were 89.84% and 35.88% in HCC and ICC, respectively, and the infection rates of hepatitis B, serum HBsAg postive rate and DNA account in HCC were higher than ICC (P < 0.001). The incidence of liver cirrhosis and hepatic schistosomiasis in HCC was also significantly different from that in ICC (both P < 0.01). Pearson’s correlation analysis showed that there was a significant negative correlation between HCC or ICC tumor type and hepatic schistosomiasis (r = -0.018, P < 0.001), and there was a significant positive correlation between HCC and hepatic cirrhosis (r = 0.179, P < 0.001, and r = 0.528, P < 0.001, respectively). However, the proportion of cirrhosis and schistosomiasis in hepatitis B positive ICC cases was not significantly different from that in HCC cases (P > 0.05). Among the biochemical indicators, there were significant differences between HCC and ICC in the abnormal rate of ALT(P < 0.01), AST(P < 0.05), ALP (P < 0.01), GGT (P < 0.01) and TBIL (P < 0.01) while there was no significant difference between ALB and pre-ALB (P > 0.05) in HCC and ICC groups. The content and abnormal rate of alpha-fetoprotein were higher in HCC (P < 0.01), while the content and abnormal rate of carcinoembryonic antigen and carbohydrate antigen 19-9 were higher in ICC (P < 0.01). The 10-year survival rate and median survival time (46.92% and 80.3 months) of HCC were higher than those of ICC (12.57% and 12.4 months) (P < 0.01).@*Conclusion@#In the study population, compared with ICC cases, the old aged male HCC cases are more common and has higher infection rate of hepatitis B virus and cirrhosis, but liver schistosomiasis is less common. The inflammatory damage, secretion and metabolic function of HCC were different from that of ICC cases, while the synthetic reserve function was similar to that of ICC and the prognosis of HCC cases was significantly better. The incidence of cirrhosis and schistosomiasis in ICC cases with positive hepatitis B virus infection was not significantly different from that of HCC cases.

ABSTRACT

Objective To evaluate the clinical outcomes of hemodialysis patients after superficial femoral artery - superficial femoral vein arteriovenous graft (AVG). Methods Hemodialysis patients with mid - thigh superficial femoral artery - superficial femoral vein AVG from August 2015 to March 2018 in department of vascular surgery, Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine were enrolled. Their clinical outcomes and complications after operation were analyzed retrospectively. Patency rates were measured by Kaplan - Meier survival curve. Results A total of 18 cases were enrolled. The success rate of the operation was 100%without complication. Follow - up time was (22.00 ± 11.77) months with 100% follow - up rate. The 6 months -, 12 months -, 24 months - primary patency rates were 83.3%±8.8%, 48.5%±12.1%, 24.2%± 13.5%, respectively; secondly patency rates were 100.0%, 100.0%, 87.5%±11.7%. There were 1 case of seroma, 1 case of puncture site infection, 11 cases of stenosis and 5 cases thrombosis during follow-up, while no heart failure, ischemia or pseudoaneurysm. Conclusion Mid - thigh AVG has low infection rate and high patency rate, so it can be as the first choice for the lower extremity AVG in hemodialysis patients.

ABSTRACT

Objective To summarize the experiences of aneurysmorrhaphy for arteriovenous fistula aneurysms with acute thrombosis in hemodialysis patients.Methods There were 7 cases of arteriovenous fistula with acute thrombosis from Nov 2015 to Feb 2017 at our department of vascular surgery,Longhua Hospital.Results In all cases thrombosis was secondary to proximal stenosis or occlusion.The stenosis and occlusion were corrected with embolectomy and aneurysmorrhaphy.The proximal part of the cephalic vein was translocated to the basilic vein in 1 case,axillary vein in 2 cases;autologous vein graft in 1 case;resection of the occlusion,end-to-end anastomosis in 1 case;autogenous patch in 1 case.No perioperative complications occurred.The operation site was cannulated within one month after operation in all cases.Patients were followed up for 7 months to 23 months,all cases were patent.Conclusions Aneurysmorrhaphy is effective,reliable and safe for arteriovenous fistula aneurysms with acute thrombosis in hemodialysis patients.

ABSTRACT

Objective To evaluate the effects of fraction of inspired oxygen (FiO2) and positive end-expiratory pressure (PEEP) on the gradient between arterial and end-tidal carbon dioxide (D(a-ET)CO2).Methods Sixty ASA Ⅰ or Ⅱ patients (body mass index < 30 kg/m2),aged 25-50 yr,weighing 45-75 kg,scheduled for elective gynecological laparoscopic surgery,were randomized into A,B and C groups (n =20 each).The patients were mechanically ventilated with pure oxygen in group A,and with air and O2 (FiO2 =50%) in groups B (PEEP =0) and C (PEEP =5 cm H2O).PETCO2 was monitored during mechanically ventilation.Blood samples were obtained from the radial artery immediately after tracheal intubation (T1) and at 1 h of pneumoperitoneum (T2) for blood gas analysis.D(a-ET) CO2 and Qs/Qt were calculated.Results D(a-ET) CO2 and Qs/Qt were significantly lower at T2 in groups B and C than in group A (P < 0.05).Compared with group B,D(a-ET)CO2 was significantly decreased at T2 (P < 0.05),and no significant change was found in Qs/Qt in group C (P > 0.05).Conclusion Decreased FiO2 and PEEP of 5 cm H2O can decrease D(a-ET)CO2 and increase the accuracy of PErCO2 for reflection of PaCO2,which is due to reduced intrapulmonary shunt.

AIM: To study the immunity modulation mechanism of achyranthes bidentata polysaccharides (ABPS). METHODS: (1) PBMC from patient with asthma or lung cancer were cultured with 800 mg/L ABPS for 18 h. IFN-? and IL-4 mRNA was detected by RT -PCR. (2) Th cells were cultured with serially diluted ABPS for 48 h and with 800 mg/L ABPS for various times. Cultured supernatant was harvested at diffevent time points and RNA was extracted, respectively. IFN-? or IL-4 transcriptional and translational levels were assay by RT -PCR and ELISA.
