ABSTRACT

PURPOSE: This study developed a web-based wound care course for undergraduate nursing students and evaluated the course's content, system, and student-satisfaction. METHODS: This study was done in three stages, the development of the web-based wound care course, the implementation and evaluation of the course. The course was developed based on the ARCS model. 80 undergraduate nursing students to Y University in Korea used the web-based wound care program during four weeks. After that, they completed questionnaires, evaluating the contents, system, and their satisfaction. RESULTS: Eighteen learning objectives were used to create the web-based wound care course and the course was developed with 7 chapters and 20 subsections. The analysis of the questionnaires showed a mean score for content and system-related items of 3.04 each, out of a possible 4 points. Student satisfaction items had a mean score of 2.89. CONCLUSIONS: The web-based course allowed students access to the course anytime and anywhere, and according to their own learning abilities. However this advantage would only be possible when nurse educators develop qualitative web-based course to meet the demand of a complex health care system as well as the needs of the students and the effectively incorporate it into traditional lectures.

ABSTRACT

PURPOSE: The purpose of this study was to develop a vital signs e-book for undergraduate student nurses and evaluate the content, system and student satisfaction. METHOD: This study was done in three stages, the development of a vital signs e-book, implementation and evaluation. The subjects were 73 undergraduate student nurses in Y university. RESULT: Thirty one learning objectives were used to create the contents. A set of 5 chapters and 18 subsections were defined after validation from nurse educators. The e-book is available at http://123.134.207.23/ebook/vitalsigns. Analysis of the questionnaires showed a mean score for content, system and students satisfaction of 3.17 +/- .73, 3.11 +/-.79, and 2.96 +/-.74 respectively out of a possible 4 points. CONCLUSION: Nurse educators should provide quality and effective web-based courses that meet undergraduate student nurses' learning needs and they should incorporate web-based learning into traditional teaching to meet the demands of nursing education.

ABSTRACT

PURPOSE: The purpose of this study was to evaluate self-study learning achievement in undergraduate nursing students who used a web-based wound care e-book. METHODS: The web-based wound care e-book was applied to 80 nursing students at Y university. The students studied the wound care e-book for four weeks and practiced wound dressing by themselves in open laboratory. Learning achievement was evaluated according to achievement of unit objectives and performance of an actual wound dressing. RESULTS: 1. The total mean score for achievement of unit objectives was 3.06 (+/-0.41) and the total mean score on the performance of the wound dressing was 89.40 (+/-5.47). 2. There was no difference between the scores in the performance test (F=1.012, p=.366) for students who used self-study and those who were given a lecture. 3. A positive correlation was found between achievement of unit objectives and performance of the wound dressing (r=0.306, p<0.05). CONCLUSION: The web-based wound care e-book was effective in facilitating self-study for nursing students, and there is a need to continuously develop and up-date web-based nursing education e-books to facilitate self-study.

ABSTRACT

PURPOSE: This study was done to provide information about issues of clinical competency from the perspective of new graduate nurses and to make suggestions for improving undergraduate education programmes. It was also done to clarify learning experiences between fundamentals of nursing and the other major areas of nursing. METHOD: For this study, 7 new graduates and 8 experienced nurses participated. The data were collected through in-depth interviews and analysed in terms of Strauss and Corbin's grounded theory methodology. RESULT: Core category and main categories, were delineated. Clinical nursing competency was found to be a dynamic process with each participant actively engaged in acquiring problem solving ability in diverse clinical settings. These findings have value in understanding the embedded meaning of clinical nursing competency. CONCLUSION: Therefore, the educational programs reflecting the experience of new nursing staff should be developed.

ABSTRACT

PURPOSE: The purpose of this study was to identify nurses' perceptions of the importance of concrete items as a basis for nursing practice. A total of 179 concrete items for basic nursing, which clinical nurses must know in order to practice appropriately, were identified. METHOD: The participants in this study were 225 nurses who had worked for two to five years in university hospitals. The nurses were asked to rate the importance of the 179 items (in 39 middle range categories) on a 5 point scale. RESULT: The mean age of the nurses was 26 and about 54% were university graduates. Of the 39 middle range categories those rated as the top ten were transfusion (4.682), IV injection (4.492), po medication (4.476), surgery (4.469), infection control (4.438), IM injection (4.413), safety (4.388), oxygenation (4.376), diagnostic test (4.366), and fluid & electrolyte balance (4.359). The categories that had the lowest scores were sexuality (3.449), conceptual nursing (3.465), spirituality (3.527) and personal hygiene (3.548). CONCLUSION: The results of this study show that most nurses evaluate items which they use frequently in their practice as important.

ABSTRACT

PURPOSE: This descriptive study was done to evaluate the attitude and practice of hand-washing by student nurses. METHOD: The subjects of the study were 463 senior and junior undergraduate student nurses and RN-BSN students. The questionnaire was developed by the researchers. Data were collected from Oct. 16 to Dec. 20. 2001. and analysed by SPSS/WIN. General characteristics, attitude and practice were analyzed by frequency and percentage and differences in attitude and practice according to grade and practice setting were analyzed with x2 test, t-test or ANOVA. RESULT: 1) The mean frequency of hand- washing was 6.63. Senior students washed more frequently than junior students. 2) The reasons for non-compliance were 'not being accustomed', 'forgetting', and 'not accessible' 3) The mean time for hand- washing was 23.79 sec., less than the stated appropriate time of 42.18 sec. 4) 90% of the students washed their hands 'after contacting excretions and contaminated items' and few washed 'before giving care to the client'. 5) Most students washed hands rotationally rubbing with soap and water. 6) While washing, they removed their rings more often than their watches. 7) Soap bar (52.8%) and paper towel (69.6%) were most frequently used as decontaminating agent and drying method, but detergent solution (74.2%) and paper towels (60%) were considered as the most appropriate agent and method. 8) Hand-washing was perceived important generally 'to protect from cross-infection', 'to protect one-self' and 'the most cost effective'. 9) All students responded nurses should practice 'hand-washing,'.

ABSTRACT

PURPOSE: This study was to analyze the trends and content of all the research (171 research papers) published in the Journal of Fundamentals of Nursing from 1994, when it began to 2000 using an objective tool developed by the researches, and to identify the direction for the future research in Fundamentals of Nursing. METHOD: Research published in the Journal of Fundamentals of Nursing from 1994 were reviewed. RESULT: Most of research (94.2%) was non-degree based and funded research was very limited. The average number of researchers per research study was 2.2 and collaborative research has been gradually increasing. The total number of key words was 331 and the average was 2.6 per research. The major key words were not different from other nursing departments and included home care nursing (10 times), health promotion (8 times), self-care (7 times), self-efficacy (7 times), and hemo-dialysis (6 times). Eighty seven percent of the research was quantitative research, and there were very few qualitative studies. Considering theory level, it was found that 40% were factor related research. For study design, non-experimental studies were most frequent (66%), and the rate, especially of surveys, has remarkably decreased. Selection of subjects by convenience sampling, was most frequent and there were very few studies that provided the rationale for the calculation of sample size. The major subjects of study were patients (44.8%) with various diagnoses. The subjects usually gave oral consent to take part in that study. Giving information (46.9%) and exercise (26.5%) were common nursing interventions, and physiological indices (16.5%), vital signs (10.3%), physical functioning (8.2%), level of knowledge/skill (7.2%) and level of activity (6.2%) were frequently measured as outcome. Variables questionnaires were the major approach used to collect data, and 57.8% of the research provided the Cronbach alpha to guarantee internal consistency of study instruments. Data were analyzed with computerized statistical packages using, ANOVA (42.0%), T test(39.5%), and chi-square test. For the last seven years, nursing research in Fundamentals of Nursing has gradually improved in both quantity and quality. CONCLUSION: It was difficult to find any uniqueness or difference compared to other departments of nursing. In fact, because the history of the Journal of Fundamentals of Nursing is rather short, we can expect that there will be further improvement in quality and content in the future.

ABSTRACT

This study is based on grounded theory methodology by Strauss & Corbin(1998). Ten hospitalized subjects were interviewed for data collection. In the process of data analysis, 'acceptance' is found to be the causal condition, while 'health professionals' skillfulness', 'ward environment', 'history of hospitalization', and 'general conditions' were identified as context, 'felling of relief' as the core phenomenon, 'self-efficacy', 'support of others', and 'life style' as the intermediate situation, 'passive reaction', 'alternative reaction' and 'active reaction' as the strategy and 'stabilization', 'satisfaction', 'hope' and 'carrying out' as consequences. 'Feeling of relief' is found to go through the three stages of recognition-generation-maintenance after the five different patterns. 1) In case the health professionals are skillful, the ward environment is favorable, the general conditions of the patients improved and as a result the feeling of relief is strong, during the first hospitalization, the self-efficacy of the subjects tends to be strong. They proceed toward the goal set for themselves with a renewed hope and active or alternative reaction toward the feeling of relief. 2) The subjects tend to proceed toward the goal set for themselves with a renewed hope and active and alternative reaction toward the feeling of relief in case health professionals are skillful, the ward environment is favorable the general conditions of the subjects improved, self-efficacy is strong, and lifestyle is autonomous, during the second hospitalization even though support of others is merely superficial. 3) The subjects tend to stabilize, and satisfy themselves with the given situation with passive and alternative reaction to the feeling of relief in case health professionals are skillful and the ward environment is favorable but the general conditions worsened and accordingly the feeling of relief, is weak and life style is dependent during the second hospitalization although the subjects' self-efficacy is strong and support of others is specific. 4) The subjects tend to stabilize and satisfy themselves with the given situation with passive and alternative reaction to the feeling of relief in case health professionals are unskillful the ward environment is unfavorable, the general conditions improved, support of others is specific but life style is dependent and self-efficacy is weak during the first hospitalization. 5) The subjects tend to stabilize and satisfy themselves with the given situation in case health professionals are unskillful the ward environment is unfavorable but the general conditions improved support of others is specific and as a result self-efficacy is strong but life style is dependent.

ABSTRACT

The purpose of the study is to evaluate the effectiveness and competence level of trainees of Cardiopulmonary resuscitation training targeted for nursing students. 70 nursing students of Y nursing college are recruited as subjects from Dec. 1st, 1998 through Dec. 8th, 1998. For the pre-test, demographic data related to CPR and knowledge of CPR were evaluated. For the post-test, the next week of pre-test, three difference groups of subjects were tested their knowledge of CPR. CPR training was designed by two components which were 90 mins lecture and demonstration by one professor and individual practice using two educational models with two professors. As the tool of measurement estimating pre or post knowledge of CPR, questionnaires were developed based on self-diagnosis questionnaires of American Heart Association (AHA). The questionnaires were multiple choices (50 questions) and open end questions regarding CPR process. Each multiple choices questions valued 2 points (Score varied min. 0 point to max. 100 points.). Collected data were computerized and analyzed by SPSS-WIN. Frequency and percentage of each questions analyzed. The differences of the knowledge and competency level of subjects between pre and post test was analyses by paired t-test. The followings are research outcome. 1. In the pre-test, 95% of subjects answered that they already knew what CPR was, but only 82% described correctly what CPR was. 49% learned CPR before, and 80s5 of them learned at high school. 2. 37 questions scores increases, and 10 questions scores decreased. 3 questions didnt change their score. After getting training, ratio of 80% correct score significantly increased 4 times. 3. In post-test, knowledge level of trainees increased compared to that of pre-test. (t=-15.075, p=0.000) 4. Competence level also increased (t= -14.86, p=0.00). In result, after getting CPR training, most CPR knowledge increased except open the air tract, toddler CPR, and alternative behavior when the air tract is blocked. CPR training needs to extend the educational scope not only CPR lecture but also psychomotor skill practice. CPR trainees are in need of appropriate feedback as well as enough opportunities of skill practice.

ABSTRACT

In 1990 Home Care Education Programs started when legislation established certification for Home Care Nurses. The Ministry of Health and Welfare proposed a home care education curriculum which has 352 class hours and 248 hours of 'family nursing and practice'. Though Home Care Education Programs have been offered in 11 home care educational institutes, there has been no formal revision for the home care education programs. Also a first and second home care demonstration projects have been carried out, but there has been no research on outcomes for home care education as applied in home care practice. The purpose of this study were to identify the important content areas for home care nursing as perceived by home care nurses, and to identify their clinical competence in each of these areas, and from these to identify the education needs. The sample was 107 home care nurses who were working in home care demonstration hospitals and community-based institutions which have been offering home care services. Responses were received from 88 nurses, comprising a 82.2% return rate, and 86 were included in the final analysis. The instrument used was a modification of the instrument developed by Caie-Lawrence et(1995) and Moon's(1991) instrument on home care knowledge. The instrument's Cronbach's coefficient was 0.982. Among the respondents, 64% were working at home care demonstration hospitals and 36% were working at community-based institutions. Their home care experiences were from one month to six years, with a mean of 20.6 months. The importance rating for home care education content was 3.42 0.325, which means importance was rated relatively high. Technical aspects of home care were identified the most important. Five items 'education skill', 'counseling skill', 'interview skill', 'wound care skill', 'bed sore care skill' received 100% importance ratings. The competency rating was 2.87 0.367 and 'technical aspects of home care' was the highest, and 'application to home care skill' was the lowest. Home care nurses' education needs were identified and compared to the importance ratings and competency ratings. Eleven items were identified as the highest in the importance areas and eleven items were in the lowest competency areas. High importance ratings matched with low competency ratings determined training needs, but there was no matching items in this study. In the lowest competency areas four items were excluded, because of not being applicable in current home care practice. Therefore total eighteen items were identified as home care education needs. These items are 'bed sore care skill', 'malpractice', 'wound care skill', 'general infection control', 'change and management of tracheostomy tubes', 'CVA patient care', 'Hospice care', 'pain management', 'urinary catheterization and management', 'L-tube insertion and managements', 'Respirator use and management skill', 'infant care', 'prevention to burnout', 'child assessment', 'CAPD', 'infant assessment', 'computer literacy', and 'psychiatry patient care'.

Injuries are important causes of death and disability. The purpose of this study was to determine how often preschoolers have injuries and what factors affect the injury occurrences. The sample consisted of 360 preschoolers who were registered at eight nurseries located in Sedaemoongu, Seoul. The data were analyzed using the SAS PC statistical package. The results of the study are as follows: During last 12 months 73.1% of 360 preschoolers had accidental injuries. In an analysis of the location where the injury took place, showed that the most places were in the nursery(17.3%) and around home(14.4%), and the most place inside home were in the bedroom(13.9%) and living room(13.9%). There were the diverse and injurious risks inside the home as well as around home. The most frequent type of injury was abrasion(27.5%) and most children injured their legs(28.3%). They were treated most frequently at home(52.8%) and usually emergency treatment was performed by family members with disinfection being the main type of first aid. To identify factors related to injury occurrence, multiple logistic regression was performed and the main factors were the age of child, active tendency, and mother's education level.

ABSTRACT

The effectiveness of dedication video tapes in teaching fundamental nursing skills was investigated using experimental research methodology. Data was collected from 153 students attending fundamental nursing practice classes of two junior colleges and two universities. The subjects were randomly assigned to two groups, one was an experimental group of 74 subjects and the other was a control group of 79 subjects. The in dependent variable was learning type and the dependent variables were the degree of knowledge achievement and nursing skills achievement regarding medication. The research hypotheses of this study were as follows : 1) There will be a significant difference between the experimental group and the control group in the degree of knowledge achievement about medication. 2) There will be a significant difference between the experimental group and the control group in the nursing skills achievement about medication. The data was analyzed with the SPSSWIN program and the t-test, number of subjects and percentiles were then examined. The findings of this study are as follows : 1) There was a significant difference between the experimental group and the control group in the knowledge achievement about medication using P<.05. 2) There was no significant difference between the experimental group and the control group in the nursing skill achievement about medication using P<.05. 3) This study suggests that medication video tapes in the education of fundamental nursing skills are effective for knowledge achievement. As a result, medication video tapes can be effectively utilized for the education of nursing skills regarding medication.

Computer-Assisted Instruction(CAI) is one kind of teaching method to teach leaners knowledge, attitude, and technical aspect of course contents through utilizing a computer as a learning material. CAI is focused on the concept of interaction and individualized learning. Thus in teaching and learning process CAI is considered the individual differences of learners. And learners can study course contents repeatedly through utilizing courseware. Based on the importance of individualized learning program through utilizing a computer the courseware of infection control was developed to acquire and apply the appropriate knowledge for controlling infection for learners.

ABSTRACT

Effectiveness of the video-record learning method in teaching bowel elimination nursing skill was investigated using an experimental research methodology. Data was collected from 63 female students attending Fundamental Nursing class from a nursing college in Seoul. The subjects were randomly assigned to two groups, one is the experimental group of 29 and the other the control group of 34. The independent variable was video-record learning method and the dependent variable were the degree of knowledge achievement, nursing skill achievement, competence on practicing elimination skill, and satisfaction about the learning method. The hypotheses of the study were as following. 1) There will be significant difference between the experimental group and the control group in dependent variables. 2) There will be significant positive correlations between nursing skill achievement and other three dependent variables-interest in nursing, adaptation in nursing, and preference of nursing job. Data was analyzed using descriptive statistics, chi-square test, t-test, and Pearson's correlation coefficient with SPSS PC+ program. Findings of the study are: 1) There was no significant difference between the experimental group and the control group in knowledge achievement using P<.05. 2) There was significant difference between the experimental group and the control group in nursing skill achievement using P<.05. 3) There was no significant difference between the experimental group and the control group in competence on practicing elimination skill using P<.05. 4) There was no significant difference between the experimental group and the control group in satisfaction about learning method using P<.05. 5) There was positive correlation between nursing skill achievement and the other variables but no significant difference was shown. 6) This study suggests that video-record learning method is an effective learning method for achieving basic nursing skills but is not effective in other areas such as knowledge achievement, competence in performing nursing practice, and satisfaction about the learning method. Further study with more developed research design and statistical analysis should be done to investigate the effectiveness of video-record learning method in learning basic nursing skill more accurately.

ABSTRACT

The circadian system represents a temporal order which is mediated by the mutual coupling of oscillators and by the synchronizing effects of zeitgebers. It is known that well-being of man depends partly on the maintenance of this order, and that repeated or long lasting disturbances to it such as shift work will cause harmful effects. This study was a quasi-experimental study to test the effect of shift directions for the clinical nurses on the circadian rhythm. Fourteen nurses working at the general units of Y hospital were selected according to the established criteria. Fourteen subjects were assigned to a weekly shift but the directions of shift work were phase delay first and then phase advance or vice versa. Oral temperature, total sleeping time, frequency of sleep-wake cycle, fatigue, mental performance, and physical symptom were measured during these days except holidays. The data collection period was from April 26, 1993 to July 3, 1993. MANOVA and Wilcoxon signed rank test were used for statistical analysis. The results are summarized as follows. 1. Having worked on evening and night shifts in either phase delay or phase advance schedules, temperature rhythms of shift workers were gradually adapted to the new sleep-wake cycles. A complete adaptation to work on the night shift was achieved the sixth day of the night shift in the phase delay schedule compared to the partial adaptation to the work on the night shift in the phase advance schedule. Accordingly, by putting evening shift between day and night shifts, it will be possible for circadian rhythm to adapt easily to the night shift. 2. There were differences in the total sleeping time, frequency of sleep-wake cycle, fatigue, and physical symptom except for mental performance between night shift and day, evening shift. This indicates further that shift workers working on the night shift have a hard time adapting to the shift work compared to the other shifts. 3. Evaluating all the acrophases of temperature rhythm either in phase delay or phase advance schedules, it was shown that night to evening shift in the phase advance schedule revealed the smallest phase move. Also phase advance schedule showed poorer adaptation to shift work than phase delay schedule in connection with total sleeping time, frequency of sleep-wake cycle, fatigue, mental performance, and physical symptom. It is suggested, taken together, these findings reflect that phase delay schedule facilitated the degree of adjustment to the shift work compared to the phase advance schedule.
