ABSTRACT

Objective:To investigate the application value of manual anastomosis of gastro-duodenum in totally laparoscopic distal gastrectomy.Methods:The retrospective cohort study was conducted. The clinicopathological data of 55 patients with gastric cancer who underwent totally laparoscopic distal gastrectomy combined with gastrointestinal anastomosis in the Tianjin Medical University Cancer Institute & Hospital from January 2020 to October 2022 were collected. There were 34 males and 21 females, aged 61(range, 29?75)years. Of 55 patients, 25 patients undergoing manual anastomosis of gastroduodenum were divided into the manual anastomosis group, 30 patients undergoing modified Delta anastomosis of gastroduodenum were divided into the modified Delta anastomosis group. Observation indicators: (1) surgical situations; (2) postoperative complications. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was conducted using the t test. Measurement data with skewed distribution were represented as M(range). Count data were described as absolute numbers, and comparison between groups was conducted using chi-square test or Fisher exact probility. Results:(1) Surgical situations. All 55 patients underwent surgery successfully, without conversion to laparotomy. The distance from the superior margin of tumor to the upper margin, anastomosis time, number of bookings used were (48±4)mm, (22.6±2.3)minutes, 3.2±0.5 in the manual anastomosis group, versus (41±4)mm, (14.0±1.4)minutes, 5.2±0.4 in the modified Delta anastomosis group, showing significant differences in the above indicators between the two groups ( t=5.04, 16.38, ?17.13, P<0.05). The location of tumor (antrum, gastric angle) was 18, 7 in the manual anastomosis group, versus 29, 1 in the modified Delta anastomosis group, showing a significant difference between the two groups ( P<0.05). (2) Postoperative complications. There was no patient undergoing anastomotic fistula in both of manual anastomosis group and modified Delta anastomosis group, and there was 1 patient undergoing anastomotic stenosis in the modified Delta anastomosis group. Conclusion:Compared with modi-fied Delta anastomosis of gastroduodenum,totally laparoscopic distal gastrectomy with manual anas-tomosis of gastroduodenum can remove more gastric tissue, and decrease the number of bookings used.

ABSTRACT

Objective@#To investigate the status of insomnia, Internet addiction, and depressive symptoms among medical students and to analyze the effect of Internet addiction on insomnia and the mediating role of depressive symptoms, in order to provide a basis for the development of targeted interventions and measurements for medical students.@*Methods@#A stratified whole group sampling method was used to select full-time college students from three medical universities in Anhui Province. The Chinese version of Insomnia Severity Index (ISI), Internet Addiction Test (IAT) scale and 9-item Patient Health Questionnaire (PHQ-9) were used to evaluate the symptoms of insomnia, Internet addiction and depressive in students. A multivariate Logistic regression analysis was used to explore the factors influencing insomnia among medical students and to analyze the relationship between insomnia with Internet addiction and depressive symptoms, respectively.@*Results@#The overall rate of Internet addiction was 49.5%, depressive symptoms was 39.5%, insomnia was 18.6%. High academic stress, and the presence of surrounding people diagnosed with COVID-19 were associated with a higher risk of insomnia ( P <0.05). The higher the level of Internet addiction (mild, OR =2.60; moderate/severe, OR =4.21) and depression. (mild, OR =6.35; moderate/severe, OR =19.32), the higher the risk of insomnia. Mediated effect analysis showed that Internet addiction had a direct predictive effect ( β =0.02, P <0.05) on insomnia and also indirectly affected insomnia through depression (indirect effect=0.07,95% CI =0.06-0.08).@*Conclusion@#The detected rates of insomnia, Internet addiction and depressive symptoms are high among medical students in Anhui Province, and Internet addiction and depressive symptoms are risk factors for insomnia, which should be given more attention and appropriate interventions when necessary to improve their physical and mental health.

ABSTRACT

The incidence of gastric cancer in the upper third of the stomach, including early gastric cancer, is increasing in China, South Korea and Japan. Function-preserving surgery for early gastric cancer is mainly aimed at minimizing postoperative complications and improving postoperative quality of life. Proximal gastrectomy can resect the tumor while preserve part of the stomach tissue which is beneficial to improve the postoperative nutritional status and the postoperative quality of life. However, acid reflux, reflux and the subsequent reflux esophagitis after proximal gastrectomy due to the loss of the esophageal sphincter and His angle would affect postoperative quality of life. Clinical surgeons choose different methods of digestive tract reconstruction to improve the effect of anti-esophagogastric reflux. But there are many kinds of digestive tract reconstruction methods, which are controversial. The authors comprehensively analyze the related research progress at home and abroad, systematically elaborate the current status of digestive tract reconstruction after proximal gastrectomy, and discuss the advantages and disadvantages of various digestive tract reconstruction methods.

ABSTRACT

Objective To investigate the dynamic changes and coordination of macrocirculation and microcirculation in rats with severe hemorrhagic shock and septic shock. Methods A total of 20 male Sprague-Dawley rats (450~550 g) were randomly divided into two groups (n = 10) : hemorrhagic shock group (H-Shock) and septic shock group (S-Shock). 40% of the systemic blood volume was withdrawn in 1 hour in H-Shock group.While in the S-Shock group, cereal ligation and puncture (CLP) was performed and then the abdomen was closed.Mean arterial pressure (MAP) and the end-tidal carbon dioxide (ETCO2) were recorded continuously. Cardiac ultrasonography and sublingual microcirculation were performed per hour till 12 hours or the rat die (End). A total of 12 hours were observed or the rat die. Arterial blood gas was measured at baseline (BL) , MAP ≤ 65 mmHg (Shock) and at the termination of observation respectively. Results The values reflect macrocirculation or microcirculation in the H-Shock group were all decreased than BL respectively at each time. In the S-Shock group, the values that reflect macrocirculation such as MAP, ETC02 and cardiac output (CO) showed no significant changes within 3 hours (P> 0.05) , while the values of microcirculation were decreased compared with BL respectively at each time (P < 0.05). With the progress of shock, the pH was decreased compared with BL in both groups, while the level of Lac were increased (P < 0.05). Conclusions The changes of macrocirculation and microcirculation in severe hemorrhagic shock are basically the same; while they are inconsistent in septic shock, the changes of microcirculation occurred earlier than that of macrocirculation.

ABSTRACT

Objective: To evaluate the impact of perineural invasion (PNI) on the overall survival (OS) of patients with gastric cancer. Methods: A total of 1,007 patients with gastric cancer who underwent curative resection between January 2011 and December 2012 at the Cancer Institute and Hospital of Tianjin Medical University were enrolled. All the patients were categorized into the following two groups according to the status of PNI: positive group, presence of PNI; and negative group, absence of PNI. Potential prognostic factors and clinical pathological variables correlated with the presence of PNI were analyzed. Results: One hundred and twenty (11.9%) patients had PNI. Multivariate analysis revealed that histology, depth of invasion, and lymphovascular invasion were indepen-dently associated with the presence of PNI. Univariate survival analysis revealed that age, tumor location, Borrmann type, tumor size, curability, TNM stage, type of gastrectomy, tumor deposit, lymphovascular invasion, PNI, preoperative CA19-9 levels, and CEA levels were significant prognostic factors. Gastric cancer patients with PNI had a significantly lower 5-year OS rate than those without PNI (5-year OS: 38.3% versus 66.6%, P<0.001). In the multivariate analysis, age, Borrmann typeⅣ, TNM stage, curability, tumor deposit, and PNI were independent prognostic factors for this population cohort. The strata analysis revealed that PNI merely had a significant im-pact on OS in patients at stagesⅠ,Ⅱ, andⅢa. Conclusions: PNI is an independent prognostic factor in patients with gastric cancer and can be used as a prognostic indicator for gastric cancer patients at stagesⅠ,Ⅱ, andⅢa.

ABSTRACT

Owing to technological developments in endoscopic treatment with continued development and application of newer en-doscopic equipment, as well as improved health consciousness among patients, a greater number of patients are being diagnosed with early gastric cancer (EGC). Both laparotomy and laparoscopic surgery can achieve satisfactory results in the treatment of EGC. Pa-tients undergoing endoscopic treatment for EGC showed improved quality of life postoperatively with lower complication rates. Endo-scopic submucosal dissection (ESD) is being widely used to treat EGC, with an overall 5-year survival rate of 90%. Patients undergoing ESD showed better perioperative outcomes in terms of lesser physical trauma, higher safety, excellent patient compliance, and lower treatment costs. ESD may gradually replace conventional surgery as the primary treatment for EGC. This paper focuses on the clinical application of ESD in the management of EGC.

ABSTRACT

Objective@#To investigate the effect of adding glutathion(GSH) to tumescent solution on autologous fat graft survival.@*Methods@#14 male and female New Zealand rabbits were divided into experimental group and control group randomly, 7 in each group. Experimental group: The donor areas of the rabbits were injected with 3 ml of tumescent solution with GSH. Control group: The donor areas of the rabbits were injected with 3 ml regular tumescent solution. DCFH-DA probe was used for fluorescent staining of harvested fat cells. Then stained fat cells were measured for the intracellular reactive oxygen species(ROS)content by fluorescence microplate. The grafts were harvested at 3 months after transplantation and assessed by general observation, volume measurement, wet weight measurement, HE staining for the number of fat cells, and CD34 immunohistochemical staining for the measurement of micro-vascular density. T test was performed by using SPSS 24.0.@*Results@#The intracellular ROS content of harvested fat cells in experimental group was lower than that in control group, and the difference was statistically significant (P<0.05). At 3 months after transplantation, the wet weight of fat grafts in experimental group was (1133.21±87.97) mg and that in control group was (718.79±79.27) mg. The volume of fat grafts in experimental group was (1.00±0.04) ml and that in control group was (0.59±0.03) ml. The number of fat cells in experimental group was (746.6±15.7)/10 high magnification vision and that in control group was (350.1±32.4)/10 high magnification vision. The density in group experimental was (8.1±2.0)/high magnification vision and that in control group was (6.7±2.4)/high magnification vision. The grafts′ volume, wet weight, and the number of fat cells in experimental group were higher than those in control group, and the differences were statistically significant (P<0.05). The difference of micro-vascular density between experimental group and control group had no statistical significance (P>0.05).@*Conclusions@#The addition of GSH to tumescent solution optimizes the process of autologous fat harvest, thereby improving the survival of fat graft.

ABSTRACT

Objective To evaluate the efficacy and safety of posaconazole for preventing invasive fungal disease (IFD) in the aplastic anemia patients receiving allogeneic hematopoietic stem cell transplantation (HSCT). Methods A total of 46 aplastic anemia patients received allogeneic HSCT. They were treated with oral posaconazole 200 mg, three times a day from HSCT pretreatment to granulocyte recovery after transplantation. G test and GM test were done 1, 2 and 4 weeks after the end of posaconazole treatment, and chest CT scan repeated 4 weeks after the end of posaconazole treatment. Posaconazole prophylaxis was defined as successful if there were no clinical manifestations indicative of fungal infection. Results All the 46 patients experienced neutropenia. The median of absolute neutrophil count (ANC) nadir was 0.02 (0-0.05)×109/L for a median time of 10 (8-19) days. The median duration of posaconazole prophylaxis was 26 (15-41) days. Neutropenic fever was reported in 45 patients, which lasted a median time of 5 (1-13) days. Six patients (13.3% of the patients with neutropenic fever) failed to respond to the empirical treatment of broad spectrum antibiotics with persistent fever over 7 days. Their treatment was switched to short-term empirical treatment with broad spectrum antifungal agents. However, subsequent G test, GM test and chest CT showed negative results. None of the six patient was consistent with IFD diagnosis. Breakthrough fungal infection was not considered. Oral posaconazole solution was resumed for preventing IFD. G test, GM test and chest CT scan did not show any sign of fungal infection 1, 2 and 4 weeks after the end of posaconazole prophylactic treatment in all the 46 patients, proving the success of oral posaconazole in preventing IFD. Posaconazole was not discontinued due to adverse drug reaction in any patient. Conclusions Posaconazole is effective for preventing IFD in the aplastic anemia patients receiving HSCT with good safety profile and few adverse reactions.

ABSTRACT

<p><b>OBJECTIVE</b>To investigated the characteristics and regularity of lymph node metastasis around gastric cancer in order to provide reference for standardized and optimal surgical treatment.</p><p><b>METHODS</b>A retrospective case series study was carried out on 1456 patients with gastric cancer undergoing surgical treatment at the Affiliated Tumor Hospital of Tianjin Medical University from March 2003 to August 2011. The number of harvested lymph node and metastasis status of various lymph node station were determined by routine pathological examination of specimens, including resected gastric tissue and dissected lymph node tissue, according to the 13th version of the Japanese Gastric Cancer Treatment Guidelines. Tumor T, N and M staging was performed to evaluate lymphatic metastasis status of different locations of gastric cancer according to the TNM staging criteria of the 8th edition of the American Joint Committee on Cancer (AJCC). The influence of gender, age, tumor diameter, Borrmann type, T staging and M staging, tumor differentiation degree, invasion of vessels, lymphatic vessels and nerves, radical surgical degree and other clinical factors on lymph node metastasis was analyzed.</p><p><b>RESULTS</b>A total of 1062 cases(72.9%) had lymph node metastasis in the 1456 patients with gastric cancer. A total of 9766 lymph nodes were positive for metastasis. Lymph node metastasis occurred in 11 of 44 (25.0%) patients with early gastric cancer and in 1051 of 1412 (74.4%) patients with advanced gastric cancer. The largest number of lymph node metastases was found in No.3 station [653 cases (44.8%)], followed by No.6 [437 cases(30.0%)], No.7 [345 cases (23.7%)], No.1 [304 cases (20.9%)], No.4sb [290 cases (19.9%)]. No.14v lymph node metastasis was observed in 23 cases, of whom No.6 (16 cases, 69.6%), No.8a (15 cases, 65.2%) and No.3 (12 cases, 52.2%) developed simultaneous metastasis. As for different locations of gastric cancer, stations with more lymph node metastasis in 309 patients with proximal gastric cancer were No.3 (133 cases, 43.0%), No.1 (96 cases, 31.1%), No.2 (90 cases, 29.1%) and No.7 (89 cases, 28.8%); in 144 patients with middle gastric cancer were No.3 (68 cases, 47.2%), No.6 (50 cases, 34.7%), No.7 (40 cases, 27.8%) and No.4sb (38 cases, 26.4%); in 700 patients with distal gastric cancer were No.3(287 cases, 41.0%), No.6 (265 cases, 37.8%), No.4sb (138 cases, 19.7%) and No.8a (138 cases, 19.7%); in 303 cases with diffuse-type gastric cancer were No.3 (165 cases, 54.4%), No.6 (100 cases, 33.0%), No.7 (88 cases, 29.0%), No.1 (84 cases, 27.7%) and No.4sb (72 cases, 23.8%). The incidence of lymph node skip metastasis was 7.2% (105/1456) in whole group. Positive lymph node metastasis was associated with tumor size (RR=2.016, 95%CI: 1.550-2.621, P=0.000), tumor differentiation(RR=1.631, 95%CI:1.405-1.894, P=0.000), tumor T staging (RR=1.886, 95%CI: 1.629-2.184, P=0.000), tumor M staging (RR=3.671, 95%CI:1.265-10.660, P=0.017) and radical surgery(RR=3.819, 95%CI: 2.023-7.207, P=0.000).</p><p><b>CONCLUSIONS</b>The main direction of peripheral lymph node drainage in gastric cancer is lesser curvature, and then the left gastric artery, the common hepatic artery and the peripheral lymph nodes of the celiac axis, and finally the peripheral lymph nodes of the abdominal aorta. Therefore the No.6 station lymph node adjacent to the perigastric area, the No.7, No.8 and No.9 lymph nodes should be the focus of the radical surgical dissection of gastric cancer. Tumor size, differentiation degree, invasion depth and distant metastasis have significant association with lymph node metastasis. For patients with adverse factors, radical surgery is necessary to ensure efficacy.</p>

ABSTRACT

Objective To investigate the target genes in RYBP-mediated leukemia cell apoptosis by high-flux sequencing. Methods The HL-60 cell line with knockdown of RYBP was set up. MRNA and microRNA sequencing was conducted by the next-generation sequencing technology. Result MRNAs and miRNAs were dys-regulated in HL-60 cell line with knockdown of RYBP. The results of KEGG analysis indicated that the down-regu-lated genes were associated with cancer transcription regulation and amino acid metabolism. SPI1 and miR-214-3p were shown downregulated after knockdown of RYBP. Conclusion The target genes in RYBP-mediated leukemia cell apoptosis include BBC3,BAI1,SESN2 ,CCNG2,JAK3,STAT4,SPI1,BCL6,CD11b and hsa-miR-214-3p.

ABSTRACT

A sensitive colorimetric method for the detection of copper ions (Cu2+) was developed based on the surface modification of silver/platinum nanoclusters (Ag/Pt NCs) and regulation of peroxidase-like activity. It was found that 3-mercaptopropionic acid (MPA) could inhibit the catalytic ability of Ag/Pt NCs; however, it lost the inhibition toward catalytic ability of Ag/Pt NCs after oxidized by oxygen through the catalysis of Cu2+. On the basis of this, a colorimetric method was developed for the detection of Cu2+ through measuring the colorimetric signal variation of the TMB-H2O2 reaction. This method exhibited high sensitivity and selectivity toward Cu2+ over a panel of other metal ions. The linear range was 10-100 nmol/L and the detection limit was 5.0 nmol/L (3σ). The above method was also applied to detect real water samples and spiked samples, and the results demonstrated that this method was simple with low cost.

ABSTRACT

The rutile structure titanium oxide (Ti-O) film was prepared on the pure titanium material TA2 (99.999%) surface by the magnetic filter high vacuum arc deposition sputtering source. The method can not only maintain the material mechanical properties, but also improve the surface properties for better biocompatibility to accommodate the physiological environment. The preparation process of the Ti-O film was as follows. Firstly, argon ions sputtered to the TA2 substrate surface to remove the excess impurities. Secondly, a metal ion source generated Ti ions and oxygen ions by the RF discharge. Meanwhile a certain negative bias was imposed on the sample. There a certain composition of Ti-O film was obtained under a certain pressure of oxygen in the vacuum chamber. Finally, X-ray diffraction was used to research the structure and composition of the film. The results showed that the Ti-O film of the rutile crystal structure was formed under the 0.18 Pa oxygen partial pressure. A Nano scratch experiment was used to test the coating adhesion property, which demonstrated that the film was stable and durable. The contact angle experiment and the platelet clotting experiment proved that the modified surface method had improved platelet adhesion performance, and, therefore, the material possessed better biocompatibility. On the whole, the evaluations proved the modified material had excellent performance.

ABSTRACT

Objective:The characteristics of lymph node metastasis were investigated to guide the range of lymph node dissection during the radical operation of distal gastric cancer. Methods:The clinical data of 773 patients with distal gastric cancer who under-went radical distal subtotal gastrectomy at the Tianjin Medical University Cancer Institute and Hospital between February 2010 and September 2014 were reviewed and analyzed to infer the potential clinical mechanisms of lymph node metastasis. Results:Among the included patients, 423 (54.72%) had lymph node metastasis. The proportion of metastasis in the patients from the highest to the lowest rate was observed in group NO. 6, 3, 4sb, and 5 lymph nodes. The metastasis rates of the N1 lymph nodes from the highest to the low-est were observed in group NO. 3, 6, 5, and 4d lymph nodes, whereas those of the N2 lymph nodes occurred in group NO. 8a, 7, and 1 lymph nodes. Group NO. 8a lymph node with skipped metastasis was observed in 50.68%of the patients. Conclusion:During radical distal gastric cancer surgery, attention should be given to the possibility of metastasis in group NO. 8a lymph node. Appropriate exten-sion of the lymph node dissection should be conducted when necessary.

ABSTRACT

The possibility of polyoxymethylene (POM) as heart valve leaflet material was investigated by comparing the hemocompatibility with that of 316L stainless steel and low-temperature isotropic pyrolytic carbon (LTIC). Surface hydrophobicity was characterized by water contact angle measurement. Platelet adhesion, APTT/PT/TT and hemolysis rate tests were applied for evaluating hemocompatibility. The results showed that POM was hydrophobic and had a low hemolytic rate, adhesion amount and activation degree of platelets on POM surface were less than 316L stainless steel, and was similar to LTIC. This research pointed out potential application of POM as heart valve leaflets.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the postoperative adverse events and survival of patients with sustained-released fluorouracil implanted during operation.</p><p><b>METHODS</b>Data of 124 patients with advanced gastric cancer undergoing radical operation in Tianjin Medical University Cancer Institute and Hospital from January 2007 to January 2009 were analyzed retrospectively. All the patients were divided into two groups according to whether intra-operative fluorouracil was implanted or not. The treatment group(n=64) was implanted with fluorouracil in abdominal cavity after radical resection. The control group(n=60) did not receive fluorouracil implant in abdominal cavity after radical resection. Abdominal drainage fluid, temperature and adverse events within 15 postoperative days and 3-year survival were observed and compared between the two groups.</p><p><b>RESULTS</b>Pathological findings of the two groups were similar. No statistical significances existed in abdominal drainage fluid, temperature and adverse events within 15 postoperative days(P>0.05). The 3-year survival rate was higher in treatment group(64.3% vs. 42.4%, P=0.018).</p><p><b>CONCLUSION</b>Intra-operative sustained-released fluorouracil implants are safe and tolerable, and can improve the survival rate of patients with advanced gastric cancer.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To elucidate the prognostic influence of preoperative weight loss for gastric cancer.</p><p><b>METHODS</b>A total of 672 gastric cancer patients who underwent gastrectomy between January 2003 and December 2007 were enrolled. The patients were categorized into three groups according to the percentage of weight loss before surgery: no weight loss group (0%), limited group ( < 10%), and severe group ( ≥ 10%). Compared the clinicopathologic characteristics and analyzed the prognostic influence of preoperative weight loss. The survival was analyzed by Kaplan-Meier survival cure and the prognostic factors were analyzed univariately and multivariately by Cox comparative hazard modal.</p><p><b>RESULTS</b>Among the 672 cases gastric cancer, no weight loss group had 275 cases, limited group 294 cases, severe group 103 cases. Tumor size (F = 4.386) , tumor location (χ² = 15.864), depth of invasion (χ² = 22.245) , the number of lymph node metastasis (χ² = 23.803), Surgical approach (χ² = 18.423) , extent of lymphadenectomy (χ² = 8.172) , curability (χ² = 15.650) were discrepant among the three groups (all P < 0.05) . The 5-year survival rate of the patients with severe group was 28.0%, limited group was 37.7%, while the no weight loss group was 40.3% (χ² = 20.148, P < 0.05). Age (95% CI: 0.480 - 0.744, P = 0.000), weight loss before surgery (95% CI: 0.371 - 0.687, P = 0.000), depth invasion (95% CI: 0.289 - 0.564, P = 0.000), lymph node metastasis (95% CI: 0.451 - 0.783, P = 0.000), extent of lymphadenectomy (95% CI: 0.647 - 0.990, P = 0.000), curability (95% CI: 0.291 - 0.486, P = 0.000), postoperative adjuvant chemotherapy (95% CI: 0.511 - 0.846, P = 0.000) were associated with survival of this group. In multivariate analysis, age (HR = 1.618, 95% CI: 1.298 - 2.016, P = 0.000), weight loss before surgery (HR = 1.258, 95%CI: 1.077 - 1.469, P = 0.004), depth of invasion (HR = 1.810, 95% CI: 1.287 - 2.545, P = 0.000), N stage (HR = 1.555, 95% CI: 1.413 - 1.172, P = 0.000) were independent prognostic factors for survival.</p><p><b>CONCLUSIONS</b>Patients with weight loss above 10% have poor prognosis. Weight loss before surgery may be an important independent prognostic factor for gastric cancer.</p>

ABSTRACT

Based on the analysis of the influence of the valve pivot distance on the performance of mechanical heart valve (MHV), such as the valve opening and closing features, flow field characteristics and the valve assembly properties, value constraints of the valve pivot distance were established, and the reasonable valve was obtained by means of the finite element method. It can be shown that the central flow characteristics of the valve could be enhanced with the increasing of the ratio of pivot distance to valve inner diameter, but the plastic deformation of the ring could be liable to occur in the MHV assembly process. It is proved that the valve of specifications can be designed in similar ratio of pivot distance to valve inner diameter according to the result of the valve performance experiment.

ABSTRACT

Objective To evaluate the diagnosis and therapy of solid pseudopapillary tumor of the pancreas (SPTP).Methods From January 2006 to October 2011,the clinicopathological data of 24 SPTP patients diagnosed and surgically treated at Tianjin Medical University Cancer Hospital were analyzed retrospectively.Results There were 22 women and 2 men,the median age was 27.5 years old,with a range from 12 to 51 years.The most common tumor localization were at the head of the pancreas ( 11 patients,45.7% ),followed by the body (5 patients,20.8% ),the tail (4 patients,16.7% ),the body and the tail (3 patients,12.5% ) and the neck (one patient,4.2% ).Clinical symptoms were nonspecific and the preoperative diagnosis of SPTP depended on imaging study such as ultrasonography,CT and MR.All patients were treated surgically and the correct diagnosis was made according to pathology after surgery.In a follow-up ranging from 2 to 55 months( median 27.2 months),23 cases were alive with no evidence of disease recurrence,one patient was alive with tumor recurrence.Conclusions SPTP was a less common neoplasm with low-grade malignancy,occurring mainly in young women.Complete tumor resection results in satisfactory prognosis.

ABSTRACT

Objective To explore the occurance and risk factors of recurrence and metastasis of gastric cancer after gastrectomy.Methods From January 2001 to December 2004, the clinic pathological data of 141 patients with recurrence and metastasis after radical gastrectomy in Tianjin Medical University Cancer Institute and Hospital were analyzed retrospectively.The possible clinic pathological factors which may affect tumor recurrence were analyzed.Results After the surgery, the 1, 2, 3 and 5-year cumulative recurrence rates were 58.2%(82/141) 、80.1%(113/141)、89.4%(126/141) 、97.9 % (138/141)respectively.The results of multivariate analysis indicated that the tumor size,invasive depth, lymph node metastasis were independent factors which affected recurrence and metastasis after radical gastrectomy (P = 0.017, 0.003, 0.000).Invasive depth, lymph node metastasis and tumor differentiation degree were independent factors which affected early recurrence and metastasis after radical gastrectomy (P=0.042, 0.000, 0.039).Conclusions The tumor size,invasive depth, lymph node metastasis are the independent risk factors to predict the recurrence and metastasis after radical gastrectomy.Most of the recurrences and metastasis is found within 2 years after radical gastrectomy.Invasive depth, lymph node metastasis and tumor differentiation degree are the independent factors to predict early recurrence after radical gastrectomy.
