ABSTRACT

ObjectiveTo develop a questionnaire on the knowledge needs of respiratory rehabilitation training for in-service healthcare workers, and test its reliability and validity. MethodsA theoretical framework was constructed based on the contents of the respiratory rehabilitation course modules of American Association of Cardiopulmonary Rehabilitation and American Association for Respiratory Care. The first draft of the questionnaire was prepared through literature searches and analyses, semi-structured interviews, and expert group discussions. The reliability and validity of the questionnaire was tested by interviewing 17 respiratory rehabilitation professionals and 16 respiratory rehabilitation experts by correspondence, and investigating 380 cases of in-service healthcare workers engaged in respiratory rehabilitation. ResultsThe knowledge needs questionnaire of respiratory rehabilitation training for in-service healthcare workers included progress in respiratory rehabilitation, physiological basis and diagnosis of chronic lung diseases, individualized assessment and management principles of chronic lung diseases, treatment of chronic lung diseases and guideline-recommended medications, oxygen therapy, exercise assessment and exercise prescription, tobacco addiction and cessation, nutritional assessment and weight management, psychological screening and intervention, self-management and health behavior promotion, and effectiveness assessment of respiratory rehabilitation, totaling eleven dimensions and 52 items. The total Cronbach's α was 0.921, the total folded half reliability was 0.904, the total retest reliability was 0.922, and the Cronbach's α of each dimension ranged from 0.909 to 0.953; the folded half reliabilities ranged from 0.882 to 0.924, and the retest reliabilities ranged from 0.908 to 0.950. The validity of the questionnaire's content was 0.902. In exploratory factor analysis, eleven metric factors were extracted, and their cumulative variance contribution rate was 75.324%. ConclusionThe questionnaire on the knowledge needs of respiratory rehabilitation training for in-service healthcare workers is reliable and valid, and can be used to assess the knowledge needs of healthcare workers participating in in-service training in respiratory rehabilitation.

ABSTRACT

OBJECTIVES@#To understand medical students' mental health, professional pride, and intention to work in the front-line during coronavirus disease 2019 (COVID-19) pandemic, and provide a reference for psychological intervention.@*METHODS@#We used the depression-anxiety-stress scale and self-designed questionnaire on professional pride, intention to work in the front-line and the extent of family support. Medical students from 4 medical schools in Fujian and Hunan were investigated. Their mental health status, professional pride and first-line work willingness with different characteristics were compared, and the influential factors for professional pride and first-line work willingness were analyzed.@*RESULTS@#A total of 266 valid questionnaires were collected. During the pandemic, there were significant differences in the proportion of depressed students among different college and universities, majors and stages (<0.05), and the professional pride was significantly different (<0.001). Medical students with different mental health status showed significant differences in professional pride (<0.01). Marriage, pressure and extent of family support were the influential factors for their professional pride (<0.05). The latter two were also influential factors for their intention to work in the front-line (<0.05).@*CONCLUSIONS@#During the pandemic, students from college and nursing have relatively better mental health and higher professional pride. The professional pride is low in medical students who married, with abnormal stress or low family support. The intention to work in front-line is decreased in students with abnormal stress or low family support.

ABSTRACT

To explore the effects of miR-21 on macrophage autophagy, proliferation and apoptosis induced by cigarette smoke extract (CSE).
 Methods: The cells was divided into a control group, a CSE interventine macrophage group (CSE group), and a miR-21 inhibitor+CSE intervention macrophage group (miR-21 inhibitor+CSE group). The expression of miR-21 in the 3 groups was detected by real-time PCR. The effects of miR-21 inhibitor on macrophage autophagy, proliferation and apoptosis were detected by Western blot, MTT assay and flow cytometry.
 Results: Compared with the control group, the levels of miR-21 and autophagy in the CSE group were significantly increased (both P<0.05). The expression of miR-21 in the miR-21 inhibitor+CSE group was significantly lower than that in the CSE group (P<0.05). Compared with the control group, the expressions of macrophage microtubule associated protein 1 light chain 3 alpha (LC3) and autophagy related 7 (ATG7) in the CSE group were increased, which was attenuated by miR-21 inhibitor. Compared with the control group, the macrophage proliferation in the CSE group was inhibited by the miR-21, which could be reversed by adding miR-21 inhibitor; the proliferative rates in the miR-21 inhibitor+CSE group in 2, 3 or 4 days were increased by 1.41, 1.54 or 1.70 times compared with those in the CSE group (all P<0.05). Flow cytometry showed that the apoptosis rate in the control group was (2.57+1.35)%, which was (18.70+2.16)% in the CSE group and (6.28+1.08)% in the miR-21 inhibitor+CSE group (P<0.05).
 Conclusion: CSE intervention macrophage increase the autophagy and apoptosis of macrophages, decrease the cell proliferation by affecting the expression of miR-21 and the level of autophagy in macrophages.

ABSTRACT

Objective To study the effects and mechanism of Xuling-Jiangu formula on bone mineral density in the osteoporosis model rats.Methods According to the random number table method,50 SD female rats were randomly divided into the sham group,the model group,the low dose group of Xuling-Jiangu formula group,the medium dose group and the high dose of group.In addition to the sham group,the other groups were osteoporosis model.After 30 days,low dose group received intragastric Xuling-Jiangu formula solution 7.5 g/kg;medium dose group and high dose group received 15 and 30 g/kg,respectively.And the sham group and the model group received normal saline 10 ml/kg.After 12 weeks treatment,the bone mineral density of the left tibia was measured by double energy X ray.Estrone,osteocalcin in serum had been detected by Enzyme linked immunosorbent assay (ELISA).The mRNA expression of osteoprotegerin (OPG),receptor activator of nuclear factor κB ligand (RANKL) in lumbar were measured by quantitative real-time PCR.Results The bone mineral density (0.215 ± 0.010 g/cm2,0.222 ± 0.013 g/cm2 vs.0.196 ± 0.016 g/cm2) of the Xuling-Jiangu formula group were significantly higher than that of the model group (P<0.01 or P<0.05).The estrone in low,middle and high dose groups showed an upward trend,but there was no significant difference compared with the model group.The OC (87.0 ± 8.9 ng/L vs.100.5 ± 16.8 ng/L) of high dose group was significantly lower than that of the model group (P<0.05).Compare with the model group,OPG mRNA level (0.97 ± 0.23 vs.0.78 ± 0.17) in high dose group increased (P<0.05),the RANKL mRNA level decreased significantly (1.12 ± 0.17,0.97 ± 0.38,1.04 ± 0.29 vs.1.31 ± 0.18) in three Xuling-Jiangu formula groups (P<0.05).Conclusions The bone mineral density of rats with osteoporosis can be improved by treating Xuling-Jiangu formula.It may be related to the increase of mRNA expression of OPG,and the reduction of RANKL.

ABSTRACT

Objective To explore the mechanism of Liver-soothing therapy on the Luteotropic hormone receptor (LHR) and Follicle stimulating hormone receptor (FSHR) in Perimenopausal Syndrome (PMS). Methods A total of 30 nature aging rats (13-month-old) were randomly assigned into three groups;PMS Liver-qi stagnation model (n=8), PMS Liver-qi stagnation model (n=8) treated with Chaihu-Shugan powder (4.0 g/kg?d) and PMS Liver-qi stagnation model (n=8) treated with Danzhi-Xiaoyao powder (4.0 g/kg?d). The PMS Liver-qi stagnation syndrome rat model were established by immobilizing the nature aging rats. Twelve-week-old female rats (n=8) were used as normal controls. Water decoctions of Chaihu-Shugan powder or Danzhi-Xiaoyao powder were administered respectively for 3 weeks while the rat models established. The serum E2, FSH, LH level were measured by radioimmunoassay. The LHR, FSHR in ovary were measured by quantitative real-time PCR. Results Compared with Liver-qi stagnation syndrome rat model, FSH in the Chaihu-Shugan powder group (4.32 ± 0.33 mIU/ml vs. 5.24 ± 0.45 mIU/ml) decreased (P<0.01), and LH in the Danzhi-Xiaoyao powder group (6.76 ± 0.52 mIU/ml vs. 8.08 ± 0.59 mIU/ml) decreased (P<0.01). Compared with normal controls, LHR mRNA, FSHR mRNA level increased in PMS Liver-qi stagnation model. Compared with Liver-qi stagnation syndrome rat model, LHR mRNA, FSHR mRNA level decreased (7.42 ± 2.54,4.91 ± 1.76 vs. 3.80 ± 1.36) in the ovary of Danzhi-Xiaoyao powder group (P<0.01), but there was no remarkable FHR, LHR expression changes in Chaihu-Shugan powder group. Conclusions The mechanism of Liver-soothing therapy may be related to the regulation of endocrine and decrease of LHR, FSHR.

Chronic obstructive pulmonary disease (COPD) is an incompletely reversible chronic airway disease that can be prevented and cured.There is a tendency toward increasing the morbidity and mortality for COPD.Establishment of an animal model for COPD is an important step to explore the pathogenesis of this disease.Presently,a well-recognized COPD animal model is not available.The key points for establishing the COPD animal models,such as selection of animal species,parameters for model evaluation,are constantly updated.

ABSTRACT

OBJECTIVE@#To explore expression of miR-21 in peripheral blood serum and mononuclear cells of patients with chronic obstructive pulmonary disease (COPD), and to discuss the significance and underlying mechanisms.
@*METHODS@#The subjects were divided into a healthy control group (n=41) and a COPD group (n=49). The miR-21 level was detected by quantitative real-time PCR. The correlations between miR-21 and lung function or forced expiratory volume in one second (FEV1) were analyzed.
@*RESULTS@#The expression levels of miR-21 in the serum and mononuclear cells in the COPD group were significantly elevated compared with those in the healthy group (P<0.05). The expression of miR-21 was correlated with the lung function of COPD patients. The expression level of miR-21 in the COPD patients was positively correlated with FEV1. 
@*CONCLUSION@#The upregulation of miR-21 in peripheral blood serum and mononuclear cells of COPD patients may contribute to the pathogenesis of COPD and the severity of this disease.

ABSTRACT

There is emerging evidence from clinical studies that the existence of liver cancer stem cells(CSCs)or tumor initiating cells is responsible for the high recurrence rates of tumor,generation of metastasis,and resistance to therapeutic regimens after therapy. Here,the characteristics of liver CSCs,clinical manifestation,molecular signaling Wnt/β-catenin,signal transducers and activators of transcription 3,NANOG,annexin A3/c-Jun N-terminal kinase,and chapter four-transmembrane 4 L six family member 5/CD44 in liver CSC self-renewal were briefly reviewed. In addition,potential targets for drug therapy were analyzed,providing some reference for drug discovery that selectively target liver CSC self-renewal signals.

ABSTRACT

Objective To research the toxicity of ethanol extracts from Poylgonum multiflorum Thunb(PMT)induced by en?dotoxin of Gram-negative bacteria lipopolysaccharide(LPS)in rat liver,and then investigate the hepatotoxicity mechanisms of PMT on immune inflammatory signal pathway Toll-Like receptor 4(TLR4)-interferon regulated factor3(IRF-3). Methods SD rats were randomly assigned into normal control,LPS(4 mg/kg),acetaminophen APAP(625 mg/kg),PMT 6 g/kg(PMT-L),PMT 12 g/kg (PMT-H),LPS+APAP and LPS+PMT-L/-H groups. The 4 groups later were injected LPS 4 mg/kg by caudal vein,after 2 h,the corre?sponding drugs were administered once a day for 7 consecutive days,respectively. The changes of weight of rats were observed every day. The tissue morphology of liver tissue of rats on 2 h,14 h,5 d,8 d after administration were detected by hematoxylin-eosin stain? ing respectively. Real time quantitative PCR(RT-qPCR)method and Western blotting were used to detect the expression of TLR4, TRIF and IRF3 in the TLR4 signaling pathway in liver cells. Results Two hours after the rat tail vein injection of LPS,the liver tiny granulomas of rats could be observed in LPS-induced groups,and then,the liver injury of rats in LPS group was gradually recovered. Eight Days after LPS induction,the liver tissue structure of rats in LPS group was clear and complete,but in LPS+APAP group and LPS+PMT 6 or 12 g/kg groups,the focal necrosis of hepatocytes,with inflammatory cell infiltration could be observed. The results of RT-qPCR and Western blotting showed that in oral administration of PMT groups,the expression of TLR4,TRIF and IRF-3 mRNA and protein in the liver cells had no significant change compared with the normal control group. But in 4 groups induced with LPS,the expression of TLR4, TRIF and IRF-3 mRNA and protein in the liver cells were significantly higher than that of the normal control group and LPS group(P<0.05). Conclusion PMT can cause liver damage induced by LPS,the hepatotoxicity is related to the positive regulation of TLR4/IRF-3 signaling pathways,which is not related to the dosage of PMT. The results show that activating TLR4/IRF3 signaling pathway is one of the mechanisms of liver injury of PMT in rats induced by LPS.

ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of Zishen jianpi huoxue decoction combined with Xiaoxintong capsule in the treatment of type 2 diabetes complicating with silent myocardial ischemia (SMI). METHODS:126 patients with type 2 diabetes complicating with SMI were selected and randomly divided into control group(62 case)and observation group(64 cases). Both group were given diet guidance and hypoglycemic drugs as sulfonylureas,biguanides. Control group was given Xiaox-intong capsule 10 mg,tid;observation group was additionally given Zishen jianpi huoxue decoction 400-500 ml,bid. Both groups received treatment for 7 d. Therapeutic efficacy of thoracic obstruction,improvement of coronary artery branch stenosis and times and duration of electrocardiogram ST segment low voltage outlet were observed in 2 groups. ADR was compared between 2 groups. RESULTS:Therapeutic efficacy of thoracic obstruction and improvement of coronary artery branch stenosis in observation group were significantly better than in control group,with statistical significance(P<0.05);the times and duration of electrocardiogram ST segment low voltage outlet in observation group were significantly lesser shorter than in control group,with statistical signifi-cance (P<0.05). No obvious ADR was found in 2 groups. CONCLUSIONS:Zishen jianpi huoxue decoction complicating with Xiaoxintong capsule shows significantly therapeutic efficacy in the treatment of type 2 diabetes complicating with SMI with good safety.

ABSTRACT

OBJECTIVE:To observe the efficacy and safety of metformin combined with rosiglitazone in the treatment of incipi-ent type 2 diabetes. METHODS:160 patients with incipient type 2 diabetes were randomly divided into observation group and con-trol group. Based on the diet control and exercise therapy,control group was orally given 0.5 g Metformin hydrochloride tablet,3 times a day;observation group was additionally given 4 mg Rosiglitazone maleate tablet,once a day. They were treated for 24 weeks. Clinical efficacy,body mass index (BMI),fasting plasma glucose (FPG),total cholesterol (TC),triglyceride (TG),2 h postprandial glucose(2 h PG),glycated hemoglobin(HbA1c),fasting plasma insulin(FINS)level,insulin resistance index(HOMA-IR)and insulin sensitivity index(HOMA-IAI)before and after treatment,and incidence of adverse reactions in 2 groups were ob-served. RESULTS:The total effective rate in observation group was significantly higher than control group,the difference was statis-tically significant(P0.05). Af-ter treatment,BMI、FPG、TC、TG、2 h PG、HbA1c、FINS、HOMA-IR in 2 groups were significantly lower than before,FPG,2 h PG, FINS,BMI and HOMA-IR in observation group was lower than control group,HOMA-IAI was significantly higher than before, and observation group was higher than control group,the differences were statistically significant(P0.05). CONCLUSIONS:The efficacy and improvement of glucose metabolism,insulin resistance and insulin sensitivity of rosiglitazone combined with metformin are superior to metformin alone in the treatment of incipient type 2 diabetes,with similar safety.

ABSTRACT

Objective To explore the effect of liver-soothing therapy on the expression of estrogen receptors mRNA in perimenopausal syndrome (PMS) rats with liver qi stagnation. Methods A total of 30 nature aging rats are assigned into control groups (n=8), model groups (n=8),Chaihu-Shugan San (CHSGS group,n=8) andDanzhi-Xiaoyao San (DZXYS group, n=8), according to the random number table. The PMS liver-Qi stagnation syndrome rat models were established by the methods of isolation raised and chronic bondage in all the groups except the control group. CHSGS group were administered 4.0 g/kg water decoctions ofChaihu ShuganSan, and DZXYS group 4.9 g/kg water decoctions ofDanzhi XiaoyaoSan respectively for 3 weeks after the rat models established. The model group and control group were administered with equal volume of normal saline. The open field test was used for the behavior test. The serum E2, FSH, LH level were measured by radioimmunoassay. The ERα, ERβ in ovary were measured by quantitative real-time PCR. Results Compared with model group on the 21st days, the CHSGS and DZXYS groups showed a significantly increase in crossings (49.6 ± 6.0, 51.6 ± 5.8vs. 40.0 ± 4.6,P0.05) increaed in the DZXYS group, but there was no remarkable difference. Conclusion The Liver-soothing therapy can improve the behavior of PMS rats with liver-Qi stagnation, and the mechanism may be related to the regulation of endocrine and ovarian estrogen receptors.

ABSTRACT

Objective To explore the effect of follow-up continuous nursing intervention on the psychological quality and medication compliance in patients with ulcerative colitis after discharge. Methods Ninety-seven patients with ulcerative colitis after discharge from October 2014 to October 2015 were recruited,and 48 cases were selected in the study group and 49 cases in the control group randomly. The control group received routine postdischarge care. The study group received postdischarge follow-up nursing intervention. After 3 months of postdischarge intervention, the psychological quality of patients in two groups was assessed by the emotion application measure scale separately. The Morisky Medication Adherence Scale 8-item version (MMAS 8-item version) was used for the assessment of the medication compliance of the two groups of patients. Results After the intervention, the patients in the study group showed better psychological quality and medication compliance than the patients in the control groups.There were significant differences between two groups. Conclusion Post-discharge follow-up nursing intervention on patients with ulcerative colitis is effective for improving the psychological quality and the medication compliance and may be helpful to improve the life quality of the patients.

Objective To study the effects ofXuling-Jiangu formula on bone mineral density and the bone biomechanic in the osteoporosis model rats.Methods According to the random number table method, 40 SD female rats were randomly divided into the caltrate D group, theXuling-Jianguformula group, the model group and the sham group,10 rats each group. In addition to the sham group, the other groups were osteoporosis model. After 30 days, the Caltrate D group received intragastric caltrate D mixed suspension 0.126 g/kg; the Xuling-Jiangu formula group receivedXuling-Jianguformula solution 15 g/kg, and the sham group and the model group received normal saline 10 ml/kg. After 12 weeks treatment, detection of left tibia bone mineral density andthree-point bending method was used for biomechanical testing.Results The mineral density of the Xuling-Jiangu formula group (0.244 ± 0.022 g/cm2,0.195 ± 0.017 g/cm2vs. 0.223 ± 0.013 g/cm2) were significantly higher than the model group and caltrate D group (P<0.01); compared with the model group, the bone biomechanictests in theXuling-Jiangu formula group (0.072 ± 0.036 kN vs.0.041 ± 0.015 kN; 843.754 ± 428.722 N/mm2vs. 482.084 ± 176.646 N/mm2) were significantly higher (P<0.05).ConclusionXuling-Jiangu formula canimprove the bone mineral density and the bone lbiomechanic of osteoporosis rats.

ABSTRACT

BACKGROUND:Diabetic lower limb ischemia is prone to involve distal lower limb arteries, and a conventional treatment is often unable to obtain the ideal effect. OBJECTIVE:To investigate the effect and safety of umbilical cord blood stem cel transplantation in the treatment of diabetic lower limb ischemia. METHODS: A diabetic rat model of lower limb ischemia was established, and along the femoral artery, five points were selected for injection of human umbilical cord mesenchymal stem cel suspension, 20 μL per point. At 1, 2, 4 weeks after transplantation, transcutaneous oxygen pressure, vascular density and vascular endothelial growth factor level in the ischemic region, and incidence of adverse reactions were recorded. RESULTS AND CONCLUSION:At 1, 2 and 4 weeks after transplantation, the transcutaneous oxygen pressure, vascular density and vascular endothelial growth factor level in the ischemic region were found increasing, which were significantly different from those before transplantation (P < 0.05). At different time after transplantation, al animals had no inflammatory reactions such as skin bleeding and dermatitis, and local red, sweling, hot, pain, and had no tumor-like growth in organs. These findings indicate that umbilical cord blood stem cel transplantation can safely and significantly improve symptoms of diabetic lower limb ischemia, which has certain application feasibility. ABSTRACT

Recently the studies on mosquito genomics, transcriptomics and small RNAomics developed rapidly with the novel biotechnologies of the next generation sequencing techniques. The genome sequences of several important vector mosquitoes including Anopheles gambiae, Culex quinquefasciatus, and Aedes aegypti have been published. The genome sizes vary among the different species of mosquitoes and are consistent with the number of the repeat regions. The released genome sequences facilitate gene cloning and identification as for OBP, OR and dsx genes. Transcriptomics provides a useful tool for functional analyses of the mosquito genes, and using this technique, the molecular basis of mosquito blooding, gland proteins and diapauses have been explored. Studies on small RNAomics suggest important roles of miRNAs and piRNAs in ovary development, blood digestion, and immunity against virus infection. The studies on mosquito omics have generated a big data platform for investigation of vector biology and vector-transmitted disease prevention.

ABSTRACT

BACKGROUND:Studies have shown the bone mineral density of postmenopausal women is closely related to parathyroid hormone. But there are differences in different areas. OBJECTIVE:To investigate the association between BstBⅠ polymorphism of parathyroid hormone gene with bone mineral density in postmenopausal women from Fuzhou area. METHODS:The bone mineral densities of the lumbar spine, femoral neck, trochanter and Ward’s triangle were measured in 150 postmenopausal women by dual energy X-ray absorptiometry. The genotype of parathyroid hormone gene was determined by polymerase chain reaction-restriction fragment length polymorphism. RESULTS AND CONCLUSION:(1) The distribution of parathyroid hormone genotypes were BB genotype 68.8%, Bb 24.1%, and bb 7.1%. The B al elic gene frequencies reached 81%, while b was 19%. The distribution fol owed the Hardy-Weinberg equilibrium. (2) Analysis of the relationship between the genotypes and bone mineral density:There was no significant difference in the bone mineral densities of the lumbar spine, femur, neck, trochanter and Ward’s triangle among the three genotypes (P>0.05). BstBⅠ gene polymorphism of parathyroid hormone gene is not correlated to bone mineral density, and there is no enough evidence to support genotype of parathyroid hormone gene as a genetic marker in predicting the risk of developing osteoperosis in Fuzhou postmenopausal women.

ABSTRACT

Objective To investigate the correlation between bone mineral density(BMD)and the PXh haplotype combining estrogen receptor (ER) gene Xba Ⅰ , Pvu Ⅱ polymorphisms and osteocalcin gene Hind Ⅲ polymorphism in postmenopausal women.Methods In 307 subjects,the BMD of lumbar vertebrae and proximal femur were measured by dual energy X-ray absorptiometry and the Xba Ⅰ and Pvu Ⅱ polymorphisms of ER gene and the Hind Ⅲ potymorphism of osteocalcin gene were detected by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP).Results (1)The BMD of greater trochanter was significantly lower in XX genotype group than in xx genotype group ( P<0.05).The BMD of femoral neck, greater trochanter and Ward's triangle were lower in Xx genotype group[(0.695±0.087)g/cm2 , (0.592±0.106)g/cm2, (0.500±0.115) g/cm2] and X allele group[(0.697±0.088)g/cm2 , (0.594±0.105)g/cm2, (0.505±0.123)g/cm2] than in xx genotype group[(0.737±0.108) g/cm2,(0.653±0.119)g/cm2 ,(0.554±0.130)g/cM2] and non-X allele group[(0.737 ± 0.108) g/CM2, (0.653 ± 0.119) g/cm2 , (0.554 ± 0.130) g/cm2] ,respectively (all P<0.05 ).(2)The BMD of Ward's triangle was lower in PP genotype group and P allele group than in pp genotype group and non-P allele group (P<0.05).(3)The BMD of femoral neck, greater trochanter and Ward's triangle were lower in hh genotype group and h allele group than in I-IH genotype group, and were lower in non-h allele group than in HH genotype group(all P<0.05).(4)Women carrying PX, PXh haplotypes combining ER gene and osteocalcin gene had lower BMD at femoral neck than those not carrying PX,not carrying PXh haplotypes, respectively (all P<0.05).ConclusionsER gene(Xba Ⅰ) polymorphism and osteocalein gene(Hind Ⅲ) polymorphism are associated with BMD in postmenopausal women.The presence of X allele or h allele shows negative influence on the BMD of postmenopausal women.The PXh haplotype is a suitable genetic marker of postmenopausal women osteoporosis in Fuzhou area.

ABSTRACT

OBJECTIVE: To study the association of the vitamin D receptor gene BSM Ⅰ, TAQ Ⅰ and APA Ⅰ genetic polymorphisms with bone mineral density and biochemical markers of bone turnover in postmenopausal women.METHODS: ①total of 576 postmenopausal Han ethnic women of 48-84 (62.17±6.37) years old in Fuzhou city were investigated, on the basis of their informed consent, through random sampling method from January 2007 to December 2008. ②The subjects were recorded regarding to their age, menopause duration, body mineral index and postmenopausal fracture incidence. ③Dual energy X-ray absorptiometry was used for measuring the bone mineral density of vertebrae L<,2-4>, left femoral neck, trochanter and Ward's triangle. ④The genetic polymorphisms of vitamin D receptor gene BSM Ⅰ, TAQ Ⅰ and APA Ⅰ were detected using polymerase chain reaction-rastriction and fragment length polymorphism (PCR-RFLP) technique. ⑤The biochemical markers of bone turnover (serum bone gla protein, serum bone alkaline phosphatase, urinary pyddinoline and urinary deoxypyridinoline) were detected with enzyme linked immunosorbent assay.RESULTS: A total of 561 subjects up to standard were involved in the result analysis. ①There was no significant difference in bone mineral density among genotypes of vitamin D receptor gene BSM Ⅰ, TAQ Ⅰ and APA Ⅰ polymorphisms (P > 0.05). ②There was no significant difference in the biochemical markers of bone tumover among genotypes of BSM Ⅰ, TAQ Ⅰ and APA Ⅰ polymorphisms (P > 0.05). ③There was no significant difference in the incidence of osteoporosis among genotypes of BSM Ⅰ, TAQ Ⅰ and APA Ⅰ polymorphisms (P > 0.05). ④There was no significant difference in the incidence of postmenopausal fracture among genotypes of BSM Ⅰ, TAQ Ⅰ and APA Ⅰ polymorphisms (P > 0,05).CONCLUSION: BSM Ⅰ, TAQ Ⅰ and APA Ⅰ polymorphisms of the vitamin D receptor gene are not obviously associated with osteoporesis in postmenopausal women, and accordingly can not be taken as genetic markers of osteoporosis in postmenopausal women in Fuzhou.

ABSTRACT

BACKGROUND: There is nearly no muscle tissue with satisfactory function and appearance applying in clinical repair and construction of injured muscles to date. OBJECTIVE: To investigate the feasibility of applying autologous fascia as a scaffold to construct muscle in vivo. DESIGN, TIME AND SETTING: The randomized, self-matched control experiment was carried out between January 2004and June 2006 at Department of Burns & Plastic Surgery, Second Hospital of Shenzhen, Shenzhen, Guangdong Province, China MATERIALS: Twenty-eight healthy New Zealand rabbits, weighing (1.7±0.5) kg, without sex restriction, establishing middle part defect model of anterior tibial muscle of rabbit hind legs. METHODS: One hind leg of each rabbit was randomized to the experimental group (n=28), the other hind leg was assigned to one of 3 control groups, scaffold-connected group (n=10), muscle particle implant group (n=10) and blank control group(n=8). In experimental group, the defect was connected with an autologous fascial scaffold and filled with the mutilated muscle particles, and subcutaneous tissue and skin were sutured in situ. In scaffold-connected group, the treatments were same to the experimental group only except muscle particle implantation. In muscle particle implant group, the defect was filled with muscle particles but without fascial scaffold and other treatments were same to the experimental group. The defect in blank control group received no treatment. MAIN OUTCOME MEASURES: The success rate of muscle transplantation, histological and ultra structural observation,and immunohistochemical identification of desmin were observed at 2, 3, 4, 6 and 9 weeks after operation. The middle parts of samples were also harvested for relative quantitative analysis of α-actin cDNA using reverse transcription-polymerase chain reaction in the experimental group and scaffold-connected group.RESULTS: In experimental group, 1 muscle broke near the proximal junction, the other 27 succeeded and the appearance of healed defects became near normal gradually. In scaffold-connected group, 4 muscles broke, 6 muscles still depressed in defect area; in muscle particle implant group and blank control group, the defects had no change. In experimental group, a large quantity of skeletal muscle satellite cells proliferated, which reached peak at 2-3 weeks, cells attached to the ends of fibrous connective tissue; in scaffold-connected group only fibrous connective tissue was seen. lmmunohistochemistry showed that 85% cells in experimental group were desmin-positive, while the positive rate in scaffold-connected group was < 25%. The relative quantitative analysis of α -actin cDNA showed that there were significant differences between the experimental group and scaffold-connected group at different time points(P < 0.05).CONCLUSION: The success rate of repairing muscle defect with autologous fascial scaffold reached 93.33%, which indicates that it is feasible to promote muscle regeneration with autologous fascial scaffold.
