ABSTRACT

Objective To identify high?risk factors of white matter damage ( WMD ) in premature infants,in order to reduce the incidence and improve the prognosis of brain injury?Methods Five hundred and seventy?seven cases of preterm infants treated from January 2012 to December 2013 in the Pediatrics Department of Affiliated Hospital of Qingdao University were slecteted?According to MRI of the brain at 2 weeks after birth, all the patients were divided into WMD group(133 cases) and the control group(444 cases)?Single?factor and multifactor logistic regression were used for the analyses of the high?risk factors of WMD?Results WMD incidence was 23?05%( 133/577) in 577 cases of premature infants,with 108 cases( 81?2%) of focal injury,5 cases(3?8%) of diffuse injury and 20 cases(15?0%) of periventricular leukomalacia(PVL) injury?There was significant statistically difference in different gestational age( P=0?017)?Multi factor Logistic regression analysis showed that metabolic acidosis(OR=4?489,P=0?000),asphyxia(OR=3?519,P=0?000),perinatal infection ( OR=2?650,P=0?003) were independent risk factors for WMD,while cesarean( OR=0?293,P=0?000) was protective factor for WMD?Conclusion WMD is a complex interaction outcome of multiple risk factors in premature infant?Perinatal hypoxia and infection are the main risk factors of WMD in premature infants.

ABSTRACT

Objective:To explore therapeutic effect of bisoprolol with CYP2D6 and CYP3A4 gene targeting therapy for hypertension .Methods :A total of 100 cases with essential hypertension (EH) were selected ,among them 20 ca-ses were randomly chosen to receive routine medication (bisoprolol 5 mg/d ,routine treatment group ) ,the other 80 patients were equally divided into group A (CYP2D6*10) and group B (CYP3A4*1G) .According to detection results of CYP2D6*10 and CYP3A4*1G gene polymorphism ,they were dived into group ACC (n=13) ,group ACT (n=14) ,group ATT (n=10) ,group BCC (n=21) ,group BCT (n=17) and group BTT (n=0) .Bisoprolol dosage was adjusted according to genotypes :group ACC and group BCC received 10 mg/d ,group ACT and group BCT received 5 mg/d ,group ATT and group BTT received 2.5 mg/d .Blood drug concentration ,blood pressure and heart rate were compared among above groups after two weeks .Relationship between plasma concentration of bisoprolol and gene polymorphism of drug metabolism enzyme was evaluated .Results:Comparison between related groups with same bi-soprolol dosage :there were no significant difference in blood drug concentration ,heart rate and blood pressure be-tween ACT and routine treatment group , P>0.05 all;compared with routine treatment group ,there was significant rise in blood drug concentration [ (33.5 ± 19.1) ng/ml vs .(50.13 ± 23.21) ng/ml] ,significant reduction in heart rate [ (71.4 ± 5.16) times/min vs .(66.5 ± 6.04) times/min] and blood pressure [ (127.22 ± 10.44/82.4 ± 7.27) mmHg vs .(119.48 ± 11.97/71.2 ± 10.30) mmHg] in BCT group , P<0.05 all .Conclusion:Because gene possesses polymorphism ,therapeutic effect of bisoprolol treating hypertension is differing ;the gene targeting therapy may significantly improve therapeutic effect for hypertension .

ABSTRACT

Objective: To study the relationship between CYP2D6*10 gene polymorphism and metoprolol therapeutic effect for hypertension. Methods: A total of 60 patients with essential hypertension (EH) received metoprolol 47.5mg/d for 3d. After 3d the plasma metoprolol concentration after oral 2h was measured. Polymorphism of CYP2D6*10 gene was detected by PCR-RFLP. According to results of gene detection, the patients were divided into CC genotype group (wild type homozygote, fast metabolism type, n=14), CT genotype group (heterozygous mutation, intermediate metabolism type, n=25) and TT genotype group (homozygous mutation, slow metabolism, n=19). Metoprolol dosage was adjusted according to CYP2D6*10 genotype. After one week, plasma concentration of metoprolol after oral 2h was measured again, and mean heart rate and blood pressure were measured before and after gene-directed therapy. Results: Before gene-directed therapy, compared with CC and CT group there was significant increase in plasma concentration of metoprolol [(26.57±19.40) ng/ml vs. (23.88±12.86) ng/ml vs. (64.74±32.94) ng/ml, P0.05 all; Compared with before gene-directed therapy, there was significant increase in plasma concentration of metoprolol, and significant decrease in mSBP, mDBP and mHR in CC group (P0.05). Conclusion: CYP2D6*10 gene polymorphism affects metoprolol metabolism and its therapeutic effect on hypertension, gene-directed therapy can significantly improve drug therapeutic effect and reach ideal therapeutic goal in short time.

ABSTRACT

A simple, fast GC method based on headspace single drop microextraction (HS-SDME) was used for the determination of menthol and methyl salicylate in Baicao oils. A special sample pretreatment method was performed by adding 10 microL the oil its methanol solution into 5 mL water in a caped 10 mL sample vial, a 1.5 microL microdrop of N,N-dimethylformamide with benzyl alcohol as internal standard was formed on the pinpoint of microsyringe needle, exposing 5 min at extraction temperature of 40 degrees C. After extraction, 0.5 microL of extract was directely injected into GC for analysis. The determination was carried out on a capillary column (0.53 mm x 30 m) with PEG as the stationary phase with FID as the detector. A temperature programm was employed. The excellent separation and detection of the target components was accomplished. The average recoveries varied between 95.4% and 99.2%, and relative standard deviations were less than 1.9%. The calibration curves were linear in the range of 1.26-80.5 mg x L(-1) for menthol (r = 0.999 0) and 2.49-1.59 x 10(2) mg x L(-1) for methyl salicylate (r = 0.999 1), respectively. The proposed method is proved to be simple, fast, accurate and low cost without complicated sample pretreatment HS-SDME is expected to be widely applied for the analysis of volatile components in traditional Chinese medicines.

ABSTRACT

OBJECTIVE:To analysis the characteristics and regularity of the adverse drug reaction(ADR)occurred in our hospital. METHODS:307 ADR cases collected from 2004 to 2008 in our hospital were analyzed statistically in respect of patient’s age and sex,route of administration,categories of drugs,organs and systems involved in ADR and clinical manifestations,etc. RESULTS:Of total 307 ADR cases,more cases were woman;45.93% of cases were aged ≤18 a and 60 a. 80.13% of ADR cases were induced via intravenous administration. The incidence of ADR cases were mainly induced by antiinfective drugs,accounted for 48.53%. The most common manifestation of the ADR was lesion of skin and its appendants(27.97%),followed by injury of digestive system(16.10%). CONCLUSION:Great importance should be attached to rational use of anti-infective drugs and its management to reduce or avoid the occurrence of ADR and improve the quality of ADR reports.

OBJECTIVE:To improve the outpatient pharmacy counseling service.METHODS:The experience of carrying out outpatient pharmacy counseling service in our hospital over the past two years was summarized.RESULTS & CONCLUSION:Due to the practice of outpatient pharmacy counseling service,the rational drug use level in the outpatient pharmacy has been improved,and a satisfactory social benefit was achieved.It is necessary to further evolve the current outpatient pharmacy counseling service system.

ABSTRACT

Objective To observed the and - inflammatory effect of Qianlie Tai Granule (QTG) on chronic prostatitis. Methods Three models, asepsis and chronic aseptic prostatitis rat model, acute inflammation mouse model and chronic inflammation rat model, were adopted to observe the pharmacocynamic action of QTG. Results QTG decreased the number of WBC, inhibited the increasing of prostate tissue weight and improve the inflammatory lesion in prostate tissue of chronic aseptic inflammation rats. QTG also had an obvious inhibitory effect on early exudative auricular swelling induced by 2% compound croton oil and late proliferative granuloma induced by 2% agar. Conclusion QTG has an obvious effect for prostatitis. It can inhibit the early and late inflammation .

ABSTRACT

OBJECTIVE: To optimize the formula and preparation technology of gel-matrix sustained release tablet of nicotinic acid(GSTNA).METHODS: The formula of GSTNA was optimized by orthogonal experiment with the amount of hydrophilic gel-matrix material HPMC(K15M,E15-LV) and that of adjuvant calcium hydrogen phosphate(CHP) as factors and with the in vitro release rates as index.Meanwhile,the verification test on the intra-and inter-batch release rates of the samples was performed.RESULTS: The optimum formula could be seen as follows: the ratios of HPMC(K15M,E15-LV) and CHP were 4%,40% and 25% respectively.The GSTNA prepared in this formula achieved a sustained drug release of up to 12 h,and both the intra-batch homogenicity and the inter-batch reproducibility were satisfactory.CONCLUSION: The GSTNA is reasonable in formula and simple in preparation technology.
