ABSTRACT

Objective:To observe the effect of sagittal traction on the internal fixation with proximal femoral nail anti-rotation (PFNA) for femoral intertrochanteric fractures in the elderly patients. Methods:A retrospective study was conducted to analyze the clinical data of 61 elderly patients with intertrochanteric fracture who had been treated from January 2016 to January 2018 at Department of Orthopaedics, The Ninth Hospital of Ningbo. Of them, horizontal traction of the affected limb was performed in 31 (16 males and 15 females with an age of 73.6 years ± 9.4 years) while 10°-20° sagittal traction of the affected limb in 30 (15 males and 15 females with an age of 75.0 years ± 7.2 years). The 2 groups were compared in terms of operation time, intraoperative blood loss, times of intraoperative fluoroscopy, fracture healing time, hip function at 6 months after operation by Harris scoring and complications. Results:There was no statistically significant difference between the 2 groups in the general clinical data, showing comparability ( P>0.05). The operation time (47.1 min ± 4.7 min), intraoperative blood loss (72.3 mL ± 9.1 mL), and times of intraoperative fluoroscopy (11.5 ± 1.4) in the sagittal traction group were significantly less than those in the horizontal traction group (56.7 min ± 4.6 min, 103.9 mL ± 19.2 mL, and 14.6±1.6) ( P< 0.05). This cohort was followed up for 6 to 18 months (12.6 months on average). There were no significant differences between the 2 groups in quality of postoperative fracture reduction, fracture healing time or hip Harris scores at postoperative 6 months ( P>0.05). Internal fixation loosening and hip pain occurred in one patient in the sagittal traction group. Follow-ups observed no complications like wound infection, bedsores, deep vein thrombosis or hip varus deformity in either group. Conclusion:As 10°-20° sagittal traction can lift the affected limb upward to avid blockage by the iliac wing, hip soft tissue and lateral abdominal wall, it lends convenience to surgeons, reducing intraoperative bleeding volume, times of fluoroscopy and operation time.

ABSTRACT

BACKGROUND:Deep vein thrombosis after hip replacement has a high incidence rate. Moreover, deep vein thrombosis can induce pulmonary embolism that can endanger patients’ life and dysfunction of distant deep vein. The appearance of deep vein thrombosis is a great obstacle for the gradual y increased hip replacement. OBJECTIVE:To observe the occurrence of deep vein thrombosis of lower limb after total hip replacement in elderly patients, and to screen the risk factors for the occurrence of deep vein thrombosis of lower limbs. METHODS:Clinical data of 128 elderly patients with hip replacement were analyzed retrospectively. Al patients were examined with color Doppler ultrasound in double lower limbs at 1 day before replacement and 7 days after replacement. Multifactor unconditional logistic analysis was conducted on clinical related factors and the formation of lower limb deep vein thrombosis. RESULTS AND CONCLUSION:At 7 days after the operation, 16 patients affected deep vein thrombosis of lower limb. The factors for deep vein thrombosis contained female, general anesthesia, bilateral hip replacement and the application of bone cement (P<0.05). The risk for deep vein thrombosis after total hip replacement significantly increased in elderly patients aged over 70 years. Multifactor unconditional logistic analysis exhibited that the multiple risks of sex, obesity and the use of bone cement in elderly patients with deep vein thrombosis increased to 11.398, 3.109 and 8.925. The patients with a blood type O at the age of over 70 years experienced a decreased risk for deep vein thrombosis after total hip replacement. The occurrence of deep vein thrombosis decreased to 0.186 times after replacement. Blood type O could be considered as a protective factor for the occurrence of deep vein thrombosis.

ABSTRACT

BACKGROUND:The materials for internal fixation of bone allografts have been used in the clinic. However, they are only used in the regions bearing little stress because of low strength. The focus of current studies is how to enhance the fixation strength of the materials to expand application range.OBJECTIVE:To compare the strength difference of cortical bone allografts among different fixation modes and to investigate the possible mechanisms of action.DESIGN,TIME AND SETTING:An in vitro biomechanical experiment was performed at the Laboratory of Biomechanics,Nanhua University between October 2005 and March 2006.MATERIALS:Twenty-seven pieces of cadaveric femurs were included in this study.A total of 45 cortical bone allografts with a size of 110 mm×10 mm×3 mm and 90 bone screws were also used.METHODS:Twenty-seven femurs were made into simulated fracture models and randomized to 3 groups, with 9 models per group:A,B,and C.In the group A,the models were fixed with two cortical bone all6grafts; the models from the groups B and C were fixed with two bone allografts plus 5 bone screws and one bone allograft plus 5 bone screws accordingly.MAIN OUTCOME MEASURES:Biomechanical tests were performed in the above-mentioned 3 groups to measure compressive, bending and torsional stiffness as well as the maximum loads.RESULTS:Different fixation models displayed different mechanical characteristics. The group A exhibited similar compressive stiffness compared to the group B,but the level was significantly higher compared to the group C (P < 0.05).The other two stiffness parameters including bending and torsion were significantly greater in the group A than in the groups B and C (P < 0.05).The maximum loads of compression, bending, and torsion were significantly greater in the group A compared to the group B (P < 0.05) and group C (P < 0.01).CONCLUSION:The strength of bone allograft is highly related to fixation mode. The two bone allografts showed greater strength and stiffness than struts fixed with bone screws, which meet the clinical requirements.

ABSTRACT

OBJECTIVE To study the effect of frequency difference and intensity difference on mismatch negativity (MMN) change in normal persons. Emphatically to observe the effect on the latency and amplitude of normal people's MMN, as well as the threshold of the acoustic difference which may induce the MMN. METHODS Twenty-four adults were included in this study. Their thresholds of pure tone audiometry were within 20dBHL. The MMN were measured by using the Smart EP ( ear potentiometer provided by the company of Intelligent Hearing) and the rule of the change by the difference of frequency and intensity to the latency and amplitude of normal people’s MMN were studied. RESULTS Along with the decreasing of the difference of the frequency, the MMN latency prolonged gradually. The amplitude of the MMN decreased gradually as the decreasing of the intensity difference. CONCLUSION A stable MMN graph is an important indicator of objective physiological test with hearing distinguish.

ABSTRACT

Objective To observe the space-time patterns of damaged outer hair cells(OHCs) in rat cochlea at the early stage after exposure to impulse noise. Methods Wistar rats were exposed to 100 emissions of impulse noise (3 seconds interval between each emission) at 154 dB SPL. Four times (10 min, 30 min, 3h and 6h) after the noise exposure, the animals were sacrificed and the organs of Corti were processed for detection of OHC death modes. The apoptotic and necrotic OHCs were distinguished by propidium iodide (PI), a fluorescent probe specifically labeling the nuclear DNA. The specimens were examined under a fluorescence microscope for assessment of OHC damage. Results Nuclear chromatin began to shrink as the chromatin condensed around the nuclear periphery. The peripheral chromatin ring condensed into discrete mass. Chromatin masses appeared to bleb off from the nuclear surface, forming apoptotic bodies at 10 min after the noise exposure. There were a few swollen nuclei appeared 30 min after the noise exposure. Loss of OHC nuclei could be seen 3 h after the noise exposure. The cochlear lesion expanded to contain a large number of missing OHCs and seriously shrunken nuclei at 6 h after the noise exposure. Conclusions The results of the study indicate that death of OHCs takes place extremely rapid after the impulse noise exposure. The apoptosis of OHCs precedes necrosis. OHC apoptosis is a quick process. Most of dead outer hair cells were eliminated 6 h after the noise exposure.
