ABSTRACT

Abstract: Introduction: EuroSCORE is a probabilistic model with good performance in the prognosis of mortality in heart surgery in many latitudes. It is recommendable to validate it in hospitals where it is employed. Objective: To validate the EuroSCORE model in adult patients at the Hospital Regional de Alta Especialidad del Bajío (HRAEB) of León, Guanajuato, Mexico. Material and methods: We conducted an observational transversal, and retrospective study, accomplishing this through the review of the clinical files of patients submitted to heart surgery with and without extracorporeal circulation pump from 01/01/2008 to 12/31/2013 at the HRAEB. This included mortality up to hospital discharge, utilizing the on-line calculator of the EuroSCORE program to estimate risk of death. In order to validate the EuroSCORE model, we assessed discrimination and calibration through the Area Under the receiver operating characteristics (ROC) curve and χ2 test with Hosmer-Lemeshow (H-L) goodness-of-fit, respectively. Results: A total of 342 patients, aged 50.02 ± 16.66 years, 181 males (52.9%) and 161 women (47.1%). The area under the ROC curve of the additive model was 0.763, and of the Hosmer-Lemeshow test was 5.30, with p = 0.62. The area under the ROC curve of the logistic model was 0.761 and of the Hosmer-Lemeshow test, 8.78, with p = 0.36. Conclusion: The EuroSCORE model is a reliable score for estimating the probabilities of death in adult patients submitted to heart surgery with or without the pump at the HRAEB.


Resumen: Introducción: EuroSCORE es un modelo probabilístico con buen desempeño en el pronóstico de mortalidad en cirugía cardiaca en muchas latitudes. Es recomendable validarlo en los hospitales donde se emplea. Objetivo: Validar el modelo EuroSCORE en pacientes adultos en el Hospital Regional de Alta Especialidad del Bajío (HRAEB) de León, Guanajuato, México. Material y métodos: Se realizó un estudio observacional transversal y retrospectivo, que se completó con la revisión de los expedientes clínicos de pacientes sometidos a cirugía cardiaca con y sin bomba de circulación extracorpórea desde el 01/01/2008 hasta el 31/12/2013 en el HRAEB. Esto incluyó la mortalidad hasta el alta hospitalaria, utilizando la calculadora en línea del programa EuroSCORE para estimar el riesgo de muerte. Para validar el modelo EuroSCORE, evaluamos la discriminación y la calibración a través de la curva de características de operación del receptor (ROC) y la prueba de χ2 con la efectividad de ajuste de Hosmer-Lemeshow (H-L), respectivamente. Resultados: Un total de 342 pacientes, de 50.02 ± 16.66 años, 181 hombres (52.9%) y 161 mujeres (47.1%). El área bajo la curva ROC del modelo aditivo fue de 0.763, y la de la prueba Hosmer-Lemeshow fue de 5.30, con p = 0.62. El área bajo la curva ROC del modelo logístico fue 0.761 y de la prueba Hosmer-Lemeshow, 8.78, con p = 0.36. Conclusión: El modelo EuroSCORE es una herramienta confiable para estimar las probabilidades de muerte en pacientes adultos sometidos a cirugía cardiaca con o sin bomba en el HRAEB.

ABSTRACT

Context: Mouthrinses are used, by many of our patients, as a complement to daily dental hygiene routine. The use of a toothbrush and an interproximal cleaning method may not be enough to control dental plaque. Essential oils and delmopinol mouth rinses are effective for the prevention of dental caries and gingivitis. To study the effect of an essential oil and a delmopinol mouth rinse on dental plaque bacteria, an in vitro study was developed. Aims: The objective of this study was to determine the antibacterial activity of an essential oil and a delmopinol mouth rinse on Streptococcus mutans, Lactobacilli, and aerobic and anaerobic dental plaque nonspecific bacteria. Design: Samples of human dental plaque were collected from consenting participants and bacteria isolated. Disk‑diffusion tests were performed to obtain the minimum concentration of the mouth rinses necessary to inhibit bacterial growth. The ability of the commercial mouth rinses to inhibit bacterial growth was studied in comparison to a positive control (0.2% chlorhexidine) and a negative laboratorial control (sterilized water). Results: The minimum inhibitory concentration was found to be inferior to the commercial essential oils and delmopinol mouth rinses concentrations. Delmopinol and essential oils have significant antibacterial properties shown in vitro only for aerobic bacteria, and for S. mutans, Lactobacillus, and anaerobic bacteria, the results were not statistically significant. Conclusions: Essential oils and chlorhexidine are statistically similar and better than delmopinol for aerobic bacteria growth inhibition. For the other bacteria, essential oils and delmopinol are not statistically promising. Results show that essential oils only may help patients to maintain good oral health as a complement to daily brushing and interproximal cleaning.

ABSTRACT

We have identified a novel DNA sequence of 500 bp (β500-DNA) on the Leishmania (Viannia) subgenus, located in the intergenic region of one of the loci of the β-tubulin gene family. The sequence analysis showed that this sequence has no homology to any other sequence described so far, including the β-tubulin gene. We improved a specific β500-PCR assay, which generated a PCR product of 375 bp for total genomic DNA from Leishmania strains belonging to the L. (Viannia) subgenus. In contrast, no amplification was found when using genomic DNA from species of L. (Leishmania) subgenus or other organisms. Under our PCR conditions, the lower detection limit was 1 fg when a purified DNA clone (pLgβ4), which contains one copy of the β500-DNA sequence, was used. The β500-DNA PCR assay confirmed the preliminary diagnosis of cutaneous leishmaniasis in clinical samples in which the Montenegro skin test was positive and parasite cultures were negative. The analytical specificity and the sensitivity of the PCR assay provide a tool for epidemiological studies of the disease.


En este trabajo identificamos una nueva secuencia de DNA de 500 pb (β500-DNA) en Leishmania del subgénero Leishmania (Viannia), localizada en la región intergénica de uno de los loci de la familia de los genes de la β tubulina. El análisis de secuencia mostró que β500 no tiene homología con ninguna otra secuencia previamente descrita, incluido el gen de la β tubulina. Nosotros implementamos un ensayo de PCR específico para β500, β500-PCR, que genera un producto de PCR de 375 pb a partir del DNA genómico de cepas de Leishmania pertenecientes al subgénero L. (Viannia). No hubo amplificación alguna cuando se utilizó el DNA genómico de especies del subgénero L. (Leishmania) o el de otros organismos. En las condiciones establecidas, utilizando DNA purificado del clon pLgβ4, que contiene una copia de la secuencia de DNA de β500, el límite de detección más bajo fue de 1 fentogramo. El ensayo β500-PCR confirmó el diagnóstico preliminar de leishmaniasis cutánea en muestras clínicas de pacientes positivas a la prueba de Montenegro y negativas en el cultivo de parásitos. La especificidad y sensibilidad analítica del ensayo de PCR proporciona una herramienta para estudios epidemiológicos de la enfermedad.

ABSTRACT

A ulceração gástrica pode causar uma série de manifestações clínicas em eqüinos adultos, mas ainda faltam conhecimentossobre essa enfermidade no Brasil. Para avaliar sua importância clínica e alguns fatores associados à etiopatogenia, foramavaliados, através de gastroscopia, 20 eqüinos adultos com suspeita clínica da presença de úlceras gástricas. Concluiu-seque: o número total de animais atendidos com ulceração gástrica foi maior no grupo dos estabulados do que no dos criadosa campo. As ulcerações devem ser consideradas como causa de manifestações clínicas (principalmente cólica recorrente),ocorrendo mais na região aglandular ao longo do margo plicatus. Quanto maior o número, a extensão e a profundidade dasúlceras, maior a probabilidade de estarem correlacionadas com manifestações clínicas.

ABSTRACT

El propósito de este trabajo fue optimizar un ensayo de reacción en cadena de la polimerasa (PCR) para efectuar el diagnóstico diferencial de Trypanosoma evansi y Trypanosoma vivax, usando cebadores previamente descritos: 21-mer/22-mer e ILO1264/ILO1265. La especificidad se evaluó efectuando pruebas preliminares sobre muestras de ADN purificado de diversas especies de parásitos y sobre mezclas de éstas. La sensibilidad se valoró empleando diluciones de ADN de aislados de referencia de T. evansi y T. vivax y concentraciones variables de cebadores. El ensayo optimizado mostró una sensibilidad de 10 pg de ADN, con especificidad para el Subgénero Trypanozoon (cebadores 21-mer/22-mer) y especificidad absoluta para T. vivax (cebadores ILO1264/ILO1265). Ambos grupos de cebadores mostraron potencialidad para detectar infecciones mixtas T. evansi/T. vivax. Un análisis de hibridación sobre el patrón de cariotipo de diversos Kinetoplastida, usando la sonda Te-ADN, mostró su carácter repetitivo y la distribución de su secuencia en diferentes cromosomas de T. evansi TE0. La PCR fue evaluada como herramienta diagnóstico de la presencia de tripanosomas en muestras sanguíneas de animales con infecciones experimentales, demostrando alta sensibilidad al detectar positividad hasta 72 horas antes que lo hicieran los métodos parasitológicos. Este ensayo también fue evaluado sobre muestras sanguíneas de búfalos de agua con infecciones naturales, mostrando alta sensibilidad y especificidad al detectar 9/86 muestras como positivas a T. vivax, revelando una tasa de infección activa de 10,47 por ciento; valor tres veces superior a los resultados de la evaluación microscópica de frotis teñidos (3,49 por ciento) y casi dos veces superior (6,98 por ciento) a la técnica parasitológica de microcentrifugación capilar. En ninguna de las muestras de búfalos evaluadas se detectó T. evansi.

Summary A survey on seedling nodulation of native leguminous forest species was made in an area near Manaus, Amazonas, Brazil, with typical humid tropical forest vegetation on yellow oxisols of varying textures. This survey was made by collecting seedling from the forest with intact root systems, registering the presence of nodules and the type of soil were the collection was made. Seedlings were transplanted to polyethylene bags with a horizon forest soil to verify if nodulation by native Rhizobium sp. occurs in nursery conditions. One year later observations were made for: presence of nodules, nitrogenase activity, as well as some nodule characteristics. Of the 34 species, 22 showed nodulation. Of these nine belonged to the subfamily Mimosoideae, six to Caesalpinioideae and four to Papilionoideae. Of these twelve were collected on sandy soils and six on clay soil and one in the two types of soil. Capacity of nodulation was observed in some species that were not reported in a recent revision as being able to nodulate before (Cedrelinga catenaeformis, Diplotropis purpurea and Tachigalia sp).


Resumo Fez-se um levantamento da nodulação em mudas de espécies florestais nativas, em área próxima de Manaus na Amazônia, caracterizada por uma vegetação do tipo tropical úmida de terra firme, em Latossolo Amarelo de várias texturas. Este levantamento foi feito com a extração de mudas com sistema radicular intacto, sendo registrada a presença de nódulos e o tipo de solo em que se fez a coleta. Estas mudas foram transplantadas para recipientes plásticos com solo de floresta, com o objetivo de verificar se ocorria nodulação por Rhizobium nativo em condições de viveiro. Após um ano, sob estas condições, foram feitas observações da presença de nódulos, atividade da nitrogenase, bem como algumas características dos nódulos. Das 34 espécies pesquisadas, 22 mostraram-se capazes de nodular. Destas, nove são da subfamília Mimosoideae, seis da Caesalpinioideae e quatro da Papilionoideae, sendo que doze foram coletadas em solo arenoso, seis em solo argiloso e uma nos dois tipos de solo. Baixas atividades da nitrogenase foram encontradas na maioria das espécies. Foi observada a ocorrência de nódulos em algumas espécies (Cedrelinga catenaeformis, Diplotropis purpurea e Tachigalia sp.) que não haviam sido mencionadas como capazes de nodular em recente revisão.
