ABSTRACT

Background@#There are several metrics for determining cultural competency in nurses, but none of them apply to Filipino nurses caring for patients with mental health issues in the Philippines setting. @*Purpose@#The goal of this study was to create a cultural competency tool for Filipino nurses (CCTFN) caring for patients with mental health issues in hospitals, communities, or schools. @*Methodology@#A sequential exploratory design was used. The qualitative phase utilized a scoping review and guided interviews, followed by the quantitative phase involving the validity test by five experts and the reliability tests participated by 140 nurses in Zamboanga City. IBM SPSS Statistics 22.0 was used to examine the data. @*Results@#The scoping review utilized PRISMA to search for eligible articles and the interviews resulted in an initial 51-item pool. The tool's content validity (S-CVI=0.992) was confirmed by a panel of experts. The inter-rater (ICC=0.773), inter-item (α =0.838) reliability, and exploratory factor analysis revealed four factors that accounted for 68.38 percent variance, resulting in 25 valid items. The four factors were “awareness on the Filipino concept of mental illness,” “attitudes towards fostering an efficient relationship between patient with mental health problems and self,” “skills in addressing cultural needs and issues of patients with mental health problems,” and “knowledge on socio-political factors affecting patients with mental health problems.” @*Conclusion/Implications for Practice@#Overall, the CCTFN was found to be valid and reliable. This tool can improve nursing care and inform training programs in the Philippines to improve mental health service provision and reduce stigma.

ABSTRACT

To perform a systematic review and meta-analysis of all randomized controlled trials (RCTs) that investigated the clinical benefits of 17-alpha hydroxyprogesterone caproate (17OHPC) in the prevention of recurrent preterm birth (PTB) among singleton pregnant women with a previous history of PTB. We searched four major databases up till April 2021 and assessed the risk of bias in the included studies. We meta-analyzed various maternal-neonatal endpoints (n=18) and pooled them as mean difference or risk ratio (RR) with 95% confidence interval (CI) using the random-effects model. Six RCTs met the inclusion criteria, comprising 2,573 patients (17OHPC=1,617, control=956). RCTs revealed an overall low risk of bias. The rates of PTB <35 weeks (n=5 RCTs; RR, 0.77; 95% CI, 0.63-0.93; P=0.008), PTB <32 weeks (n=3 RCTs; RR, 0.68; 95% CI, 0.51-0.91; P=0.009), neonates with low birth weight (<2.5 kg) at delivery (n=3 RCTs; RR, 0.63; 95% CI, 0.5-0.79; P<0.001), and neonatal death (n=4 RCTs; RR, 0.41; 95% CI, 0.20-0.84; P=0.02) were significantly reduced in the 17OHPC group compared with the control group. Moreover, 17OHPC treatment correlated with a significantly decreased rate of retinopathy (n=2 RCTs; RR, 0.42; 95% CI, 0.18-0.97; P=0.004). However, there were no significant differences in the rates of neonatal intensive care unit admission, cesarean delivery, and other pretermrelated complications between both the groups. Among singleton pregnant women with a prior history of PTB, 17OHPC may favorably decrease the risks of recurrent PTB and reduce the rate of neonatal death.

ABSTRACT

Novel functionalized 2-phenylindole derivatives, their derived imidazolethione and 1,2,4-triazinethione weresynthesized. The bicyclic compounds thiazoloimidazole and thiazolotriazine compounds in addition to their arylidinederivatives 5-8 were synthesized. Furthermore, a thioglycoside, as well as sugar hydrazone derivatives of theimidazolylindole system, were prepared. Some of the prepared compounds were screened against four cancer celllines and compounds 2, 3a and 10 showed high cytotoxic activities. The imidazolylindol-3-one derivatives 3a showedsignificant cytotoxic effect superior to the reference drug, doxorubicin, against breast adenocarcinoma cell line.

@#<p><strong>OBJECTIVE:</strong> To evaluate the effects of Dexamethasone-impregnated absorbable nasal pack versus saline-impregnated nasal packing on postoperative outcome of nasal cavities after endoscopic sinus surgery using the Perioperative Sinus Evaluation Scoring System (POSE) and Lund and Kennedy Endoscopic Scoring System. </p> <p><strong>METHODS:</strong></p> <p><strong>Design:</strong> Prospective, randomized, double blinded, placebo-controlled trial</p> <p><strong>Setting:</strong> Single Center Tertiary Government Hospital</p> <p><strong>Participants:</strong> Nineteen (19) patients aged 15 years old and above, diagnosed with chronic rhinosinusitis, with nasal polyposis grade 3, who underwent endoscopic sinus surgery from January 2015 to August 2015 </p> <p><strong>RESULTS:</strong> Nasal cavities that received postoperative dexamethasone-impregnated nasal packs showed significantly lower POSE scores than placebo on post-op Days 14 (p value 0.0022; 95% CI: -2.113 to -0.5116) as well as lower Lund-Kennedy Scores on post-op day 14 (p value of 0.0180; 95% CI: -2.493 to - 0.2571) and day 28 (p value of 0.007; 95% CI: -1.56275 to -0.2832) </p> <p><strong>CONCLUSION:</strong> Dexamethasone-impregnated absorbable nasal packing affords better postoperative outcomes: less edema, crusting, secretions, and synechiae, than saline-impregnated absorbable packing in later postoperative days.</p>

ABSTRACT

Objectives: To compare associations between different indices of socio-economic status (SES) and child malnutrition. Methods: A cross-sectional survey in poor urban areas in the Philippines collected information on child wasting, stunting, anemia, iron deficiency, and vitamin A deficiency, as well as data to calculate three measures of SES: household income; a Philippines government measure using housing conditions, income, education, and occupation; and a wealth index calculated from ownership of durable goods and other factors. Results: All forms of malnutrition were associated with all three SES measures; however, the government measure and the wealth index were more strongly associated. The odds ratios for anemia comparing the poorest group to the richest group were 1.7 for household income, 3.3 for the government measure, and 3.5 for the wealth index. Similarly, for iron deficiency, the odds ratios were 1.5 for household income, 2.9 for the government measure, and 2.4 for the wealth index. The odds ratios for vitamin A deficiency were 3.4 for income, 3.4 for the government measure, and 11.9 for the wealth index. In addition, the correlations with anthropometric z-scores, hemoglobin, ferritin, and retinol binding protein concentrations were higher for wealth index than for household income when analyzed as continuous variables. Conclusions: In this study, measurements of wealth based on a broader variety of household characteristics were better correlated with malnutrition than household income alone. Estimates of SES in surveys assessing nutritional status should thus be based on a broader mix of data than just household income.

ABSTRACT

Introduction :Les tumeurs malignes de la glande submandibulaire se caracterisent par une grande diversite histologique. de ce travail est de rapporter notre experience dans la prise en charge de ces tumeurs et de suggerer une attitude therapeutique apres revue de la litterature. Patients et methode : Il s'agit d'une etude retrospective a propos de 20 patients colliges sur 17 ans. La decision therapeutique etait prise au sein d'un comite multidisciplinaire. Les moyens therapeutiques etaient la chirurgie tumorale et ganglionnaire; la radiotherapie et la chimiotherapie. Les pieces operatoires ont ete adressees pour examen anatomopathologique extemporane et definitif. Une surveillance clinique et radiologique a ete realisee. Le recul moyen etait de 35 mois. Resultats: L'age moyen etait de 60;75 ans. Le delai moyen de consultation etait de 6;15 mois. La tumefaction submandibulaire etait le motif de consultation chez tous les patients. Des adenopathies cervicales etaient notees chez 8 patients (40). Seize patients avaient beneficie d'une tomodensitometrie cervicale et du massif facial et un autre d'une Irm. La radiographie du thorax montrait un aspect de lacher de ballon chez un patient et une opacite mediastinale chez un autre. L'abstention chirurgicale avait ete decidee pour 2 patients. Les patients metastatiques ont ete operes uniquement dans un but diagnostique. Dix-huit patients (85) ont eu une submandibulectomie; dont une elargie a la mandibule. Pour le geste ganglionnaire; 11 patients ont eu un curage selectif triangulaire et 3 un curage fonctionnel complet. Une radiotherapie postoperatoire a ete realisee chez 14 patients (70). Une radiotherapie palliative isolee a ete realisee chez 2 patients. Une chimiotherapie palliative a ete indiquee chez les 2 patients presentant des metastases a distance lors du diagnostic; mais n'a ete administree que dans un cas. La remission complete etait notee dans 11 cas (55). Un seul patient avait presente une poursuite evolutive. Une recidive loco-regionale avait ete decelee chez 2 patients. Deux patients avaient developpe secondairement des metastases a distance. Le taux de mortalite dans notre serie etait de 25. Conclusion: Les cancers de la glande submandibulaire representent une pathologie peu frequente. Le diagnostic a grandement beneficie de l'apport de l'imagerie. La strategie therapeutique depend du stade tumoral au moment du diagnostic et de l'etat general du patient. Le pronostic de ces tumeurs reste toujours reserve; surtout pour les tumeurs de haut grade diagnostiquees a un stade avance

ABSTRACT

O alecrim é planta exótica e o interesse no seu cultivo tem crescido ao longo dos anos, pois pode ser utilizado tanto para fins medicinais como aromáticos; no entanto, o conhecimento fitotécnico sobre a espécie é bastante limitado. Dessa forma, o trabalho objetivou avaliar a influência da altura de corte e do intervalo destes sobre a produção de biomassa e no rendimento do óleo essencial de alecrim. O experimento foi realizado no Instituto Agronômico, em Campinas - SP, no período de janeiro de 2006 a julho de 2007. O delineamento experimental utilizado foi em blocos ao acaso, com 8 tratamentos em esquema fatorial 2 x 4, sendo duas alturas de corte (20 e 40 cm) e quatro intervalos entre cortes (60, 80, 100, 120 dias) e três repetições. Foram avaliadas as características altura da planta, massa seca da parte aérea, rendimento e qualidade do óleo essencial. Observou-se que maiores intervalos entre cortes proporcionaram maior altura da planta e maior massa seca da parte aérea. O rendimento e a qualidade do óleo essencial não foram afetados ao longo do período experimental.


Rosemary is an exotic plant and the interest in its cultivation has been growing along the years, because it can be used for medicinal or aromatic ends, however the agricultural knowledge about the species is quite limited. The goal of this research was to evaluate the influence of the cutting height and of the interval of these cuts on the biomass production and on the yield of the rosemary essential oil. The experiment was conducted at the Agronomical Institute, in Campinas - SP, from January 2006 through July 2007. It was organized in a completely randomized design with 8 treatments in a 2 x 4 factorial system, with two cutting heights (20 and 40 cm) and four intervals between cuts (60, 80, 100, 120 days) and three replications. Characteristics such as plant height, dry mass of the aerial part, yield and quality of its essential oil were evaluated. It was observed that longer intervals among cuttings provided longer height of the plant and greater dried mass of the aerial part. The yield and the quality of the essential oil were not affected throughout the experimental period.

ABSTRACT

The improvement of transfusion medicine technology is an ongoing process primarily directed at increasing the safety of allogeneic blood component transfusions for recipients. Over the years, relatively little attention had been paid to the leukocytes present in the various blood components. The availability of leukocyte removal [leukoreduction] techniques for blood components is associated with a considerable improvement in various clinical outcomes. These include a reduction in the frequency and severity of febrile transfusion reactions, reduced cytomegalovirus transfusion-transmission risk, the reduced incidence of alloimmune platelet refractoriness, a possible reduction in the risk of transfusion-associated variant Creutzfeldt-Jakob disease transmission, as well as reducing the overall risk of both recipient mortality and organ dysfunction, particularly in cardiac surgery patients and possibly in other categories of patients. Internationally, 19 countries have implemented universal leukocyte reduction [ULR] as part of their blood safety policy. The main reason for not implementing ULR in those countries that have not appears to be primarily concerns over costs. Nonetheless, the available international experience supports the concept that ULR is a process that results in improved safety of allogeneic blood components

ABSTRACT

Objectif : Demontrer l'interet du prelevement ganglionnaire sus-claviculaire et jugulaire inferieur dans la prise en charge des cancers differenties de la thyroide (CDT) N0 clinique. Materiel et methodes : Nous rapportons une etude retrospective a propos de 170 patients colliges sur 12 ans 2005). Tous les patients ont eu une thyroidectomie totale en 1 ou 2 temps. Le curage mediastino-recurrentiel a ete pratique soit systematiquement soit en cas de decouverte d'adenopathies en peroperatoire. Le curage lateral a ete realise soit de principe soit apres un prelevement ganglionnaire sus-claviculaire et jugulaire inferieur positif a l'examen extemporane. Des doses ablatives d'iode radioactif ont ete administrees en cas d'elevation du taux de la thyroglobuline ou d'une fixation d'iode lors du balayage cervico-thoracique. Resultats : L'examen anatomopathologique trouvait un carcinome papillaire (74;1); vesiculaire (16;5) ou papillaire a composante vesiculaire (9;4). Trente patients ont eu un curage fonctionnel systematique uni ou bilateral; une metastase ganglionnaire a ete notee chez 9 d'entre eux. Cent quarante patients ont eu un prelevement ganglionnaire uni ou bilateral; un curage fonctionnel a ete pratique chez 6 d'entre eux devant un examen extemporane positif. Les taux de remission; de recidive et de metastases etaient respectivement de 83;3; 6;7et 10dans le premier groupe; et de 89;3; 4;3et 6;4dans le deuxieme groupe. Par ailleurs; un deces a ete observe chez deux patients du deuxieme groupe (1;4) . Aucune difference statistiquement significative n'a ete retrouvee entre les differents taux. Conclusion : Les resultats de notre serie mettent en evidence l'interet du prelevement sus-claviculaire et jugulaire inferieur permettant d'eviter le curage fonctionnel systematique

ABSTRACT

Objectif : Le but de notre etude est l'evaluation de l'atteinte mandibulaire dans les carcinomes epidermoides de la cavite orale et de l'oropharynx. Materiel et methodes : Il s'agit d'une etude retrospective a propos de 34 patients colliges sur 6 ans (1999-2004) et ayantun carcinome epidermoide de la cavite orale et/ou de l'oropharynx avec envahisse- ment mandibulaire confirme a l'anatomopathologie.L'atteinte mandibulaire a ete evaluee par l'examen clinique; l'orthopantomographie; la tomodensitometrie; et par les constata- tions peroperatoires du chirurgien. Tous les patients ont eu un evidement ganglionnaire; une exerese tumorale associee a une mandibulectomie segmentaire interruptrice ou conservatrice. Resultats : L'envahissement mandibulaire a ete suspecte cliniquement chez 70;5des patients devant l'adherence de la tumeur a l'os. Apres examen clinique et imagerie; cette atteinte a ete diagnostiquee chez 88;2des patients. Dans 11;8des cas; l'atteinte n'a ete suspectee qu'en peroperatoire. L'examen histologique a confirme l'atteinte osseuse chez tous les patients. La mandibulectomie segmentaire a ete pratiquee chez 17 patients devant l'atteinte du canal mandibulaire. Vingt-huit patients ont eu une radiotherapie postoperatoire a la dose moyenne de 64 Gy. Les taux de recidive; de metastase et de deces etaient respectivement de 11;7; 17;6et 17;6pour les patients qui ont eu une mandibulectomie segmentaire; et de 23;5; 17;6et 23;5pour ceux qui ont eu une mandibulectomie conservatrice. Par ailleurs; 76;5et 29;5 des patients ayant eu respectivement une mandibulectomie segmentaire et une mandibulectomie conservatrice avaient une gene fonctionnelle importante lors de l'alimentation.Conclusion : L'examen clinique; l'imagerie et l'examen peroperatoire sont d'un apport capital dans l'evaluation de l'atteinte mandibulaire. Une atteinte epargnant le canal mandibulaire justifie une mandibulectomie conservatrice permettant d'avoir de meilleurs resultats esthetiques et fonctionnels tout en assurant un controle carcinologiquement satisfaisant

ABSTRACT

Introduction : Les cancers de l'orbite sont rares. Ils sont caracterises par leur diversite histologique et leur agressivite locoregionale rendant le traitement difficile et mutilant. Dans ce travail; nous rapportons notre experience dans la prise en charge diagnostique et therapeutique de ces tumeurs. Materiels et methodes : Notre etude retrospective a concerne 31 cas de cancers de l'orbite colliges sur 13 ans (1993- 2005). Tous les patients ont beneficie d'un examen clinique complet; d'une imagerie du massif facial (TDM et/ou IRM) et d'une biopsie de la tumeur. Le traitement a ete base sur la chirurgie; la radiotherapie et/ou la chimiotherapie. Resultats : La symptomatologie clinique etait dominee par les signes ophtalmologiques et les algies faciales. L'imagerie a montre dans tous les cas un processus expansif tissulaire a point de depart orbitaire; avec lyse osseuse orbitaire chez 16 patients (51;6); une extension au massif facial chez 7 patients (22;6); endocraniennes chez 6 patients (19;4) et des formes bilaterales atteignant les deux orbites dans 3 cas (9;7) L'anatomopathologie montrait une predominance des lymphomes malins non hodgkiniens (32;3) et des carcinomes epidermoides (32;3); suivis des rhabdomyosarcomes embryonnaires (19;4). Douze patients ont ete traites par chirurgie et radiotherapie postoperatoire; dix patients par une association radio-chimiotherapie; et neuf autres par une chimiotherapie neo-adjuvante. La survie globale etait de 67;8 a 3 ans; 48;4 a 5 ans et 22;6a 10 ans

ABSTRACT

The aim of the study was to declare the presence of oral candidiasis in debilitating children with oral lesions and the association between this candida infection and the existene of certain risk factors was studied. moreover, species identification of the yeasts isolated was carried on by utilizing the new enzymatic culture [chrom agar]. Seven hundred and ninety one child who were admitted to pediatric hospitals in mosul [north iraq] for different diseases were screened clinically for the presence of suspected lesion of oral thrush. Out of them,101 child showed oral lesions from which swabs were taken and send to the department of microbiology, mosul medical college, for direct and cultural examination. Cultural identification was performed by the conventional and chrom agar methods. Out of 101 cases that showed the presence of oral lesions,93 of them revealed the presence of yeasts by both direct and cultural examination. The higher incidence [55%] was in children less than 6 month of age. Gastroenteritis was the main associated risk factor in the studied cases [51%]. The infection among bottle-feeding infants was higer [53%] than breast-feeding infants [15%].the main isolated species was candida albicans [59%], identified by chrom agar culture method. oral thrush can occur in significant rate among debilitating children, especially those with gastroenteritis and bottle feeding manner. The main isolated species by the chrom agar was candida albicans
